Garry Tan 55fbe77195 v1.91.24.0 test: audit wave — PR evals keep finished work, honest ceilings, EVAL_POLICY v2, faster free suite (#3031)
* chore(scripts): retire hollow and legacy paid package scripts behind one-release stubs (W8a, DX-4, CEO-31)

test:codex*, test:evals*, test:e2e*, test:gate, test:periodic, eval:bg,
eval:bg:all, eval:flake-rank, eval:watch and test:audit now print their
replacement and exit 1 via scripts/retired-command.ts. CONTRIBUTING.md
carries the same table; test/retired-commands.test.ts pins both. Eval CLI
empty-state hints point at eval:bg:pr.

* chore: delete dead update-readme-throughput, eval-watch, orphan fixtures and QA_FUNCTIONAL_INPUTS (W8b, CEO-24)

Retired with named behavior:
- update-readme-throughput.ts + readme-throughput test: replaced a README
  anchor that no longer exists from a JSON file that was never produced;
  the script was a no-op. Guarantee withdrawn (nothing to protect).
- eval-watch.ts + its dashboard tests: rendered ~/.gstack-dev/e2e-live.json
  and _partial-e2e.json, written only by unsharded runs, so it showed
  nothing for every supported (sharded) entry point. Guarantee withdrawn;
  live progress is the gstack-detach log or gh run watch. Manual-review
  rendering stays covered by eval-list and eval-summary tests.
- Orphan fixtures (autoplan-pending-artifact-ae.json, autoplan-existing-app/,
  autoplan-sign-in-ui/, parity baselines v1.47.0.0/v1.57.7.0/v1.69.1.0):
  no test, touchfile or script reads them; superseded baselines stay in
  git history.
- QA_FUNCTIONAL_INPUTS: exported, never imported.

* refactor(test-free): delete dead WORKER_HOSTILE list and buildShardArgs parallel mode

Neither had a production caller: full-suite mode runs shard processes, so the
worker quarantine was inert and --parallel was never passed (W8b).

* docs(test-value): narrow prompt-byte contract to machine-read tokens (D2)

Define prompt-byte contract as machine-read tokens (markers, field names,
command lines, enum values, file paths, security-boundary strings) and add a
sentence-pin glossary entry: convert to structural checks or delete when a
behavioral eval covers the behavior.

* test(ratchet): count free tests that slice a paid test's source

A free test that binds readFileSync(<paid test file>) and cuts it with
.slice() tests the paid harness's code out of context even when it imports a
lib/ symbol. Record the 15 existing files with reasons; new ones fail with fix
text. Self-test covers positive and negative shapes.

* test(browse): un-quarantine compare-board; all 16 board tests pass headless (W8d, CEO-24)

The GSTACK_COMPARE_BOARD_TESTS opt-in hid a headless mismatch that no
longer reproduces: all 16 tests pass on Bun 1.4.0 with the bundled
headless Chromium, solo, repeated, and beside other in-process browser
files. Protected behavior (the comparison-board DOM contract the
plan-design-review/office-hours/design-consultation feedback loop polls)
is checked on every free run again.

* test: replace silent existsSync early returns with explicit skips or assertions (W8d)

- browse/test/build.test.ts: server-node.mjs checks skip only when the
  bundle is absent and GSTACK_EXPECT_BINARIES is unset; the free CI lane
  and test:ubicloud build it and set the flag, so a dropped build step
  fails with the build command instead of passing with no assertions.
- gstack-update-check: VERSION is tracked; the guard could only hide a bug.
- gen-skill-docs host leakage/frontmatter checks assert the host tree
  exists (the module-level render always produces it).
- cso-docker staged-runtime test uses test.skipIf so reports show it as
  skipped, not passed.

* test(ship): convert ship-workflow-clarity sentence pins to structural checks

Add test/helpers/prompt-structure.ts (between, expectTokens, expectAbsent,
expectOrdered, expectMentions) with fix text on failure. ship-workflow-clarity
now pins headings, step order and machine-read tokens (commands, fields, enum
and status strings); safety rules are meaning-level keyword checks. Wording is
judged by the 'ship/SKILL.md workflow' judge in skill-llm-eval.
Sentence pins: 90 -> 0.

* ci: add platform-qualification lane for opt-in ML, gitleaks and Swift tests (W8d, CEO-24)

security-bench + security-live-playwright L4 (SECURITY_BENCH), the real
gitleaks 8.30.1 memory-ingest cases (GSTACK_TEST_GITLEAKS) and the
DebugBridge Swift build invariants (GSTACK_TEST_SWIFT) self-skip in every
lane today. No scheduled lane has their prerequisites, so per CEO-24 they
move to an owned dispatch + quarterly workflow instead of being deleted.
Each job was run locally (ML 10 pass, gitleaks 130 pass; Swift needs
macOS). test/platform-qualification-workflow.test.ts pins gate variable,
job and file together.

* fix(scripts): capture-baseline and preflight-agent-sdk no longer act on --help or typos (W8e)

capture-baseline parses its flags strictly: --help prints usage, an unknown
or valueless flag exits 2, and only a valid invocation writes a fixture
(--help used to write test/fixtures/parity-baseline-current.json).
preflight-agent-sdk gains --help, rejects unknown flags, and runs the live
SDK query (API spend) only with --live.

* test(evals): register the three keyless periodic paid files with case ids (W1b)

skill-e2e-auq-consistency, skill-e2e-auq-verbose-vs-carved-ab and
codex-e2e-recommendation-substance now carry touchfiles, a periodic tier and
kind rule (one trial, unchanged), so editing one selects its own case instead
of restoring the full PR gate. The verbose A/B case also registers the vendored
pre-cut fixture it reads, and review-army-migration-safety registers the SQL
fixture it reads.

* feat(evals): narrow PR fallback for free-only inputs (W1a, CEO-13/20, ENG-13)

The PR profile now exempts the duration seeds, every free-lane workflow
(explicit list; inventory test fails on an unclassified workflow, .yaml
and workflow_call included), scripts/ubicloud/** and fixtures declared in
the explicit FREE_FIXTURES map. Each remaining unknown file carries a label
and fix (needs touchfile entry / add to FREE_ONLY_PR_FILES / real unknown
dependency). evals.yml itself still restores the full gate.

* ci: delete ci-image.yml; the eval workflows already build and push the same content-hash tag (W8f)

evals.yml, evals-periodic.yml and evals-marathon.yml each compute the
identical hashFiles() tag and build/push it (with :buildcache and :latest)
when missing, so ci-image's push trigger and weekly cron were no-ops
(30-day median 0.5 min, 36/36 success). ci-image-tag-binding now pins the
three eval workflows.

* ci(evals): key periodic/marathon concurrency on ref+event; branch periodic dispatch skips the gate census (W2a, ENG-9, CEO-04)

* refactor(paid): drop ~160 unused imports; leaf paid-types/paid-select modules break the paid-* import cycle (W8c)

* ci(quality-gate): per-PR concurrency, free runner, drop unscoped bun audit (W2a, W8f)

- Concurrency keys on pull_request.number || run_id, so back-to-back main
  pushes no longer cancel each other's credential scan.
- ubuntu-24.04 instead of a billed 8-vCPU Ubicloud runner for a ~1 min job.
- bun audit --audit-level=critical ran on every PR regardless of diff, so a
  newly published advisory turned unrelated PRs red, with its own ignore
  policy. dependency-review (lockfile PRs, fail >= high) and the weekly OSV
  sweep remain the dependency gates.

* chore(test-free): refresh the free duration seed from the audit's Ubicloud recording

Adds the 19 files merged after the last refresh, including the 115 s
claude-overlay-setup that packed as ~1 s and doubled a shard (W6a). The seed
is re-recorded on the final tree at the end of this wave.

* ci: move tiny ubicloud-standard-2 jobs to free ubuntu-24.04 runners (W8f)

actionlint, skill-docs, dependency-review, version-gate and pr-title-sync
are sub-minute jobs; GitHub-hosted runners are free for this public repo.

* feat(test-free): pessimistic unseeded packing, seed growth ratchet and CI health summary

- Unseeded files pack at the seed's p99 instead of p75 (W6a).
- --ci-plan warns in the job summary past 5 unseeded files; --ci-verify
  lists shards that outran their prediction by 1.5x (and 30 s) plus every
  flaky pass absorbed by the serial retry (W6a, W7a job summary).
- test/free-seed-ratchet.test.ts fails when a seeded file over 60 s is not
  on scripts/free-test-seed-allowlist.json, when an entry is stale, and when
  the allowlist grows versus its merge-base copy unless the new entry is an
  annotated exception with a todo (CEO-10, DX-7). free-plan runs it with
  full history so the shrink check is real in the required lane.
- Every message names the fix command and docs anchor (DX-6).

* feat(evals): state PR coverage mode, counts, reuse and fallback files in the summary and comment (CEO-13)

* test: make the tracked-tree skill-doc freshness check local-only; tidy skill-docs header (W8f)

Every free CI shard regenerates the tracked SKILL.md files before running
tests, so the --dry-run freshness assertion compared the regenerated tree
with itself and could not fail there. It now skips under GitHub Actions;
skill-docs.yml's git diff --exit-code is CI's freshness gate. Removes a
stale concurrency comment in skill-docs.yml.

* ci(free-tests): install zsh so the bash+zsh portability arms run (W6f)

~42 zsh-arm tests (aside-probe-shell, qa-browser-preservation,
generated-docs-fences) skipped in CI because zsh is not in the runner image.
All 141 tests in those files pass with zsh present.

* feat(evals): executors write negative receipts at the execution boundary (CEO-25, ENG-6)

A completed single-trial rule/judge shard that did not pass now writes its
negative receipt beside the pass receipts in the slice artifact, so a
cancelled run's FAIL still blocks an older PASS when its report never runs.
Receipts merge negatives first and order by (run attempt, completedAt); a
same-second FAIL/PASS tie resolves to FAIL. The runner call site is a
one-line hunk in scripts/test-paid-shards.ts.

* docs(cso): state that contained runtime/scanner execution is unavailable today

The scanner catalog is empty while its build inputs await review and the
runtime catalog holds no qualified profile until the private evaluator
exists, so /cso runs static assessment only. AGENTS.md, README and
docs/cso-release-qualification.md now say so; docs/cso-scanners.md
already did.

