+8 28f1385eac v1.91.32.0 fix: Codex works from zsh, free text never enters shell commands, honest failures (#3057)
* fix: sourced helpers locate themselves under zsh (#3024)

Skill blocks source bin/gstack-codex-probe and bin/gstack-egress-lib.sh into
the host's shell. Under zsh (macOS default) BASH_SOURCE is empty, so Codex
model selection resolved /../scripts/... and every outside pass reported
CODEX_MODEL: invalid; the egress lib resolved to the caller's cwd. Resolve
the file once at source time (BASH_SOURCE under bash, %x under zsh), and
replace the bash-only ${!name} indirection in the auth probe. A free test
derives every sourced helper from the generated skills and sources each
under bash and zsh.

* fix: reply and issue templates never pass untrusted text in double quotes (#3046)

greptile-triage's reply APIs posted -f body="<reply text>" while the
templates quote SHAs and diff code in backticks; in double quotes bash ran
each span (replies posted as 'Fixed in .', and diff or reviewer text ran as
a command). The pre-existing-failure issue template and /spec's issue title
had the same shape. All three now go through quoted heredocs. The
generated-bash lint gains untrusted-in-quotes and now also covers runtime
reference docs in skill directories (review/greptile-triage.md).

* fix: a Codex usage limit is its own state, not 'inconclusive; proceeding' (#3051)

The model probe matched only model and broken-install errors, so a usage or
rate limit fell through to MODEL_PROBE_INCONCLUSIVE: Codex's own message
(with the reset time) was dropped, the hint pointed at model config, and
every later skill re-probed for 30 s. The probe now returns
MODEL_QUOTA_EXHAUSTED (exit 4) on a failed, non-timeout call that names a
usage limit, rate limit, insufficient_quota or 429, relays Codex's line
verbatim and caches it for 15 minutes. Preflights map it to
CODEX_MODE: quota_exhausted (missing coverage, no further Codex calls), and
classifyOutsideReview reports a mid-run limit as unavailable (quota) with
its own troubleshooting anchor.

* test: record ship/review union sizes after the quota branch and issue template

* fix: Codex watchdog freezes the command before reaping its children

The bash-native watchdog (stock macOS, no timeout binary) ran pkill -P on
the command's children and then killed the command. Between the two, the
command continued past its killed child and printed output after its
deadline. Stop it first. The test's minimal PATH also lacked pkill, which
the watchdog uses to reap children (stock macOS ships it), so the orphaned
child held stdout open and the capture waited out its sleep.

* fix(autoplan): an oversize session journal is a hard too_large denial, not a retryable identity failure (#3050)

The guard capped journal reads at 32 MiB and reported a longer journal as
'identity', which the hook treats as deny-and-retry. A journal only grows,
so /autoplan could never enter Phase 1 in a long session, and the
troubleshooting row sent people chasing symlinks. Oversize journals now
report too_large as a hard cause that names the size and the supported
fallback (run the reviews by hand or start a new session).

* fix(deps): clear the weekly OSV findings (#3040)

Raise overrides to fixed releases: ip-address 10.7.3 (our own pin was the
vulnerable 10.3.1; reachable through socks in browse's proxy path),
fast-uri 3.1.8, hono 4.13.13, proxy-addr 2.0.8, @modelcontextprotocol/sdk
1.31.0, sharp 0.35.5, and in lib/diagram-render dompurify 3.4.16 (bundled
by mermaid) and source-map-js 1.2.2; the render page is rebuilt. Three
advisories without a usable fix get justified, expiring ignores: braces
(build-time only, excalidraw -> sass -> chokidar; a test checks the page
carries no chokidar code), sprintf-js (roarr's constant-format logger) and
katex (needs pre-existing prototype pollution; fix outside mermaid's range).
osv-scanner with the repo config: no issues found.

* fix(autoplan): name a shell-built snapshot init as the cause of the missing-evidence denial (#3045)

The guard binds a phase entry only to a literal snapshot-init Bash call.
When the model resolved the tool path into a variable, chained or piped the
call, init ran and succeeded but the denial said 'Complete the existing
snapshot init step', which reads as already done, so the run gave up. When
no init binds but a Bash call looks like one, the denial now says the shape
cannot be bound and prints the literal re-run (answers reused:true). The
binding rule is unchanged, and the skill says to run init on its own.

* fix(browse): chain reads piped stdin synchronously, so Windows runs the flow (#3039)

On Windows, an awaited Bun.stdin.text() inside the CLI's un-awaited main()
let the process exit 0 before reading stdin: a replayed flow reported
success with nothing sent to the daemon. Read fd 0 synchronously (skipped
for a terminal); empty input now gets the server's usage error and a
non-zero exit.

* fix(browse): clearer config-mismatch refusal and a WebGL note for the macOS GPU flags (#3030)

A plain call refused because another session started the daemon with
--headed/--proxy was told to 'apply --proxy/--headed', flags it never
passed. It now says who started the daemon and offers both remedies. The
BROWSER.md env table notes that the default macOS GPU flags disable WebGL
and that GSTACK_DISABLE_GPU=off restores it for WebGL QA.

* fix(ship): skip Greptile triage when no PR exists yet (#3020)

/ship's first run on a branch dispatched a triage subagent only to learn
there was no PR. Step 10 now checks for a PR inline (gh's 'no pull requests
found') and records that Greptile runs on the PR once it exists; other gh
errors still dispatch so the child can report unavailable.

* fix(router): route only to loaded skills, and say when not to invoke (#3018)

The root router listed every skill and ended 'When in doubt, invoke the
skill'. Current Claude models follow that literally: they tried to invoke
skills a user had turned off in Claude Code's skillOverrides (which gstack's
own disabled_skills render cannot see), and borderline requests tipped into
full workflows. The router now routes only to skills in the session's
available-skills list and names when to answer directly; the CLAUDE.md
routing block skill-start offers says the same.

* fix: spawned sessions never auto-grant consent or publish off the machine (#3016)

Spawned sessions auto-choose the recommended option except for destructive
or irreversible choices. Main already withholds the artifacts-sync consent
prompt from spawned sessions; the remaining gap was any other prompt whose
recommended option grants a consent or syncs data off the machine. Every
spawned directive (skill-start's block, the escape sentence, the Conductor
deny reason and the AUQ-error fallback) now names that as irreversible. A
consent the user already gave interactively stays in force.

* test: fix the spawned-block slice and type the #3045 re-init events

* fix(redact): TypeScript types and JSX expressions are code, not env.kv secrets (#3048)

A credential-named parameter or prop with an unquoted code value
(`session: SessionState,`, `key={turn.requestId + turn.role}`,
`apiKey={settings.apiKey}`) cleared the entropy gate and blocked pushes as
a MEDIUM env.kv finding. One rule replaces the PR's two keyword heuristics:
a value that reads as a code name (no digits, word-like camelCase or dotted
parts) with a code terminator, or a brace expression of such names with no
quoted literal, is source syntax. Paired controls keep firing on quoted,
brace-wrapped, concatenated, YAML-bare and random-letter literals.

* fix(setup): refuse to register a hook that does not parse (#3032)

Claude Code runs gstack's hook shims through /bin/sh; a shim that does not
parse exits 2, which for a PreToolUse hook blocks the tool call in every
session until someone finds the file. Before any hook registration is
healed or written, setup now runs bash -n on each registered shim and a Bun
parse of its TypeScript entry, and stops with the file, line and fix. A
lean rewrite of the PR's idea (one gate block plus a test that runs it).

* fix(browse): Windows runtime roots start the server from the source checkout's bundle (#3026)

On Windows a host runtime root (~/.codex/skills/gstack and the other
env-var hosts) holds file copies of browse/dist and no node_modules, so
its server-node.mjs could not import playwright and every browser skill
died (the module-scope server.ts half was fixed in v1.91.19.0). setup now
records the source checkout in every runtime root's .source-path, and the
CLI runs that checkout's bundle when the adjacent one cannot reach
playwright. Verified by unit layout tests and the Windows CI lane, not
live on Windows.

* fix(copilot): on Windows, run bash blocks in Git for Windows Bash (#3047)

Copilot CLI on Windows ran gstack's extensionless helpers straight from
PowerShell, which opened an app picker, and bare bash can resolve to WSL.
The Copilot tool glossary (rendered into every Copilot skill) now says to
run each bash block in Git for Windows Bash by its full path and never to
translate a block into PowerShell. The PR's PowerShell repair skill and
README section are not taken.

* test: record ship's union size after the Greptile no-PR check

* fix: keep issue numbers out of model-read Copilot text; seed the new routing sentence in the autoplan fixture

* docs: Oct 6 fix-wave plan, reviewed through /autoplan

* fix(autoplan): every oversize-journal path reports too_large with a recovery that keeps the work

Review follow-up to the #3050 fix. The summed batch read and the narrowed
parent loop now report too_large with the real size instead of a plain
error naming a hard-coded 32 MiB. The denial names the size and the cap,
and gives the recovery that keeps work (/context-save, new session,
/context-restore, /autoplan) with a docs anchor. A test seam that can only
lower the cap replaces the 33 MiB fixture.

* test(autoplan): negative controls and more shapes for the shell-built init denial

* fix(redact): decide TS/JSX source syntax by file context, not by value shape

Review follow-up to the #3048 fix. The word-shape heuristic also exempted
real secrets made of words (API_KEY=VelvetRiverOrbitSunset; in a .env).
The scanner now takes the file path (ScanOptions.sourcePath): the pre-push
hook scans the pushed diff per file with its +++ b/ path, and gstack-redact
--from-file passes its path. Only in a TypeScript/JSX file is an unquoted
value after ':' or a JSX brace of names treated as source; the same line in
.env/YAML, or with no path, is scanned as a literal, and the jwt pattern
still fires in .ts files.

* docs: gstack's own CLAUDE.md routing block matches the new router wording

Live check (Claude Code 2.1.284): a skill set to "off" in skillOverrides
is absent from the session's skill list while its sibling is listed, and
both are listed without the override, so 'route only to skills in your
available-skills list' holds.

* fix(sync-gbrain): don't offer a call-graph build for a source with no code pages (#3029)

Step 3.5 offered --dream for any source with pages, including one that
holds only markdown (synced with gbrain's default strategy), where a build
cannot help, and the injected CLAUDE.md guidance told future sessions to run
--dream for any count: 0. On gbrain >= 0.60, code-def for an impossible
symbol answers .status (probed live against gbrain 0.60.96: ready for a
code source, out_of_scope for a markdown-only one). Step 3.5 now probes the
pinned source first, reports the cause instead of asking on out_of_scope,
and keeps today's offer when the probe is unknown (older gbrain, an error).
gstack's own sync uses --strategy code, so the PR's soft-delete claim does
not apply to gstack's path.

