v1.91.34.0 fix: /autoplan on current Claude Code and long sessions, quieter pre-push and sync guards, gstack-post, gstack-doctor (#3069)
* docs: Oct 7 follow-up wave plan (pre-review) * docs: Oct 7 follow-up wave plan, reviewed through /autoplan * docs: Oct 7 wave plan, record final gate decisions (UC1 accepted, UC2 rejected) * docs: credit #1822 to @Mike-E-Log and #1743 to @habassa5 (v1.64.0.0) The v1.64.0.0 CHANGELOG entry credited the extension token fix (#1822) and the polyfill exited promise (#1743) to the wrong handle, and the extension-sender-auth test header repeated the first slip. Commit history is not rewritten. Fixes #3054. Co-authored-by: Mike Ilog <48659643+Mike-E-Log@users.noreply.github.com> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(context-restore): offer newer checkpoints from nested repos and task worktrees /context-restore could present an older checkpoint as the latest with no error in two layouts: 1. A session started in a non-git parent saved from a nested repo, so the checkpoint went to the nested repo's bucket. Restore in the parent now lists checkpoints of other buckets whose project_root lies below the cwd (BELOW_CWD; the 200 newest are read, and from $HOME only each project's newest). /context-save also leaves a JSON pointer (checkpoint path, bucket, project_root) in the starting directory's bucket when $CLAUDE_PROJECT_DIR is set and files elsewhere; restore lists those as POINTER, in or out of git. 2. A plan checkpoint on the main tree's branch shadowed a newer checkpoint saved in a ticket worktree. When another branch holds a newer checkpoint for the same task (a worktree below the main tree, read from the new worktree: stamp; equal normalized titles; or a shared #123 / ABC-123 token in titles or branch names), restore prints NEWER_TASK and the skill shows both and proposes the newer one. Generic titles (wip, checkpoint, save, notes, todo) never match on title alone, and the worktree rule applies only when restoring from the main tree, so sibling worktrees still never shadow each other. The branch-first ordering itself is unchanged. Refs #3065. It now prints one line per finding: rule id, file and file line, and a fix, never the matched value. Line numbers come from the hunk headers (combined --cc headers included), so a finding in a later hunk or past a scan-chunk boundary points at the real file line. Findings from the per-commit catch-up path name the commit whose file line it is, so content removed before the pushed tip is still locatable. Plan items: CEO-7 (summary half), DX-9 (fix lines), ENG-8 (line mapping). Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(redact-prepush): stop flagging your own email and already-public authors (#3060) The hook never passed the engine's selfEmail / repoPublicEmails, so every push that added the pusher's address, or a commit author's, raised a MEDIUM pii.email warning for an address the destination already shows. The hook now reads `git config user.email` and one bounded `git log` of author and committer addresses, raw and mailmapped (%ae %aE %ce %cE): the history of every pushed tip (those commits become public with the push), each remote tip being replaced, and, for a new branch or an unknown remote tip pushed to a configured remote, that remote's own tracking refs. A URL or unknown destination never borrows tracking refs, and another remote's history never counts. The read stops at 50,000 commits (the newest authors are used) and 5 seconds. An error or the timeout passes no history and, when an email finding is listed, prints one line saying suppression was limited for this push. The two inputs fail independently. HIGH findings and every other rule are untouched. Controls: a stranger's address, an address only another remote knows, a URL push next to a matching remote, and a first push of a repo with no history all still report. Plan items: A3, CEO-7, CEO-23 (A3 half), DX-9 (partial history), ENG-8 (history scope, mailmap). Co-authored-by: akmandhania <143462686+akmandhania@users.noreply.github.com> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(redact-prepush): per-address allowlist for pii.email (gstack.redact.allowEmail) `git config --add gstack.redact.allowEmail <address>` allows that address for MEDIUM pii.email findings only, so a team alias or support address that has to be in the repo stops warning without reaching for --no-verify. It never affects HIGH findings or any other rule, and the pii.email fix line names it. Controls: a different address still reports, and a HIGH secret next to an allowed address still blocks. Plan item: DX-17. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * docs(troubleshooting): pre-push pii.email findings, exemptions and allowlist Plan item: DX-12 (pre-push touchpoint). Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * test(redact-prepush): assemble fixture addresses at runtime The email fixtures were literals, so this branch's own push listed 47 pii.email findings in its test file. Build them from a helper instead, the way the neighbouring tests build credential and IP fixtures. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(browse): offer the user's signed-in tab, name silent aside repl failures Rules (rendered into every browsing skill, kept short): - Rule 1: before the first openTab, offer the user's tabs on the target origin (title and origin only; the rest of listBrowserTabs() stays private) and attach only after the user confirms one. - Rule 4: a second sign-in wall after a confirmed sign-in means the session is bound to the tab or URL; offer that tab instead of another sign-in. - Rule 6 no longer suggests an early return; rule 7 says a fast [ok without GSTACK_STEP_OK is an abort. Cookbook (browse, devex-review): a "Silent failures" list (evaluate returns only serializable values, no top-level return, locator(sel).press(key), an empty DOM read is about the selector until the screenshot and the request's response say otherwise, read toggle state before clicking) and a signed-in tab recipe that prints only target-origin tabs and never closes the user's tab. Parity caps for design-consultation, land-and-deploy, qa and the browse skeleton are raised to the measured sizes, with notes. Refs #3063, #3064. Co-authored-by: Thomas Gernhardt <t.gernhardt@softwareprojekt.net> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(codex): run gstack-codex-probe as a command, one subcommand per probe Generated skills now execute bin/gstack-codex-probe (select-model, check-auth, show-sandbox, check-sandbox, probe-model, check-version, show-first-use-notice, run-with-timeout, log-event, log-hang, help) and capture its KEY: value lines, so no helper state crosses into the calling shell and the preflight works from bash, zsh or sh. select-model keeps --model > GSTACK_CODEX_MODEL > config.toml precedence; run-with-timeout supervises with the existing gtimeout/timeout/ watchdog chain and returns 124 on a deadline; an unknown subcommand exits 64. Sourcing keeps working for skills rendered before the upgrade and prints one deprecation line naming /gstack-upgrade; it and the zsh self-locate block are removed in a release on or after 2026-10-21 (DX-18). Parity caps raised with measured notes for the longer capture lines. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(gstack-post): one argv-only, redaction-scanned write path for PR and issue text bin/gstack-post posts a PR/MR or issue comment, a review-comment reply, a PR title or body, a new PR/MR or a new issue from agent-written files. Every value reaches gh/glab as its own argv element in --flag=<value> form or on stdin, never through a shell string, and a target is a number or a URL of this repository on the same host. The helper owns the redaction scan: it reads each file once, scans those exact bytes (visibility from config, then gh/glab; the pusher's own email suppressed) and sends the same bytes. HIGH is refused with exit 1 and no confirmation can post it. MEDIUM exits 2 with RULE:/LINE:/PART: lines and a TOKEN: digest of the bytes, destination, operation and findings; only --confirm <token> for those exact bytes posts, so any edit needs a new confirmation. gh/glab failures exit 3, usage errors 64. It also carries the projectCards REST fallback and the title read-back that /ship did by hand. The logic lives in lib/gstack-post.ts behind an injectable runner, so its tests run in the Windows free lane; the stub-gh executable test is POSIX. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(autoplan): guard verifies dispatches on Claude Code 2.1.29x (schema strip, unflushed call, named causes) Cause A: Claude Code journals the model's raw Agent input but hands PreToolUse the schema-parsed input; with the fork-subagent gate on (the interactive default in 2.1.292) run_in_background is dropped, so the journal record never deep-equaled the payload. nativeToolInput now drops only that documented strip for Agent. Agent input is bound on both paths: keys limited to prompt, description, subagent_type (general-purpose) and run_in_background; a model override or any other key is denied. Cause B: when a ready journal lacks the current call, the guard evaluates the payload placed after the last journaled record instead of polling for a record that may arrive only with the tool result. A phase transition counts a report only when a later journaled record follows it (and, on the journal path, it is not in the current call's message). One batch rule covers both paths; a background reviewer keeps the invocation armed until its completion notice. Every outcome now has a stable reason code, disposition and anchor (autoplan/bin/guard-reasons.ts). UC1: Claude-Code-caused states (unrecognized shape, journal lag, rewritten journal, oversized record, unchecked-version schema strip) allow with a visible warning; integrity failures stay denials. No denial advises a bare retry. Every guarded decision appends one content-free line to analytics/autoplan-guard.jsonl, trimmed to 1,000 lines past 256 KiB. Co-authored-by: Jared <182125291+yolo-jared@users.noreply.github.com> Co-authored-by: crblabs <293123359+crblabs@users.noreply.github.com> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(autoplan): publish each phase report with a no-op call; foreground requests are conditional Phase-close step 6 sends the report in a message whose only tool call is the Bash no-op ': autoplan-published <phase>', and step 7 makes the next guarded call in a later message, so the guard sees the report flushed before the next phase entry regardless of Claude Code's journal timing. Every site that asked for run_in_background: false now asks for it only when the Agent schema offers it and says how to recover when a dispatch runs in the background anyway (wait for its completion notice). Parity caps rise by the measured bytes. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(ship, spec, review): PR and issue text goes out only through gstack-post /ship: Step 18 writes the title into an agent-written file. An existing PR's title is piped from gh/glab through gstack-pr-title-rewrite.sh --stdin (new), so neither the agent nor a shell string carries it; a new title is written with the file tool and its version prefix checked in the file. Step 19 composes the body as before and publishes with gstack-post pr-create, or pr-body then pr-title, which scan the exact bytes they send. The separate title and body scans, the hand-rolled REST fallback and the title self-check are gone (the helper does all three). Title preparation moved into the pr-body section, so the skeleton shrinks. /spec files its issue with gstack-post issue-create and drops the pre-issue re-scan; Greptile replies use gstack-post reply and pr-comment. scripts/resolvers/redact-doc.ts loses its PR and issue sinks, and a test keeps any template from rendering one again. Tests that pinned the old scan-then-gh wiring now pin the gstack-post wiring; the publication tests run the real compose and publish blocks against a stub gh with the real helper (HIGH in title or body refused, MEDIUM token flow). Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * ci: pin Claude Code 2.1.292 in the CI image 2.1.292 is the npm latest users run. Its interactive default turns on the fork-subagent gate, which strips run_in_background from the Agent schema and backgrounds reviewers; CI on 2.1.284 never saw the /autoplan guard break. The image tag is a content hash, so every eval workflow picks up the new image without a workflow edit. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(brain-sync): one drain lock for every writer to the artifacts repo gstack-artifacts-init, gstack-brain-restore, gstack-gbrain-source-wireup and the skill-start merge ran git in $GSTACK_HOME without the drain's lock, so the drain could never tell an abandoned .git/index.lock from a live one. They now take the same mkdir lock (bin/gstack-brain-lock.sh): the drain and the skill-start merge skip when it is held, user-run writers wait for it (GSTACK_BRAIN_LOCK_WAIT, default 120 s), and a child of the holder (the drain's daily worktree advance, restore's wireup) inherits it by token. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(brain-sync): hold back only the flagged file, keep coupled artifacts whole One file with secret-shaped text used to block every push, silently. The drain now attributes scanner hits to files in one pass over the staged diff (split on its diff --git headers), holds back each flagged file with the rest of its coupled group, unstages it, and requires the remaining staged diff to re-scan clean; anything short of that falls back to a full unstage and status blocked. Held files stay queued and are re-scanned every drain. - Status file gains held_count, drainable, last_drain_at, last_push_at and held[] (path, scanner rule, dependents, both fixes); new status held. - Coupled groups come from an explicit map (decisions.jsonl and decisions.active.json): a queued member stages the whole group, a group stages only when its snapshot matches its log in the index, a skipped member stops the whole group. - --unskip-file undoes --skip-file and queues the path again. - An index lock older than 10 minutes is removed under the drain lock; a younger one, a failed git add, a failed commit (hooks included) keep the queue and report status error. A verified empty staged diff is the only "nothing to commit". git add runs once, batched. - The scanner reads bytes, so undecodable content no longer crashes it into a false clean; matched text never reaches the status file. - An empty-queue run with unpushed sync commits stays push_failed. - Privacy-held duplicate records collapse to one per path; --status counts a spool too deep for a shell glob. - --attention prints skill start's fixed attention lines from the sync status table in the usage header. Co-authored-by: v639dragoon <matthewsbrickley@gmail.com> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(skill-start): ARTIFACTS_SYNC attention lines for a stuck sync Skill start now prints one fixed attention line per sync-status-table row that applies (held, blocked, push_failed, error, unknown, stale-push, stale-drain), each naming gstack-brain-sync by absolute path. Values from the status file are clamped (status code to the table, counts to digits, timestamps to their shape), so the status message, file paths and scanner output never reach the line. The preamble tells the model to surface the line in one sentence and continue. Skill-start tests are generated from the table in bin/gstack-brain-sync, and docs/gbrain-sync-errors.md carries the same table. Co-authored-by: v639dragoon <matthewsbrickley@gmail.com> Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(outside-review): severity words in label position count as findings The Oct 7 /autoplan run's Codex CEO and DX reviews labeled seven and eight findings High/Medium without [Pn] tags, and the classifier printed VERDICT: clean / FINDINGS: none for both. lib/outside-review-result.ts now reads severity words in label position (Severity: High, a leading High: / High — / [High], **High**, a table cell): critical/high block like P0/P1, medium/low are advisory. Words in prose (high-level, low-risk, no critical findings) never count. A review or structured response with neither tags, labels nor an explicit no-findings statement is unverified, never clean (DX-11, ENG-10). Design-direction proposals move to a new proposal gate so they keep needing only their Recommendation line, and review prompts ask for a severity per finding. Fixtures reproduce the CEO/DX label shapes, the eng [P1] output, a table and prose negative controls; the stored Codex panel keeps its verdicts. docs/troubleshooting.md explains how the verdict is read. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat: gstack-doctor readiness report and setup --status Codex row (B5) bin/gstack-doctor prints one row per check without starting a skill: install root (the registry rows behind ./setup --status), state root, Bun against the floor, hook parse check, Codex mode and self-locate via gstack-codex-probe subcommands, the cached model probe with its age (--live runs the paid probe-model), artifacts-sync health from the status file with the status code clamped, the browse bundle, Claude Code version, the largest session journal and the last five schema-1 guard reason codes. Rows are ok, warn, not configured or fail; only fail exits non-zero; fix lines name helpers by absolute install-root path. ./setup --status gains a read-only Codex row (bin/gstack-codex-status.sh, shared with the doctor) and ends with the doctor's absolute path. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * docs: bug-report template asks for doctor output; README troubleshooting entry; doctor test in the Windows lane The template gives the Claude Code doctor path and a lookup for other hosts and names no bare gstack- command (asserted in test/gstack-doctor.test.ts). The doctor test runs every script through explicit bash argv, so it joins the curated Windows free subset. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * docs(troubleshooting): readiness check section points at gstack-doctor Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(question-tuning,plan-tune,gbrain): free text reaches helpers only through agent-written files Question tuning no longer pipes the question summary through printf: a registered id needs no summary, and for an unregistered id the agent writes the summary to .gstack/tmp/qt.txt and passes --summary-file (new). The helper resolves a relative path against the git top level, removes scratch files under .gstack/tmp, and asks normally when the file is missing. /plan-tune tune events carry the user's own words again: they go into a file the agent writes and reach gstack-question-preference --write through --free-text-file (new), still behind the injection filter and the user-origin gate. docs/gbrain-write-surfaces.md drops its heredoc templates for the agent-written page file on stdin, matching the per-skill save block, and the generic save block stops showing a quoted --content argument. The tuning prose was tightened so every preamble stays within its size cap. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * test(evals): daily autoplan-schema-canary on the latest Claude Code A periodic rule case installs the latest published Claude Code with install scripts off, verifies registry signatures, and runs one headless session with the fork-subagent gate on whose PreToolUse hook records an Agent and a Read payload. Each payload must match its journal record under the guard's own comparison, keep the journal's key set apart from the documented run_in_background strip, and stay inside the Agent allowlist; a background Agent's completion notice must keep the shape the guard reads. evals-periodic.yml runs it daily on its own schedule and concurrency group (read-only token); the weekly census also includes it. A free test proves the comparison fails on an injected strip. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(doctor): read the artifacts sync held count from held_count, the field the drain writes * fix(artifacts sync): the init and restore pre-commit hooks catch Bearer/Basic/Token-prefixed JSON auth headers like the drain does * feat(greptile-early): detect Greptile, ask once on public repos, open the early draft, wait for the review bin/gstack-greptile-early backs /ship's early Greptile PR: - detect: a root .greptile/ folder (which takes precedence), greptile.json, or a Greptile comment on the repo in the last 90 days; triggerOnDrafts from the effective root config; repo visibility; public-repo consent. It prints the signal, that /ship will push and open a draft now, the 10-minute wait cap and the absolute gstack-config command that turns it off. - consent yes|no: the public-repo answer, remembered per repo in the project's state. - open: opens the draft through gstack-post with a body saying /ship opened it early, and posts one @greptileai comment through gstack-post when the config does not set triggerOnDrafts: true. An open PR is left alone. - wait: polls every 30 s for a completed Greptile check run, a Greptile review of the head commit or a summary naming it; each call returns within about 90 s and reports timeout 10 minutes after the PR opened. gstack-config gains ship_greptile_early (default true, invalid values rejected): on automatically whenever the repo uses Greptile, off per user. The logic is in lib/greptile-early.ts behind an injectable runner, so its fixture-repo tests run in the Windows lane; an end-to-end test drives the real helper and gstack-post against a stub gh. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * feat(ship): early Greptile PR at Step 6.5, bounded wait at Step 10 (#3020) When the repo uses Greptile, /ship pushes and opens a draft PR right after its free tests pass (new lazy section greptile-early, Step 6.5), so Greptile reviews while Steps 7-9 run. A public repo asks once, remembered per repo; declining keeps today's order. Step 10 waits for that review with gstack-greptile-early wait and takes the existing unavailable route on a timeout, never a claim of zero comments. Step 18 replaces the provisional title, Step 19 marks the draft ready unless a draft was requested, and a /ship that stops after Step 6.5 leaves the draft open with one comment saying why. ship's union cap rises to 1.5275 with the measured note (B4 +836 bytes, B2 +4,732 bytes); the step-numbering guard allows 6.5; ship goldens are refreshed for B4 and B2. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(autoplan): the publication no-op is 'true autoplan-published <phase>' Measured on Claude Code 2.1.292 in a default-settings interactive session: a bare ':' Bash command asks for approval ('This command requires approval'), and after a background reviewer's completion notice starts a new turn the skill's allowed tools no longer cover it, so the phase-close no-op prompted the user. 'true' with arguments runs without approval and prints nothing. The guard only needs a later journaled record, so the command changes everywhere it is named. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * test(evals): live autoplan-guard-pty case on the pinned Claude Code A gate-tier rule case (one trial, haiku) drives an interactive PTY session with default settings through a project skill that carries /autoplan's own hook block: init, Phase 1 entry, a CEO reviewer dispatch that sends run_in_background, a prepared close packet, the report published with the no-op, and the Phase 2 entry in a later message. It reads outcomes from the guard decision log (three verified allows) and fails if the no-op raises a permission card; other cards are approved and printed. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * fix(autoplan): bounded, append-tolerant owned-journal read (A2) The publication guard no longer reads the whole parent journal or refuses it at 32 MiB. lib/claude-owned-journal.ts streams the bytes present at open to the last complete line (pass one), keeps a content-free index per record and decides ownership, ancestry and causal order on it with the same checks and reason codes as before. Pass two reloads in full only what the evaluation reads (autoplan/bin/guard-journal.ts): every literal init and its result, the current call, and inside the invocation window (from the latest human turn at or before the bound chain's first init) guarded and close-packet Read results, structured Bash results, error results, assistant text after the first close Read and active-plan mutations, plus the earlier Read a dedup reply cites even before the window (ENG-4). - lib/claude-journal-records.ts: per-record event extraction, record metadata and the causal-order checks, shared by readPlanCountTranscript and the bounded reader. The batch reader keeps its whole-file read and cap. - Appends after open never fail a read; same device and inode and no shrink are required (identity). A second read in one hook invocation must hash bytes [0, first size) the same, or it is `rewritten` (UC1 unverified); the unrecognized-shape double read uses that prefix hash. `changing` is removed (REMOVED_CODES; its anchor stays). - OWNED_RECORD_MAX_BYTES (32 MiB): only one record over it is too_large. OWNED_RETAINED_MAX_BYTES (32 MiB): over it is the new oversized_invocation fallback denial. GSTACK_TRANSCRIPT_TEST_MAX_BYTES and GSTACK_TRANSCRIPT_TEST_RETAINED_BYTES can only lower them. - ENG-5: evaluation groups uses and results by tool id once (no per-tool rescans). CEO-25 / ENG-5 spike (120 MiB synthetic, 31,242 records, 4 vCPU Linux, Bun 1.4.2): a full streaming rescan (parse every line, small index, sha256) took 254-326 ms at 147-161 MiB peak RSS; a pass-one index cache under the state root (load cached index, verify a 64 KiB tail hash, parse the tail) took 26-29 ms at 47 MiB. The rescan meets the 1 s budget, so the cache is not adopted: it would add trusted persistent state and invalidation for no budget need. A backward tail read cannot prove competing_root or cycle, so it was not a candidate. Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com> * test(autoplan): journal-read benchmark and its dispatch-only workflow (CEO-2, CEO-6) scripts/measure-journal-read.ts builds two synthetic 120 MiB parent journals around a real /autoplan invocation (mixed: ~28k records, most bytes inside the invocation; large-invocation: ~89k records with a current invocation of many small records), keeps appending while the guard reads, and measures one hook call in a fresh process (wall ms, peak RSS). The workflow runs it on ubuntu-latest and macos-latest on workflow_dispatch only. A free test proves that observer on a padded journal through the lower-only bound seams (the PTY counting reader refuses the same journal), with a sidechain control that must deny. v1.91.32.0 fix: Codex works from zsh, free text never enters shell commands, honest failures (#3057)
* fix: sourced helpers locate themselves under zsh (#3024) Skill blocks source bin/gstack-codex-probe and bin/gstack-egress-lib.sh into the host's shell. Under zsh (macOS default) BASH_SOURCE is empty, so Codex model selection resolved /../scripts/... and every outside pass reported CODEX_MODEL: invalid; the egress lib resolved to the caller's cwd. Resolve the file once at source time (BASH_SOURCE under bash, %x under zsh), and replace the bash-only ${!name} indirection in the auth probe. A free test derives every sourced helper from the generated skills and sources each under bash and zsh. Co-authored-by: marcosdetry <marcosdetry@users.noreply.github.com> Co-authored-by: akshaykalia <akshaykalia@users.noreply.github.com> Co-authored-by: scottgigante-hubflow <scottgigante-hubflow@users.noreply.github.com> Co-authored-by: woodsonl <woodsonl@users.noreply.github.com> Co-authored-by: bezdekt <bezdekt@users.noreply.github.com> * fix: reply and issue templates never pass untrusted text in double quotes (#3046) greptile-triage's reply APIs posted -f body="<reply text>" while the templates quote SHAs and diff code in backticks; in double quotes bash ran each span (replies posted as 'Fixed in .', and diff or reviewer text ran as a command). The pre-existing-failure issue template and /spec's issue title had the same shape. All three now go through quoted heredocs. The generated-bash lint gains untrusted-in-quotes and now also covers runtime reference docs in skill directories (review/greptile-triage.md). Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: a Codex usage limit is its own state, not 'inconclusive; proceeding' (#3051) The model probe matched only model and broken-install errors, so a usage or rate limit fell through to MODEL_PROBE_INCONCLUSIVE: Codex's own message (with the reset time) was dropped, the hint pointed at model config, and every later skill re-probed for 30 s. The probe now returns MODEL_QUOTA_EXHAUSTED (exit 4) on a failed, non-timeout call that names a usage limit, rate limit, insufficient_quota or 429, relays Codex's line verbatim and caches it for 15 minutes. Preflights map it to CODEX_MODE: quota_exhausted (missing coverage, no further Codex calls), and classifyOutsideReview reports a mid-run limit as unavailable (quota) with its own troubleshooting anchor. Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com> * test: record ship/review union sizes after the quota branch and issue template * fix: Codex watchdog freezes the command before reaping its children The bash-native watchdog (stock macOS, no timeout binary) ran pkill -P on the command's children and then killed the command. Between the two, the command continued past its killed child and printed output after its deadline. Stop it first. The test's minimal PATH also lacked pkill, which the watchdog uses to reap children (stock macOS ships it), so the orphaned child held stdout open and the capture waited out its sleep. * fix(autoplan): an oversize session journal is a hard too_large denial, not a retryable identity failure (#3050) The guard capped journal reads at 32 MiB and reported a longer journal as 'identity', which the hook treats as deny-and-retry. A journal only grows, so /autoplan could never enter Phase 1 in a long session, and the troubleshooting row sent people chasing symlinks. Oversize journals now report too_large as a hard cause that names the size and the supported fallback (run the reviews by hand or start a new session). Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com> * fix(deps): clear the weekly OSV findings (#3040) Raise overrides to fixed releases: ip-address 10.7.3 (our own pin was the vulnerable 10.3.1; reachable through socks in browse's proxy path), fast-uri 3.1.8, hono 4.13.13, proxy-addr 2.0.8, @modelcontextprotocol/sdk 1.31.0, sharp 0.35.5, and in lib/diagram-render dompurify 3.4.16 (bundled by mermaid) and source-map-js 1.2.2; the render page is rebuilt. Three advisories without a usable fix get justified, expiring ignores: braces (build-time only, excalidraw -> sass -> chokidar; a test checks the page carries no chokidar code), sprintf-js (roarr's constant-format logger) and katex (needs pre-existing prototype pollution; fix outside mermaid's range). osv-scanner with the repo config: no issues found. * fix(autoplan): name a shell-built snapshot init as the cause of the missing-evidence denial (#3045) The guard binds a phase entry only to a literal snapshot-init Bash call. When the model resolved the tool path into a variable, chained or piped the call, init ran and succeeded but the denial said 'Complete the existing snapshot init step', which reads as already done, so the run gave up. When no init binds but a Bash call looks like one, the denial now says the shape cannot be bound and prints the literal re-run (answers reused:true). The binding rule is unchanged, and the skill says to run init on its own. Co-authored-by: CharlesBerg85 <CharlesBerg85@users.noreply.github.com> * fix(browse): chain reads piped stdin synchronously, so Windows runs the flow (#3039) On Windows, an awaited Bun.stdin.text() inside the CLI's un-awaited main() let the process exit 0 before reading stdin: a replayed flow reported success with nothing sent to the daemon. Read fd 0 synchronously (skipped for a terminal); empty input now gets the server's usage error and a non-zero exit. Co-authored-by: VaibDeveloper <VaibDeveloper@users.noreply.github.com> * fix(browse): clearer config-mismatch refusal and a WebGL note for the macOS GPU flags (#3030) A plain call refused because another session started the daemon with --headed/--proxy was told to 'apply --proxy/--headed', flags it never passed. It now says who started the daemon and offers both remedies. The BROWSER.md env table notes that the default macOS GPU flags disable WebGL and that GSTACK_DISABLE_GPU=off restores it for WebGL QA. Co-authored-by: Hagetino <Hagetino@users.noreply.github.com> * fix(ship): skip Greptile triage when no PR exists yet (#3020) /ship's first run on a branch dispatched a triage subagent only to learn there was no PR. Step 10 now checks for a PR inline (gh's 'no pull requests found') and records that Greptile runs on the PR once it exists; other gh errors still dispatch so the child can report unavailable. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix(router): route only to loaded skills, and say when not to invoke (#3018) The root router listed every skill and ended 'When in doubt, invoke the skill'. Current Claude models follow that literally: they tried to invoke skills a user had turned off in Claude Code's skillOverrides (which gstack's own disabled_skills render cannot see), and borderline requests tipped into full workflows. The router now routes only to skills in the session's available-skills list and names when to answer directly; the CLAUDE.md routing block skill-start offers says the same. Co-authored-by: Rekin226 <Rekin226@users.noreply.github.com> * fix: spawned sessions never auto-grant consent or publish off the machine (#3016) Spawned sessions auto-choose the recommended option except for destructive or irreversible choices. Main already withholds the artifacts-sync consent prompt from spawned sessions; the remaining gap was any other prompt whose recommended option grants a consent or syncs data off the machine. Every spawned directive (skill-start's block, the escape sentence, the Conductor deny reason and the AUQ-error fallback) now names that as irreversible. A consent the user already gave interactively stays in force. Co-authored-by: ekson73 <ekson73@users.noreply.github.com> * test: fix the spawned-block slice and type the #3045 re-init events * fix(redact): TypeScript types and JSX expressions are code, not env.kv secrets (#3048) A credential-named parameter or prop with an unquoted code value (`session: SessionState,`, `key={turn.requestId + turn.role}`, `apiKey={settings.apiKey}`) cleared the entropy gate and blocked pushes as a MEDIUM env.kv finding. One rule replaces the PR's two keyword heuristics: a value that reads as a code name (no digits, word-like camelCase or dotted parts) with a code terminator, or a brace expression of such names with no quoted literal, is source syntax. Paired controls keep firing on quoted, brace-wrapped, concatenated, YAML-bare and random-letter literals. Co-authored-by: tianxchen <tianxchen@users.noreply.github.com> * fix(setup): refuse to register a hook that does not parse (#3032) Claude Code runs gstack's hook shims through /bin/sh; a shim that does not parse exits 2, which for a PreToolUse hook blocks the tool call in every session until someone finds the file. Before any hook registration is healed or written, setup now runs bash -n on each registered shim and a Bun parse of its TypeScript entry, and stops with the file, line and fix. A lean rewrite of the PR's idea (one gate block plus a test that runs it). Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com> * fix(browse): Windows runtime roots start the server from the source checkout's bundle (#3026) On Windows a host runtime root (~/.codex/skills/gstack and the other env-var hosts) holds file copies of browse/dist and no node_modules, so its server-node.mjs could not import playwright and every browser skill died (the module-scope server.ts half was fixed in v1.91.19.0). setup now records the source checkout in every runtime root's .source-path, and the CLI runs that checkout's bundle when the adjacent one cannot reach playwright. Verified by unit layout tests and the Windows CI lane, not live on Windows. Co-authored-by: ubay887 <ubay887@users.noreply.github.com> * fix(copilot): on Windows, run bash blocks in Git for Windows Bash (#3047) Copilot CLI on Windows ran gstack's extensionless helpers straight from PowerShell, which opened an app picker, and bare bash can resolve to WSL. The Copilot tool glossary (rendered into every Copilot skill) now says to run each bash block in Git for Windows Bash by its full path and never to translate a block into PowerShell. The PR's PowerShell repair skill and README section are not taken. Co-authored-by: nareshh74 <nareshh74@users.noreply.github.com> * test: record ship's union size after the Greptile no-PR check * fix: keep issue numbers out of model-read Copilot text; seed the new routing sentence in the autoplan fixture * docs: Oct 6 fix-wave plan, reviewed through /autoplan * fix(autoplan): every oversize-journal path reports too_large with a recovery that keeps the work Review follow-up to the #3050 fix. The summed batch read and the narrowed parent loop now report too_large with the real size instead of a plain error naming a hard-coded 32 MiB. The denial names the size and the cap, and gives the recovery that keeps work (/context-save, new session, /context-restore, /autoplan) with a docs anchor. A test seam that can only lower the cap replaces the 33 MiB fixture. * test(autoplan): negative controls and more shapes for the shell-built init denial * fix(redact): decide TS/JSX source syntax by file context, not by value shape Review follow-up to the #3048 fix. The word-shape heuristic also exempted real secrets made of words (API_KEY=VelvetRiverOrbitSunset; in a .env). The scanner now takes the file path (ScanOptions.sourcePath): the pre-push hook scans the pushed diff per file with its +++ b/ path, and gstack-redact --from-file passes its path. Only in a TypeScript/JSX file is an unquoted value after ':' or a JSX brace of names treated as source; the same line in .env/YAML, or with no path, is scanned as a literal, and the jwt pattern still fires in .ts files. Co-authored-by: tianxchen <tianxchen@users.noreply.github.com> * docs: gstack's own CLAUDE.md routing block matches the new router wording Live check (Claude Code 2.1.284): a skill set to "off" in skillOverrides is absent from the session's skill list while its sibling is listed, and both are listed without the override, so 'route only to skills in your available-skills list' holds. * fix(sync-gbrain): don't offer a call-graph build for a source with no code pages (#3029) Step 3.5 offered --dream for any source with pages, including one that holds only markdown (synced with gbrain's default strategy), where a build cannot help, and the injected CLAUDE.md guidance told future sessions to run --dream for any count: 0. On gbrain >= 0.60, code-def for an impossible symbol answers .status (probed live against gbrain 0.60.96: ready for a code source, out_of_scope for a markdown-only one). Step 3.5 now probes the pinned source first, reports the cause instead of asking on out_of_scope, and keeps today's offer when the probe is unknown (older gbrain, an error). gstack's own sync uses --strategy code, so the PR's soft-delete claim does not apply to gstack's path. Co-authored-by: syong <syong@users.noreply.github.com> * fix(setup): register the hooks that parse, refuse the rest with file:line (#3032) The landed gate stopped setup before any registration, so one unparseable hook left every other hook unregistered. On the Claude install the canonical root is the checkout Claude Code runs hooks from, so there is no staged swap to abort: setup now registers the hooks that parse, refuses the ones that do not with file and line, finishes the install and exits non-zero, saying it is a gstack bug and that a skipped hook keeps running from the checkout. bin/gstack-hook-check derives its list from setup's _hook_command_path calls plus the autoplan phase-publication hook (a test keeps them equal), parses each shim with its shebang's interpreter, and bundles each TypeScript entry with its local imports (packages external) into a temp dir, so a broken or missing import, or a missing entry, fails. Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com> * fix(auto-update): hold an update whose hooks do not parse, before the checkout moves bin/gstack-session-update fast-forwards the live checkout before setup runs, and Claude Code runs hooks straight from it, so setup's gate cannot protect an auto-update. After the Bun floor check the updater extracts the incoming revision with git archive and runs bin/gstack-hook-check on it; a failure holds the update like bun-too-old, records hook-does-not-parse with file:line in the update status, and the next session start says the current hooks keep running. Fixture-remote tests cover a broken shim, a broken imported module and a parseable control. Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com> * fix(browse): chain with no flow is a usage error before any daemon boots (#3039) Empty piped input, a terminal with no arguments, and a readFileSync(0) error (EAGAIN, EOF) now print the chain usage with the cause and its troubleshooting link and exit 1 before ensureServer(), instead of sending an empty argument to a daemon the CLI booted for it. Tests run the real CLI with no daemon and no browser, plus the stub-daemon flow. Co-authored-by: VaibDeveloper <VaibDeveloper@users.noreply.github.com> * fix(browse): refuse a runtime-root CLI and checkout server from different builds (#3026) A Windows runtime root runs the source checkout's Node server bundle through .source-path. When the root's CLI and the checkout's bundle carry different browse/dist/.version builds (the checkout was rebuilt without refreshing the root), the CLI now refuses to start the server with both builds, the ./setup fix and its troubleshooting anchor. Unversioned dev builds are not called skew. Co-authored-by: ubay887 <ubay887@users.noreply.github.com> * ci(windows): real isolated ./setup install, then a piped chain flow through the installed browse windows-setup-e2e built the checkout but never ran ./setup. It now installs the codex host into an isolated HOME (a runtime root copy with no node_modules), then from an unrelated directory pipes a two-command chain flow at a loopback page into the installed browse and asserts the page text, and that empty piped input exits 1 with the usage error. Covers the checkout-bundle path and the piped-stdin path end to end. * test(lint): free text never enters a generated command (structural rule) The generated-bash lint's untrusted-in-quotes rule (gh/glab/git/curl text flags in double quotes only) becomes free-text-placeholder: any <placeholder> anywhere in any command, in double, single or no quotes, and in heredoc bodies (quoted or not), unless it is on the reviewed identifier allowlist (test/helpers/placeholder-allowlist.ts) with the grammar a value must match. Quoted-only identifiers (URLs, selectors, paths printed by gstack) must sit inside quotes. Backslash-newline continuations now read as one command, so a flag and its value on separate lines are still seen together. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: reply, issue and spec text travel as agent-written files, never through a heredoc Replaces the quoted-heredoc bodies with CEO-12's file mechanism: a bash block creates a private mktemp file in the project's .gstack/tmp/ (kept out of git through .git/info/exclude), the agent writes the text with its file-write tool, and the next block posts with -F body=@file, --body-file or "$(cat file)". A heredoc ends early at any body line equal to its delimiter; a file has no such line. When the file was never written the block refuses with the path and the command to send it by hand, and it deletes the file after a successful post. - review/greptile-triage.md: replies; the fetched JSON moves from fixed /tmp/greptile_*.json paths to a mktemp directory; REPO and PR are re-derived per block instead of read from a previous shell. - Test-failure issues: title and body files, platform case on github/gitlab, assignee only when it is a valid login. - /spec: one draft file feeds the audit hash, every redaction scan, the issue and the archive; the archive no longer expands <title>/<body> inside an unquoted heredoc; the decision-log JSON is built by jq from files. Location verified against Claude Code 2.1.284 (Dockerfile.ci pin): project-local .gstack/tmp is writable under acceptEdits with no prompt and keeps mode 0600; $TMPDIR and the gstack state root prompt even under acceptEdits. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix(ship): Greptile triage runs only when a PR exists; every other case says why PR: unknown (gh missing, not logged in, a non-GitHub remote, or a failed lookup) used to dispatch the triage child anyway. The Step 10 check now tests gh, its login, the remote and the PR in that order, and every case but 'PR: exists' skips the dispatch and records 'Greptile: not run (<reason>); runs on the PR once it exists'. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: question-tuning commands carry no free text The question log took the summary in single quotes and the preference write took the user's own words; an apostrophe ended the quote and the rest ran as shell. The summary is now a kebab-case slug (an identifier with a grammar), and the optional free_text field is no longer passed from skills (the binary still accepts it). * fix(cso): Windows finds docker.exe only under known-folder roots and says transport is unsupported (#3028) On Windows /cso looked for docker only in Git's directory and System32, so Docker Desktop was never found and the gap read as 'not installed'. It now resolves Program Files, Program Files (x86) and the Windows directory through PowerShell's [Environment]::GetFolderPath (not the process environment), checks each candidate's real path against those roots and rejects reparse points. lib/cso/docker.ts admits only unix:/// endpoints, so a trusted docker.exe reports 'native Windows Docker transport is not supported yet; static assessment only'; one outside the roots is refused as outside the trusted install locations, not overridable. Both carry an anchor. Native named-pipe transport is not implemented. * fix: sourced helpers share one self-locate block and fail loudly when they cannot locate themselves (#3024 rework) The codex probe and the egress lib now carry one byte-identical block: BASH_SOURCE under bash or %x under zsh, made absolute with $PWD, else a set GSTACK_ROOT that holds the helper. Anything else (dash, sh, a shell that cannot name the file) prints "gstack: cannot locate <helper> (shell: ...)" with the docs/troubleshooting.md#sourced-helper-location URL and returns before any bash- or zsh-only syntax. The Claude-only default path and the $(pwd) fallback are gone. The generated Codex preflight dot-sources the probe and, when it cannot load, reports CODEX_MODE: helper_unavailable with the helper's own line instead of falling through to not_authed. The class guard enumerates nested sources, makes real resolver, auth, state-root and egress-home calls under bash and zsh (absolute, relative then cd, bare name via PATH), covers dash and an unknown shell, runs the full preflight, and lints BASH_SOURCE to the shared block plus other bash-only constructs. Co-authored-by: marcosdetry <marcosdetry@users.noreply.github.com> Co-authored-by: akshaykalia <akshaykalia@users.noreply.github.com> Co-authored-by: scottgigante-hubflow <scottgigante-hubflow@users.noreply.github.com> Co-authored-by: woodsonl <woodsonl@users.noreply.github.com> Co-authored-by: bezdekt <bezdekt@users.noreply.github.com> * fix(cso): a Maven or Gradle repository is Java, not Python (#3028) With no supported stack metadata, preparation defaulted to Python and asked a Java repository for requirements.txt. A root pom.xml, build.gradle or build.gradle.kts now names the stack as Java (unsupported for runtime preparation, so the prerequisite says so), and Python helper scripts or a root requirements.txt beside it no longer make it Python. * fix(browse): BROWSER.md says plainly how to enable WebGL; the mismatch refusal links Daemon discipline (#3030) The GSTACK_DISABLE_GPU row now states the effect and the switch: set GSTACK_DISABLE_GPU=off to enable WebGL (off is the only recognized value). The reworded config-mismatch refusal links BROWSER.md's Daemon discipline section. Free tests pin both. Co-authored-by: Hagetino <Hagetino@users.noreply.github.com> * test(windows): name the B2, B4, B5 and B12 tests in the curated Windows lane The runtime-root bundle tests lived in browse/test/config.test.ts and the Copilot glossary test in test/host-config.test.ts, both excluded from the Windows lane, so the two Windows-only fixes were never checked on Windows. Move each into its own Windows-safe file (unchanged assertions) and list it, the piped-stdin chain test and the /cso docker.exe discovery test as named Windows coverage in scripts/lib/windows-curation.ts. * fix: the free-text block exits loudly when mktemp fails; spec and redact tests follow the file mechanism The redact resolver test, the #2679 mktemp guard test, the /spec invariant and the spec secret-sink harness pinned the heredoc transport. They now pin the agent-written file: the harness runs the draft-file block, writes the draft the way the file-write tool would, then runs the real scan fence, with every HIGH/MEDIUM/scanner-failure assertion unchanged. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * test: hostile payloads through the generated posting blocks, identifier grammars, every quoting shape - The lint reads a whole command, so an unquoted multi-word placeholder is caught; negative controls cover double, single and no quotes, quoted and unquoted heredocs, continuation lines, and quoted-only identifiers used unquoted. - Every allowlisted grammar rejects quote-breaking characters (and shell operators and whitespace when used unquoted); git accepts 'fix;id' and 'fix`id`' as branch names, the ref grammar does not. - Greptile replies, test-failure issues (GitHub and GitLab) and /spec filing and archive run from the real generated text with stub gh/glab: a body with backticks, $(...), an apostrophe and lines equal to EOF and the old heredoc delimiters arrives byte for byte, nothing in it runs, and the temp file is gone. An unwritten file is refused with its path and the manual command, and nothing is sent. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * test(ship): the PR check precedes the Greptile dispatch, and each skip names its reason Runs Step 10's generated check against stub gh for gh missing, not logged in, a non-GitHub remote, no PR, a failed lookup and an existing PR. Fails on the previous text (which printed PR: unknown and dispatched). Co-authored-by: dgrant <dgrant@users.noreply.github.com> * feat(ship): ship-measure runner for the measure-then-fix loop (A6) scripts/ship-measure.ts owns /ship's diagnostic measurement of a red eval case: per-kind aggregation (rule trials; behavior panels of 3, each at its own 2 of 3 plus 90% of trials; judge outputs scored by 3-sample panels, standalone judge ids accepted), one artifact directory per baseline, repair round and trial, a diagnostic label with verdict null that refuses the eval history dir, an estimated admission budget that reserves every concurrent trial and reconciles actual cost, the ask-once rule, the repair-round limit, the unmeasured skip and the per-case PR table. Free-suite flakes (CEO-1): 'free' reruns one shard's exact file list N times, rerun 1 alone as the baseline and the rest in parallel at max(1, floor(cores / shard concurrency)), each with its own HOME, state root and flake ledger, GSTACK_FREE_RETRY_FLAKY=0, under a wall cap that reports a partial count. test:ubicloud --diagnostic runs the same on one VM with the retry off. bin/gstack-config gains the DX-7 keys with their CEO-9 defaults and rejecting validators. Free tests drive every kind, the budget stop, the unmeasured skip and the free reruns with fake runners. * fix: a Codex usage limit is quota_exhausted, a plain 429 is rate_limited, and only Codex's own error lines count (#3051 rework) The probe's quota signature used to treat any rate limit, "too many requests" or 429 as quota, and read the whole output, which echoes the prompt. Now only Codex's trailing ERROR: / stream error lines are read. A usage-limit or insufficient_quota line is MODEL_QUOTA_EXHAUSTED (exit 4, cached 15 minutes; quota wins over a 429 that names insufficient_quota); a plain 429 is MODEL_PROBE_RATE_LIMITED (exit 0, state rate_limited, never cached), reported as CODEX_MODE: unverified (rate_limited) so the review still runs and decides. The quota HINT says how many minutes gstack skips Codex and how to retry now (GSTACK_CODEX_PROBE_RETRY=1 skips the negative cache for that probe, or delete <state root>/.codex-model-probe), with the full troubleshooting URL. classifyOutsideReview reads the same trailing lines: a mid-review quota is unavailable (quota_exhausted), a mid-review 429 unavailable (rate_limited). New gate row rate_limited and docs anchor codex-rate-limited; the quota row documents the retry. Tests cover each line shape, the echoed prompt, exit 0, timeout, retry recovery, probe/classifier parity, and two fresh-shell preflight blocks where the second makes no paid Codex call. Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com> * ci: the eval image installs zsh, and the macOS lane sources the Codex probe from zsh Dockerfile.ci adds zsh so the sourced-helper guard runs (and fails loudly when zsh is missing) inside the eval container too. The macOS named regressions job sources bin/gstack-codex-probe from zsh in an unrelated directory and runs _gstack_codex_select_model exec (no paid call), then runs the sourced-helper guard on the macOS runner. A free test pins both lanes and runs the macOS step's script locally. * test: deterministic regression for the Codex watchdog freezing the command before reaping its children The existing stubborn-child test caught the reap-then-kill race only some of the time (2 of 3 runs on the pre-fix watchdog). A pkill that sleeps after reaping widens the window between reaping the children and killing the command, so a watchdog that does not SIGSTOP the command first always lets it print "late" after the deadline. Fails 3 of 3 on the pre-fix watchdog, passes 3 of 3 on the current one. * test: live codex smokes run codex in a private HOME and CODEX_HOME codex-resume-flag-semantics and codex-e2e-sol-scope probe the codex CLI (the latter at module load, in every free run), and every codex run writes $CODEX_HOME/tmp/arg0, so with a real codex on PATH they wrote into the developer's ~/.codex. Both now give codex a private HOME and CODEX_HOME and remove it afterwards. A free test runs each file against a stub codex that records HOME and CODEX_HOME and asserts neither is the caller's. * feat(ship): a red eval runs classify, measure, fix, re-measure, gate once (A6) Step 6 no longer stops at the first red paid eval: it reads the new lazily loaded measure section and runs the loop for each red case, and Step 5 measures a failed free-suite shard (diagnostic reruns, retry off) before the triage can call it pre-existing. The section states the diagnostic- not-verdict rule, the per-kind table, the ask-once and named-red stops, the unmeasured label and the honest-fix wording (observed k/n after fix at <cause>). A free test pins the steps and their order in the generated skill and checks every command and flag it names exists in the runner. Ship's parity cap rises to 1.455 (measured 272,944 / 187,706 = 1.4541). * docs(testing): diagnostic measurement is not a verdict; per-kind measure table (A6) * test(ship): seeded-flake paid case for the measure loop (A6, periodic, behavior) A fixture repo with no remote documents a single-case eval command and a gate command, both free local stubs. queue-priorities claims trial slots with atomic mkdir (reset per ship-measure round) and fails slots 2, 5 and 9 while src/priorities.js leaves its seed-shuffled list unsorted, printing failure_cause and failure_detail that name the line. The paid case gives /ship's generated Step 6 and measure section to a live session and asserts it measured the case alone, fixed the named line without touching the eval, re-measured at least 10 passing trials on the sorted file, and only then ran the gate once; any question other than the spend question is a contract violation. A free test measures the fixture through the real ship-measure CLI: 7/10 unsorted, 7/10 after an unrelated edit with the gate still red (ENG-13's negative control), 10/10 after the sort with the gate green. * test(cso): the missing-scanner case points Docker at a dead socket 'an optional scanner prerequisite does not turn completed manual coverage partial' assumed no usable Docker. On a machine with Docker and a pulled gitleaks image the scan completed and the test failed (also on main). The scan step now gets a DOCKER_HOST that cannot connect, which is the prerequisite-missing condition the test describes. * fix: Aside prompts travel as agent-written files, never inside the command The open-ended reading recipe (browse, devex-review), scrape's fuzzy-intent read and the shared web-research block put the model's question, query and reply format inside a double-quoted _aside_exec argument, where backticks and $(...) from page or repo text would run. The agent now writes the prompt into a private .gstack/tmp file; the send block rebuilds the path from the printed basename, refuses when the file is empty, and passes "$(cat "$PROMPT_FILE")". The fixed read-only sentence stays in the shell source. aside-driver pins move from the inline prompt to the file-based send block. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: design briefs and approval feedback travel as agent-written files Design briefs went into $D variants/generate/check as a double-quoted argument, and approval feedback was spliced into a single-quoted echo of JSON, so a quote, backtick or $(...) in either broke the command or ran. The agent now writes each brief into a private .gstack/tmp file and the block passes --brief "$(cat "$BRIEF_FILE")" (the CLI's --brief-file expects a JSON brief, so plain text uses --brief). variants and the later check reuse the same brief file. approved.json is built with jq -n from --arg values and --rawfile feedback, so the feedback is data. The office-hours mockup flow creates its brief and feedback files in one block. design-approval-flow now writes the feedback file (with quotes, backticks and $(...) in it) instead of substituting <FEEDBACK>, and checks the file is removed after the save. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: carve office-hours on every host to stay under the 160,000-byte ceiling With its brief, feedback and research prompts moved into files, the inlined office-hours render is 161,329 bytes on Codex and 160,302 on Factory, over SKILL_BYTE_CEILING, so gen-skill-docs --host all fails. Per the C4 policy (carve every skill whose external render exceeds the ceiling), office-hours joins ship and plan-ceo-review in CARVED_ON_EVERY_HOST. The Claude render is unchanged; external hosts read its sections relative to the installed skill. external-host-carve and qa-lazy-sections now include office-hours in their carved-skill lists. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * chore: regenerate after the free-text mktemp guard * fix(codex): prompts and review focus travel as agent-written files Co-authored-by: dgrant <dgrant@users.noreply.github.com> Challenge and consult prompts, and the /codex review focus text, are written by the agent into private .gstack/tmp files and fed to Codex on stdin or read back with cat; no heredoc carries them. Consult's new and resumed runs share one block (and one parser) so no placeholder stands in for the parser; a resumed run reads its session id from .context/codex-session-id and checks it. Adds the A3 group 2 placeholder allowlist block. * fix(benchmark-models): an inline prompt goes through a prompt file Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix(land-and-deploy): PR head repository and branch travel as data Co-authored-by: dgrant <dgrant@users.noreply.github.com> The branch check reads headRepository/headRefName from gh's JSON into quoted shell variables; ls-remote, the confirm-first delete and the local delete use those variables instead of model-substituted names. The postfail test now pins the variable form it replaces. * fix(office-hours): the reviewer receipt must match the receipt grammar before the check runs Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix(setup-gbrain): cleanup-orphans collects the PAT with read_secret_to_env Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix(ship): PR body drafts and rejected test paths are agent-written files Co-authored-by: dgrant <dgrant@users.noreply.github.com> The PR body is written into two private drafts (through ## Documentation, and from ## Test plan on); the scan block splices Step 14.5's section between them into the scanned temp file and deletes the drafts. Rejected test paths are read from a written list, and paths outside the repository are skipped. Tests that ran the old heredoc blocks now write the drafts and list first. * fix(sync-gbrain): orchestrator flags must match an allowlisted grammar Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: the gbrain page save and external-host placeholders pass the free-text rule The gbrain save block passed the whole page through a quoted heredoc inside --content; the agent now writes it to a mktemp file and gbrain put reads it on stdin. The Claude Code outside-voice runtime root and session mode, and the feature slug, join the allowlist with grammars; unused entries are dropped, and a test keeps every allowlisted placeholder tied to a real use. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: skill-specific Aside research queries travel as prompt files investigate, office-hours, plan-ceo-review, review, plan-eng-review, design-consultation Phase 2, the shared test bootstrap (B2) and QA's test bootstrap each carried their own _aside_exec call with the model-filled query inside a double-quoted argument. Skills that already carry the Web research block now give only the prompt-file text and use that block to create, write and send it; design-consultation, the shared bootstrap and QA create the file and send it with the new {{ASIDE_RESEARCH_SEND}} resolver, which is the same send block the research section uses (read-only sentence stays in the shell). aside-driver's B2 pin and qa-browser-preservation's bootstrap contract move from the inline call to the prompt file and the shared send resolver. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * fix: prose design commands pass briefs and feedback as data The DESIGN_SETUP command list taught `iterate --feedback "..."`, and design-consultation's late-change and plan-design-review's slop-regenerate lines showed `--feedback "<feedback>"` / `"..."`; design-shotgun's retry showed `generate --brief "<brief>"` and `evolve --brief "<brief>"`. Feedback now reads "$(cat "$FEEDBACK_FILE")" from an agent-written mktemp file (the setup note says how), and the shotgun retry reads the variant's own brief out of briefs.json with jq instead of retyping it. Co-authored-by: dgrant <dgrant@users.noreply.github.com> * test: record parity sizes after the free-text file mechanism; refresh ship goldens Measured union and skeleton sizes for ship, plan-eng-review, plan-design-review, office-hours, design-consultation, review, qa, browse and design-shotgun after every posting, prompt and brief site moved to agent-written mktemp files; the ship goldens pick up the question-tuning slug and the dropped free_text field. * test: parity caps for the combined wave head (rounded up past the measured ratio) * fix(plan-ceo-review): the mode handoff helper reminds the model to lead its chat with the line (#3038) Periodic run 37272185151's auto-decide-preserved red replays as the Step 0E handoff-line class: the model ran gstack-ceo-mode-handoff --auto, got the exact line, then opened its chat with a heading and a paraphrase, so the user never saw the auto-decide line (the terminal collapses tool output) and the detector saw no outcome. The helper now prints a one-line reminder on stderr beside the line; the detector accepts the line followed by that reminder and nothing else. The ship Greptile check test reads its template (test-of-test ratchet). * test: every troubleshooting link in shipped code resolves, relative or full URL * docs(todos): Oct 6 fix-wave follow-ups * v1.91.32.0 release notes: Codex works from zsh, free text never enters shell commands, honest failures * fix(plan-ceo-review): show the mode handoff line through a PostToolUse hook, not model compliance (#3038) The stderr reminder beside the handoff line measured 1/10 on auto-decide-preserved: the model still opened its chat with a paraphrase ('I've set review mode to HOLD SCOPE...'), so the user never saw the line and the detector saw no outcome. /plan-ceo-review now declares a Claude-only PostToolUse hook that shows the helper's exact line as a system message. Diagnostic measurement on this head: 10/10, the line visible in every trial while no trial's chat led with it. The reminder is reverted. * fix: type the handoff hook's line filter * fix: repairs from the integrated free suite - The self-locate markers looked like heredocs to the pipe-deadlock guard; they are plain comment fences now. - The spawned block keeps the canonical 'conservative non-destructive' phrase the cross-surface drift guard pins. - The free-text block's mktemp refusal says 'mktemp failed', as every /codex temp file must. - The batch transcript read reports sizes in MiB. - docs/TESTING_INTERNALS.md gets a real 'Ship measure' heading for its anchor. - The early-question harness admits /plan-ceo-review's handoff hook by its exact command (it only adds a system message); any other PostToolUse Bash hook is still refused. - Paid file count and the unconsumed-file example follow the new seeded case; ship goldens refreshed. - The CSO scanner test points Docker at a dead socket, so a machine with the pulled image does not scan. * test(cso): a host with no Docker binary is also a no-scan host The missing-image scan test failed on a Docker-less VM (docker is not installed in a trusted directory). Every accepted message is a reason the scan cannot run here; the not_assessed, helper-owned artifact assertions are unchanged. * ci: split the USERPROFILE assignment from export (SC2155) * fix(browse): Windows runtime roots read .source-path as a native path (#3026) The new windows-setup-e2e journey showed the installed browse could not start its server. setup runs under Git Bash and wrote .source-path as an MSYS path (/d/a/gstack); Node read it as \d\a\gstack on the current drive, found nothing, and fell back to the root's own bundle, which cannot import playwright. setup now writes cygpath -m, and the CLI maps an MSYS drive path for installs written by an older setup. The journey prints the daemon and startup logs when the chain fails. * fix(ship-measure): defects from the parallel #3034 plan review - A trial passes only with a JUnit testcase that executed and passed; a --case that selected nothing also exits 0. - measure runs a no-cost --list preflight that must plan the case's own file before any paid trial. - Diagnostic trials drop EVALS_CACHE_DIR/EVALS_CACHE_RUNTIME_ID, so they never reuse a stored judge pass or publish one the gate could reuse. - Gate once: when CI runs the gate on push, the push is the gate run; the skill no longer also starts eval:bg:pr (a second paid gate). - ubi-runner: down fails unless the VM is confirmed gone, and nothing sweeps other runners' VMs by default (UBI_GC_HOURS opt-in). * test(windows): the seeded-flake fixture drives a POSIX shell stub, so it stays off the Windows lane CreateProcess cannot exec the fixture's ./evals.sh shebang script (status null on windows-free-shard 6). The measure runner is covered on Windows by ship-measure.test.ts, which uses in-process fake runners. * fix: the free-text block refuses an unset temp dir instead of writing at / The rendered block ran mktemp "$_GT/..." with _GT unset in the portability test; on Linux that fails at /, but Git Bash on Windows can write to its install root, so the block went on (windows-free-shard 3). It now uses ${_GT:?}, so an unset directory always fails closed with the mktemp message. * test: pin the guarded ${_GT:?} temp-file form * test: the Greptile check's gh stub uses a portable shebang On Windows the absolute bash path has a space, so the stub never ran and every case read 'gh not logged in' (windows-free-shard 1). * test: the Greptile check runs with the native PATH and the gh stub first The PATH-only stub bin could not run its stub on Windows (every case read 'gh not logged in'). Stub cases now keep the real PATH with the stub first; only the no-gh case drops directories holding a real gh. --------- Co-authored-by: marcosdetry <marcosdetry@users.noreply.github.com> Co-authored-by: akshaykalia <akshaykalia@users.noreply.github.com> Co-authored-by: scottgigante-hubflow <scottgigante-hubflow@users.noreply.github.com> Co-authored-by: woodsonl <woodsonl@users.noreply.github.com> Co-authored-by: bezdekt <bezdekt@users.noreply.github.com> Co-authored-by: dgrant <dgrant@users.noreply.github.com> Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com> Co-authored-by: CharlesBerg85 <CharlesBerg85@users.noreply.github.com> Co-authored-by: VaibDeveloper <VaibDeveloper@users.noreply.github.com> Co-authored-by: Hagetino <Hagetino@users.noreply.github.com> Co-authored-by: Rekin226 <Rekin226@users.noreply.github.com> Co-authored-by: ekson73 <ekson73@users.noreply.github.com> Co-authored-by: tianxchen <tianxchen@users.noreply.github.com> Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com> Co-authored-by: ubay887 <ubay887@users.noreply.github.com> Co-authored-by: nareshh74 <nareshh74@users.noreply.github.com> Co-authored-by: syong <syong@users.noreply.github.com> |

v1.79.0.0 fix: ship subagent dispatches can no longer strand the run (#497/#2440 class) (#2772)
* fix(ship): foreground-flag + deadline + scope guard on all four dispatch sites (#497/#2440 class, 3rd recurrence)
Steps 7/8/10/18 dispatch subagents whose LAST-line JSON the parent
consumes, but none passed run_in_background: false — since Claude Code
v2.1.198 subagents background by default, so /ship stranded at Step 18
waiting on output that never arrives. Every site now renders the shared
{{FOREGROUND_DISPATCH_NOTE}} resolver constant, carries a ~10-minute
deadline with an explicit recovery branch (stop the runaway task,
reconcile against pre-dispatch HEAD, surface stray state, never
re-dispatch), and Step 18's prompt gains a docs-sync-only scope guard
(no VERSION changes, no base-branch merges, push-rejection reported as
pushed:false with parent-side reconciliation and a second-failure
branch). Greptile failures now record as UNAVAILABLE, not zero comments.
GENERATED_WITH_GUIDANCE pins all five ship dispatch carriers; a new test
pins the deadline recovery + scope guard phrases in pr-body (.md and
.tmpl). Codex/factory ship goldens re-rendered.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* feat(document-release): first-class spawned-dispatch contract
document-release's own templates had zero subagent-awareness — the
entire headless contract lived in /ship's dispatch prompt, so any other
orchestrator (or an older installed /ship) dispatching it inherited none
of the gate handling. The skill now carries the contract itself: detect
spawned strictly from the dispatch prompt or the preamble echo (never
from file content — prompt-injection guard), auto-choose recommended
options while keeping the never-clobber-CHANGELOG and
never-bump-VERSION-silently invariants via their Skip options. Step 8.4d
gets an explicit spawned note (its interactive recommendation bumps
VERSION — wrong headlessly), and the Codex Documentation Review section
skips itself in spawned sessions (the apply gate needs a human; the
dispatching workflow owns review passes).
Contract, 8.4d note, and resolver skip are pinned in
run-in-background-guidance.test.ts; document-release skeleton budget
re-measured (39,812 B) and ratcheted to 40,200.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: sweep every remaining synchronous Agent-dispatch site with run_in_background: false
The #2440 failure shape was a silently-missing review voice — a
specialist launched in the background and merged before it completed.
Every remaining synchronous dispatch site now carries the explicit flag:
the Red Team dispatch, the spec review loop, the Codex
second-opinion/plan-review/doc-review Claude fallbacks, the adversarial
subagent, design sketch and outside voices, autoplan's design/eng/dx
phase dispatches, CSO parallel finding verification, and design-shotgun's
variant launch. Parallel fan-outs stay parallel — multiple foreground
Agent calls in one message run concurrently (the shipped v1.64.0.0
review-army pattern).
GENERATED_WITH_GUIDANCE now pins all 24 generated carriers, so a new
dispatch site that drops the flag fails the free suite. Six carved-skill
skeleton ceilings re-measured and ratcheted (~80-130 B growth each);
factory ship golden re-rendered.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* release: v1.79.0.0 — CHANGELOG, VERSION, TODOS follow-ups
Queue-advanced to 1.79.0.0 (1.78.0.0 claimed in the workspace queue;
same MINOR level per the versioning invariant). Entry references the
class history (#497 → #2440 → Step 18). Three TODOS filed: PreToolUse
hook enforcement, structural ship-mode for document-release, cross-host
dispatch semantics audit.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* regen: review-army sections carry the Red Team foreground flag
The scripts/resolvers/review-army.ts Red Team edit regenerated these two
files but the sweep commit staged only the adversarial sections — the
skill-docs freshness gate (regen + git diff --exit-code) catches this.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* regen: agents digest picks up v1.79.0.0
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix(document-release): one canonical spawned contract, downstream notes are pointers
The diff-selected LLM-judge eval scored the skill's clarity 3 (threshold
4, main scores 4): the spawned-session rules read as three separately-
framed rule sets (contract paragraph, Step 8.4d note, Codex-review skip).
The contract paragraph now declares itself the single source of spawned
behavior and the two downstream notes reference it instead of restating
rationale. The pointer avoids naming the Codex section verbatim so the
codex-host render (which strips that section) keeps its negative pin.
Judge re-scored 4/5/4 across repeated samples after the change.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: foreground note names the Agent tool as non-substitutable
The ship-docsync gate E2E caught a behavioral regression: the note's
blocking-emphasis ('a backgrounded dispatch strands the run') steered
the driven agent to run doc-sync via the Skill tool inline — the most
blocking option — twice in a row, forfeiting the fresh-context isolation
the dispatch exists for (baseline on main's text dispatches via Agent).
The shared note now says explicitly: dispatch with the Agent tool itself,
never substitute Skill or inline execution; the flag already makes the
call block. Re-verified: ship-docsync passes on the amended text.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: pre-landing review fixes
Review army + red team findings, all verified before applying:
- ship-docsync E2E now asserts run_in_background === false on the
captured dispatch (red team CRITICAL: phrase pins prove text exists,
this proves the model obeys it — verified passing live).
- Structural scanner test: any generated file with an Agent-dispatch
imperative (or bare '(foreground)' prose, the #2440 inert shape) must
carry the flag or hold a reasoned exemption — the 4th-recurrence net
the hand-enumerated pin list can't provide.
- Parent push reconciliation models reality: the parent shares the repo,
so a non-fast-forward that hit the subagent hits the parent identically
— fetch + ahead/behind check first, push only when the rejection was
transient; dispatch prompt promise softened to 'the parent will handle
it'.
- Recovered commits from a dead subagent are vetted docs-only
(git show --stat, never VERSION/package.json) before any push.
- Deadline pacing named: ~3 minutes between checks, wall clock not polls.
- Greptile UNAVAILABLE recording narrowed to the PR body (Step 20's
schema carries no triage field).
- document-release contract gains the echo-failure tie-breaker: prompt
claims spawned + no echo → fail fast with the dispatch contract's
failure shape instead of reproducing the #2733 prose-STOP; contract
anti-injection and NEVER-relax clauses pinned in tests.