* test(helpers): expectMentions splits on sentence ends only

Clauses joined by ';' or ':' are one rule; splitting there made a reworded
rule fail for punctuation.

* test(plan-ceo): convert skill-ceo-section-ordering sentence pins

Safety and authority rules (never/do not/stop/only/without/wait/approval) are
meaning-level expectMentions checks; other sentence pins are dropped. Headings,
labels, option rows, placeholders and machine-read lines (auto-decide
annotation, Mode:/approved decisions format, STATUS/SOURCE fields,
CROSS-MODEL marker, gstack-ceo-mode-handoff) stay as tokens; ordering checks
unchanged. Behavior is covered by the periodic PTY case in
skill-e2e-plan-ceo-review-section-loading.test.ts.
Sentence pins: 210 -> 23.

* test(helpers): expectMentions treats wrapped lines as one sentence

Templates hard-wrap prose; only sentence ends and blank lines separate rules.

* test(gen-skill-docs): convert sentence pins to meaning checks or drop them

Safety/authority sentences become expectMentions keyword checks; other English
sentences are dropped (wording is judged by the per-skill workflow judges in
skill-llm-eval and the E2E cases in touchfiles-data). Commands, config keys,
markers (PLAN MODE EXCEPTION, NO UNRESOLVED DECISIONS, Gate outcome), field
names and alternation-based meaning regexes stay. Removed the now-empty
'contains opt-in AskUserQuestion text' test (only pinned a sentence; the
resolver's AskUserQuestion wiring is covered by the codex second-opinion
tests in the same describe). Lane E's existsSync hunks are untouched.
Sentence pins: 216 -> 65 (remaining lines carry machine tokens).

* ci(windows): shard the Windows lane via --ci-plan/--ci-run/--ci-verify; move native campaigns to native-qualification.yml

- windows-free-tests.yml: a Linux plan job curates and packs the Windows-safe
  files into 6 shards, one strict serial shard per windows-latest job, and a
  Linux aggregate verifies exact coverage and every result (8.1 min on one
  runner today). --windows-only now combines with the CI modes; the plan binds
  the curated file set (W6e).
- Every Windows plan and --list run prints the curated count with its top
  exclusion reasons in the job summary; a free ratchet fails when the curated
  subset drops below its floor, with the fix (W6e).
- The four Dia/cookie diagnostic dispatch campaigns move to the dispatch-only
  native-qualification.yml with one exclusive `mode` choice input, so their
  red dispatch runs no longer read as a red PR lane (W8f). Their workflow pins
  move to test/windows-native-workflows.test.ts; the boolean-exclusivity
  guard is replaced by the choice input.

* feat(paid): per-slice CI job ceilings and an in-process slice deadline with not_run/hung outcomes and checkpoints (W2c, ENG-2)

* fix(pass-rates): segment trial history strictly by EVAL_POLICY version

Reader-compatibility floor for EVAL_POLICY v2 (ENG-14/CEO-35/DX-12). Only
trials under the reader's own policy version feed labels and alarms; older
policy trials stay visible but are never scored or used as a drift baseline;
trials from a newer policy are ignored with a printed count and fix text.
Reverting the later v2 writer commits leaves a reader that cannot pool v2
records into v1 series (the pre-PR reader at 2db0b3a does: it reports a
mixed fixture as FAILING with drift alarms).

* docs: one contributor command table; retired commands, seed sources and current workflows (W8g, DX-5, DX-13, CEO-12)

- CONTRIBUTING.md gains 'Which command do I run?' (task -> command ->
  cost/time -> needs, including branch validation via gh workflow run);
  CLAUDE.md, AGENTS.md and TESTING_INTERNALS.md link to it instead of
  repeating command lists. Tier tables drop retired commands and stale
  per-run dollar estimates.
- Retired names (test:evals, test:e2e, eval:bg, eval:flake-rank, eval:watch,
  ...) are gone from CLAUDE.md, CONTRIBUTING.md, TESTING_INTERNALS.md,
  ARCHITECTURE.md and USING_GBRAIN_WITH_GSTACK.md.
- The flake-ledger claim now says what happens: CI uploads flake-ledger-*
  artifacts; eval:pass-rates reads only the local ledger.
- TESTING_INTERNALS states that only test:ubicloud --record-durations
  produces an acceptable duration seed (DX-13).
- CI section: quality gate no longer runs bun audit; dependency-review's
  real trigger; the platform-qualification lane.
- docs/test-audit-2026-09.md gets a corrections section for its stale claims.
- TODOS.md: audit follow-ups (post-merge metrics, stub removal, name reclaim,
  diff-aware planner); eval-watch shard-awareness and the image freshness
  tripwire closed as won't-do.

* feat(evals): recover receipts of cancelled PR runs (W1c, CEO-14/34, ENG-4/5/7)

A new recover-receipts job runs base-ref code with actions: read, restores
the PR's store, merges the receipts of completed cancelled evals.yml runs
for this PR (provenance-checked, at most 5, oldest first, negatives first),
reconstructs completed panel negatives with panelVerdict() (partial panels
get no credit) and keeps a recovery.json checkpoint that never advances past
an unresolved run. plan-slices keeps contents: read, takes the store as an
artifact, turns reuse off when recovery degraded or a run is unresolved, and
carries the whole store to slices-report so nothing is lost. Any gh problem
degrades to no recovered receipts with a named reason.

* feat(evals): classify this wave's free-only tools and workflows for PR selection

platform-qualification.yml, retired-command.ts, test-health-report.ts,
bump-harness-version.ts and eval-bg.ts never run inside a paid case.

* feat(evals): debounce PR push bursts before planning paid slices (W1d, CEO-15, ENG-11, DX-7)

A tiny ubuntu-24.04 job waits 90 s only on synchronize when another run for
the PR started in the last 15 minutes; cancel-in-progress supersedes, and a
run that outlives the wait reports "superseded by <sha>" and plans nothing.
Labels: evals-no-debounce skips the wait, evals-fresh disables receipt reuse.

* perf(test): move the real-process browser settlement cases out of test-free-shards.test.ts (W6b)

The 19 owned-browser settlement modes were 85 s of the file's 95 s and could
not be packed apart from the fast runner unit cases. They move to a shared
fixture with two files (lifecycle 47 s, ownership 39 s); the unit file drops
to 10 s. Same modes, same assertions.

* chore(evals): delete the finished validation_phase dispatch and its inline planner (W8f)

Retirement (CEO-24): the protected behavior was a one-off cookie-repair
validation subset (eight dependent cases) and a quality/behavior split of
the census; that validation is complete, so the guarantee is withdrawn.
Branch validation dispatches evals.yml with evals_all.

* perf(test): inject the office-hours record grace in non-cooperative deadline tests

* perf(test): shrink aside-render process slack via GSTACK_RENDER_SLACK_MS in timeout tests

* perf(test): make the memorable lock give-up configurable for lock-contention tests

* perf(test): check install-timeout knob normalization through its own setup lines

* perf(test): inject a 2 s storage-reset deadline in the late isolated dispatch test

* perf(test): shorten the slow gbrain fake to 1 s, still past the 300 ms probe timeout

* ci(free-tests): run only cso-docker-integration in the free CSO Docker job (F16)

cso-node-lifecycle-integration and cso-stack-cold-integration need
GSTACK_CSO_TEST_STACK images and always skipped there; they stay in the CSO
runtime workflows through test:cso:docker.

* test(gen-skill-docs,plan-ceo): split mixed sentence pins into tokens and rules

Sentences that wrap a backticked or bold token keep the token (expectTokens)
and, when they state a safety rule, a keyword check; short safety sentences
(25-39 chars) get the same treatment.

* test(plan-ceo): convert ceo-workflow-clarity.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: periodic PTY case skill-e2e-plan-ceo-review-section-loading and the plan-ceo workflow judges.
Sentence pins: 13 -> 1.

* test(plan-eng): convert eng-review-routing.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: plan-eng E2E cases (skill-e2e-plan-eng-*) and the plan-eng workflow judge.
Sentence pins: 28 -> 6.

* test(plan-eng): convert eng-scope-entry-ap.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: plan-eng E2E cases (skill-e2e-plan-eng-*) and the plan-eng workflow judge.
Sentence pins: 16 -> 5.

* test(plan): convert plan-scope-selection.test.ts sentence pins

checks stay. Behavior and wording: plan-review E2E cases and workflow judges in skill-llm-eval.
Sentence pins: 19 -> 2.

* test(qa): convert qa-lazy-sections.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: qa/qa-only E2E cases (skill-e2e-qa-*) and the qa workflow judge.
Sentence pins: 71 -> 11.

* test(qa): convert qa-browser-preservation.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: qa/qa-only E2E cases (skill-e2e-qa-*) and the qa workflow judge.
Sentence pins: 25 -> 11.

* test(qa): convert qa-probe-gates.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: qa/qa-only E2E cases (skill-e2e-qa-*) and the qa workflow judge.
Sentence pins: 20 -> 5.

* test(qa): convert qa-checkpoint-evidence.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: qa/qa-only E2E cases (skill-e2e-qa-*) and the qa workflow judge.
Sentence pins: 7 -> 5.

* test(qa): convert qa-evidence.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: qa/qa-only E2E cases (skill-e2e-qa-*) and the qa workflow judge.
Sentence pins: 4 -> 3.

* test(design): convert design-consultation-contract.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: design-consultation E2E/judge cases in touchfiles-data.
Sentence pins: 44 -> 11.

* test(outside-voice): convert run-in-background-guidance.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: outside-voice E2E cases and plan-review workflow judges.
Sentence pins: 35 -> 5.

* test(spec): convert spec-template-invariants.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: spec E2E/judge cases in touchfiles-data.
Sentence pins: 36 -> 21.

* perf(evals): 420 s census slice budget; case-shard skill-e2e-qa-workflow; seed its case walls (W5c)

* feat(evals): evals.yml dispatch contract for eval:bg (CEO-17/29, DX-1, ENG-8/12)

workflow_dispatch gains base_ref, base_sha, expected_sha, nonce and
pr_receipts. The nonce appears in run-name so the dispatcher resolves its
own run; evals_all=false runs the PR profile against base_sha/base_ref; a
checkout that differs from expected_sha fails before any paid work with a
named fix; pr_receipts reads that PR's receipts read-only through the
recover-receipts artifact path (never the cache) and never saves.

