fix: REPO_MODE defaults to unknown when helper emits nothing

Changed preamble from `source <(...) || REPO_MODE=unknown` (which doesn't
catch empty output) to `source <(...) || true` followed by
`REPO_MODE=${REPO_MODE:-unknown}`. Regenerated all SKILL.md files.