* fix(setup): register the hooks that parse, refuse the rest with file:line (#3032)

The landed gate stopped setup before any registration, so one unparseable
hook left every other hook unregistered. On the Claude install the canonical
root is the checkout Claude Code runs hooks from, so there is no staged swap
to abort: setup now registers the hooks that parse, refuses the ones that do
not with file and line, finishes the install and exits non-zero, saying it
is a gstack bug and that a skipped hook keeps running from the checkout.

bin/gstack-hook-check derives its list from setup's _hook_command_path calls
plus the autoplan phase-publication hook (a test keeps them equal), parses
each shim with its shebang's interpreter, and bundles each TypeScript entry
with its local imports (packages external) into a temp dir, so a broken or
missing import, or a missing entry, fails.

Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com>

* fix(auto-update): hold an update whose hooks do not parse, before the checkout moves

bin/gstack-session-update fast-forwards the live checkout before setup runs,
and Claude Code runs hooks straight from it, so setup's gate cannot protect
an auto-update. After the Bun floor check the updater extracts the incoming
revision with git archive and runs bin/gstack-hook-check on it; a failure
holds the update like bun-too-old, records hook-does-not-parse with file:line
in the update status, and the next session start says the current hooks keep
running. Fixture-remote tests cover a broken shim, a broken imported module
and a parseable control.

* fix(browse): chain with no flow is a usage error before any daemon boots (#3039)

Empty piped input, a terminal with no arguments, and a readFileSync(0)
error (EAGAIN, EOF) now print the chain usage with the cause and its
troubleshooting link and exit 1 before ensureServer(), instead of sending
an empty argument to a daemon the CLI booted for it. Tests run the real CLI
with no daemon and no browser, plus the stub-daemon flow.

Co-authored-by: VaibDeveloper <VaibDeveloper@users.noreply.github.com>

* fix(browse): refuse a runtime-root CLI and checkout server from different builds (#3026)

A Windows runtime root runs the source checkout's Node server bundle through
.source-path. When the root's CLI and the checkout's bundle carry different
browse/dist/.version builds (the checkout was rebuilt without refreshing the
root), the CLI now refuses to start the server with both builds, the
./setup fix and its troubleshooting anchor. Unversioned dev builds are not
called skew.

Co-authored-by: ubay887 <ubay887@users.noreply.github.com>

* ci(windows): real isolated ./setup install, then a piped chain flow through the installed browse

windows-setup-e2e built the checkout but never ran ./setup. It now installs
the codex host into an isolated HOME (a runtime root copy with no
node_modules), then from an unrelated directory pipes a two-command chain
flow at a loopback page into the installed browse and asserts the page text,
and that empty piped input exits 1 with the usage error. Covers the
checkout-bundle path and the piped-stdin path end to end.

* test(lint): free text never enters a generated command (structural rule)

The generated-bash lint's untrusted-in-quotes rule (gh/glab/git/curl text
flags in double quotes only) becomes free-text-placeholder: any <placeholder>
anywhere in any command, in double, single or no quotes, and in heredoc
bodies (quoted or not), unless it is on the reviewed identifier allowlist
(test/helpers/placeholder-allowlist.ts) with the grammar a value must match.
Quoted-only identifiers (URLs, selectors, paths printed by gstack) must sit
inside quotes. Backslash-newline continuations now read as one command, so
a flag and its value on separate lines are still seen together.

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix: reply, issue and spec text travel as agent-written files, never through a heredoc

Replaces the quoted-heredoc bodies with CEO-12's file mechanism: a bash
block creates a private mktemp file in the project's .gstack/tmp/ (kept out
of git through .git/info/exclude), the agent writes the text with its
file-write tool, and the next block posts with -F body=@file, --body-file
or "$(cat file)". A heredoc ends early at any body line equal to its
delimiter; a file has no such line. When the file was never written the
block refuses with the path and the command to send it by hand, and it
deletes the file after a successful post.

- review/greptile-triage.md: replies; the fetched JSON moves from fixed
/tmp/greptile_*.json paths to a mktemp directory; REPO and PR are
re-derived per block instead of read from a previous shell.
- Test-failure issues: title and body files, platform case on
github/gitlab, assignee only when it is a valid login.
- /spec: one draft file feeds the audit hash, every redaction scan, the
issue and the archive; the archive no longer expands <title>/<body>
inside an unquoted heredoc; the decision-log JSON is built by jq from
  files.

Location verified against Claude Code 2.1.284 (Dockerfile.ci pin):
project-local .gstack/tmp is writable under acceptEdits with no prompt and
keeps mode 0600; $TMPDIR and the gstack state root prompt even under
acceptEdits.

* fix(ship): Greptile triage runs only when a PR exists; every other case says why

PR: unknown (gh missing, not logged in, a non-GitHub remote, or a failed
lookup) used to dispatch the triage child anyway. The Step 10 check now
tests gh, its login, the remote and the PR in that order, and every case
but 'PR: exists' skips the dispatch and records
'Greptile: not run (<reason>); runs on the PR once it exists'.

* fix: question-tuning commands carry no free text

The question log took the summary in single quotes and the preference write
took the user's own words; an apostrophe ended the quote and the rest ran as
shell. The summary is now a kebab-case slug (an identifier with a grammar),
and the optional free_text field is no longer passed from skills (the
binary still accepts it).

* fix(cso): Windows finds docker.exe only under known-folder roots and says transport is unsupported (#3028)

On Windows /cso looked for docker only in Git's directory and System32, so
Docker Desktop was never found and the gap read as 'not installed'. It now
resolves Program Files, Program Files (x86) and the Windows directory through
PowerShell's [Environment]::GetFolderPath (not the process environment),
checks each candidate's real path against those roots and rejects reparse
points. lib/cso/docker.ts admits only unix:/// endpoints, so a trusted
docker.exe reports 'native Windows Docker transport is not supported yet;
static assessment only'; one outside the roots is refused as outside the
trusted install locations, not overridable. Both carry an anchor. Native
named-pipe transport is not implemented.

* fix: sourced helpers share one self-locate block and fail loudly when they cannot locate themselves (#3024 rework)

The codex probe and the egress lib now carry one byte-identical block:
BASH_SOURCE under bash or %x under zsh, made absolute with $PWD, else a set
GSTACK_ROOT that holds the helper. Anything else (dash, sh, a shell that
cannot name the file) prints "gstack: cannot locate <helper> (shell: ...)"
with the docs/troubleshooting.md#sourced-helper-location URL and returns
before any bash- or zsh-only syntax. The Claude-only default path and the
$(pwd) fallback are gone.

The generated Codex preflight dot-sources the probe and, when it cannot load,
reports CODEX_MODE: helper_unavailable with the helper's own line instead of
falling through to not_authed. The class guard enumerates nested sources,
makes real resolver, auth, state-root and egress-home calls under bash and
zsh (absolute, relative then cd, bare name via PATH), covers dash and an
unknown shell, runs the full preflight, and lints BASH_SOURCE to the shared
block plus other bash-only constructs.

* fix(cso): a Maven or Gradle repository is Java, not Python (#3028)

With no supported stack metadata, preparation defaulted to Python and asked
a Java repository for requirements.txt. A root pom.xml, build.gradle or
build.gradle.kts now names the stack as Java (unsupported for runtime
preparation, so the prerequisite says so), and Python helper scripts or a
root requirements.txt beside it no longer make it Python.

* fix(browse): BROWSER.md says plainly how to enable WebGL; the mismatch refusal links Daemon discipline (#3030)

The GSTACK_DISABLE_GPU row now states the effect and the switch: set
GSTACK_DISABLE_GPU=off to enable WebGL (off is the only recognized value).
The reworded config-mismatch refusal links BROWSER.md's Daemon discipline
section. Free tests pin both.

Co-authored-by: Hagetino <Hagetino@users.noreply.github.com>

* test(windows): name the B2, B4, B5 and B12 tests in the curated Windows lane

The runtime-root bundle tests lived in browse/test/config.test.ts and the
Copilot glossary test in test/host-config.test.ts, both excluded from the
Windows lane, so the two Windows-only fixes were never checked on Windows.
Move each into its own Windows-safe file (unchanged assertions) and list it,
the piped-stdin chain test and the /cso docker.exe discovery test as named
Windows coverage in scripts/lib/windows-curation.ts.

* fix: the free-text block exits loudly when mktemp fails; spec and redact tests follow the file mechanism

The redact resolver test, the #2679 mktemp guard test, the /spec invariant
and the spec secret-sink harness pinned the heredoc transport. They now pin
the agent-written file: the harness runs the draft-file block, writes the
draft the way the file-write tool would, then runs the real scan fence, with
every HIGH/MEDIUM/scanner-failure assertion unchanged.

* test: hostile payloads through the generated posting blocks, identifier grammars, every quoting shape

- The lint reads a whole command, so an unquoted multi-word placeholder is
caught; negative controls cover double, single and no quotes, quoted and
unquoted heredocs, continuation lines, and quoted-only identifiers used
  unquoted.
- Every allowlisted grammar rejects quote-breaking characters (and shell
operators and whitespace when used unquoted); git accepts 'fix;id' and
'fix`id`' as branch names, the ref grammar does not.
- Greptile replies, test-failure issues (GitHub and GitLab) and /spec filing
and archive run from the real generated text with stub gh/glab: a body
with backticks, $(...), an apostrophe and lines equal to EOF and the old
heredoc delimiters arrives byte for byte, nothing in it runs, and the temp
file is gone. An unwritten file is refused with its path and the manual
  command, and nothing is sent.

* test(ship): the PR check precedes the Greptile dispatch, and each skip names its reason

Runs Step 10's generated check against stub gh for gh missing, not logged
in, a non-GitHub remote, no PR, a failed lookup and an existing PR. Fails on
the previous text (which printed PR: unknown and dispatched).

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* feat(ship): ship-measure runner for the measure-then-fix loop (A6)

scripts/ship-measure.ts owns /ship's diagnostic measurement of a red eval
case: per-kind aggregation (rule trials; behavior panels of 3, each at its
own 2 of 3 plus 90% of trials; judge outputs scored by 3-sample panels,
standalone judge ids accepted), one artifact directory per baseline,
repair round and trial, a diagnostic label with verdict null that refuses
the eval history dir, an estimated admission budget that reserves every
concurrent trial and reconciles actual cost, the ask-once rule, the
repair-round limit, the unmeasured skip and the per-case PR table.

Free-suite flakes (CEO-1): 'free' reruns one shard's exact file list N
times, rerun 1 alone as the baseline and the rest in parallel at
max(1, floor(cores / shard concurrency)), each with its own HOME, state
root and flake ledger, GSTACK_FREE_RETRY_FLAKY=0, under a wall cap that
reports a partial count. test:ubicloud --diagnostic runs the same on one
VM with the retry off.

bin/gstack-config gains the DX-7 keys with their CEO-9 defaults and
rejecting validators. Free tests drive every kind, the budget stop, the
unmeasured skip and the free reruns with fake runners.

* fix: a Codex usage limit is quota_exhausted, a plain 429 is rate_limited, and only Codex's own error lines count (#3051 rework)

The probe's quota signature used to treat any rate limit, "too many
requests" or 429 as quota, and read the whole output, which echoes the
prompt. Now only Codex's trailing ERROR: / stream error lines are read.
A usage-limit or insufficient_quota line is MODEL_QUOTA_EXHAUSTED (exit 4,
cached 15 minutes; quota wins over a 429 that names insufficient_quota); a
plain 429 is MODEL_PROBE_RATE_LIMITED (exit 0, state rate_limited, never
cached), reported as CODEX_MODE: unverified (rate_limited) so the review
still runs and decides.