* ci(windows): pack the Windows lane with Windows-measured durations (W6e)

The first sharded run (37221451711) finished in 6.0 min: the Linux seed has
browse/test/cookie-import-native-job.test.ts at ~0 s (its cases skip on
Linux) but it takes 122 s on Windows, so one shard ran 242 s against an 84 s
prediction. The plan now reads scripts/free-test-durations-windows.json; a
record_durations dispatch times every Windows-safe file alone and uploads it.

* test(free): settlement case files import the runner rule they exercise

Keeps the split files out of the test-of-test ratchet: the retry-eligibility
rule is a direct runner import passed to the shared fixture.

* fix(marathon): no hollow plan-decision-classification shard; office-hours marathon scenario scoped to the pilot (W2f)

* test(free): satisfy the typecheck:test ratchet in the seed ratchet test

* refactor(ci-history): one shared GitHub history reader with the CI flake-ledger reader

Extract pass-rates' gh fetch layer into scripts/lib/ci-history.ts (CEO-09) and
add the W7a reader: paged run listings that fail whole on any page error,
per-run artifact downloads (cached, size-capped, data only), flake-ledger
parsing, the pre-registered flaky-file rule (> 5% of main free-tests runs over
a window of at least 20 runs, widened to the newest 20 runs when the window is
thinner), a job-summary table, and named unavailable reasons with next steps.

* feat(test-health): test:health report of the audit success metrics

scripts/test-health-report.ts (CEO-09/06/22/32, DX-8/9/12/13) prints every
success metric with its counting rule from CI history: PR full-fallback rate,
reused PR trial records, PR-lane spend per merged PR and per validated
revision, cancelled slice-minutes per week, push-to-verdict p50/p95, census
expected failed trials per run with raw failure rate per kind/class and
P(lane red) under the recorded panels vs single-trial aggregation, INFRA
re-dispatches, quarantine evidence (insufficient evidence distinct from
healthy), free flaky-pass rate, free/Windows wall, free serial time and the
net-complexity line. An unreadable input prints 'unavailable: <reason>; next:
<step>', never 0. --enforce fails on a flaky file (W7a) or > 5 unseeded free
files (CEO-10); unavailable checks never fail and keep the issue open.

* test(review): convert review-workflow-clarity.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: review E2E cases (skill-e2e-review*) and the review workflow judge in skill-llm-eval.
Sentence pins: 85 -> 17.

* test(review): convert review-entry-and-design-clarity-au.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: review E2E cases (skill-e2e-review*) and the review workflow judge in skill-llm-eval.
Sentence pins: 15 -> 4.

* test(review): convert review-enum-lifecycle.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: review E2E cases (skill-e2e-review*) and the review workflow judge in skill-llm-eval.
Sentence pins: 8 -> 0.

* test(ship): convert ship-control-flow.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: ship E2E cases (skill-e2e-ship*, skill-e2e-workflow) and the 'ship/SKILL.md workflow' judge.
Sentence pins: 74 -> 9.

* test(ship): convert ship-skip-requeue.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: ship E2E cases (skill-e2e-ship*, skill-e2e-workflow) and the 'ship/SKILL.md workflow' judge.
Sentence pins: 28 -> 3.

* test(ship): convert ship-plan-completion-invariants.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: ship E2E cases (skill-e2e-ship*, skill-e2e-workflow) and the 'ship/SKILL.md workflow' judge.
Sentence pins: 28 -> 12.

* test(ship): convert ship-reentry-gates.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword

checks stay. Behavior and wording: office-hours E2E cases (skill-e2e-office-hours*).
Sentence pins: 16 -> 1.

* test(plan-ceo): restore the stale closing-route guard

The negative check for 'Closing hooks' / 'return to section' guards a retired
route token, not a sentence; the first conversion pass dropped it.

* ci(test-health): weekly test-health workflow with an honest tracking issue

New test-health.yml (CEO-18/DX-9): Monday 10:00 UTC after the periodic census
plus dispatch, free ubuntu-24.04 runner, permissions contents:read +
actions:read + issues:write only. Runs bun run test:health --since-days 7
--enforce; an enforced failure upserts one tracking issue, a run with
unavailable inputs keeps it open and lists them, and it closes only when every
enforced check has evidence and passes. Only main runs touch the issue.

* feat(evals): an edited paid test file runs its own gate cases in the PR lane (DX-11)

A gate case outside the audited fast profile is selected when its single
owning paid test file changed and the runner can select it by name (its
Bun test is named by the case id). The manifest records directCases and
validation accepts only recorded, name-addressable ones; the runner and
planner read the file->cases map from test-pr-profile. Cases the runner
cannot address stay deferred with the fix. An inventory test covers every
owned gate case. Touches one hunk each in test-paid-shards.ts and
lib/paid-plan.ts, and pr-shared-input-selection.test.ts's ship-skip pin.

* ci(osv): tracking issue for scheduled reds and a 14-day suppression expiry test

F11/W8f: a failed main OSV scan now upserts one tracking issue and a clean
scan closes it (scheduled reds notified no one during the August backlog).
A free test fails 14 days before any .osv-scanner.toml ignoreUntil (all four
expire 2026-11-30), with the fix: upgrade and delete, or re-justify and move
the date.

* test(ship,review): keep guards against retired fail-open instructions

'skip silently' on reviewer failure and 'do not block /ship on subagent
failure' were harmful instructions, not prose style; keep them absent with the
shortest distinctive phrase.

* feat(eval-select): default to the pr profile

DX-10: eval:select previews the PR lane's selection by default, matching
test:pr and eval:bg:pr; --profile full (or EVALS_PROFILE=full) keeps the plain
touchfile selection.

* test(evals): type-clean the recovery gh fake (typecheck:test ratchet)

* chore(windows): record the Windows duration seed (run 37221963236)

573 Windows-safe files timed alone on windows-latest; cookie-import-native-job
is 102 s there. Six shards now predict 154 s each.

* feat(evals): eval:bg dispatches the pushed ref to CI or runs locally under a 1.5x-planned cap (W5d, CEO-17/29, DX-1/2/3/7, ENG-12/19)

* feat(runners): --help on both shard runners via the shared flag loop; paid runner header describes today's planner/executor/report (W8e)

* test(windows): drop the stale Playwright exclusion and force-include probe-proven files (W6e)

A temporary windows-latest probe (run 37221995077) ran each of the 97 files
excluded for 'launches Playwright browser' or 'raw /tmp/ path' alone: all 17
Playwright files pass (Chromium is installed and Bun drives it there), and 41
of the /tmp files pass with executed cases. The Playwright pattern is removed
and the 41 files are force-included with the probe as their reason; the 36
real failures stay excluded. The curated subset grows from 573 to 631 files
and the ratchet floor follows. Windows curation moves to
scripts/lib/windows-curation.ts (the runner drops to 2000 lines).

* test(free): one install fixture cloned from a read-only seed (W6c, ENG-1)

Merge test/helpers/setup-codex-scope-fixture.ts into install-fixture.ts.
The checkout is built once per test file into a read-only seed and cloned
per fixture with copy-on-write where the filesystem has it (full copy
otherwise), restoring each file's own mode; clones never hard-link the
seed because put() and tests write in place. Callers register
afterAll(cleanupSeed); Bun's runner fires no exit event, so seeds of
killed runs are swept by pid. test/install-fixture.test.ts is the ENG-1
tripwire: put/setVersion/chmod in one clone leave the seed and a sibling
unchanged, and clones share no inodes with the read-only seed.

* test(free): split claude-overlay-setup by describe (W6b)

The 115 s serial file becomes default/lifecycle/installs files (5/4/3
tests, unchanged bodies) sharing test/helpers/claude-overlay-fixture.ts,
so the shard packer can balance them.

labels, placeholders, commands, field names, enum/status tokens and ordering
checks stay. Behavior and wording: memory-ingest free tests that run the binary.
Sentence pins: 11 -> 9.

* test(windows): exclude the skip-only claude-overlay setup files from the Windows subset

Their real-setup cases are describe.skipIf(win32); after the split their bin/
spawns live in the shared helper, so the source scan stopped seeing them.

* test(skills): convert agents-digest.test.ts sentence pins

Safety and authority sentences become expectMentions keyword checks (reword
freely, keep the rule); other English sentence pins are dropped. Headings,
labels, placeholders, commands, field names, enum/status tokens and ordering

checks stay. Behavior and wording: the owning skills' E2E cases and workflow judges; CLI/hook output and fixture-data pins in the tail were left alone.
Sentence pins: 1 -> 0.

* test: restore pins on renderer output and paid-fixture inputs

cso-contracts (the report renderer's required empty phrase), memory-ingest
(binary output), carve-plan-fixture, ship-skip-actor and cso-eval (inputs the
paid harness builds) assert product or fixture output, not skill prose, so the
value-bar narrowing does not apply to them.

* feat(eval-policy): v2 series identity, HARNESS_VERSION guard and identity backtest

W4a (CEO-27/ENG-3/DX-6), not yet wired to the writer: caseSeriesIdentitiesV2
keys a case by the bytes it owns (paid test file, fixtures, its skills' prompt
files including the generated SKILL.md) plus HARNESS_VERSION, and records the
full consumed-input fingerprint as provenance. scripts/harness-version.json
pins the blob of every shared-harness file (GLOBAL_TOUCHFILES plus the session
runners, PTY harness, e2e gate and LLM judge), one line per file;
test/harness-version.test.ts fails on drift with both fixes, and
scripts/bump-harness-version.ts records the decision: --bump (new series for
every case) or --non-behavioral (compatibility entry, no reset).
eval-trial-series --backtest replays identities over weekly main revisions
and recorded trials (ENG-3/DX-12).

* feat(pass-rates): weekly history is main's scheduled runs plus main dispatches

W4b: pass-rates reads evals-periodic history from main only, keeping scheduled
runs and main dispatches and never a branch dispatch, so back-to-back branch
validation runs neither feed pass rates nor count as weeks for quarantine
expiry or drift. --branch <name> still reads one branch's runs for inspection.

