Resolves conflicts in CHANGELOG.md (ordering), CONTRIBUTING.md (eval
tools list merge), VERSION (take main's 0.4.1), qa/SKILL.md.tmpl
(keep full methodology + baseline line), eval-store.test.ts (drop
redundant comment).