The quota HINT says how many minutes gstack skips Codex and how to retry
now (GSTACK_CODEX_PROBE_RETRY=1 skips the negative cache for that probe, or
delete <state root>/.codex-model-probe), with the full troubleshooting URL.
classifyOutsideReview reads the same trailing lines: a mid-review quota is
unavailable (quota_exhausted), a mid-review 429 unavailable (rate_limited).
New gate row rate_limited and docs anchor codex-rate-limited; the quota row
documents the retry. Tests cover each line shape, the echoed prompt, exit
0, timeout, retry recovery, probe/classifier parity, and two fresh-shell
preflight blocks where the second makes no paid Codex call.

* ci: the eval image installs zsh, and the macOS lane sources the Codex probe from zsh

Dockerfile.ci adds zsh so the sourced-helper guard runs (and fails loudly
when zsh is missing) inside the eval container too. The macOS named
regressions job sources bin/gstack-codex-probe from zsh in an unrelated
directory and runs _gstack_codex_select_model exec (no paid call), then runs
the sourced-helper guard on the macOS runner. A free test pins both lanes and
runs the macOS step's script locally.

* test: deterministic regression for the Codex watchdog freezing the command before reaping its children

The existing stubborn-child test caught the reap-then-kill race only some of
the time (2 of 3 runs on the pre-fix watchdog). A pkill that sleeps after
reaping widens the window between reaping the children and killing the
command, so a watchdog that does not SIGSTOP the command first always lets
it print "late" after the deadline. Fails 3 of 3 on the pre-fix watchdog,
passes 3 of 3 on the current one.

* test: live codex smokes run codex in a private HOME and CODEX_HOME

codex-resume-flag-semantics and codex-e2e-sol-scope probe the codex CLI (the
latter at module load, in every free run), and every codex run writes
$CODEX_HOME/tmp/arg0, so with a real codex on PATH they wrote into the
developer's ~/.codex. Both now give codex a private HOME and CODEX_HOME and
remove it afterwards. A free test runs each file against a stub codex that
records HOME and CODEX_HOME and asserts neither is the caller's.

* feat(ship): a red eval runs classify, measure, fix, re-measure, gate once (A6)

Step 6 no longer stops at the first red paid eval: it reads the new lazily
loaded measure section and runs the loop for each red case, and Step 5
measures a failed free-suite shard (diagnostic reruns, retry off) before
the triage can call it pre-existing. The section states the diagnostic-
not-verdict rule, the per-kind table, the ask-once and named-red stops,
the unmeasured label and the honest-fix wording (observed k/n after fix
at <cause>). A free test pins the steps and their order in the generated
skill and checks every command and flag it names exists in the runner.

Ship's parity cap rises to 1.455 (measured 272,944 / 187,706 = 1.4541).

* docs(testing): diagnostic measurement is not a verdict; per-kind measure table (A6)

* test(ship): seeded-flake paid case for the measure loop (A6, periodic, behavior)

A fixture repo with no remote documents a single-case eval command and a
gate command, both free local stubs. queue-priorities claims trial slots
with atomic mkdir (reset per ship-measure round) and fails slots 2, 5 and
9 while src/priorities.js leaves its seed-shuffled list unsorted, printing
failure_cause and failure_detail that name the line. The paid case gives
/ship's generated Step 6 and measure section to a live session and asserts
it measured the case alone, fixed the named line without touching the
eval, re-measured at least 10 passing trials on the sorted file, and only
then ran the gate once; any question other than the spend question is a
contract violation.

A free test measures the fixture through the real ship-measure CLI: 7/10
unsorted, 7/10 after an unrelated edit with the gate still red (ENG-13's
negative control), 10/10 after the sort with the gate green.

* test(cso): the missing-scanner case points Docker at a dead socket

'an optional scanner prerequisite does not turn completed manual coverage
partial' assumed no usable Docker. On a machine with Docker and a pulled
gitleaks image the scan completed and the test failed (also on main). The
scan step now gets a DOCKER_HOST that cannot connect, which is the
prerequisite-missing condition the test describes.

* fix: Aside prompts travel as agent-written files, never inside the command

The open-ended reading recipe (browse, devex-review), scrape's fuzzy-intent
read and the shared web-research block put the model's question, query and
reply format inside a double-quoted _aside_exec argument, where backticks and
$(...) from page or repo text would run. The agent now writes the prompt into
a private .gstack/tmp file; the send block rebuilds the path from the printed
basename, refuses when the file is empty, and passes "$(cat "$PROMPT_FILE")".
The fixed read-only sentence stays in the shell source.

aside-driver pins move from the inline prompt to the file-based send block.

* fix: design briefs and approval feedback travel as agent-written files

Design briefs went into $D variants/generate/check as a double-quoted
argument, and approval feedback was spliced into a single-quoted echo of JSON,
so a quote, backtick or $(...) in either broke the command or ran. The agent
now writes each brief into a private .gstack/tmp file and the block passes
--brief "$(cat "$BRIEF_FILE")" (the CLI's --brief-file expects a JSON
brief, so plain text uses --brief). variants and the later check reuse the
same brief file. approved.json is built with jq -n from --arg values and
--rawfile feedback, so the feedback is data. The office-hours mockup flow
creates its brief and feedback files in one block.

design-approval-flow now writes the feedback file (with quotes, backticks and
$(...) in it) instead of substituting <FEEDBACK>, and checks the file is
removed after the save.

* fix: carve office-hours on every host to stay under the 160,000-byte ceiling

With its brief, feedback and research prompts moved into files, the inlined
office-hours render is 161,329 bytes on Codex and 160,302 on Factory, over
SKILL_BYTE_CEILING, so gen-skill-docs --host all fails. Per the C4 policy
(carve every skill whose external render exceeds the ceiling), office-hours
joins ship and plan-ceo-review in CARVED_ON_EVERY_HOST. The Claude render is
unchanged; external hosts read its sections relative to the installed skill.
external-host-carve and qa-lazy-sections now include office-hours in their
carved-skill lists.

* chore: regenerate after the free-text mktemp guard

* fix(codex): prompts and review focus travel as agent-written files

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

Challenge and consult prompts, and the /codex review focus text, are written
by the agent into private .gstack/tmp files and fed to Codex on stdin or read
back with cat; no heredoc carries them. Consult's new and resumed runs share
one block (and one parser) so no placeholder stands in for the parser; a
resumed run reads its session id from .context/codex-session-id and checks it.
Adds the A3 group 2 placeholder allowlist block.

* fix(benchmark-models): an inline prompt goes through a prompt file

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix(land-and-deploy): PR head repository and branch travel as data

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

The branch check reads headRepository/headRefName from gh's JSON into quoted
shell variables; ls-remote, the confirm-first delete and the local delete use
those variables instead of model-substituted names. The postfail test now pins
the variable form it replaces.

* fix(office-hours): the reviewer receipt must match the receipt grammar before the check runs

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix(setup-gbrain): cleanup-orphans collects the PAT with read_secret_to_env

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix(ship): PR body drafts and rejected test paths are agent-written files

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

The PR body is written into two private drafts (through ## Documentation, and
from ## Test plan on); the scan block splices Step 14.5's section between them
into the scanned temp file and deletes the drafts. Rejected test paths are read
from a written list, and paths outside the repository are skipped. Tests that
ran the old heredoc blocks now write the drafts and list first.

* fix(sync-gbrain): orchestrator flags must match an allowlisted grammar

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix: the gbrain page save and external-host placeholders pass the free-text rule

The gbrain save block passed the whole page through a quoted heredoc inside
--content; the agent now writes it to a mktemp file and gbrain put reads it
on stdin. The Claude Code outside-voice runtime root and session mode, and
the feature slug, join the allowlist with grammars; unused entries are
dropped, and a test keeps every allowlisted placeholder tied to a real use.

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix: skill-specific Aside research queries travel as prompt files

investigate, office-hours, plan-ceo-review, review, plan-eng-review,
design-consultation Phase 2, the shared test bootstrap (B2) and QA's test
bootstrap each carried their own _aside_exec call with the model-filled query
inside a double-quoted argument. Skills that already carry the Web research
block now give only the prompt-file text and use that block to create, write
and send it; design-consultation, the shared bootstrap and QA create the file
and send it with the new {{ASIDE_RESEARCH_SEND}} resolver, which is the same
send block the research section uses (read-only sentence stays in the shell).

aside-driver's B2 pin and qa-browser-preservation's bootstrap contract move
from the inline call to the prompt file and the shared send resolver.

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix: prose design commands pass briefs and feedback as data

The DESIGN_SETUP command list taught `iterate --feedback "..."`, and
design-consultation's late-change and plan-design-review's slop-regenerate
lines showed `--feedback "<feedback>"` / `"..."`; design-shotgun's retry
showed `generate --brief "<brief>"` and `evolve --brief "<brief>"`. Feedback
now reads "$(cat "$FEEDBACK_FILE")" from an agent-written mktemp file
(the setup note says how), and the shotgun retry reads the variant's own brief
out of briefs.json with jq instead of retyping it.

* test: record parity sizes after the free-text file mechanism; refresh ship goldens

Measured union and skeleton sizes for ship, plan-eng-review,
plan-design-review, office-hours, design-consultation, review, qa, browse
and design-shotgun after every posting, prompt and brief site moved to
agent-written mktemp files; the ship goldens pick up the question-tuning
slug and the dropped free_text field.

* test: parity caps for the combined wave head (rounded up past the measured ratio)

* fix(plan-ceo-review): the mode handoff helper reminds the model to lead its chat with the line (#3038)

Periodic run 37272185151's auto-decide-preserved red replays as the Step 0E
handoff-line class: the model ran gstack-ceo-mode-handoff --auto, got the
exact line, then opened its chat with a heading and a paraphrase, so the
user never saw the auto-decide line (the terminal collapses tool output)
and the detector saw no outcome. The helper now prints a one-line reminder
on stderr beside the line; the detector accepts the line followed by that
reminder and nothing else. The ship Greptile check test reads its template
(test-of-test ratchet).

* test: every troubleshooting link in shipped code resolves, relative or full URL

* docs(todos): Oct 6 fix-wave follow-ups

* v1.91.32.0 release notes: Codex works from zsh, free text never enters shell commands, honest failures

* fix(plan-ceo-review): show the mode handoff line through a PostToolUse hook, not model compliance (#3038)

The stderr reminder beside the handoff line measured 1/10 on
auto-decide-preserved: the model still opened its chat with a paraphrase
('I've set review mode to HOLD SCOPE...'), so the user never saw the line and
the detector saw no outcome. /plan-ceo-review now declares a Claude-only
PostToolUse hook that shows the helper's exact line as a system message.
Diagnostic measurement on this head: 10/10, the line visible in every trial
while no trial's chat led with it. The reminder is reverted.