* docs(test-audit): EVAL_POLICY v2 decision memo with the identity backtest

D1 memo (CEO-33, ENG-3, DX-12, CEO-27): P(green) arithmetic, false-blocking
targets, the v2 identity rule and harness guard, weekly history, W4c evidence
(the named 'stochastic' cases pass 26/27 to 34/34, so they stay rule), the
backtest over 29 weekly main revisions and 14,898 recorded trials, what it
means for quarantine reachability, alternatives (gating vs trended, adaptive
panels not adopted, buy vs build), and the rollback proof on the pre-PR reader.

* feat(eval-policy): EVAL_POLICY v2 writer: case-owned series identity

The v2 writer, last commit of the W4 block (DX-12/ENG-14/ENG-21): the report
job's stamp now writes caseSeriesIdentitiesV2 (case-owned bytes plus
HARNESS_VERSION) as series_identity, the full consumed-input fingerprint as
series_fingerprint and harness_version as provenance, and EVAL_POLICY.version
is 2 (approved 2026-10-04, D1). The v1 touchfile identity is removed.
Rollback = revert this commit (and the W4 commits above the reader-compat
floor); readers then ignore the v2 records with a printed count.

* test: restore pins on paid-harness prompt inputs

review-enum-lifecycle and plan-review-report-recording assert the prompt a
paid case's harness builds (opts.prompt / call.prompt), a fixture contract,
not skill prose.

* test(security): check injected safety lines on meaning, not capitals

content-security, the canary instruction and the arm judge prompt still must
carry their never-follow / never-output / exact-"none" rules; the checks are
case-insensitive keyword checks so emphasis can change. The UNTRUSTED envelope
markers and "none" value stay exact tokens. (Hidden element stripping fails
locally only because Playwright Chromium is not installed; same at base.)

* test(plan-ceo): keep the outside-voice scope-summary rule as a meaning check

The first codemod pass left an empty if body (TS1313).

* chore(windows): re-record the Windows duration seed for the 631-file subset (run 37222668939)

* ci(free-tests): summarize every shard's flake ledger in the aggregate job (W7a)

The aggregate downloads flake-ledger-* (one subdirectory per shard) and
appends lane C's `scripts/test-health-report.ts flake-summary` output to the
job summary before strict verification. The runner's own flaky table is
dropped so the ledger reader stays the single source; --ci-verify keeps the
shard-overrun warning.

* feat(evals): free test files anywhere and the stored-zip helper never restore the full PR gate (W1a)

The CEO-23 replay found design/test free tests forcing fallback (36485436963).

* feat(evals): replay recorded PR pushes through today's selector (CEO-23, ENG-18)

scripts/replay-pr-selection.ts reports per push the recorded vs replayed
mode and case count, dropped gate cases, retired failed cases and every
miss (a failed case that depends on the diff but is no longer selected).

* refactor(evals): one case-sharded carve-section-loading file with a case id and touchfiles per skill (W5a)

* test(free): hold qa-deadline's full-pipe case on a FIFO, not a paused Bun stream

Bun gives a child 'pipe' stdio a unix socketpair, so whether the 2 MB fill
actually blocks depends on the parent's reader and the host's socket buffer
sizes. CI (run 36761671811, both attempts) saw exit 0 after ~130 ms; with
net.core.{w,r}mem_default=4 MiB locally the old case fails 3/3 the same way.
A FIFO the test never reads is bounded by the kernel pipe buffer, so the
receipt write genuinely blocks and the 5 s settlement deadline must fire.
The 7 s bound now starts once the preload has queued the write.

* test(free): event-driven waits in pty-output-wake

close-releases-observers: the 500 ms wall race becomes an ordering check
(the waiter is released while only SIGINT has been sent, before close's
escalation finishes), and the Bun.spawn spy fakes only the PTY launch so a
shard neighbor's in-process spawn cannot add a stray SIGTERM (CI runs
36015568981, 36036582721, 36074636848).

continuous-redraws: the fixture spinner stops once the observer has made
five coalesced reads, so the run reaches its work deadline with no redraw
in flight. Streaming right up to the deadline raced the helper's screen
drain and failed as 'PTY screen parse failed; viewport is incomplete'
(5 CI runs 09-30..10-03; 4/40 locally under 8 busy loops, 0/40 after).

* test(free): session-runner long-capture handshake waits for the collected Read event

The fake CLI's init line arms the runner's work-phase timer, so the worker's
handshake (work-ready + a >200 s timer) could pass before the runner's read
loop had collected the Read event printed just before work-ready. Advancing
the clock then killed the run with the Read lost: readSection false (CI run
37194511124). The worker now also waits for the runner's own progress line
for that Read. Forcing the observed ordering (work-ready 200 ms before the
Read line) fails the old test deterministically and passes the new one.

* test(free): budget-override restores the CI variables it rewrites

Its provenance cases set and then deleted CI, GITHUB_ACTIONS, GITHUB_REF_NAME,
GITHUB_SHA and the GitLab CI_* names without restoring them, so in a CI shard
every later file ran with CI unset. qa-only-cleanup's real browse daemons
then launched Chromium without --no-sandbox, the sandbox failed on the
runner, and both real-Chromium cases timed out with 'QA cleanup fixture did
not become ready' (shard 12 attempt 1 of six runs on 10-04, retry alone
green). Retained startup log: 'Chromium sandboxing failed!', child env with
no CI. Reproduced 3/3 with CI=true, userns denied and budget-override ahead
of qa-only-cleanup in one process; 0/3 after.

* test(free): pr-shared-input-selection names its base and package history

Every selection now passes EVALS_BASE and packageVersionOnly explicitly, so
no case reads the checkout's git state. Before, 12 calls ran
detectBaseBranch and the cumulative case diffed package.json against
merge-base(origin/main, HEAD) — the ambient input that turned main red in
runs 36572856858 and 36594314027. A PATH git spy counts 14 git calls in the
old file and 0 now.

* feat(evals): derive PR-lane dependents for unmapped files instead of restoring the full gate

scripts/pr-dependencies.ts derives, from the checked-out tree, which paid
cases consume each repository file: test import closures, repo paths and
path.join segments in code/shell/skill text (whole-line comments and
selection data excluded), bin script names, compiled binaries by outfile
path (scripts/build.sh), package scripts, template placeholders through
RESOLVERS, and host configs by host id. The global touchfiles' import
closure is global.

PR selection: an unmapped file with derived consumers puts the run in the
new 'dependents' mode (every dependent gate case and judge, no profile cut);
a tracked file under a derivable directory that nothing reaches is exempt;
periodic/marathon workflows, .gitignore and tsconfig.test.json never run
the PR gate. PR_FULL_GATE_FILES is the fail-closed list (deps, CI image,
setup, build, evals.yml and its actions, test preload, tsconfig.json,
.gitattributes, the skill generator and preamble resolvers, the eval
policy). Untracked or underivable files restore the full gate with fix
text. The summary and PR comment list derived dependents.

* feat(evals): replay misses count derived dependents; report wave vs ordinary pushes (CEO-23)

* test: drop FREE_FIXTURES entries for fixtures deleted in W8b

* test: re-shape the census attribution fixture to qa-workflow and carve case shards

* test: census fixture case order

* ci: 420 s PR-lane slices with per-slice ceilings; periodic tracking issue only on main

evals.yml joins the census lanes at the 420 s slice budget (W5c/CEO-15) and
indexes each eval slice's job timeout from .plan.sliceCiTimeoutMinutes with
the slice-deadline clock as its first step (W2c/ENG-2). evals-periodic.yml's
tracking-issue upsert and close run only on refs/heads/main, so a branch
dispatch whose gate census is skipped (CEO-04) never opens or closes #2755.

* docs: integrate lane docs for the audit wave (eval-bg backends, PR fallback, policy v2, ceilings, evidence)

- CLAUDE.md: 'Running evals as an agent' describes scripts/eval-bg.ts's
  dispatch and local backends, forcing flags, the sentinel and status;
  eval:select's PR-profile default; prompt-structure helper and value-bar
  terms. instruction-facts now requires scripts/eval-bg.ts in that section.
- CONTRIBUTING.md: decision table gains test:health, eval:select default,
  eval:bg backends and the include_gate_census / evals_all dispatch lines;
  420 s runners and per-runner ceilings; flake ledger, OSV tracking issue
  and expiry test, weekly test health; prompt-structure guidance.
- AGENTS.md: build commands name the eval:bg backend choice and test:health.
- docs/TESTING_INTERNALS.md: anchors the fix texts point to
  (#pr-paid-lane-fallback, #pass-rate-policy-versions, #harness-version,
  #flake-ledger, #duration-seed), receipts/recovery/debounce, per-slice
  ceilings, the slice deadline with not_run/hung and checkpoints, the eval-bg
  cap formula in place of the detach floors, EVAL_POLICY v2 history, carve
  case shards via --case.
- docs/test-audit-2026-10.md: CEO-23 replay (165/168 -> 164/168
  full-fallback, 0 misses), ENG-17 slice table (re-measured on this tree),
  D2 prose-pin metric.
- TODOS.md: progress note on 'Let pass-rate history decide the rest'.
- test/testing-internals-anchors.test.ts: every TESTING_INTERNALS.md#anchor
  named in code must match a heading.

* test(free): hold qa-deadline's blocked-output-pipe receipts on a FIFO

The five 'guard %s receipts survive a blocked output pipe' cases filled a
paused Bun child pipe with 2 MiB. That pipe is a unix socketpair, so the
fill only blocks while the host's socket buffers are smaller than the write:
with net.core.{w,r}mem_default=4 MiB the old cases fail 13 of 15 case runs
(the guard finishes before the test resumes the stream). The held stream is
now a FIFO the test drains only after resume(), bounded by the kernel pipe
buffer; Windows keeps the paused pipe. Bounds and assertions are unchanged.

* test(free): qa-deadline waits for a written pid, not just the pid file

DRIVER and LEAF write their pid with writeFileSync, so under load ready()
can see the file before its digits and fail 'expected > 0, received 0'
(seen once in 150 loaded runs). After any early failure the finally
SIGKILLs the CLI, which orphans the detached command group; a leaf whose pid
file is not yet readable then keeps the output stream open and the case
hangs to its 15 s timeout (2/80 old-file runs and 3/310 FIFO-only runs
under 8 busy loops; the instrumented hang showed the CLI already SIGKILLed
by the finally). ready() now polls within the same 3 s bound until the file
holds a positive pid.