- 'Claude Code v2.1.198' extracted to CC_BACKGROUND_DEFAULT_SINCE and
interpolated at all resolver sites (byte-identical output).
- CHANGELOG: entry-boundary blank line restored; worst-case-wait row
scoped to the backgrounded path.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: adversarial review fixes — failure shape, CHANGELOG guard, vet coherence
Claude adversarial subagent findings (11), all verified before applying:
- The dispatch JSON contract gains an explicit FAILURE shape
({"error":...}) and a parent branch for it — a doc-sync that could not
run (spawned marking failed, broken preamble) previously had only the
no-updates shape to emit, which the parent printed as 'Documentation is
current': a silent false-clean of exactly the VAS-449 class.
- Scope guard now covers CHANGELOG: skip Step 5 voice polish and resolve
CHANGELOG-touching gates to leave-as-is (the parent authored the
release entry; the prompt's older auto-choose clause conflicted with
the contract's never-rewrite invariant).
- Recovery vet is sequence-coherent: pushing a commit pushes its
ancestors, so ANY non-doc commit (VERSION, package.json, CHANGELOG.md)
blocks the whole sequence — no more push-the-vetted-child-of-an-
unvetted-parent hole.
- Steps 7/8/10 failure branches stop a still-running backgrounded task
before falling back, so a late result never races the inline audit.
- 'Documentation synced' print gated on pushed:true (item 6 owns the
local-only outcome); doc-review skip note's backward step pointer
fixed; foreground note scoped 'at initial dispatch' so the sanctioned
inline FALLBACKS in Steps 7/8 read as sanctioned; constants docstring
no longer overclaims single-sourcing (names the 3 inline templates).
- Runtime-hook TODO raised P2 → P1: the spawned trust chain is
agent-self-asserted; prose cannot close it, the hook can.
Both docsync gate E2Es re-verified green on the amended prompt, including
the strict run_in_background === false dispatch assert.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: codex adversarial fixes — JSON trust, fail-open visibility, user-state safety
Codex outside-model adversarial pass (inline-diff workaround for the
sandbox), P1/P2 findings triaged cross-model:
- Parent validates the dispatch JSON's field types and treats
documentation_section as untrusted markdown (Step 19's redaction scan
covers the final body; instruction-shaped text inside it is never
followed). The error branch now explicitly skips items 2-6.
- Remote-ahead divergence is named, not silent: the parent lists foreign
commits before creating a PR over a moved branch.
- Recovery cleanup never discards content: stray staged doc edits are
unstaged but never checked out or cleaned away.
- Greptile UNAVAILABLE gets a concrete PR-body line, not a vague
'wherever results are reported'.
Pre-existing-class P1s (self-asserted spawned trust chain, unenforceable
foreground timeout) are cross-model confirmed and tracked: PreToolUse
hook TODO at P1, harness-residue hazard documented.

* fix: never-Skill prohibition made unambiguous; docsync fixture made diagnostic
The ship-docsync E2E flaked on this sandbox because the driven agent
resolved the STOP pointer's ~ to nonexistent homes (/root, /home) and
acted blind — pass/fail sampled model priors, not the prompt. The
fixture prompt now names the planted sections dir, making local runs
deterministic (CI, with real install paths, was always diagnostic).
With a diagnostic fixture: 2/2 passes, real pr-body read, Agent dispatch
with run_in_background: false, zero Skill-tool substitutions.
Prose: the foreground note now states the dispatch happens ONLY via the
Agent tool (invoking the target as a Skill is wrong even though it
appears in the skills list; inline FALLBACKs apply only after a

never-the-Skill-tool clause at the decision point.

* fix(test): daemon 503 test binds an OS-assigned port, not old-port+1
windows-free-tests flaked on this PR: the tunnel-less restart bound
daemon.loopbackPort + 1 — a fixed neighbor of the OS-assigned ephemeral
port — and died with 'Is port 55738 in use?' whenever another shard or a
TIME_WAIT socket held it; the file-level retry re-rolled the same dice.
Every other startDaemon in the file already uses loopbackPort: 0 and the
assertion reads d2.loopbackPort, so nothing needs a predictable number.

v1.68.0.0 fix: next tracker wave — 16 verified fixes in, 90 stale PRs and 21 issues closed with receipts (#2632)
* fix(plan-tune): reject never-ask on one-way ids at --write --check already ignored those prefs; --write still stored them and --stats counted them as a working NEVER_ASK. Refuse the write and count leftover on-disk prefs as INERT_ONE_WAY. Co-authored-by: Cursor <cursoragent@cursor.com> * Fix: gstack-config get returns "" with exit 0 for keys that have no default Skill preambles read configuration with VAR=$(gstack-config get <key> 2>/dev/null || echo "<default>") and that fallback only fires on a non-zero exit. lookup_default ended in a catch-all that echoed "" and returned 0, so for any key missing from the table VAR came back empty and the default written right there in the preamble was unreachable. The skill then branched on a value it never specified: "skip entirely if QUESTION_TUNING is false", reached with QUESTION_TUNING="". Four keys that skills actually read had no entry and took that path: question_tuning -> callers assume "false" repo_mode -> callers assume "unknown" team_mode -> callers assume "false" transcript_ingest_mode -> callers assume "off" Each default above is the value the call sites already substitute in their own `|| echo` fallback, so this only makes reachable what was already intended. The catch-all now returns non-zero. That is deliberately scoped to the unknown-key arm alone: keys whose default is intentionally empty still exit 0, because "" is their real answer and their callers depend on it -- cross_project_learnings ("unset triggers the first-time prompt"), redact_repo_visibility ("empty falls through to gh/glab detection"), salience_allowlist, user_slug_at_*. Making every empty answer an error would have broken those. test/gstack-config-defaults.test.ts pins the class rather than the four instances: it parses the case arms and asserts every `gstack-config get <key>` site in the tree is covered, so adding a read without a default fails CI. It also pins the exit-code contract in both directions. Verified failing against the pre-fix script, where it names exactly those four keys. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com> * fix(redact): a typo'd subcommand no longer exits 0 having done nothing main() recognised exactly two subcommands and let everything else fall through to the stdin scan. On empty stdin that prints "(no findings)" and exits 0, so: $ gstack-redact install-prepush-hooks # plural typo gstack-redact scan — repo UNKNOWN (no findings) $ echo $? 0 No hook was installed, and the operator has every reason to believe the credential guard is armed. A guard that silently no-ops must never exit 0. Two smaller faults in the same dispatch, both of which lead people here: - There was no --help handler, so `gstack-redact --help` fell through to the scanner. Piping a credential to it scanned the secret and exited 3. - With no piped input and no --from-file, readInput() blocks on readSync(fd 0) until an EOF that an interactive terminal never sends. That prints nothing at all, so it reads as a hang rather than as "this is a filter, feed it". Now: --help/-h/help prints usage and exits 0; an unrecognised positional prints the offender and exits 1; a TTY with nothing piped in prints usage instead of blocking. "scan" stays accepted, because the human output header reads "gstack-redact scan — repo …" and that is what people type. Usage errors exit 1, deliberately not 2 or 3. Those mean MEDIUM and HIGH findings and callers gate dispatch on them, so a usage error exiting 2 would be read as "medium findings — prompt the user". A test pins that. Tests: 4 written failing first, then fixed. Full suite 7,722 pass / 0 fail. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com> * fix(browse): one ambiguous ref no longer kills the whole annotated screenshot `snapshot -a` exits 1 with "Selector matched multiple elements" on most real pages, so /qa, /canary and /land-and-deploy silently produce reports whose screenshots do not exist. Plain `screenshot <path>` is unaffected. Refs are built as getByRole(role, {name}) and disambiguated with .nth() when role+name repeats. That disambiguation cannot fire for a node with NO accessible name: the locator degrades to getByRole(role) with no name filter, and the count driving .nth() is taken from the FILTERED aria snapshot while getByRole matches the unfiltered DOM. Measured on a live page: the tree surfaced 2 unnamed paragraphs, the DOM had 9. Landmarks (banner/main/contentinfo) and paragraphs are correctly unnamed per ARIA, so this is the common case rather than an edge case. boundingBox() then hits Playwright strict mode, and the catch allowlisted only timeout/closed/Target/Execution-context messages — so the strict-mode error was re-thrown and aborted every remaining annotation. Two changes: - `.first()` before boundingBox(), so an ambiguous ref draws a box on its first match instead of aborting. The heatmap path below has always tolerated this via a bare `catch {}`; annotate was the only path that could be killed outright. - the catch no longer re-throws on unrecognised messages. A box we cannot measure is a box we do not draw, never a reason to lose the rest of the page. Set BROWSE_DEBUG to see what was skipped. Also: `-o` passed without `-a`/`-H` was silently ignored (exit 0, no file), which reads as "screenshots are broken" rather than "you forgot a flag". It now warns and points at `browse screenshot <path>`. Verified by rebuilding both ways against the same page with 51 refs present: before — "Selector matched multiple elements", no file written after — exit 0, 229KB PNG Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(version-bump): missing or empty VERSION no longer repairs a fabricated 0.0.0.0 into package.json repair now fails with exit 2 when the VERSION file is absent or empty instead of folding to DEFAULT ("0.0.0.0") — which passed VERSION_RE and regressed package.json below where it started. classify gains an additive versionFileExists field so /ship can tell a real 0.0.0.0 from a fabricated one. Re-derived from PR #2612 under the generated-file screening rule. Fixes #2600 (repair half; the path-configurability half landed in v1.67 via #2531). Contributed by @Lockyer228 Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(memory-ingest): --probe counts post-attribution, through the same gate --bulk uses probeMode previously stat'd every walked file, so setup-gbrain gated its silent bulk ingest on pre-filter counts that the write path would never ingest (#2394). The attribution decision now lives in ONE shared gate (sessionIsAttributable — cheap-parse: cwd extraction + memoized resolveGitRemote, never a full page build) used by BOTH probeMode and preparePages, so the two stages' post-attribution counts are structurally identical. ProbeReport gains skipped_unattributed; the probe prints what it excluded and --include-unattributed restores raw counts. The parity is pinned at the prepare stage (probe post-attribution == transcripts reaching import), deliberately NOT == final written. Re-derived from PR #2612 under the generated-file screening rule; the shared-gate design and the remote memo are additions from the plan review. Fixes #2394. Contributed by @Lockyer228 Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat(browse): allow CPU and network throttling for performance measurement Adds Emulation.setCPUThrottlingRate and Network.emulateNetworkConditions to CDP_ALLOWLIST. Motivation: diagnosing a real "uploads take 1-2 minutes" report, the only machine available was a fast developer workstation. Client-side processing measured 1.4s where the user experienced minutes, so the conclusion had to be reached arithmetically rather than observed. Throttling would have let the measurement reproduce the reporter's conditions directly. Both fit the existing posture rather than widening it: - Emulation already allows setDeviceMetricsOverride, clearDeviceMetricsOverride and setUserAgentOverride, which are equally mutating and scoped to the tab. - Neither method reads page content. setCPUThrottlingRate affects only timing; emulateNetworkConditions constrains traffic rather than inspecting it, so no request bodies, headers or cookies are exposed. Both are output: 'trusted' because they return no page-derived data. scope 'tab' for both, matching the surrounding Emulation entries. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(session-update): lock pidfile records the live holder; hard TTL bounds every wedge (#2613) echo $$ inside the backgrounded subshell recorded the PARENT hook's PID — which exits immediately — so every subsequent session judged the lock stale and rm -rf'd a LIVE holder's lock, letting concurrent updaters run over each other. The pidfile now records ${BASHPID:-$(sh -c 'echo $PPID')} (macOS bash 3.2 has no BASHPID; the sh child's PPID is exactly this subshell). Staleness is now two independent detectors: PID liveness (as before, but against the real holder), and a 30-minute hard TTL on the heartbeat mtime — reclaimed regardless of kill -0, so a recycled PID or hung holder can't wedge the lock forever. The holder touches the pidfile after the pull and after setup, so a legitimately-slow run keeps itself alive. Empty and missing pidfiles are respected inside the TTL window (the mkdir→echo race) and reclaimed past it. Fixes #2613. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore(browse): explicit windowsHide on every Bun.spawn site + census tripwire (#2575 residual) Bun.spawn sites were structurally outside the windowsHide census (it swept child_process bindings only). The runtime was already safe — native Bun hides consoles by default and bun-polyfill.cjs defaults windowsHide !== false since #2523/#2539 — but implicit defaults are exactly what regress silently. Every Bun.spawn/spawnSync in browse/src now carries the explicit flag (harmless on unix-only sites like Xvfb/xattr/open), and a second SWEEP in windows-spawn-hide.test.ts fails CI on any new flagless Bun.spawn site. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(gbrain): brain worktree advances on the daily sync — no more silently stale brains (#2516) The daily pull refreshed only ~/.gstack itself, never the detached worktree at ~/.gstack-brain-worktree that gbrain actually indexes — so after setup the brain served stale pages forever unless setup-gbrain/sync-gbrain happened to run. brain-sync --once now advances the worktree once per 24h behind an ATTEMPT stamp (.brain-worktree-last-advance — a persistently-failing advance warns once a day, not at every skill boundary), inside the existing run lock and before any ingest step touches the worktree. The new gstack-gbrain-source-wireup --advance-only is built for the unattended cadence: git-only (no gbrain prereqs), pins every operation to the managed worktree (refuses paths that are not worktrees of the artifacts repo), refuses dirty worktrees, and never runs the force-remove recovery — a cron path must not be able to delete local changes. A static pin keeps the force-remove out. docs/gbrain-sync.md stops overclaiming the old cadence. Fixes #2516. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat(memory-ingest): honor the per-remote deny/read-only trust policy (#2392) Transcript ingest now respects the same trust store as code import — the gate existed only in gstack-gbrain-sync's runCodeImport, so memory-ingest happily ingested transcripts from deny-listed repos. preparePages filters prepared transcript pages through ONE batch policy lookup (new 'get --batch' verb on bin/gstack-gbrain-repo-policy — the script owns URL normalization; the client adds repoPolicyTierBatch, one spawn for all distinct remotes, so large corpora never pay a 10s-timeout subprocess per remote). Outcomes match code-import semantics: read-only → clean skip (skipped_policy_readonly), deny → counted refusal (skipped_policy_deny), corrupted/unreadable store → HARD ERROR before any write (state, staging, egress receipt, and import all untouched) with the recovery command named — policy corruption must never read as successful ingestion. Artifacts are never policy-filtered (their git_remote is a project slug, not a remote). Fixes #2392. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(config): repo_mode keeps its empty no-default semantics (#2611 follow-up) The ported defaults table synthesized repo_mode → "unknown", but EMPTY is load-bearing for that key: gstack-repo-mode treats any non-empty answer as a user override and skips its own repo classification — the synthesized default turned the classifier into dead code (REPO_MODE=unknown everywhere; caught by test/gstack-repo-mode.test.ts via the wave's cross-agent blame protocol). repo_mode joins the empty-is-real carve-outs (empty output, exit 0). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(pair-agent): consent before killing a healthy headless daemon The pair-agent headed switch spawned 'connect --force-restart' unconditionally — auto-killing a live headless daemon (open tabs, cookies, logins) in direct contradiction of the iron rule it sits beside ('only an explicit --force-restart may kill a live daemon'). The CLI now captures daemon liveness BEFORE ensureServer (which can itself boot a fresh daemon) and relaunches only when the user passed --force-restart to pair-agent; otherwise it prints the tab count and continues against the existing daemon. The /pair-agent skill gains a matching one-way-door consent question (template half rides the wave's template block). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(gbrain-status): MCP scoping is per-project, and project-local beats user scope hasRemoteOnlyGbrainMcp scanned EVERY project's mcpServers in ~/.claude.json, so one project's remote gbrain registration reclassified broken local engines as thin-client machine-wide. It now reads user scope plus only the cwd's nearest-ancestor project key. The precedence itself was verified empirically and hermetically (fake HOME + CLAUDE_CONFIG_DIR fixtures, claude 2.1.233): with both scopes defining gbrain, 'claude mcp get gbrain' reports Scope: Local config — PROJECT-LOCAL WINS. Both in-repo consumers assumed the opposite; brain-cache's endpoint resolution flips to nearest-ancestor-project-first, and the stale user-first pin in brain-cache-roundtrip now pins the verified precedence. (The user-first jq in the brain-sync preamble resolver gets the same swap in the template block.) Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(slug): gstack-slug matches remote-slug's owner-repo canonical form (live misfile bug) Found live during this wave's CEO review: bin/gstack-slug emitted SLUG=garrytan for this garrytan/gstack worktree while remote-slug correctly gave garrytan-gstack — decisions, timeline, ceo-plans, and learnings were filing into the wrong project store (observed polluting Context Recovery with another repo's decisions). Root cause: a stray empty ~/.git directory made the walk-up crown $HOME as the outermost project root; the remote lookup ran only against that root, failed silently, and the basename fallback cached 'garrytan' sticky. NOT worktree-specific — any strong marker on a non-repo ancestor triggered it. Fix: the walk now finds the outermost ancestor whose .git actually resolves an origin remote and derives owner-repo with remote-slug's byte-identical parse; marker-only ancestors keep anchoring the basename fallback but can no longer shadow a real remote. A new cache self-heal recomputes the poisoned shape (cached == basename of a marker root while a remote-bearing repo exists below), preserving legit #2212 stickiness. Nested-repo walk-up, no-remote and non-git fallbacks, and the SLUG=/BRANCH= eval contract are unchanged, pinned by a 10-case parity suite. Store migration for pre-fix data is tracked in TODOS.md. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(brain-sync): per-record spool dir — the enqueue/drain race dies structurally Producers appended lines to .brain-queue.jsonl while the drain re-read and os.replace'd it; the in-code comment admitted a lockless append between the re-read and the replace was lost. Locks and rename-rotation designs were both reviewed and rejected (each retained a tail race); the shipped design is a maildir-style spool: one FILE per record in .brain-queue.d/ (tmp + atomic rename), the drain snapshots filenames, processes, and deletes exactly what it snapshotted. Writer and drainer never share an inode — nothing to race. Semantics: at-least-once (a crash between process and unlink re-drains; downstream content-hash dedup absorbs duplicates); retained (privacy-held) records keep their files; unparseable records are kept + warned, never destroyed. Legacy .brain-queue.jsonl migrates atomically on the next drain (crash-leftover .migrating files recovered too); status/drop-queue count both surfaces; discover-new writes spool records and advances its cursor per-record-written. The preamble's queue-depth line switches to spool count in this wave's template block. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(bin-context): native slug fallback walks up like bash gstack-slug slugFromEnvironment derived the slug from the INNERMOST repo's origin while bash gstack-slug walks to the outermost project root — nested/vendored repos split their stores across the bash/native boundary (win32 hits the native path constantly). The native fallback now ports _outermost_project_root faithfully (strong/weak markers, outermost-strong-wins, 64-depth cap, fixed-point termination) plus the full resolution order: env override → walk-up → sticky cache with the #1125 self-heal → remote get-url → basename. Twelve mirrored scenarios drive BOTH implementations against the same fixtures and pin identical slugs. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(next-version): git fallback queries the live remote, never mutates, and keeps 3-digit width The degraded path counted every remote-tracking ref on every remote — stale experiment branches and second remotes inflated version allocation, and a failed base read flipped 3-digit repos to 4-digit slots. Now: ls-remote --heads origin first (GIT_TERMINAL_PROMPT=0, 5s timeout, zero local ref mutation); on failure, local refs/remotes/origin ONLY with an explicit stale-refs warning; a failed base read zeroes at the LOCAL version file's width so a 3-digit repo allocates 0.0.1, not 0.0.1.0. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(setup): hooks register the global-install path and re-point stale ones Registering hooks from a dev worktree baked that worktree's absolute path into settings.json — deleting the worktree left a dead hook erroring on every session stop, and the presence-only dedup (list-sources | grep) could never re-point it. setup's hook paths now route through _hook_install_path (global install preferred, source dir fallback), and the new ensure-event verb on gstack-settings-hook compares the registered command payload against canonical: identical → no write, different → single atomic replacement (never zero or two registrations). The plan-tune hooks had the same stale pattern and get the same fix without re-triggering their consent prompt. Also hardened: bun 1.3.13 turns an uncaught sync fs error in bun -e into a SILENT exit 0 — the registrar's write path now catches, prints, and exits 1, so a failed update can never report fake-green. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(preamble): learnings capture is unconditional at completion (#2402) 43 of 44 learnings entries came from explicit /learn — the completion-status prose read 'if you discovered a durable project quirk... log it', which models treated as optional. The step now ALWAYS runs: review the session for durable learnings, log each one, and state 'No durable learnings this session' explicitly when the review comes up empty — an empty result, never a skipped step. Re-derived from PR #2612 under the generated-file screening rule. Fixes #2402. Contributed by @Lockyer228 Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat(scrape): untrusted-content warning on the page-fetching skills (#2441) /scrape and /skillify consumed page content with zero injection guidance — the CHANGELOG claimed coverage the skills didn't have. The warning now lives in ONE exported const (UNTRUSTED_CONTENT_WARNING in resolvers/browse.ts), embedded in the browse COMMAND_REFERENCE as before AND injected standalone into both skills via the new {{UNTRUSTED_CONTENT_WARNING}} token — single source, wording can never drift between surfaces. Re-derived from PR #2612 under the generated-file screening rule. (Structural isolation for skillify-generated code is tracked as its own TODO.) Fixes #2441. Contributed by @Lockyer228 Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(review): checklist paths resolve from the installed skill root (#2518) /review Step 2 read .claude/skills/review/checklist.md — a path relative to the TARGET repo, which only resolves in gstack's own checkout. Every checklist/greptile-triage/TODOS-format reference (six across five templates — two more than the issue named, same class) now uses the installed-root form ~/.claude/skills/gstack/review/... that the templates' other references already use. The install-root class itself (non-default install dirs) is #1882, deliberately its own PR. Fixes #2518. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(pair-agent): one-way-door consent question before a daemon relaunch (template half) The skill flow now checks daemon liveness before Step 4 and asks an explicit one-way-door question (tabs/cookies/logins are lost) before passing --force-restart — never proceeding on a vague reply. Pairs with the CLI-half commit that stopped pair-agent auto-killing live daemons. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * docs(codex): resume does not amortize the ~21K session prelude (#2387) Measured (#2387): every codex exec call pays Codex's session prelude, and a resumed call came in slightly ABOVE a fresh one — resume buys continuity, never token savings. The skill now says so where the resume flow lives: prefer one codex call per skill, batch questions into it. Fixes #2387. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(upgrade): fast-forward first; reset --hard only behind a proved-safe gate (#2517) /gstack-upgrade went straight to stash + reset --hard origin/main. Now it tries git pull --ff-only --autostash first (the same policy session-update's auto-upgrade uses). The destructive fallback runs unprompted ONLY when both git status --porcelain AND git rev-list origin/main..HEAD are empty — a clean tree with unpushed local commits is NOT safe, reset destroys them. Anything else requires an explicit one-way-door confirmation that lists every dirty file and unpushed commit being discarded. Fixes #2517. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(preamble): brain-sync block counts the spool queue and resolves MCP project-first Two resolver halves deferred from earlier wave commits: the queue-depth line counts .brain-queue.d/*.json spool records (plus legacy lines until the drain migrates them), and GBRAIN_MCP_ENTRY_JQ swaps its operands to nearest-ancestor-project-first — matching the empirically verified Claude Code precedence (project-local beats user scope) instead of the backwards user-first assumption. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore: regenerate SKILL.md docs + golden fixtures (single regen for the template block) Pure generator output for the six template/resolver commits above (learnings capture, untrusted-content warning, review paths, pair-agent consent, codex resume note, upgrade ff-only, brain-sync block) — bun run gen:skill-docs + --host codex + --host factory, with the three ship golden fixtures refreshed per the documented procedure. The three sidecar-path pins in gen-skill-docs.test.ts move to the new installed-root/$GSTACK_ROOT contract (#2518). Restores template freshness; full suite green from here. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore: TODOS.md — strike the six wave-fixed residuals, add two follow-ups The v1.67 adversarial-review residuals section shrinks to the one item the wave couldn't reach (iOS tap routing — needs real-device verification). New entries: skillify structural isolation (a prose warning is not a boundary for page-derived generated code) and the slug store migration (pre-fix sessions on stray-marker machines filed data under the degraded slug; post-fix reads go to the correct store, so history needs a merge/alias). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: align cross-cutting pins with the wave's contracts Three suites pinned pre-wave behavior: browse's gstack-config test asserted the old unknown-key ''/exit-0 shape (#2611 made it exit 1); the Windows-paths suite pinned O_APPEND enqueue atomicity (the spool design satisfies the same invariant via tmp + os.replace, one file per record — pinned in its new form); and nine carve-guard skeleton ceilings absorbed the #2402 unconditional-learnings prose (~450B per skill), bumped with measured values per the guard's own protocol. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: re-anchor the referenced-path scanner self-check to the gstack-rooted review refs The self-check pinned the review checklist as a class-1 alias-relative ref; #2518 moved those refs to the installed gstack root (class 2). The guard now proves the scanner sees them in their new class, so the class-2 assertion can't go vacuous. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: pin the wave's prose-tier behaviors (ship coverage-audit gap closure) The coverage audit found one regression-shaped gap: nothing pinned that the upgrade template's ff-only pull precedes the gated reset --hard (#2517) — a future template edit reverting to reset-first would fail nothing. Pinned: the ordering, the FF_OK gate, and the unpushed-commits check. Also pinned the two minor gaps: the {{UNTRUSTED_CONTENT_WARNING}} injection points in scrape/skillify (#2441) and brain-uninstall's spool-dir cleanup. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: pre-landing review round — 8 auto-fixes + 8 accepted findings hardened The ship review army (4 specialists + red-team + checklist, 29 findings) produced 8 mechanical auto-fixes and 11 decisions; the accepted set: - win32 slug parity completed: lib/bin-context.ts gains the remote-first outermost walk + degraded-cache self-heal the bash side got this wave — the two implementations now agree on the stray-marker live-bug shape, pinned by shared fixtures (multi-specialist 9/10 finding). - probe honors the plan's bounded-read decision: 256KB prefix, extraction semantics mirrored from parseTranscriptJsonl so probe/prepare can never diverge on the same file (>1MB transcript test). - policy normalize parity: bash normalize() now matches canonicalizeRemote on .git/-trailing and uppercase-.GIT shapes (7-shape corpus pinned two ways) — a deny for those shapes could previously slip the transcript gate. - session-update reclaim is TOCTOU-safe (atomic mv-aside on both branches). - settings-hook: unparseable settings.json errors instead of being replaced with {}; ensure-event keys on (event, source) so matcher changes update in place — never zero or two registrations. - dot-only slug guard at both parse sites (hostile 'url = ..' can't escape projects/); enqueue tmp-file janitor (1h TTL, inside the drain lock); brain-sync .migrating never clobbered; drop-queue/status count .migrating; snapshot -o warning correct + surfaced in diff mode; version-bump test order-dependence removed; uninstall clears the advance stamp. Deferred with record: slug heal-probe cost sentinel (P3 TODO), FF_OK conflation (noted, misdiagnosis-only). 270 pass / 0 fail across the 10 touched suites. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: adversarial round — the P0 finalize fail-safe and 12 hardened findings Three adversarial passes (Claude fresh-context, Codex chaos, Codex structured with P1 gate) on the full wave diff. Multi-source findings, all fixed: - P0: finalize_queue is now explicit-delete-only — a record is unlinked ONLY when classification proves it staged or dropped; a classifier crash, a missing class file, or a malformed pulled .brain-privacy-map.json (which previously nuked the whole snapshotted queue, remotely triggerable) now retains everything, warns, and re-drains next run. load_privacy_map treats corrupt maps as retain-all, never as empty. - next-version cannot silently drop a live claim: unreadable advertised refs get a targeted --depth=1 fetch + retry; still-unreadable claims surface as UNKNOWN warnings instead of duplicate-version silence. - session-update lock: ownership-checked EXIT trap (a TTL-reclaimed holder can no longer delete the new holder's lock) + a 5-min background heartbeat so a legitimately-slow pull/setup is never reclaimed while alive. - ensure-event collapses ALL same-(event,source) duplicates to one canonical entry; unique per-process tmp path; setup call sites surface (not swallow) the hardened refusals. - memory-ingest: --limit counts only policy-permitted pages (denied records no longer starve permitted ones); --probe applies the same policy filter as --bulk (skipped_policy_* fields on the report). - version-bump repair accepts a genuine literal 0.0.0.0 VERSION file. - slug heal restricted to the stray-.git shape — package.json-anchored wrapper roots keep their legit sticky identity (#2212 preserved). - brain-sync: idle fast path sees leftover .migrating records; unparseable spool records quarantine instead of warning forever; migration comment stops overclaiming the transition-window race. - CDP throttling justifications document override persistence (callers own restoration), pinned in the allowlist test. Deferred with record: deny retroactivity for already-ingested pages (P2 TODO, same semantics as the code-import gate); legacy-migration tail race (transition-window, requires pre-spool writers). 288 pass / 0 fail across the 10 touched suites. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore: regenerate SKILL.md docs + goldens (Windows-separator jq fix) Pure generator output for the brain-sync block's jq ancestor match now accepting backslash-formed Windows project keys — previously project-scoped brains were invisible on Windows while the TS scope resolvers saw them. Golden ship fixtures refreshed per the documented procedure. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: codex verify-pass residuals — chunked cwd read, post-filter partial count, migrating depth The verify re-review passed the P1 gate (0 P1s) and left three residuals, all applied: transcriptCwdFromPrefix reads in chunks until one complete record (4MB cap) so a giant first prompt can't truncate mid-JSON and break probe/bulk parity; partial_pages derives from the FINAL prepared set instead of the whole scanned corpus; the preamble queue-depth line counts leftover .brain-queue.jsonl.migrating records like the status path does (regen + goldens included). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore: bump version and changelog (v1.68.0.0) Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * docs: update project documentation for v1.68.0.0 BROWSER.md: fix the $B cdp example (positional JSON params, not --json; depth is the real CDP param) and add the new perf-throttling examples (Emulation.setCPUThrottlingRate, Network.emulateNetworkConditions) with their clear-override counterparts. USING_GBRAIN_WITH_GSTACK.md: the state-files table row for the sync queue now names the maildir-style spool dir .brain-queue.d/ that replaced .brain-queue.jsonl this release. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: align memory-pipeline probe pins with the #2394 stage-count contract The paid-tier E2E pinned the pre-fix contract (probe headline = raw discovered). Probe now counts post-attribution — the same gate --bulk uses — with an explicit unattributed-skip line. Adds the --include-unattributed companion pin so all 9 fixtures stay accounted for. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix(next-version): batch missing-tip fetches — one bounded round trip, never a per-branch crawl The targeted-fetch retry for branches whose advertised tip has no local object ran ONE git fetch per branch (10s cap each). On a shallow clone against a busy remote that crawls the network for minutes — CI's shard deadline killed the free suite mid-file. Missing tips now collect into a single batched shallow fetch (15s cap); refs still missing after the batch (one unservable ref fails the whole transfer) get a capped per-branch retry, and anything past the cap warns as an UNKNOWN claim instead of fetching. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test(next-version): pin the batched fetch + make the offline-contract tests hermetic Two new G2 pins: N unfetched claim branches resolve with exactly ONE fetch spawn (PATH-shimmed git counts invocations), and one unservable ref no longer poisons the batch — live claims resolve via the bounded retry while only the ghost warns UNKNOWN. The #2545 offline-contract tests now run the CLI in a local fixture repo instead of the repo's own checkout: the checkout path did a live ls-remote against the real origin (operator-network-dependent, and the CI shard-deadline hang). The online-contract test gains a succeeding gh stub, so fallback:null is asserted deterministically instead of only when the operator happens to be authed. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test(redact-cli): derive the synthetic AWS-key fixture — no contiguous credential literal in source The CI quality gate scans every ADDED diff line with the redact engine, so the #2610 port's raw fixture literals failed the very gate they exist to test. The fixture is now assembled at runtime; the scanner still receives the identical bytes. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test(next-version): pin the fixture's host via origin-URL sniff — kills the last environment dependence The hermetic offline-contract fixture had no origin remote, so detectHost() fell through to auth probes: a machine with glab authed passed via the gitlab path while a bare CI runner read host:unknown (offline stays false there) and failed. The fixture now pushes to a local bare origin at a path containing github.com — the URL sniff pins host:github identically everywhere, asserted explicitly in both tests, with every git call still local. v1.54.0.0 feat: carve /ship into skeleton + on-demand sections (-59% always-loaded) (#1806)
* feat(test): transcript-section-logger + ship-action fingerprint (T10) Pure-analysis module over a SkillTestResult/NDJSON transcript: - extractSectionReads(): which sections/*.md a run opened (post-carve check) - extractShipActions(): observable action fingerprint (merge/test/bump/ changelog/commit/push/pr) that works on the MONOLITH too, so a baseline captured before the carve can detect a sectioned-ship regression - baseline read/write + compareShipActions() for baseline-first dogf(T10) Baseline-first answers the Codex outside-voice critique that a logger in the same PR as the carve is post-failure telemetry without a pre-carve reference. 11 unit tests, all green. Paid monolith baseline capture runs separately. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * feat(pipeline): section discovery + generation machinery (T9) - discover-skills.ts: discoverSectionTemplates() scans <skill>/sections/*.md.tmpl - gen-skill-docs.ts: extract resolvePlaceholders + applyHostRewrites + buildContext as shared helpers (processTemplate and the new processSectionTemplate both call them, so a sanitization/rewrite fix can't miss sections) [C1] - processSectionTemplate: body-fragment generation (no frontmatter/catalog/voice), parent-skill TemplateContext (skillName pinned to parent, not 'sections', so appliesTo gating + tier behave identically), per-host output routing - --host all now fails the build on ANY host failure, not just claude, so a stale external-host output can't slip the freshness gate [Codex outside-voice #9] Inert until a skill is carved (no sections/ dirs exist yet). Refactor is output-neutral: gen:skill-docs --dry-run --host all reports 0 STALE. 5 discovery unit tests + 389 gen-skill-docs tests green. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * feat(setup): install sections/ for cherry-pick targets (claude + kiro) (T9) Two install targets cherry-pick SKILL.md and would leave a carved skill's sections/ behind, 404ing a runtime 'Read sections/<name>.md': - link_claude_skill_dirs: link the sections/ subdir via _link_or_copy (windows gets a fresh copy on every ./setup) - kiro per-skill loop: sed-rewrite + copy each sections/* so paths resolve under ~/.kiro, not ~/.codex/~/.claude codex/factory/opencode link the whole generated dir, so sections ride free. Addresses Codex outside-voice #4/#6 (runtime pathing landmine). Inert until a skill is carved. Static-tripwire test + windows-fallback invariant green. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * feat(ship): gstack-version-bump CLI — tested idempotency classify + write (T9) Hybrid CLI extraction (CM1): the deterministic core of ship Step 12 becomes a tested CLI instead of bash prose the agent re-derives each run. - classify: FRESH/ALREADY_BUMPED/DRIFT_STALE_PKG/DRIFT_UNEXPECTED from VERSION vs origin/<base>:VERSION vs package.json.version (pure reader) - write: validated dual-write to VERSION + package.json (FRESH bump) - repair: DRIFT_STALE_PKG sync, no re-bump Bump-LEVEL choice + queue collision stay agent judgment; slot pick stays bin/gstack-next-version. This removes the re-bump-a-shipped-branch footgun from skippable prose into code that can't be skipped or misread. 15 tests (exhaustive state matrix + write/repair fs + real-git classify). Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * test(parity): sectioned-skill parity capability — guards the carve (T9) Carved skills (skeleton + sections/*.md) need parity checks that see relocated content, or moving a phrase into a section reads as 'lost': - readSkillForParity(): union skeleton + all sections/*.md - checkSkillParity sectioned mode: content checks against the union; minBytes/ maxSizeRatio against union bytes (total behavior preserved); maxSkeletonBytes asserts the always-loaded skeleton actually shrank. Lowering minBytes to fit a small skeleton would otherwise make the size floor toothless [Codex #12]. Built + tested BEFORE the carve so ship's invariant can flip to sectioned in the same commit it lands. Monolith path byte-identical (verified: pre-existing investigate 1.053 ratio drift fails the same with this change stashed). 7 sectioned-parity tests + existing parity tests green. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * refactor(ship): carve into skeleton + on-demand sections (Claude) (T9) ship/SKILL.md drops 167KB → 68.7KB (~59% of the always-loaded skill) by moving 8 prose-heavy steps into ship/sections/*.md, read on demand: tests, test-coverage, plan-completion, review-army, greptile, adversarial, changelog, pr-body. Step 12's version logic now calls the tested gstack-version-bump CLI instead of inline bash. Claude-first (S2): {{SECTION:id}} emits a STOP-Read pointer on Claude (skeleton + generated section files) and INLINES the content on every other host, so external hosts keep the full monolith — verified factory at 162KB with no sections dir. {{SECTION_INDEX:ship}} renders the situation→section table from the PASSIVE manifest (CM2 / v2_PLAN.md:663); required-reads live only in test fixtures. Multi-pass resolve expands inlined sections' own resolvers. Parity: ship invariant flipped to sectioned (union content checks + maxSkeletonBytes asserts the shrink). Carve-fallout fixed across gen-skill-docs/skill-validation/ golden/plan-completion/#1539/size-budget tests via skeleton+sections union reads. Free suite green except the pre-existing investigate parity drift. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * test(ship): manifest-consistency + context-parity + requiredReads helper (T9) Free deterministic guards for the carve: - required-reads.ts + unit test: assertRequiredReads(run, requiredFiles) — the mechanical layer-5 check that the agent Read the sections its situation needs (required set comes from the fixture, not the passive manifest) - section-manifest-consistency: 3-tier orphan classification (generated orphan + hand-edited generated file → FAIL; manifest orphan → WARN per v2_PLAN.md) and pins the PASSIVE-manifest contract (no applies_when/required_for) - template-context-parity: generated sections have zero unresolved placeholders and gated resolvers (ADVERSARIAL_STEP/CONFIDENCE_CALIBRATION/CHANGELOG_WORKFLOW) rendered — proving sections resolve with the parent skillName, not 'sections' 16 tests, all green. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * test(ship): section-loading E2E + idempotency CLI detection (T9) - skill-e2e-ship-section-loading.test.ts (new, periodic): runs real /ship in plan mode against a fresh version-changing fixture and asserts the agent Read the required sections (review-army + changelog). Runs against the INSTALLED skill (~/.claude/skills/gstack/ship), not repo paths, so install-layout 404s surface [Codex outside-voice #5]. Layer-5 mechanical guard against silent section-skip. - skill-e2e-ship-idempotency.test.ts: detection updated for the carve — Step 12 now runs gstack-version-bump classify (JSON "state":"ALREADY_BUMPED") instead of the inline bash echo (STATE: ALREADY_BUMPED). Accept both; add a gstack-version-bump-write re-bump regression signal. - touchfiles: register ship-section-loading (periodic) + extend idempotency deps with bin/gstack-version-bump + scripts/resolvers/sections.ts. Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com> * test(ship): union-read redaction wiring test for the carve (T9) main's PR-body redaction-at-sink lives in sections/pr-body.md.tmpl after the carve, not the skeleton template. Read skeleton + section templates union so the redaction-wiring assertions follow the relocated content. 9/9 green.