* fix: type the handoff hook's line filter

* fix: repairs from the integrated free suite

- The self-locate markers looked like heredocs to the pipe-deadlock guard;
they are plain comment fences now.
- The spawned block keeps the canonical 'conservative non-destructive'
phrase the cross-surface drift guard pins.
- The free-text block's mktemp refusal says 'mktemp failed', as every
  /codex temp file must.
- The batch transcript read reports sizes in MiB.
- docs/TESTING_INTERNALS.md gets a real 'Ship measure' heading for its
  anchor.
- The early-question harness admits /plan-ceo-review's handoff hook by its
exact command (it only adds a system message); any other PostToolUse Bash
  hook is still refused.
- Paid file count and the unconsumed-file example follow the new seeded
  case; ship goldens refreshed.
- The CSO scanner test points Docker at a dead socket, so a machine with the
  pulled image does not scan.

* test(cso): a host with no Docker binary is also a no-scan host

The missing-image scan test failed on a Docker-less VM (docker is not
installed in a trusted directory). Every accepted message is a reason the
scan cannot run here; the not_assessed, helper-owned artifact assertions are
unchanged.

* ci: split the USERPROFILE assignment from export (SC2155)

* fix(browse): Windows runtime roots read .source-path as a native path (#3026)

The new windows-setup-e2e journey showed the installed browse could not
start its server. setup runs under Git Bash and wrote .source-path as an
MSYS path (/d/a/gstack); Node read it as \d\a\gstack on the current drive,
found nothing, and fell back to the root's own bundle, which cannot import
playwright. setup now writes cygpath -m, and the CLI maps an MSYS drive path
for installs written by an older setup. The journey prints the daemon and
startup logs when the chain fails.

* fix(ship-measure): defects from the parallel #3034 plan review

- A trial passes only with a JUnit testcase that executed and passed; a
--case that selected nothing also exits 0.
- measure runs a no-cost --list preflight that must plan the case's own
  file before any paid trial.
- Diagnostic trials drop EVALS_CACHE_DIR/EVALS_CACHE_RUNTIME_ID, so they
never reuse a stored judge pass or publish one the gate could reuse.
- Gate once: when CI runs the gate on push, the push is the gate run; the
skill no longer also starts eval:bg:pr (a second paid gate).
- ubi-runner: down fails unless the VM is confirmed gone, and nothing
sweeps other runners' VMs by default (UBI_GC_HOURS opt-in).

* test(windows): the seeded-flake fixture drives a POSIX shell stub, so it stays off the Windows lane

CreateProcess cannot exec the fixture's ./evals.sh shebang script (status
null on windows-free-shard 6). The measure runner is covered on Windows by
ship-measure.test.ts, which uses in-process fake runners.

* fix: the free-text block refuses an unset temp dir instead of writing at /

The rendered block ran mktemp "$_GT/..." with _GT unset in the portability
test; on Linux that fails at /, but Git Bash on Windows can write to its
install root, so the block went on (windows-free-shard 3). It now uses
${_GT:?}, so an unset directory always fails closed with the mktemp
message.

* test: pin the guarded ${_GT:?} temp-file form

* test: the Greptile check's gh stub uses a portable shebang

On Windows the absolute bash path has a space, so the stub never ran and
every case read 'gh not logged in' (windows-free-shard 1).

* test: the Greptile check runs with the native PATH and the gh stub first

The PATH-only stub bin could not run its stub on Windows (every case read
'gh not logged in'). Stub cases now keep the real PATH with the stub first;
only the no-gh case drops directories holding a real gh.

Garry TanandClaude Fable 5 e76f65a8da v1.77.0.0 feat: test-infrastructure overhaul wave 1 — matrix deletion, flake telemetry, sync-spawn wedge class extinct (#2746)
* fix: pin the claude CLI to an exact version in the CI image + tripwire

The image installed @anthropic-ai/claude-code UNPINNED and rebuilt weekly
'to pick up CLI updates' — while bun sat carefully pinned at 1.3.13 two RUN
lines above. The PTY harness screen-scrapes this CLI's TUI, and that drift
broke it three separate times (welcome-screen wedge on 2.1.233, skillify
HOME discovery on 2.1.237, guard/freeze hooks on 2.1.162), each debugged as
a flake first. Pin 2.1.251 (current latest), bump deliberately via a PR
that runs the PTY gate, and enforce with test/ci-image-cli-pin.test.ts:
any global npm install in Dockerfile.ci without an exact @X.Y.Z pin fails
the free suite. The weekly ci-image cron stays as a cheap tag self-heal.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: stamp the claude CLI version into every eval-store run record

Three harness breakages were traced to claude-CLI TUI drift only after long
flake hunts, because no run record said which CLI it actually exercised.
EvalCollector now stamps claude_cli_version (claude --version, cached once
per process, 'unknown' when the binary is absent) into both partial and
finalized records — schema-additive optional field, no SCHEMA_VERSION bump.
Correlating a flake wave with a CLI release becomes a grep over
~/.gstack/projects/<slug>/evals/ instead of archaeology.

* fix: give the spinning-shard kill test load headroom (30s -> 90s)

The test spawns and group-kills three real children (one a busy-loop
burning a full core) while five sibling shard processes compete for eight
vCPUs. Under full-suite load it blew bun's default 30s per-test ceiling at
30,009ms — while passing in isolation in 1.4s — and red the only required
lane. Every assertion in it is event-based (statuses, group-kill proof,
heartbeat lines); the sole latency claim is the <30s kill-deadline sanity
bound, which stays. Explicit 90s headroom, not a weakened oracle.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: green-by-skip census — skip counts in the classifier, all-skipped labeling in the paid runner

bun's 'Ran N tests' line COUNTS skipped tests, so a codex/gemini shard
whose every test self-skipped (binary absent on the runner — true of every
CI runner today) exits 0, dodges the hollow-shard guard, and reads as
coverage in the weekly census. The classifier now parses bun's ' N skip' /
' N pass' recap lines; ShardOutcome carries skippedTests; formatSummary and
the fail-closed slices report label an all-skipped pass explicitly:
'all N tests SKIPPED — verified nothing'. Status stays 'passed' (external
service availability is host state, not a repo regression) but the census
can no longer mistake absence for coverage.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* refactor: extract composite actions for eval-lane setup; surviving lanes gain the fail-fast registry verification

'Fix bun temp' x3, 'Restore deps' x5, 'Seed claude interactive config' x3,
and 'Register gstack skills' x3 were byte-near-identical copies across the
legacy matrix, the sliced lane, and the periodic lane — and only the MATRIX
copy of register-skills carried the 19-line dangling-symlink + frontmatter
fail-fast loop written after a silent 'Unknown command' + 35-min-timeout
incident. Extract all four into .github/actions/ composites; the register
composite carries the verification loop (generalized over the skill list),
so the sliced and periodic lanes — the lanes that SURVIVE the matrix
deletion — now inherit the check they had silently dropped. Matrix-job
inline copies are left untouched: that job is deleted next.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: delete the legacy 17-row eval matrix — the sliced lane is the only paid lane

Every PR paid twice: the hand-enumerated matrix (18 test files, 22.6 min,
~$21 API measured on run 33263204465) ran serialized AHEAD of the strictly
superior sliced lane via 'needs: evals' — 35.5 min wall and ~2x paid spend
for the same diff. 14 of 17 rows carried no tier:, so periodic Opus
benchmarks leaked into every PR (the e2e-plan row alone: 12/12 tests,
21.7 min, $7.28 — the wall-clock bound of ALL of CI).

Parity receipt (static, pre-deletion): the sliced lane's gate census (49
files, derived from the runner itself) strictly contains all 18 matrix test
files, plus 31 files the matrix never ran. Pure deletion — one revert
restores it. The PR comment moved into slices-report (same '## E2E Evals'
upsert marker, now sourced from slice artifacts + carrying the fail-closed
reconciliation verdict). plan-slices loses the needs edge; the dead
workflow-level EVALS_TIER env goes with it.

test/evals-workflow-matrix.test.ts (and its KNOWN_MATRIX_GAPS /
KNOWN_TIER_UNSET burn-down ratchets — retired: the sliced census makes
'every gate file runs' true by construction) is rewritten as
test/evals-workflow-wiring.test.ts: matrix stays deleted, planner/executor/
report tier + slice-count agreement, both surviving lanes on the shared
register-skills composite with its fail-fast verification loop, PR comment
survival. Expected: PR eval wall 35.5 -> ~13 min, per-PR paid spend ~halved.

* fix: provider-runner timeouts kill the whole process GROUP; codex/gemini inherit the orphan-drain hardening

All three provider runners (claude/codex/gemini) killed only the direct
child on timeout: tool subprocesses the CLI spawned survived as orphans
holding our pipes open and burning shared API rate (observed: a 600s
timeout stretching past 1400s; a stalled run once burned a core for 15
hours). gstack-detach's watchdog had the same shape one level up — killpg
SIGTERM, 5s grace, then a direct-child proc.kill() that orphaned
grandchildren.

Fix: spawn provider children via node:child_process with detached (own
process group) and killProcessGroup(SIGKILL) in the timeout handler —
runShardChild's proven pattern, EPERM/ESRCH fallbacks included. The codex
and gemini copies also gain the reader.cancel() + stderr Promise.race
hardening only the claude copy had (they still carried the blocked-drain
hang it fixed). gstack-detach's watchdog now group-SIGKILLs after the
grace.

Regression net: test/session-runner-groupkill.test.ts drives the REAL
runSkillTest against a fake claude shim (PATH override) that spawns a
grandchild and wedges — the run must classify timeout within budget and
leave neither shim nor grandchild alive — plus source pins on all three
runners (detached + killProcessGroup, no bare timeout kill, no Bun.spawn
reversion).

* fix: skill-e2e-opus-47 renders SKILL.md fixtures into a mkdtemp — never the live tree

mkEvalRoot ran gen-skill-docs with cwd=ROOT, regenerating every in-repo
SKILL.md mid-run while concurrent paid shards copyFileSync those same files
in their beforeAll (EVALS_JOBS>=4 locally, 2 per CI slice) — a sibling
could capture a half-regenerated or opus-rendered SKILL.md, and a timeout
before afterAll stranded the whole tree at the wrong model for every later
shard. A cross-shard race that could flake ANY concurrent paid test.