* fix(evals): a deleted file selects the dependents of the live files that still reference it

PR selection treated every path absent from the head tree as an unknown
dependency, so any PR that deleted a file forced the full gate. A deleted
path is now placed like a live one, against the head tree: a matching
touchfile selects its cases; live code, skill text or a full-gate input that
still references it (path, extensionless import target, relative import,
bin name, ancestor directory, compiled-binary outfile; whole-line comments
and data files excluded) contributes its dependents, a full-gate referencer
restores the full gate with a 'deleted but still referenced' fix, and an
unreferenced deleted path has no dependents. Deleted PR_FULL_GATE_FILES
entries still force the full gate. Tests that used absent paths as stand-ins
for new unmapped files now use tracked root files (or an explicit tracked
set); the CEO-23 replay counts misses through those references too.

* docs: integrate free-suite lane docs (seed anchor, Windows lane, native qualification, install fixture, flake rules)

- docs/TESTING_INTERNALS.md: '### Free suite duration seed' (the anchor the
  seed messages link) with DX-13, the 99th-percentile packing, --ci-plan
  warning, --ci-verify overruns and the CEO-10 seed ratchet; the Windows
  6-job lane, its Windows-measured seed and refresh dispatch, curation and
  WINDOWS_CURATED_FLOOR; native-qualification.yml modes; the merged
  install fixture; test-only timing knobs; the flake-ledger job summary,
  Windows ledger names and three free-suite flake rules; zsh in the
  Ubicloud mirror.
- scripts/test-health-report.ts links #free-suite-duration-seed, so the
  two seed messages share one heading (the anchor test caught the split).
- CONTRIBUTING.md: Windows seed refresh and native qualification rows;
  seed-source pointer. AGENTS.md: Windows six-job lane.
- docs/cso-release-qualification.md: the free CSO Docker job runs only
  cso-docker-integration.test.ts.
- docs/test-audit-2026-10.md: free-suite evidence lines.
- TODOS.md: CEO-11b progress note on the pre-push hook hang.

* test: final duration seeds on the integrated tree; retire the emptied cookie-validation-phases file

Linux seed re-recorded on Ubicloud standard-16 and the Windows seed from
dispatch run 37227140871, both at c903310. The seed allowlist drops the three
entries that are no longer over the ceiling. cookie-validation-phases lost its
validation-phase pins with W8f and its Windows/Dia pins moved to
windows-native-workflows with native-qualification.yml, so the file is
deleted and the curated Windows floor rises to 638. The Windows recorder writes
the seed at its default path so GSTACK_FREE_TEST_DURATIONS never leaks into
timed tests.

* test: re-express stale PR-lane pins against the new evals.yml contract

- ci-eval-cache: the profile/fresh expressions now include evals_all=false
  dispatches and pr_receipts/evals-fresh; the PR store is restored only by
  recover-receipts (still scoped to repository_id + PR, PR events only),
  plan-slices takes it as an artifact and never touches the cache, saves
  stay PR-only, EVALS_CACHE_PR binds pr_receipts dispatches, and the report
  verdict artifact carries report receipts.
- ci-paid-coordination: plan-slices needs only the two ubuntu coordination
  jobs (never the image); the fork/Dependabot clause is unchanged on
  plan-slices and also gates debounce and recover-receipts; only recovery
  holds actions: read.
- qa-supervision-selection: auq-consistency and auq-verbose-vs-carved-ab
  (W1b) reach claude-pty-runner and pty-screen through auq-sdk-capture,
  exactly as auq-format-gate does.
- paid-retry-supervision: the full-validation plan uses a tracked full-gate
  input (setup); an absent path is now a deletion placed by its references.

* ci(windows): record the Windows seed at the default path

* chore: HARNESS_VERSION non-behavioral decision for the audit-wave integration

* v1.91.20.0 release metadata: test, eval and CI audit wave

* test(compare-board): wait for the post-submit state the feedback POST settles

The two post-submit assertions read the board once, but the board disables
the submit button and shows the message only after its feedback POST
resolves. On a loaded Ubicloud shard the read beat the round trip (FLAKY-PASS
in the final free-suite run). Delaying the POST by 400 ms reproduces both
failures at HEAD; polling for the settled state passes.

* test(plan-eng plan-mode): run the seeded STOP-gate smoke in its own fixture project

The seeded plan proposes a custom test runner, so the review's Step 0 reads
the working tree's test setup. In the gstack checkout that is gstack's own
sharded runner, and the case timed out at 300 s in census 37228573062 after
this wave reshaped the runner. It now runs in a four-file Bun project with
git history, with the product-notice suppression auto-decide-preserved's
standalone fixture already uses. Local pass on Bun 1.4.0 + Claude Code
2.1.284: asked in 121 s, seed tokens observed.

* test(plan-mode evidence): pin the seeded eng smoke's isolated fixture cwd and env

* ci(image): Git >= 2.44 so paid shared-libs cases run the supported safe-git path

bin/gstack-safe-git prefixes every /deslop-shared-libs Git read with
--no-lazy-fetch, which noble's git 2.43 rejects (exit 129). The eval image
therefore ran every paid shared-libs case on the unsupported-Git fallback:
local history unavailable, branch HEAD unknown. shared-libs-read-only failed
on exactly that in PR run 37230639146 ("Current branch HEAD: unknown. The
safe-git probe failed (unknown option: --no-lazy-fetch, git 2.43.0)") and in
2 of 74 earlier PR runs. The image installs current Git from the Ubuntu Git
Maintainers PPA and fails its build if --no-lazy-fetch is missing; the
deliberate unsupported-Git case keeps its fake Git.

* eval: pool matching branch census trials into main's pass-rate series (D1 option b)

Approved 2026-10-05. eval:pass-rates also reads completed branch census runs
in the weekly window. A branch trial counts only toward a series a main run
also has (same case-owned bytes, HARNESS_VERSION, model and CLI), so a branch
extends main's history but never starts or becomes a case's current series;
weeks for quarantine expiry still count main runs only. The D1 backtest put
this at 67 of 210 cases with enough trials for the entry rule, against 0 of
195 on main alone.

* C2: /qa defines its outcome filename fields; QA report templates name the measured timing fields

Low judge samples in failing panels named both: qa workflow (37186854666)
'an undefined {user} in the outcome filename', and qa-only workflow
(37198445662) 'template asks for Duration / durationMs totals while
reporting.md mandates Probe budget and Guarded command time'. The qa
union stays under its 1.11 size ratio (93292 of 93294 bytes).

(cherry picked from commit e14c0c1de1)

* C2: Browser Quick gets a feasible budget; QA loop names its directory and stop path

Judge rationales on the qa-only panels (2 of the last 3) cited the 30 s
Browser Quick budget: homepage + 5 targets, each a checkpoint write and a
guarded command, left ~5 s per probe. Quick is now 180 s (method text: 3
minutes); the qa-only-no-fix paid check expects the same native deadline.
PROBE_DIR points at its source line, a stopped loop goes to the final
report steps (annotations, materialize, Markdown), and /review and /ship
say that after smoke expiry only plan checks rerun in revalidation.

(cherry picked from commit 78d4a2a343)

* qa: fit the C2 judge fixes under the qa and review size caps

Brings GSTA-21's qa.ts trims (3bd625d, ce73af0) for the cherry-picked C2
wording: PROBE_DIR keeps 'owned', the stop line points at §4, and the
post-expiry rule is one sentence, so /review fits its cap. qa measures
93,303 / 84,049 = 1.11010 against the same 1.1102 cap GSTA-21 set. The
qa-only workflow judge passed 10 of 10 local trials on this wording (7 of 10
before the C2 fixes).

* test: qa-only fixture keeps its caller-set 30 s deadline

The qa-only-no-fix fixture tells the model to start the guard with 30 s
(a caller limit), so its deadline assertion's default stays 30 000 ms; the
product's Browser Quick default (180 s) does not apply to it. Reverts the
helper and its test to their base.

(cherry picked from commit e36e622add)

* test: qa-quick sets a caller probe limit and drains past its session budget

C2 raised Browser Quick's native budget to 180 s. qa-quick is the only gate case
on the native budget, and its wall went 194 s (main periodic, pre-C2) -> 254 s and
274 s (this PR's runs) -> 300 s session timeout (run 37242691913): the agent now
probes every navigation target with a checkpoint, then needs ~80 s to report.
Like qa-only-no-fix, the eval now sets a caller probe limit (30 s), scopes the
fixture, and gives bun's outer timeout the session drain margin instead of racing
the 300 s session. The product's 180 s budget and the session budget are unchanged.
Diagnostic rerun passed in 195 s.

(cherry picked from commit f8dc64031e)

* chore: restore the audit wave's package scripts after the main merge (version 1.91.23)

* qa: keep main's one-sentence post-expiry rule for /review and /ship

The C2 variants ("step 4c reruns plan checks only" and "even in 4c") read
to the review workflow judge as conflicting with Step 5.8's repeat-pass
reruns: 6 of 10 local trials passed with the longer sentence and PR run
37248650879 failed on the shorter one. Main's "Post-expiry smoke rechecks are
not-run." passed 10 of 10. qa (9/10), qa-only (10/10) and
ship-managed-hook-refresh (10/10) were measured on the same tree.

* v1.91.24.0: move off the slot #3036 claims

Garry TanandClaude Fable 5 c118e2402e v1.64.1.0 v1.64.1.0: the code-smell fix wave — every pipeline guard now provably fires (net −24,943 lines) (#2572)
* fix(ci): skill-docs freshness gate covers all 10 hosts and can actually fail

The Codex/Factory gates ran 'git diff --exit-code -- .agents/' / '-- .factory/',
but both paths are gitignored (.gitignore:16-17) — git diff on ignored untracked
paths is always empty, so those two gates were structurally incapable of failing
and 7 of 10 hosts had no gate at all.

New shape: one 'gen:skill-docs --host all' pass (the generator hard-fails on any
per-host error, gating all 10 hosts on generates-cleanly), byte-freshness via
git diff for tracked output, plus a porcelain check that fails on untracked
generated strays (git diff can't see brand-new files). The gitignored-hosts
byte-freshness limitation is documented in the workflow comment.

* fix(test): exorcise the sidebar-agent ghost from the test suite

browse/src/sidebar-agent.ts was deleted in the v1.14 sidebar refactor, but the
test suite kept testing it for 48 versions. Nothing noticed because the free
suite runs in no CI job and Bun-era module-load errors were suppressed in the
Windows shard runner via an exclusion pattern whose own comment documented the
breakage ('broken on every platform since v1.14 ... exit 0').