Render via the --out-dir flag gen-skill-docs grew for exactly this reason
(mirrors the repo layout, which is all the fixture reads), read the skill
heads from the render dir, delete it, and drop the afterAll restore-regen
entirely.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: claude CLI version resolves in the runner parent, never on a test thread

Eng-review finding: getClaudeCliVersion's fallback is a SYNCHRONOUS
spawnSync on the same thread that polls concurrent PTY/session tests — the
judgePtyState blocking class this overhaul kills elsewhere. The paid runner
parent now resolves it once (cached) and stamps GSTACK_CLAUDE_CLI_VERSION
into every shard's env; eval-store short-circuits on the env var, and the
fallback spawn's budget tightens 10s -> 3s (bounded one-time stall, records
'unknown' on a slow CLI).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* test: wire skippedTests end-to-end through runPaidShard

The census unit tests hand-built outcomes and the classifier tests parsed
strings; nothing proved a real child's ' N skip' recap flows into
outcome.skippedTests and the formatSummary label. A commandFor fake now
prints the recap shape and the test asserts the parsed counts, the
all-skipped predicate, and the 'verified nothing' label.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: make the setup composites rerun-safe (codex diff-review hardenings)

restore-deps: 'cp -r SRC node_modules' with an existing node_modules NESTS
the copy and leaves stale deps active — rm first. register-gstack-skills:
'ln -snf' hard-errors under set -eu when a REAL directory occupies the
gstack slot — clear a non-symlink leftover first. CI workspaces are fresh
today; a reusable composite must survive dirty reruns.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: sweep — every sync spawn in the test trees carries a timeout (436 sites, 157 files)

spawnSync/execSync/Bun.spawnSync BLOCK the main thread, so bun's in-process
per-test timeout can never fire while one waits — a hung child (stdin read,
network probe, dead daemon) wedges the whole shard until the runner's
external wall-clock SIGKILL. This exact class reached main: free-tests run
33262077256, test/gstack-memory-ingest.test.ts (normally 2.3s) held shard 2
at the 360s wall while its five siblings finished in ~65s.

Mechanical sweep in two waves (12 + 4 fan-out agents, every edit verified
against its call site): default timeout: 30_000 (matches the free runner's
per-test budget), 120_000 for genuinely slow ops (installs, builds,
playwright, provider CLIs), helper wrappers fixed ONCE where call sites
route through them. Sites that only LOOK like calls (string fixtures, grep
needles, comments) were skipped with reasons — the enforcement commit that
follows marks them exempt.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* test: sync-spawn timeout tripwire — the wedge class stays extinct

Free scanner over all test trees (test/, browse/test/, design/test/,
make-pdf/test/, ios-qa, browser-skills): every spawnSync/execSync/
Bun.spawnSync call site must carry a timeout within a 30-line options
window, or an explicit '// tripwire-exempt: <reason>' marker. Comment
lines are skipped; exemptions are counted and ratcheted shrink-only
(ceiling 6 = the 6 string-fixture/grep-needle sites where the pattern is
CONTENT, not a call — marked in this commit). A scan-sanity test pins that
the scanner still sees >100 real call sites so it can never rot to a
vacuous green. Companion to the 436-site sweep in the previous commit.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: paid-lane flake telemetry — record-level attempts, flaky_retries, report surfacing

bun --retry leaves a retried pass INVISIBLE in its output: a fail-then-pass
prints the error detail but no (fail) result line and recaps as a clean
pass (probed live on 1.3.10). So attempts are recorded where they cannot
lie: EvalCollector.addTest stamps a 1-based attempt on same-name re-records
(a retried test runs its body again and re-records), finalized runs carry
flaky_retries, printSummary warns loudly, and the fail-closed slices report
lists every passed-only-on-retry test — recorded and ranked, never blocking
and never silent. Cross-model confirmed (codex reached the same don't-parse
-the-stream conclusion independently).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: free-lane flake ledger — retry ON in CI, flaky-passes recorded and uploaded

The runner's attribution-gated flaky-retry pass (cap 5, truncation veto)
was OFF in the required lane and its FLAKY-PASS evidence was console-only —
so a single timing flake red the merge gate while repeat offenders stayed
unenumerable. free-tests.yml now sets GSTACK_FREE_RETRY_FLAKY=1 and points
GSTACK_FLAKE_LEDGER at runner.temp; every flaky-pass appends a JSONL entry
(SINGLE writer: the parent runner — no concurrent-append hazard by
construction; fail-open with a loud warning so a broken ledger can never
red the lane) and the artifact uploads UNCONDITIONALLY — a flaky-pass run
is green, which is exactly when the evidence matters. Wiring pinned by
free-tests-workflow-wiring; ledger behavior unit-tested incl. the fail-open
path. Matches 2026 industry practice (retry for data, quarantine out of
merge-blocking but never out of logging) with the repo's own receipts.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: eval:flake-rank — the flake-telemetry dial

Aggregates per-test series across every finalized eval-store run (shard
dirs included) plus the free flake ledger: runs, fails, RETRIED PASSES
(the flake signature), avg duration — ranked retries-first. This is the
readable dial behind two policies: a flaky pass never blocks a merge but
is always ranked here, and the WS16 required-check promotion needs weeks
of clean flake-rank, not vibes. --json for machines, --dir for downloaded
CI artifacts.

* feat: two-phase session timeout — silent APIs die at the startup grace, named

The single spawn-armed timer charged API queue latency to the work budget:
the recurring '0 turns / $0.00 / x3 attempts' failure with four budget-bump
receipts (180->300s, 240->360s, 300->420s, 90->300s). Split: startup phase
(no NDJSON byte yet) kills EARLY at min(grace, timeout) with the distinct
exitReason 'timeout_startup' — an availability verdict, not transcript
archaeology — and the work phase arms on the first byte for the REMAINING
budget, so total wall never exceeds the timeout (tier envelopes are
margin-free: tests pass timeout: CAPTURE_MS and bun-budget the same tier).
Local grace 90s (observed queue latency 60-90s), CI floor 300s (TODOS-filed;
shared runners queue harder), both pinned by the new grace tests with fake
-claude shims covering the late-first-byte and silent-API paths.

* fix: census integrity — 17 phantom selection keys deleted, reverse invariant added, gitignored dep patterns replaced, local map forks derived

The merge-blocking gate census counted tests that could not run. Deleted
(critic-verified against both quoted-occurrence and dep-registration
liveness): 7 *-prosons-format keys with no declaring test, ship-plan-
completion/-verification, review-plan-completion, design-shotgun-path/
session/full, autoplan-core (dead ~10 months), e2e-harness-audit (its
namesake is a FREE-suite file), plus 2 dead LLM-judge keys and 2 free-file
keys (budget-regression-pty, global-discover) misplaced in the PAID maps.
Census: 191 -> 174 keys, gate 86 -> 78 honest.

The new reverse invariant in touchfiles.test.ts makes the class structurally
impossible: every key must be quoted in a living paid test file OR
registered to an existing paid test file via its dep list (the constructed-
name binding the 2026-08 self-registration sweep established) — zero
exceptions needed today, with a live-file check on any future exception.

Also: '.agents/skills/**' dep patterns replaced with the generator
(scripts/gen-skill-docs.ts) — .agents/ is gitignored, so those patterns
could NEVER match a git diff and review-template edits silently stopped
selecting codex/gemini tests; the codex/gemini local touchfile maps are now
DERIVED from the canonical map (loud throw if a key vanishes) instead of
hand-forked copies that had already drifted. ios-qa-e2e demoted gate ->
periodic: its gate declaration was never executable in CI (hardware
exclusion only applies at tier=periodic), so every Linux PR planned a
hollow shard.

* fix: routing journeys lose their answer key and end at the routing decision

The journey tests exist to catch skill-DESCRIPTION regressions (touchfiles:
*/SKILL.md.tmpl), but the fixture CLAUDE.md shipped an explicit
prompt->skill lookup table — with the answer key in context, a badly
regressed frontmatter description still routed correctly, so the tests
could not fail on the exact class they select for. The fixture now carries
only the generic invoke-skills nudge; the frontmatter carries the routing
load. Also capped all 10 journeys at maxTurns 2 / tools [Skill, Read]:
only the FIRST Skill call is asserted, so 5 turns of Read/Bash/Glob/Grep
was pure spend — roughly halves each journey's cost.

* fix: retire decided A/B experiments; vendor the pre-cut fixture; ban raw-SHA fixtures

Three one-shot decision experiments kept re-running weekly as N=1
stochastic comparisons — flaky by construction with near-zero remaining
information: skill-e2e-auq-repetition-cut-ab (its own header: gate "passed
pre-landing, approved 2026-08-25"), skill-e2e-preamble-script-ab ("demoted
post-Phase-3"), and opus-47's fanout arm-vs-arm (parA >= parB across two
SINGLE stochastic runs — a coin flip). Deleted, with their selection keys;
the SDK overlay-harness stays as the maintained instrument for the next
experiment, and opus-47 keeps its routing-precision cases.

verboseSkill() now reads the VENDORED test/fixtures/auq-pre-cut-...-SKILL.md
instead of `git show ab66193e^:...` — a branch-local ref that dies on
branch prune and already failed on shallow clones. New free tripwire
(test/git-ref-fixture-tripwire.test.ts) bans the raw-SHA fixture class
outright: quoted SHA:path rev-specs and gitRef-style hex defaults in the
test trees fail the suite with the vendor-instead instruction.

* fix: demote plan-ceo-review-expansion-energy to periodic

Opus generator + a subjective 2-axis >=4/5 LLM-judge threshold sat in the
MERGE-BLOCKING gate — the exact class its sibling posture tests were
demoted for, with a receipt (a +21-line preamble change once flipped the
score). CLAUDE.md's own tiering rule: Opus model test -> periodic. The
weekly lane keeps the regression signal; merges stop paying a judge-
temperament tax.

* fix: paid shards get per-shard TMPDIR + CHROMIUM_PROFILE isolation and a kill-path cleanup backstop

The free runner treats this isolation as MANDATORY (two concurrent shards
on one Chromium profile kill each other's browser; shared tmp
cross-contaminates) — the paid lane had none of it. Doubly load-bearing
here: a shard that hits its 30-min wall is group-SIGKILLed, so per-test
afterAll cleanup never runs; the rmSync backstop is the only thing keeping
wedged runs from accumulating full git-repo workspaces in the shared
tmpdir forever. This is the DAG prerequisite for raising EVALS_JOBS (next
commit) — more concurrency on shared state amplifies exactly the
shared-tree race class opus-47 exhibited.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat: paid-runner defaults 4x4 -> 8x2 — halve the local gate worst case

39 of 75 skill-e2e files hold exactly ONE test, so within-shard
concurrency was dead weight for most shards: 4 jobs x 4 concurrency
yielded only ~4-6 real in-flight sessions and a 13-wave local gate worst
case (~6.5h). 8 jobs x 2 gives ~10-13 in-flight — under the
documented-safe ~15 — and ~7 waves (~3.3h worst case). CI lanes keep
their explicit EVALS_JOBS env (2 per slice; 4 for gate-census); this
changes local defaults. Rollback trigger: sustained 429 storms in the WS1
telemetry across 2 PR cycles. test/eval-detach-timeout-floor.test.ts
recomputed green (the raise LOWERS the worst-case floor).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: SHA-pin every action in the secrets-bearing eval lanes

evals.yml and evals-periodic.yml execute PR-authored code with three
provider API keys in env, yet rode mutable action tags (@v7/@v8/@v2/@v4)
— while quality-gate.yml, osv-scanner.yml, and dependency-review.yml
already model the SHA-pin pattern. All 30 uses sites across both lanes now
pin the exact commit (tag noted in a trailing comment); dependabot's
github-actions ecosystem keeps them fresh via PRs instead of silent tag
moves. Pulled forward from the plan's endgame on the CEO-review + outside-
voice agreement: supply-chain pins on secret lanes go first, not last.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: sweep wave 3 — the execFileSync family gets timeouts (90 sites, 17 files)

The tripwire's regex covered spawnSync/execSync/Bun.spawnSync but not
execFileSync — an entire blocking sync-spawn API family that could
reintroduce the shard-wedge class undetected (ship review army). Same
mechanical recipe as waves 1-2: timeout: 30_000 default, 120_000 for slow
ops, shared wrappers fixed once, string-needle sites skipped with reasons.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* test: review-army + adversarial test hardening

- Tripwire scans execFileSync too (ceiling 8: two more grep-needle string
  exemptions); merge-introduced timeout-less spawnSync in
question-preference-hook fixed — the tripwire caught a site that landed
on main AFTER the sweep, on its first day.
- gstack-detach gains TWO watchdog kill regression tests: TERM-immune
grandchild (the killpg-after-grace escalation) and the leader-dies
variant (the pgid-at-spawn fix — the case the first test cannot see).
- eval-flake-rank gets its unit suite (final-attempt accounting, artifact
  exclusion, shard recursion, recency bound).
- Groupkill/startup-grace shim markers are per-run unique (pid-suffixed
sleep durations): sibling Conductor worktrees run free suites with no
machine lock, and fixed markers let one run pgrep/pkill the other's
  shims — a cross-run flake inside the anti-flake tests.
- flake-ledger test pins the project-scoped local default; stale empty
section headers in touchfiles-data deleted (they invited entries under
  deliberately retired categories).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: adversarial-review runtime fixes across the telemetry + kill paths

- session-runner: exit-labeling keys off 'exit', not 'close' — an orphan
holding the pipes could relabel a REAL exit (auth failure) as
'timeout_startup' availability noise; the kill path still always
group-kills and cancels the reader (labeling and unblocking are separate
concerns). Work phase arms on a flag, not firstResponseMs===0 (a same-ms
first byte left the startup timer live all run). The CI startup grace is
now a real FLOOR (Math.max), matching its name and pinning test.
- gstack-detach: pgid captured AT SPAWN (== child pid under
start_new_session) — resolving it after the grace raised ESRCH once the
leader died on SIGTERM, orphaning TERM-immune grandchildren forever.
- test-free-shards: ledger entries carry branch + git_sha (rev-parse split:
'--abbrev-ref HEAD HEAD' printed the branch twice and recorded it as the
sha); local ledger default is per-PROJECT, not the machine-global tmpdir.
- eval-flake-rank: per-LINE ledger parse (one torn JSONL line vanished the
whole series), 60-day recency bound (transcript-bearing files are MBs),
shared isFinalizedEvalResultFile predicate (the artifact-taxonomy rule
lived in three places); eval-store exports the predicate and finalize
stops computing flakyRetries twice; paid-shards cleanup uses async rm
  (a SIGKILLed shard's git-workspace teardown blocked every sibling's
stream classification on the parent event loop).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: CI trust-boundary + fail-closed repairs (adversarial findings)

- Token/exec separation restored: slices-report (runs PR-authored code:
bun install + the reconcile runner) drops to contents:read; the PR
comment moves to a NEW slices-comment job holding the write token with
ZERO repo code — no checkout, no bun, only downloaded artifacts + jq/gh.
$GITHUB_ENV/BASH_ENV persistence is job-scoped, so the split is the
boundary. The matrix-era report job had this property; the consolidation
had regressed it. Pinned by the wiring test.
- Reconcile exit captured via PIPESTATUS[0] in BOTH lanes: GitHub's default
  run-step shell has no pipefail, so `$?` after `| tee` was tee's exit —
the fail-closed gate was silently fail-open. Wiring test pins it.
- PR comment: final-attempt accounting restored the dropped COST
accumulation (the dial read $0 forever), flaky passes render as the
warning they are (never as failures), and a malformed tests[] artifact
skips that file instead of aborting the whole comment under bash -e.
- Remaining mutable action tags pinned (free-tests upload-artifact,
  ci-image checkout/docker trio — the image publisher holds packages:write
and feeds the secret-bearing lanes). restore-deps fallback installs
--frozen-lockfile; register-gstack-skills validates skill names before
  its rm -rf.

* docs: correct CHANGELOG receipt numbers to measured values

Gate census keys: 78 -> 77 (bun-imported E2E_TIERS count). Sweep receipt:
586 sites/176 files -> 499 sites/146 files, measured by running this
branch's spawnsync-timeout-tripwire against origin/main (exit 1, 499
violations across 146 unique files; green on this branch).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix: slices-comment creates the PR comment via REST — the write-token job has no git context

The token/exec split gives slices-comment NO checkout by design, and gh's
pr-comment subcommand resolves the repo FROM git — it died with 'not a git
repository' on PR #2746's first run (the update-existing PATCH path was
already explicit-repo REST and worked). Create now posts through
gh api repos/.../issues/N/comments, and the wiring test pins that no
git-context-requiring comment call can creep back into the job.

* fix: startup-grace probes clear CI for local semantics; new probe pins the floor clamp

The two shim probes pass explicit 2s/4s graces, but in CI the runner clamps
any explicit grace up to the 300s floor (deliberate adversarial-review fix),
so 'silent API killed at the grace' died at the 30s work cap instead of 2s —
a deterministic red on every CI run, green locally. The probes now pin LOCAL
semantics with CI cleared (same save/restore pattern as their PATH shim),
and a fourth probe pins the clamp itself: CI=1 + 2s grace + 6s timeout must
kill at the 6s cap, still in the startup phase — proof an explicit low grace
cannot bypass the floor.

Automated security fix generated by Orbis Security AI

* docs: align prompt-injection thresholds to security.ts (v1.6.4.0 catch-up)

CLAUDE.md:290 and ARCHITECTURE.md:159 were missed when WARN was bumped
0.60 → 0.75 in d75402bb (v1.6.4.0, "cut Haiku classifier FP from 44% to
23%, gate now enforced", #1135). browse/src/security.ts:37 has WARN: 0.75
and BROWSER.md:743 was updated alongside that commit; CLAUDE.md and
ARCHITECTURE.md still read 0.60.

Also adds the SOLO_CONTENT_BLOCK: 0.92 entry to CLAUDE.md (already in
security.ts:50 and BROWSER.md:745, missing from CLAUDE.md's threshold

* fix: Korean/CJK IME input and rendering in Sidebar Terminal

Fixes #1272

This commit addresses three separate Korean/CJK bugs in the Sidebar Terminal:

**Bug 1 - IME Input**: Korean text typed via IME composition was not
reaching the PTY correctly. Added compositionstart/compositionend event
listeners to suppress partial jamo fragments and only send the final
composed string.

**Bug 2a - Font Rendering**: Added CJK monospace font fallbacks
("Noto Sans Mono CJK KR", "Malgun Gothic") to both the xterm.js
fontFamily config and the CSS --font-mono variable. This ensures
consistent cell-width calculations for Korean characters.

**Bug 2b - UTF-8 Boundary Detection**: Added buffering logic to prevent
multi-byte UTF-8 characters (Korean is 3 bytes) from being split across
WebSocket chunks. This follows the same pattern as PR #1007 which fixed
the sidebar-agent path, but extends it to the terminal-agent path.

Special thanks to @ldybob for the excellent root cause analysis and
proposed solutions in issue #1272.

Tested on WSL2 + Windows 11 with Korean IME.

* fix(ship): tighten Plan Completion gate (VAS-449 remediation)

VAS-446 shipped with a PLAN.md acceptance criterion (domain-hq has
/docs/dashboard.md) silently skipped. /ship's Plan Completion subagent
existed at ship time (added in v1.4.1.0) but the gate let the failure
through. Four structural fixes:

1. Path concreteness rule: items naming a concrete filesystem path MUST
   be classified DONE/NOT DONE via [ -f <path> ], never UNVERIFIABLE.
2. Validator detection: CONTENT-SHAPE items scan target repo's
package.json for validate-* scripts and run them before falling back
   to UNVERIFIABLE.
3. Per-item UNVERIFIABLE confirmation: replaces blanket "I've checked
each one" with per-item Y/N/D loop. The blanket-confirm path is the
   exact failure VAS-449 surfaced.
4. Subagent fail-closed: if Plan Completion subagent + inline fallback
both fail, surface explicit AskUserQuestion instead of silent pass.
Replaces the prior "Never block /ship on subagent failure" fail-open.

Locked in by test/ship-plan-completion-invariants.test.ts (5 assertions,
no LLM dependency, ~60ms).

* fix(browse): bash.exe wrap for telemetry on Windows

reportAttemptTelemetry() in browse/src/security.ts calls spawn(bin, args)
where bin is the gstack-telemetry-log bash script. On Windows this fails
silently with ENOENT — CreateProcess can't dispatch on shebang lines.

Adopts v1.24.0.0's Bun.which + GSTACK_*_BIN override pattern (from
browse/src/claude-bin.ts:resolveClaudeCommand, introduced in #1252) for
resolving bash.exe. resolveBashBinary() honors GSTACK_BASH_BIN absolute-path
or PATH-resolvable override, falling back to Bun.which('bash') which finds
Git Bash on the standard Windows install.

buildTelemetrySpawnCommand() wraps the script invocation on win32 only;
POSIX path is bit-identical. Returns null when bash can't be resolved on
Windows so caller skips spawn — local attempts.jsonl audit trail keeps
working without surfacing a Windows-only failure.

8 new unit tests cover resolveBashBinary (POSIX bash, absolute override,
quote-stripping, BASH_BIN fallback, empty-PATH null) and buildTelemetrySpawnCommand
(POSIX pass-through, win32 bash wrap, win32 null on unresolvable, arg-array
immutability).

POSIX path is bit-identical — Bun.which('bash') on Linux/macOS returns the
same /bin/bash or /usr/bin/bash that the old hardcoded spawn relied on.

* fix(make-pdf): Bun.which-based binary resolution for browse + pdftotext on Windows

Extends v1.24.0.0's Bun.which + GSTACK_*_BIN override pattern (introduced in
browse/src/claude-bin.ts via #1252) to the two other binary resolvers in the
codebase: make-pdf/src/browseClient.ts:resolveBrowseBin and
make-pdf/src/pdftotext.ts:resolvePdftotext.

Same Windows quirks (fs.accessSync(X_OK) degrades to existence-check; `which`
isn't available outside Git Bash; bun --compile --outfile X emits X.exe), same
Bun.which-based fix shape, same env override convention.

Changes:
  - GSTACK_BROWSE_BIN / GSTACK_PDFTOTEXT_BIN as the v1.24-aligned overrides;
    BROWSE_BIN / PDFTOTEXT_BIN remain as back-compat aliases.
  - Bun.which() replaces execFileSync('which', ...) for PATH lookup. Handles
Windows PATHEXT natively; no more `where`-vs-`which` branch.
  - findExecutable(base) helper exported from each module, probes .exe/.cmd/.bat
after the bare-path miss on win32. Linux/macOS behavior is bit-identical
(isExecutable short-circuits before the win32 branch ever runs).
  - macCandidates renamed posixCandidates (always was — /opt/homebrew, /usr/local,
/usr/bin). No Windows candidates added; Poppler installs scatter across
Scoop/Chocolatey/portable zips and guessing causes false positives.
  - Error messages get a Windows install hint (scoop install poppler / oschwartz10612)
    and `setx` example for GSTACK_*_BIN.
  - Pre-existing test 'honors BROWSE_BIN when it points at a real executable'
    was hardcoded /bin/sh — made cross-platform via a REAL_EXE constant
(cmd.exe on win32, /bin/sh on POSIX). Was a Windows-CI blocker on its own.