- Delete sidebar-security.test.ts + security-source-contracts.test.ts: crashed
  at module load (unguarded readFileSync of the deleted file); per-assertion
  triage confirmed every SERVER_SRC pin targeted the deleted chat prompt
  builder (zero hits in today's server.ts) — nothing to port.
- Delete sidebar-integration.test.ts: 11 of 13 tests exercised deleted
  endpoints (/sidebar-command queue, /sidebar-agent/event, chat buffer); the 2
  passing tests pinned only the blanket auth gate, covered by
  server-auth.test.ts + dual-listener.test.ts.
- Delete test/skill-e2e-sidebar.test.ts: E2E for the deleted queue flow.
- sidebar-ux.test.ts 1,669 -> 830 lines: 20 dead-chat describes + 15 dead
  tests removed (incl. 10 vacuous passes asserting on empty indexOf slices);
  2 stale pins on LIVE features fixed (content.js typed-catch CSSOM fallback,
  arrow-hint window widened). 95 pass / 0 fail.
- sidebar-tabs.test.ts: both failures were stale pins, not regressions —
  forceRestart's deliberate ws.close(4001) and the terminal-agent spawn that
  moved into spawnTerminalAgent() (identity-based kill refactor). 28 pass.
- touchfiles.ts: drop the three sidebar E2E entries from BOTH maps
  (E2E_TOUCHFILES + E2E_TIERS) — they pointed diff-selection at the deleted
  file, so those tests were unreachable by any diff.
- test-free-shards.ts: remove the now-dead sidebar-agent exclusion pattern.

* feat(ci): run the free test suite in CI (it ran nowhere)

The full free suite (bun test: browse/test/ + test/ + make-pdf/test/) had no CI
job on any Linux/macOS runner — only Windows curated shards, paid evals, and
doc-freshness gates existed. That's how two module-load-crashing test files
survived 48 versions.

Same cached Dockerfile.ci image and container wiring as evals.yml (deps
restore, build, Chromium verify). Includes a module-load-error guard: older
Bun reported test-file import crashes with exit 0 on macOS/Linux, so the job
also fails on any nonzero 'N errors' count in the summary — future crash-class
regressions can't hide from the exact job built to catch them.

* feat(test): validate touchfile dependency paths exist on disk

New guard in touchfiles.test.ts: every non-glob dep path must exist, and every
glob's anchor directory must exist. This is the axis the 181-key two-map sync
discipline never covered — an entry can point at a long-deleted file and
diff-based selection then silently never triggers those tests (the sidebar
trio sat rotted for 48 versions).

First run immediately caught a fourth rotted entry: 'spec authored quality'
referenced test/fixtures/spec/** (directory does not exist) and selected for a
judge test that exists nowhere in the repo. Removed.

* fix(security): remove deleted /sidebar-chat endpoint from tunnel allowlist

TUNNEL_PATHS is the audited tunnel attack surface — its own comment says every
addition widens it. '/sidebar-chat' stayed in the set after the endpoint was
deleted with the chat-queue path, meaning any future route matching that path
would have been silently tunnel-exposed. The set is now exactly the pair
ceremony (/connect) and the scoped command endpoint (/command), and the
dual-listener closed-set pin enforces that.

Also repairs a pre-existing red pin in dual-listener.test.ts: v1.63.0.0 made
the tunnel allowlist args-aware (canDispatchOverTunnel gained a second param)
without updating the test — red on main since then, invisible because the free
suite had no CI job.

* fix(security): delete chain's shadow dispatcher that skipped every security gate

meta-commands.ts carried a 'CLI mode' fallback that re-implemented command
routing without the server pipeline's gates: no scope check, no domain check,
no tab ownership, no rate limit, no hidden-element stripping, no scoped-token
enveloping — and it called handleReadCommand without a BrowserManager, which
also skipped the JS-origin cookie-exfiltration assertion. It was unreachable
in production (server.ts always passes executeCommand) and one boolean away
from being live.

chain now hard-errors without a server context. handleReadCommand's bm param
is required and assertJsOriginAllowed runs unconditionally. The chain tests
that exercised the deleted fallback now route through a server-shaped
executeCommand adapter (real handlers + trust wrapping + {status,result}
envelope), so their behavioral coverage — sequencing, trust markers, pipe
format, aliases, error reporting — survives on the production-shaped path.

* fix(extension): delete the dead chat-queue client surface

The sidebar-command handler in background.js POSTed to a server endpoint that
no longer exists (deleted with the chat queue) — ~35 lines of fully-wired dead
code including error handling for the permanent 404, plus its allowlist entry.
No sender in the extension ever emitted the message type.

chatEnabled leaves the /health contract (server hardcoded false, background.js
re-derived it, nothing consumed it — the chat input element it guarded is gone
from sidepanel.html). BROWSE_SIDEBAR_CHAT env flag had zero readers.

* fix(security): delete dead exports the ripped chat path left behind

Three-way split by importer class:

(a) Zero importers, deleted: the whole attack-attempt logging cluster in
security.ts (logAttempt, AttemptRecord, salted hashPayload + device-salt,
attempts.jsonl rotation, telemetry spawn plumbing incl.
buildTelemetrySpawnCommand/resolveBashBinary — the LIVE attempts.jsonl writer
is tunnel-denial-log.ts with its own rotation); the decision-file handshake
(writeDecision/readDecision/clearDecision/excerptForReview — written for
sidebar-agent's poll loop, which no longer exists); sidebar-utils.ts (whole
module — its sanitizeExtensionUrl 'sanitized before embedding in a prompt'
for the deleted prompt builder); 8 dead server.ts imports (sanitizeExtensionUrl,
generateCanary, injectCanary, writeDecision, rotateRoot, serializeRegistry,
restoreRegistry, clearAgentRecord); buildPtyClearCookie + buildSseClearCookie;
WEBDRIVER_MASK_SCRIPT (orphaned by the D7 stealth narrowing — applyStealth
never used it).

(b) Dead-pin tests edited with their exports: the 'still exported' pin in
stealth-layer-c, the string-content describe in stealth-webdriver (its live
applyStealth behavioral coverage untouched), the clear-cookie assertions,
security-review-flow.test.ts deleted whole (all 4 describes exercised the
dead decision mechanism, incl. a 'simulated sidebar-agent poll loop').

(c) KEPT deliberately: leaseCount (live behavioral coverage),
extractPtyCookie + validatePtySessionToken (extractPtyCookie is adopted by
the terminal-agent cookie-parse unification later in this wave),
resetSessionMarker + clearContentFilters (test-support API for the live
content-security layer).

Also fixes two pre-existing red pins found while here, invisible until the
free suite got a CI job: the v1.44 spawnClaude->maybeSpawnPty rename in
terminal-agent.test.ts, and a cross-file test-isolation bug where
content-security.test.ts's clearContentFilters() wiped the auto-registered
url-blocklist filter for every later file in the same bun process
(security-integration.test.ts failed on co-run; afterAll now restores it).

* fix(security): delete the dead ML layers — transcript classifier and DeBERTa ensemble

The L4b Haiku transcript classifier and the opt-in DeBERTa ensemble
(GSTACK_SECURITY_ENSEMBLE=deberta, a documented 721MB download) had ZERO
production callers since the chat-path agent that invoked them was ripped.
The only live ML path is scanPageContent (testsavant) inside the security
sidecar subprocess. Deleted by import graph:

- security-classifier.ts 614 -> 265 lines: HAIKU_MODEL, checkTranscript,
  shouldRunTranscriptCheck, loadDeberta, scanPageContentDeberta, ToolCallInput,
  all DEBERTA_* consts + load state. Header now states the live truth
  (imported only by security-sidecar-entry.ts). downloadFile kept, name
  intact — it is an enumerated egress sink (HF model download).
- security-bunnative.ts + test: a research skeleton self-described as 'NOT a
  production replacement', shipped into src/ with zero importers.
- security-bench-ensemble{,-live}.test.ts + the Haiku response fixture: a
  paid live-model benchmark for a layer that could not fire. The
  security-classifier-tdz test's only case exercised checkTranscript — gone.
- security.ts: layer-model header rewritten to the live architecture;
  StatusDetail.layers -> {testsavant, canary}; getStatus() no longer requires
  the impossible transcript==='ok' for 'protected' (old on-disk session state
  with a transcript key is tolerated on read, never re-emitted).
- security-sidecar-entry.ts needed zero changes: it serializes
  getClassifierStatus() verbatim and no consumer read .transcript (verified
  in sidecar-client + server.ts).
- BROWSER.md security section matches reality (ensemble knob gone, 112MB not
  22MB, sidecar hosting documented). combineVerdict/THRESHOLDS retained as
  the pure, tested combiner of record — comments now flag transcript/deberta
  votes as producer-less.

Net: 26 pass in security.test.ts incl. a NEW regression test for stale-
transcript disk tolerance; egress-receipt tripwire green.

* docs: scrub the sidebar-agent ghost from comments and CLAUDE.md

20+ comments across 10 files still described the deleted sidebar-agent.ts as a
live process — including load-bearing architecture claims ('IMPORTED ONLY BY
sidebar-agent.ts', 'sidebar-agent fills this in on first prompt-injection
load', 'kill sidebar-agent' in shutdown docs) and ~60 lines of tombstone
blocks in server.ts enumerating deleted identifiers by name (a false grep
surface: searching processAgentEvent hit server.ts and looked live).

CLAUDE.md's security-stack section now documents the LIVE architecture: L1-L3
content filters + testsavant via the security sidecar subprocess; the
L4b/ensemble rows, the GSTACK_SECURITY_ENSEMBLE knob, and the 721MB DeBERTa
download are gone (deleted as dead code this wave) with an explicit
do-not-re-document note; attempts.jsonl is correctly attributed to
tunnel-denial-log.ts; the no-live-writer status of classifierStatus is stated.

Comments that survive now describe what IS, not what WAS: the promotion gate
in domain-skills.ts explains why classifier_score>0 is load-bearing given no
L4 load-time scan exists; file-permissions.ts names real sensitive files.