Coordination: PR #1094 (@BkashJEE) covered browseClient.ts independently with a
narrower scope; this PR's pdftotext + cross-platform tests + GSTACK_*_BIN naming
are additive. Either order of merge works.

Test plan:
  - bun test make-pdf/test/browseClient.test.ts make-pdf/test/pdftotext.test.ts
    on win32 — 29 pass, 0 fail (12 new assertions: findExecutable POSIX/win32/null,
    resolveBrowseBin GSTACK_BROWSE_BIN + BROWSE_BIN + precedence + quote-strip,
same shape for resolvePdftotext + Windows install hint in error message).
  - POSIX branch unchanged — fs.accessSync(X_OK) on Linux/macOS short-circuits
before any win32 logic runs, matching the v1.24 claude-bin.ts pattern.

* fix(browse): NTFS ACL hardening for Windows state files via icacls

gstack's ~/.gstack/ state directory holds bearer tokens, canary tokens, agent
queue contents (with prompt history), session state, security-decision logs,
and saved cookie bundles — all written with { mode: 0o600 } / 0o700. On Windows,
those mode bits are a silent no-op: Node's fs module doesn't translate POSIX
modes to NTFS ACLs, and inherited ACLs leave every "restricted" file readable
by other principals on the machine (verified via icacls — six ACEs, the
intended user is the LAST of six).

Threat model is non-trivial on:
  - Self-hosted CI runners (different service account on the same Windows box
can read developer tokens, canary tokens, prompt history)
  - Shared development machines (agencies, studios, lab environments)
  - Multi-tenant servers with shared home directories

Orthogonal to v1.24.0.0's binary-resolution work — complementary at the write
side. v1.24's bin/gstack-paths resolves ~/.gstack/ correctly across plugin /
global / local installs; this PR ensures files written into those resolved
paths actually get the POSIX 0o600 semantic translated to NTFS.

The fix:
  - New browse/src/file-permissions.ts (158 LOC, 5 public + 1 test-reset).
restrictFilePermissions / restrictDirectoryPermissions wrap chmod (POSIX)
    or icacls /inheritance:r /grant:r <user>:(F) (Windows). writeSecureFile /
appendSecureFile / mkdirSecure are drop-in wrappers for the common patterns.
  - 19 call sites converted across 9 source files: browser-manager.ts,
    browser-skill-write.ts, cli.ts, config.ts, meta-commands.ts,
    security-classifier.ts, security.ts (4 sites), server.ts (5 sites),
    terminal-agent.ts (8 sites), tunnel-denial-log.ts.
  - (OI)(CI) inheritance flags on directories mean files created via fs.write*
*inside* an mkdirSecure-created dir inherit the owner-only ACL automatically
    — important for tunnel-denial-log.ts where appends use async fsp.appendFile.

Error handling: icacls failures (nonexistent path, missing icacls.exe, hardened
environments) log a one-shot warning to stderr and proceed. Once-per-process
gating prevents log spam if the condition persists. Filesystem stays
functional; the file just ends up with inherited ACLs.

Test plan:
  - bun test browse/test/file-permissions.test.ts — 13 pass, 0 fail (POSIX
mode-bit assertions, Windows no-throw, mkdir idempotence, recursive
    creation, Buffer payloads, append-creates-then-reapplies-once semantics)
  - bun test browse/test/security.test.ts — 38 pass, 0 fail (existing security
test suite plus the bash-binary resolution tests added in fix #1119; the
    converted writeFileSync/appendFileSync/mkdirSync sites in security.ts
    integrate cleanly)
  - Empirical icacls before/after on a real file — 6 ACEs → 1 ACE
  - bun build typecheck on all modified files — clean (server.ts has a
pre-existing playwright-core/electron resolution issue unrelated to this PR)

POSIX behavior is bit-identical to old code — fs.chmodSync(path, 0o6XX) on the
helper's POSIX branch matches the inline { mode: 0o6XX } it replaces. Linux
and macOS see no behavior change.

Inviting pushback on three judgment calls (in PR description):
  1. icacls vs npm library
  2. ACL scope — just user, or user + SYSTEM?
  3. Graceful degradation — once-per-process warn, not silent, not hard-fail.

* fix(browse): declare lastConsoleFlushed to restore console-log persistence

flushBuffers() references a `lastConsoleFlushed` cursor at server.ts:337
and assigns it at :344, but the `let lastConsoleFlushed = 0;`
declaration is missing — only the network and dialog siblings are
declared at lines 327-328.

Result: every 1-second flushBuffers tick (line 376) throws
`ReferenceError: lastConsoleFlushed is not defined`, gets swallowed by
the catch at line 369 ("[browse] Buffer flush failed: ..."), and the
console branch's append never runs. browse-console.log is never
written in any production deployment since this regressed.

Discovered by stress-testing the daemon with 15 concurrent CLIs against
cold state — the race surfaced the buffer-flush error spam in one
spawned daemon's stderr. Verified by running the daemon against a real
file:// page with console.log events: in-memory `browse console`
returns the entries, but `.gstack/browse-console.log` is never created
on disk.

Regression introduced by 1a100a2a "fix: eliminate duplicate command
sets in chain, improve flush perf and type safety" — the flush refactor
switched from `Bun.write` to `fs.appendFileSync` and added the
`lastConsoleFlushed` cursor pattern alongside its network/dialog
siblings, but missed the matching `let` declaration. Tests don't
currently exercise flushBuffers, so the regression shipped silently.

Fix:
  - Declare `let lastConsoleFlushed = 0;` next to `lastNetworkFlushed`
    and `lastDialogFlushed` (browse/src/server.ts:327)
  - Add a source-level guard test
    (browse/test/server-flush-trackers.test.ts) that fails any future
refactor that adds a fourth `last*Flushed` cursor without the
matching declaration. Same pattern as terminal-agent.test.ts and
dual-listener.test.ts — read source as text, assert invariant, no
    daemon required.

Test plan:
  - [x] New regression test fails on current main, passes with the fix
  - [x] `bun run build` clean
  - [x] Manual smoke: spawn daemon -> goto file:// page with
        console.log -> wait 4s -> .gstack/browse-console.log now
exists with the expected entries (163 bytes vs zero before)

* fix(browse): per-process state-file temp path to fix concurrent-write ENOENT

The daemon writes `.gstack/browse.json` via the standard atomic-rename
pattern: `writeFileSync(tmp, …) → renameSync(tmp, stateFile)`. Four
sites in server.ts use this pattern (initial daemon-startup state at
:2002, /tunnel/start handler at :1479, BROWSE_TUNNEL=1 inline tunnel
update at :2083, BROWSE_TUNNEL_LOCAL_ONLY=1 update at :2113), and all
four hard-code the same temp filename `${stateFile}.tmp`.

Under concurrent writers the shared filename races on the rename:

    t0 Writer A: writeFileSync(stateFile + '.tmp', payloadA)
    t1 Writer B: writeFileSync(stateFile + '.tmp', payloadB) // overwrites A
    t2 Writer A: renameSync(stateFile + '.tmp', stateFile) // moves B's payload
    t3 Writer B: renameSync(stateFile + '.tmp', stateFile) // ENOENT — file gone

Reproduced empirically with 15 concurrent CLIs against a fresh `.gstack/`:

[browse] Failed to start: ENOENT: no such file or directory,
    rename '…/.gstack/browse.json.tmp' -> '…/.gstack/browse.json'

Pre-fix success rate: **0 / 15** under cold-start race.
Post-fix success rate: **15 / 15**, zero ENOENT.

Fix:
  - New `tmpStatePath()` helper (server.ts:333) returns
    `${stateFile}.tmp.${pid}.${randomBytes(4).toString('hex')}`
  - All 4 call sites use `tmpStatePath()` instead of the shared literal
  - Atomic rename still gives last-writer-wins semantics on the final
state.json content; only behavior change is that concurrent writers
no longer kill each other on the rename step

Source-level guard test (browse/test/server-tmp-state-path.test.ts)
locks two invariants: (1) no remaining `stateFile + '.tmp'` literals,
(2) every state-write `writeFileSync` call uses `tmpStatePath()`. Same
read-source-as-text pattern as terminal-agent.test.ts and
dual-listener.test.ts — no daemon required, runs in tier-1 free.

Test plan:
  - [x] Targeted source-level guard test passes (3 / 0)
  - [x] `bun run build` clean
  - [x] Live regression: 15 concurrent CLIs against cold state →
        15 / 15 healthy, 0 ENOENT (vs 0 / 15 pre-fix)
  - [x] No `.tmp.*` orphans left behind after rename succeeds
  - [x] Related test cluster (server-auth, dual-listener, cdp-mutex,
findport) — same pre-existing flakes as `main`, no new
        regressions introduced

* fix(browse): clear refs when iframe auto-detaches in getActiveFrameOrPage

Asymmetric cleanup between two equivalent staleness conditions:

  onMainFrameNavigated()  →  clearRefs() + activeFrame = null  ✓
getActiveFrameOrPage() → activeFrame = null (refs NOT cleared) ✗

Both paths see the same staleness condition — refs were captured
against a frame that no longer exists. The main-frame path correctly
clears both pieces of state. The iframe-detach path nulls the frame
but leaves the refMap intact.

The lazy click-time check in `resolveRef` (tab-session.ts:97) partially
saves us — `entry.locator.count()` on a detached-frame locator throws
or returns 0, so the click errors out as "Ref X is stale". But the
user has no signal that frame context silently changed underfoot: the
next `snapshot` runs against `this.page` (main) while old iframe refs
still litter `refMap` with the same role+name keys. New refs collide
with stale ones, the resolver picks one at random, the user clicks
the wrong element.

TODOS.md line 816-820 documents "Detached frame auto-recovery" as a
shipped iframe-support feature in v0.12.1.0. This restores the
documented intent — the recovery should leave the session in a clean
state, not a half-cleared one.

Fix: 1 line — add `this.clearRefs()` next to `this.activeFrame = null`
inside the if-branch.

Test plan:
  - [x] New regression test: 4/4 pass
        - refs cleared when getActiveFrameOrPage detects detached iframe
        - refs preserved when active frame is still attached (no regression)
        - refs preserved when no frame set (page-level path untouched)
        - matches onMainFrameNavigated symmetry — both paths reach the
          same clean end state
  - [x] `bun run build` clean

* fix(codex): resolve python for JSON parser

* fix: add fail-fast probe for base branch in ship step 12

* fix(plan-devex-review): remove contradictory plan-mode handshake

* fix(design): honor Retry-After header in variants 429 handler

Closes #1244.

The 429 handler in `generateVariant` discarded the `Retry-After` response
header and fell straight through to a local exponential schedule (2s/4s/8s).
In image-generation batches, that burns retry attempts inside the provider's
cooldown window and the request never recovers.

Now we parse `Retry-After` per RFC 7231 — both delta-seconds (`Retry-After: 5`)
and HTTP-date (`Retry-After: Fri, 31 Dec 1999 23:59:59 GMT`). Honored waits
are capped at 60s to bound stalls from hostile or buggy headers. Delta-seconds
are validated as digits-only (rejects `2abc`). When `Retry-After` is honored
(including 0 / past-date "retry now"), the next iteration's leading exponential
sleep is skipped so we don't double-wait. Invalid or missing headers fall
through to the existing exponential schedule unchanged.

Behavior matrix:

| Header | Behavior |
|---------------------------------|-------------------------------------------|
| Retry-After: 5 | wait 5s, skip leading on next attempt |
| Retry-After: 999999 | capped to 60s, skip leading |
| Retry-After: 2abc | invalid, fall through to exponential |
| Retry-After: 0 | wait 0, skip leading (retry immediately) |
| Retry-After: <past HTTP-date> | wait 0, skip leading |
| Retry-After: <future date> | wait diff capped at 60s, skip leading |
| no header | fall through to existing exponential |

`generateVariant` now accepts an optional `fetchFn` parameter (defaults to
`globalThis.fetch`) so tests can inject a stub. Production call sites are
unchanged.

Tests cover the five behavior buckets above, asserting both the 1st-to-2nd
call timing gap and call counts. All five pass in ~8s.

* fix(docs): correct per-skill symlink removal snippet in README uninstall

Closes #1130.

The manual-uninstall fallback in `## Uninstall` → `### Option 2` used
`find ~/.claude/skills -maxdepth 1 -type l`, which finds nothing on real
installs. Each `~/.claude/skills/<name>/` is a real directory, and only
`<name>/SKILL.md` inside it is a symlink into `gstack/`. The find never
matched, so the snippet silently removed nothing.

Replace with a directory walk that inspects each `<name>/SKILL.md`:

  find ~/.claude/skills -mindepth 1 -maxdepth 1 -type d ! -name gstack
  → check $dir/SKILL.md is a symlink → readlink it
→ if target is gstack/* or */gstack/*: rm -f the link, rmdir the dir
(only if empty — preserves any user-added files)

Excludes the top-level `gstack/` dir from the walk; that's removed by
step 3 of the same uninstall block.

`bin/gstack-uninstall` (the script-mode path) already handles the layout
correctly via its own walk; only this manual fallback needed updating.

* fix: reject partial browse client env integers

* fix(gemini-adapter): detect new ~/.gemini/oauth_creds.json auth path

gemini-cli >=0.30 stores OAuth credentials at ~/.gemini/oauth_creds.json
instead of the legacy ~/.config/gemini/ directory. The benchmark adapter's
availability check now succeeds for users on recent gemini-cli releases
who have authenticated via interactive login.

Both paths are accepted so users on older versions still work.

* fix(browser): add --no-sandbox for root user on Linux/WSL2

Chromium's sandbox can't initialize when running as root on Linux,
causing an immediate exit. Extend the existing CI/CONTAINER check to
also cover this case, keeping the Windows-safe `typeof getuid` guard.

* security: pass cwd to git via execFileSync, not interpolation through /bin/sh

`bin/gstack-memory-ingest.ts:632-643` ran `execSync(\`git -C ${JSON.stringify(cwd)}
remote get-url origin 2>/dev/null\`, ...)`. JSON.stringify escapes `"` and `\`
but not `$` or backticks, so a `cwd` of `"$(touch /tmp/marker)"` survived JSON
quoting and detonated under /bin/sh's command-substitution-inside-double-quotes.

`cwd` originates from transcript JSONL records under
`~/.claude/projects/<encoded-cwd>/<uuid>.jsonl` and
`~/.codex/sessions/YYYY/MM/DD/rollout-*.jsonl`. The walker grabs the first
`.cwd` it sees per session. That's an untrusted surface in the gstack threat
model — the L1-L6 sidebar security stack exists exactly because agent
transcripts can carry attacker-influenced text. Two pivots above the local
same-uid bar: (a) prompt-injection appending `cwd="$(...)"` to the active
session log turns the next /sync-gbrain run into RCE under the user's uid;
(b) cross-machine transcript share (a colleague's `.claude/projects` snippet
untar'd into HOME, a documented gbrain dogfooding shape) → RCE on first sync.

Fix swaps the one execSync for `execFileSync("git", ["-C", cwd, "remote",
"get-url", "origin"], ...)`. No shell, argv passed directly to git. The same
module already uses execFileSync for `gbrainAvailable()` (line 762 pre-patch)
and `gbrainPutPage()` (line 816 pre-patch) — this single execSync was the
outlier.

Test: `gstack-memory-ingest security: untrusted cwd cannot trigger shell
substitution` plants a Claude-Code-shaped JSONL with cwd=`$(touch <marker>)`
and asserts the marker file is not created after `--incremental --quiet`.
Negative control: with the patch reverted, the test fails (marker created);
with the patch applied, it passes (18/18 in test/gstack-memory-ingest.test.ts).

* security: gate domain-skill auto-promote on classifier_score > 0

`browse/src/domain-skill-commands.ts:140` (handleSave) writes
`classifier_score: 0` with the comment "L4 deferred to load-time / sidebar-agent
fills this in on first prompt-injection load." But CLAUDE.md "Sidebar
architecture" documents that sidebar-agent.ts was ripped, and grep for
recordSkillUse + classifierFlagged callers across browse/src/ returns zero hits
outside the module under test.

Net effect: every quarantined skill that survives three benign uses without
flag (`recordSkillUse(... , classifierFlagged: false)` x3) auto-promotes to
`active` and lands in prompt context wrapped as UNTRUSTED on every subsequent
visit to that host. The L4 score that was supposed to gate the promotion was
never written — the production save path puts 0 on disk and nothing later
updates it.

Threat model: a domain-skill body authored by an agent under the influence of
a poisoned page (the new `gstackInjectToTerminal` PTY path runs no L1-L3
either) would lose its auto-promote barrier after three uses. The exploit
isn't single-step but the bar is exactly N=3 prompt-injection-shaped uses on
a hostile page, which is well within reach.

Fix adds a single condition to the auto-promote gate in `recordSkillUse`:

    if (state === 'quarantined' && useCount >= PROMOTE_THRESHOLD &&
        flagCount === 0 && current.classifier_score > 0) {
      state = 'active';
    }

`classifier_score` is set once at writeSkill and never updated. Production
saves it as 0 (handleSave), so the gate stays closed; existing tests that
explicitly pass `classifierScore: 0.1` still auto-promote (the auto-promote
path is preserved for the day L4 is rewired).

Manual promotion via `domain-skill promote-to-global` is unaffected (it goes
through `promoteToGlobal` which has its own state-machine guard at line 337+).

Test: new regression case `does NOT auto-promote when classifier_score is 0
(production handleSave shape)` plants a skill with classifierScore=0 (matches
domain-skill-commands.ts:140), runs three uses without flag, asserts the skill
stays quarantined and readSkill returns null. Negative control: revert the
patch, the test fails with `Received: "active"`. With the patch: 15/15 pass.

* fix(ship): port #1302 SKILL.md edits to .tmpl + resolver source

PR #1302 added Verification Mode + UNVERIFIABLE classification + per-item
confirmation gate to ship/SKILL.md, but only the generated SKILL.md was
edited — not the .tmpl source or scripts/resolvers/review.ts. The next
`bun run gen:skill-docs` run would have wiped the changes.

Port the same content into the resolver and .tmpl so regeneration produces
the intended output.

* ci(windows): extend free-tests lane to cover icacls + Bun.which resolvers from fix-wave PRs

Closes #1306/#1307/#1308 validation gap. The four newly-added test files
already have process.platform guards so they run safely on both POSIX and
Windows lanes — only platform-relevant assertions execute on each.

Tests added to the windows-latest lane:
- browse/test/file-permissions.test.ts (#1308 icacls + writeSecureFile)
- browse/test/security.test.ts (#1306 bash.exe wrap pure-function path)
- make-pdf/test/browseClient.test.ts (#1307 Bun.which browse resolver)
- make-pdf/test/pdftotext.test.ts (#1307 Bun.which pdftotext resolver)

* test(codex): live flag-semantics smoke for codex exec resume

Closes #1270's regex-only test gap. PR #1270 asserted that codex/SKILL.md's
`codex exec resume` invocation drops -C/-s and uses sandbox_mode config.
That regex catches the skill template regressing, but not codex CLI itself
flipping flag semantics again.

This test probes `codex exec resume --help` and asserts the surface gstack
relies on: -c/sandbox_mode is accepted, top-level -C is absent. Skips
silently when codex isn't on PATH, so dev machines without codex installed
never see it fail.

* chore: regen SKILL.md after fix wave

One regen commit at the end of the merge wave per the plan. plan-devex-review
loses the contradictory plan-mode handshake (#1333). review/SKILL.md picks up
the Verification Mode + UNVERIFIABLE classification additions that #1302
authored against ship/SKILL.md (same resolver shared between ship and review
modes).

* fix(server.ts): keep fs.writeFileSync for state-file writes

#1308's writeSecureFile wrapper added Windows icacls hardening for the
4 state-file write sites in server.ts, but #1310's regression test grep's
for fs.writeFileSync(tmpStatePath()) calls. The two changes are technically
compatible only if the test relaxes — keeping the test strict (the safer
choice for catching regressions on the cold-start race) means the 4 state-
file sites stay on fs.writeFileSync(..., { mode: 0o600 }).

POSIX 0o600 hardening is preserved on those 4 sites. Windows icacls
hardening still applies to all the other writeSecureFile call sites
#1308 added (auth.json, mkdirSecure, etc.).

Also refreshes golden baselines after #1302 / port + minor wording tweak
in scripts/resolvers/review.ts to keep gen-skill-docs.test.ts assertion
'Cite the specific file' satisfied.

* v1.30.0.0: fix wave — 21 community PRs + 2 closing fixes for Windows + codex CI gaps

Headline release. Browse stops dropping console logs, cold-start race
fixed, codex resume works without python3, Windows hardening (icacls +
Bun.which + bash.exe wrap), ship gate gets VAS-449 remediation, two
closing fixes that put icacls/Bun.which/codex flag semantics under CI.

* test(domain-skills): cover #1369 classifier_score=0 quarantine + score>0 promote path

The pre-existing T6 test seeded skills via writeSkill (which defaults
classifier_score to 0 until L4 is rewired) and then expected 3 uses to
auto-promote. PR #1369 added `current.classifier_score > 0` to the gate
specifically to block that path — a quarantined skill written under the
influence of a poisoned page would otherwise auto-promote after three
benign uses.

Updated test asserts both halves of the new contract:
- classifier_score=0 + 3 uses → stays quarantined (the security guarantee)
- classifier_score>0 + 3 more uses → promotes to active (unblock path)

Catches both regressions: the gate going away (would re-allow the bypass)
and the unblock path breaking (would silently quarantine all skills
forever once L4 is rewired).