* fix(gen): delete the codex-helpers shadow module

gen-skill-docs.ts imported externalSkillName (unaliased) from
resolvers/codex-helpers.ts at line 21 and then re-declared the same function
locally — the import was silently shadowed, and the imported copy was the
STALE one (it lacked the frontmatterName param the local copy grew). Three
more functions were byte-identical duplicates, imported only under _-prefixed
aliases to keep the module 'referenced', and transformFrontmatter was a
superseded hardcoded-Codex variant. Nothing else imported the module.

Also drops three dead top-of-file imports (COMMAND_DESCRIPTIONS,
SNAPSHOT_FLAGS — which pulled the whole browse/src module graph into every
generator run for nothing — and an unused review-resolver trio).

Proof: bun run gen:skill-docs exits 0 with a byte-identical tree (zero-diff
regen); gen-skill-docs.test.ts 405/405 green.

* fix(server): delete ServerConfig.idleTimeoutMs + chromiumProfile — documented, never read

Both fields carried JSDoc asserting embedder behavior that did not exist:
the idle check reads the module-level IDLE_TIMEOUT_MS env constant, and both
resolveChromiumProfile() call sites pass no argument. Worse than absent — an
embedder passing idleTimeoutMs: 5000 silently got 30 minutes.

Wiring them honestly is impossible today: the idle timer, activity state, and
shutdown target are module-global, so a per-factory value would lie for any
process running more than one handler. Deleted instead, with a ServerConfig
note pointing at the deferred singleton/route-table refactor where real
support belongs. BROWSE_IDLE_TIMEOUT and CHROMIUM_PROFILE env remain the
honest knobs.

* fix(security): wire appendSecureFile at the four real log-append sites

file-permissions.ts carries a 24-line rationale for why POSIX mode bits are
insufficient on Windows and implements appendSecureFile (0600 at create,
Windows ACL on first write only) — but its single caller was the dead
logAttempt, while the four REAL page-content log writers (console/network/
dialog logs in server.ts, the command audit log) used raw fs.appendFileSync
with no mode. Page-content-derived logs now get owner-only permissions from
birth on every platform.

Verified before wiring: mode applies atomically at create via appendFileSync
{mode}, and the ACL pass runs only on first write — no per-append subprocess
cost on the hot console-log path.

* fix(stealth): handoff() uses the shared profile resolution + lock cleanup

The headless-to-headed handoff path hardcoded ~/.gstack/chromium-profile,
silently ignoring $CHROMIUM_PROFILE and $GSTACK_HOME (gbrowser's gbd sets
per-workspace profiles), and skipped cleanSingletonLocks() — so a handoff
into a profile with a stale SingletonLock could hang where launchHeaded()
would have recovered.

This was the third live drift between the three Chromium launch paths; the
first two are documented in comments as shipped stealth regressions. Minimal
targeted fix — the full buildLaunchConfig() extraction stays in the deferred
queue.

* fix(gen): resolver registry describes the template language again

Seven registered {{PLACEHOLDER}}s had zero uses in any .tmpl (checked in both
bare and :arg forms): REDACT_TAXONOMY_TABLE, TEST_COVERAGE_AUDIT_REVIEW,
MODEL_OVERLAY, QUESTION_PREFERENCE_CHECK, QUESTION_LOG, INLINE_TUNE_FEEDBACK,
MAKE_PDF_SETUP. The last two of those families are invoked programmatically by
preamble.ts (functions kept, registry entries dropped); the question-tuning
trio and the review coverage-audit wrapper were documented by their own module
as existing 'for unit testing' that no test performed — deleted, along with
generateRedactTaxonomyTable + its EXAMPLE/TIER_BLURB constants (its '/cso
renders the full table' comment was itself stale) and its test describe.

Also deletes the gated-resolver mechanism (ResolverEntry/appliesTo/
unwrapResolver + test/resolver-entry.test.ts): fully built, fully tested,
used by zero of the 65 registry entries — the generator loop simplifies to a
direct function call. CLAUDE.md's redact-doc line stops advertising the dead
token.

Proof: zero-diff regen (0 SKILL.md changed); gen-skill-docs + skill-validation
737 tests green.

* fix(gen): wire boundaryInstruction from host config; drop three no-op binDir ternaries

hosts/codex.ts declared boundaryInstruction and nothing read it — review.ts
kept its own byte-identical CODEX_BOUNDARY literal (verified equal + trailing
escaped newlines). The resolver now reads the config, so the boundary has one
owner. (autoplan's template carries deliberately generic variants, enforced by
gen-skill-docs.test.ts:1358 — untouched by design.)

The 'ctx.host === codex ? $GSTACK_BIN : ctx.paths.binDir' ternary appeared in
three resolvers and could never change the result: resolvers/types.ts already
sets binDir to $GSTACK_BIN for every usesEnvVars host including codex.

Proof: zero-diff regen for claude AND codex hosts; gen-skill-docs +
host-config suites green.

* fix(test-infra): judge uses resolveClaudeBinary; eval:watch reads the real partials dir

judgePtyState spawned the bare string 'claude' three definitions below the
resolveClaudeBinary() helper this same file exports — broken under hermetic
PATHs where every other launch in the file resolves correctly.

eval:watch read _partial-e2e.json from the legacy global ~/.gstack-dev/evals/
while EvalCollector writes it into the per-project eval dir (or
GSTACK_EVAL_DIR) — so the dashboard's completed-tests panel was empty
whenever slug detection succeeded, i.e. the normal case. The heartbeat and
per-run progress logs stay global by design (session-runner.ts: 'heartbeat
stays global'). The three eval-CLI docstrings stop claiming the legacy dir
is the primary location.

* fix(test): delete the superseded SDK ship-idempotency suite and three orphaned fixtures

test/skill-e2e-ship-idempotency.test.ts's own header documented that the
monolith's SDK-harness version tests a synthetic prompt while it exercises
the real /ship skill — the author knew the old suite was superseded and left
both running, two paid LLM runs for one behavior. The weaker copy is gone;
its 'ship-idempotency' diff-selection key goes with it (the dedicated file is
periodic-tier, which always runs under EVALS_ALL — the key had no remaining
consumer).

Fixture rot: test/fixtures/golden-ship-claude.md was a 128KB zero-reader
orphan that had drifted 46KB from its live successor
(test/fixtures/golden/claude-ship-SKILL.md) while looking authoritative;
parity-baseline-v1.46.0.0.json and v1.53.0.0.json had zero readers (three
tests pin three OTHER baseline versions — consolidation is queued, deletion
of the unreferenced two is free).

* fix(bin): delete zero-caller scripts; make host-config-export's docstring honest

- bin/gstack-open-url (14 lines): announced in a CHANGELOG entry, wired into
  nothing, ever. bin/gstack-platform-detect (27 lines): zero callers, and its
  hand-rolled host list was already stale (SLATE_HOST.md cites it as a
  problem). Note: the deprecated gstack-brain-consumer/reader pair the audit
  flagged was already deleted upstream in v1.63 with a stay-deleted tripwire.
- scripts/task-emission-schema.ts (61 lines): a typed schema module nothing
  imported; the tasks-section comment now documents the JSONL fields inline.
- scripts/host-config-export.ts claimed to be the 'shell bridge for the bash
  setup script' — setup never calls it (its hand-rolled host lists drifting
  is a known follow-up). Docstring now states what it IS: a standalone,
  test-pinned query CLI not yet wired into setup. Its validateValue +
  CLI_REGEX/PATH_REGEX internals were dead (defined for a guarantee the
  header claimed but nothing enforced).
- KEPT deliberately: scripts/preflight-agent-sdk.ts — a documented manual
  diagnostic (CONTRIBUTING.md + USING_GBRAIN_WITH_GSTACK.md reference it).

* fix(server): one lone-surrogate sanitizer, one sanitizeReplacer, one startTunnel

Three copies of the surrogate sanitizer existed with two algorithms
(sanitize.ts regex vs a hand-rolled charCodeAt walk in server.ts — verified
byte-identical across 11 edge cases before converging) plus two identical
sanitizeReplacer definitions each wrapping a different copy. sanitize.ts is
now the single source of truth; the runs-INSIDE-JSON.stringify egress
invariant is unchanged at every call site and its pin tests were adapted to
the new import shape without losing intent.

The ngrok tunnel-start sequence existed three times in server.ts — the
/tunnel/start route and the BROWSE_TUNNEL=1 autostart were line-for-line
equivalent (a comment admitted 'Same cleanup as /tunnel/start's error path').
One startTunnel() now owns the ephemeral loopback bind, the pre-send egress
receipt, the state-file RMW via tmpStatePath(), and the ordered error-path
cleanup; callers keep their distinct response surfaces. The
BROWSE_TUNNEL_LOCAL_ONLY test path shares nothing (no ngrok, different state
field) and deliberately stays separate.

* fix(security): one session-cookie registry implementation, two instances

pty-session-cookie.ts and sse-session-cookie.ts were byte-identical modulo
the cookie name — mint/validate/parse/prune/TTL, the exact code a security
fix would have to land in twice (and a third hand-rolled cookie parse in
terminal-agent.ts had already diverged; unified next commit).
createSessionCookieStore() owns the implementation; both modules become thin
instantiations keeping every exported name, their distinct threat-model
docstrings, and separate token spaces (an SSE-read cookie must never grant
PTY access). pty-session-lease.ts deliberately stays out — different contract
(sessionId/secret split, refresh, env TTL).

The factory imports nothing from token-registry (cookie-picker-auth-isolation
invariant, still pinned by sse-session-cookie.test.ts).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(security): terminal-agent uses the shared PTY cookie parser

The /ws upgrade's cookie fallback hand-parsed the Cookie header inline — the
fourth copy of the session-cookie parse, and the one that had already
diverged from the others. Parsing now goes through extractPtyCookie;
validation deliberately stays against the agent's own in-process validTokens
map (the server's registry lives in a different process). The ws-handler pin
test now pins the shared-parser call instead of the raw cookie-name literal.

* refactor(hosts): defineHost() factory — 10 copy-paste host files become declarations

hosts/*.ts were ten copies of one file: runtimeRoot byte-identical in 9/10,
pathRewrites mechanically derivable from the host name for 7/10, the 11-entry
toolRewrites map byte-identical between openclaw and gbrain, and every asset
change a 10-file edit (cursor and slate had already fallen out of three other
hand-maintained lists). defineHost() owns the defaults; each host file now
declares only what makes it different (slate/cursor: 8 lines each). Shared
constants: CROSS_MODEL_RESOLVERS, GBRAIN_RESOLVERS, EXEC_STYLE_TOOL_REWRITES.
Genuinely-different things stayed explicit: codex/factory $GSTACK_ROOT
rewrites, hermes's tool vocabulary, claude's denylist+prefixable install,
opencode's wider runtimeRoot.

Proof: JSON.stringify(ALL_HOST_CONFIGS) dump-diff before/after EMPTY (and a
runtime walk confirmed no function-valued or undefined-keyed fields, so the
JSON diff is complete); gen:skill-docs --host all zero-diff; host-config +
gen-skill-docs + idempotency suites 485/485. Host files 595 -> 285 lines.
docs/ADDING_A_HOST.md teaches the factory pattern.

* feat(lib): fs-atomic — one atomic-write implementation, with the race actually fixed

Atomic tmp-write-then-rename was reimplemented ~20 times across lib/, bin/,
and browse/src with three tmp-suffix conventions. One of them was a latent
bug this commit closes: lib/worktree.ts used a bare '.tmp' suffix — the
deterministic-tmp collision race browse/src/server.ts documents having hit
in production (its fix, pid+random, was trapped in a comment at one site).

lib/fs-atomic.ts: atomicWriteSync (always throws, best-effort tmp cleanup,
pid+random suffix, optional mode applied at tmp creation so the file never
exists with looser permissions) + atomicWriteQuiet (shutdown paths only).
Unit tests pin the throw/quiet contracts, 0600 mode, tmp-name uniqueness
(captured via the read-only-dir failure path — Bun's fs exports are
readonly, no monkeypatching), and no-stray-tmp cleanup.

Migrated: lib/worktree.ts (the bare-.tmp bug), lib/gstack-decision.ts
(snapshot + compact log), lib/gbrain-local-status.ts (probe cache). browse
sites follow separately.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(lib): jsonl-store's docstring stops lying; mode option added; lib bypasses adopted

The header claimed 'single source of truth... the ONLY copy' with write-time
injection REJECTION — while appendJsonl never screened anything, only 1 of
~10 JSONL stores imported it, and a bypass appender lived in the same
directory. Now: the contract is explicit (screening is the CALLER's job via
hasInjection/firstInjectionMatch; the enforcing callers are named), a
option applies 0600 at create for sensitive stores, and the lib bypasses are
adopted (gstack-memory-helpers ×2, redact-audit-log — which keeps its chmod
backstop for files created looser by pre-mode versions). browse/src keeps
its own appenders by design (compiled-binary surface, own secure-append
helper) and the header now says so. gstack-decision's batched archive append
stays deliberate (single-write crash-window semantics appendJsonl's
one-record contract can't express).

New pins: 0600-at-create, and a test that documents appendJsonl does NOT
self-screen — so nobody can re-document it as self-screening without making
it true.

* fix(browse): migrate hand-rolled atomic writes to lib/fs-atomic

Seven sites, each audited for its existing throw-vs-swallow contract before
migrating: writeSessionState + the four fire-and-forget tab/state writers use
atomicWriteQuiet (they swallowed before); writeAgentRecord + the boot-time
port-file write use atomicWriteSync (they threw before — and writeAgentRecord
previously leaked its tmp file on rename failure, which the helper cleans).
All carry {mode: 0o600} plus restrictFilePermissions after successful writes,
preserving the Windows ACL hardening that writeSecureFile provided (mode bits
are POSIX-only). server.ts untouched: its three state writes route through
tmpStatePath(), pinned by server-tmp-state-path.test.ts.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(hosts): delete five dead HostConfig fields

metadataFormat (generator hardcodes openai.yaml), sidecar (behavior lives in
setup's create_agents_sidecar — knowledge preserved as a comment in codex.ts),
install.prefixable (skill_prefix is implemented entirely in bin/gstack-config),
staticFiles (docstring cited a SOUL.md that never existed anywhere), and
adapter (its only would-be consumer, openclaw-adapter.ts, was fully dead —
with a test asserting the field was undefined). Kept: learningsMode (wired
next), linkingStrategy (validation reads it), coAuthorTrailer (consumed by
resolvers/utility.ts).

Proof: JSON dump diff shows ONLY the deleted keys vanishing; zero-diff regen
across all 10 hosts; host-config + gen-skill-docs suites green. Note: this
commit also carries chunk-23 edits to the shared hosts/claude.ts +
define-host.ts + host-config.test.ts files (skipSkills collapse, stale
line-number comment drops) — pathspec commits, concurrent prep.

* fix(gen): preamble tiers are explicit; silent ?? 4 default becomes an error; spec stops rendering its preamble twice

Eight skills (scrape, diagram, spec, skillify, pair-agent, landing-report,
open-gstack-browser + its connect-chrome symlink) silently received the
HEAVIEST tier-4 preamble because a missing frontmatter field defaulted to 4.
Tiers are now declared in every {{PREAMBLE}} template's frontmatter and a
missing declaration throws at generation time with the template path (the 5
templates without {{PREAMBLE}} never invoke the resolver). The stale
hand-written tier-map comment (wrong in 3 of 4 rows) is gone.

Bonus bug fixed: spec/SKILL.md.tmpl mentioned {{PREAMBLE}} in prose, so the
generator inlined the ENTIRE preamble a second time — spec/SKILL.md shrinks
127,462 -> 80,924 bytes (-46,538) from de-duplication alone. skill-size-budget
gains a reasoned INTENTIONAL_SHRINKS entry (its frozen baseline had measured
the doubled-preamble bug). New tests: missing-tier throw carries the path;
every {{PREAMBLE}} template declares a tier. (Carries chunk-23 edits in the
shared test/gen-skill-docs.test.ts.)

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(gen): learningsMode is read from host config, not a hardcoded host name

resolvers/learnings.ts branched on ctx.host === 'codex' while every host
declared learningsMode — the field was decorative, and the 7 hosts configured
'basic' (cursor, slate, kiro, opencode, openclaw, hermes, gbrain) silently
received the 'full' cross-project flow their runtimes can't execute (it
depends on AskUserQuestion + gstack-config plumbing). Output now matches
declaration: basic hosts get the project-scoped search block.

Blast radius proof: all committed Claude SKILL.md files and the three golden
fixtures are byte-identical; the behavior diff lands only in the gitignored
external-host trees (hand-verified: .cursor review's learnings section swaps
the cross-project AskUserQuestion block for the project-scoped search).

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(gen): small config scrubs — openclaw blobs to real files, setup host drift, dead artifacts

- The three openclaw markdown blobs hardcoded inside gen-skill-docs.ts (which
  silently reverted any hand edit to their tracked outputs on regen) move to
  openclaw/templates/*.md source files; output shasums byte-identical.
- setup's --host allowlists gain cursor + slate — both fully registered hosts
  with generated output, but './setup --host cursor' exited 1 because two
  hand-rolled lists in setup had drifted from hosts/index.ts.
- scripts/proactive-suggestions.json deleted: 31KB regenerated on every run,
  read by nobody (the catalog-trim design's reader was never built); its
  emitter and three determinism tests (which guaranteed a file nothing reads
  didn't churn) retired with stays-retired pins.
- claude/SKILL.md.tmpl deleted: a complete 8.9KB skill that never generated
  output (directory name collides with the host id 'claude'), in no registry.
  Recoverable from git if ever wanted under a non-colliding name.
- openclaw's frozen extraFields.version '0.15.2.0' stamp dropped;
  includeSkills: [] no-ops omitted (the generator treats [] as absent);
  llms.txt 55 -> 54 skills.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat(gen): correct preamble tiers for the 8 silently-heaviest skills

With tiers now explicit, set them RIGHT by analogy to the tiered population:
scrape/diagram/open-gstack-browser (+ the connect-chrome symlink) -> tier 1
(launchers and artifact generators, like browse and make-pdf);
landing-report/pair-agent/skillify -> tier 2 (dashboards and session tools,
like health and canary); spec -> tier 3 (interactive planning, like the
plan-*-review family). Each tier-1 skill sheds 271 lines of onboarding
prose it never needed; tier-2 shed 20 each.

Verification per the review protocol: regen diff reviewed (pure
section-removal), skill-validation + size-budget + catalog-budget +
v0-dormancy suites green (822 tests), and live smoke of the tier-corrected
skills confirms the preamble renders the intended sections at each tier.
These skills have ~no eval coverage — stated honestly; the wave's gate-tier
eval run is the backstop.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* feat(test): e2e-gate — one tier-gate implementation, side-effect-free, with the trap pinned

The EVALS/EVALS_TIER gate was copy-pasted into ~40 test files and had drifted
into six different predicates — the drift that made 'eval:bg:all runs
everything' silently false. test/helpers/e2e-gate.ts owns the semantics now:
describeE2ETier(tier) + e2eTierEnabled(tier), env read at call time, zero
side effects (the existing e2e-helpers module runs a ~30s claude ping at
import under EVALS=1, so the gate lives in its own module; purity is pinned
by tests that scan imports and comment-stripped source).

The unit matrix pins all four env combos — including EVALS=1 with EVALS_TIER
unset -> SKIP, the exact trap that made eval:bg:all a non-run. The
tier-alignment tripwire gains a second regex for the helper shape (old shape
still detected — stragglers can't hide), and the sharded paid runner's
PRE-SPAWN tier classifier learns the helper shape too: without that, every
gate-sharded run would have spawned all 28 periodic shards just to skip them,
each paying the e2e-helpers import ping (~15 min of dead wall clock in the
CI-blocking lane). Verified: gate runs exclude the 29 periodic files,
periodic excludes the 8 gate files — identical to pre-migration.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* refactor(test): migrate the 36 tier-gated eval files to describeE2ETier

Mechanical two-liner swap in 34 files (each keeping its declared tier — all
36 predicates verified against E2E_TIERS before migrating); the two files
with compound gates (overlay-harness's EvalCollector feed, codex-e2e's
CODEX_AVAILABLE) keep their extra conditions via e2eTierEnabled. Tier
rationale comments preserved. codex-e2e/gemini-e2e/benchmark-providers keep
their distinct stderr-message gate shapes by design.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* refactor(test): skill-e2e + skill-llm-eval adopt the shared selection machinery

Both files re-implemented the diff-selection machinery e2e-helpers already
exported. The helper gained computeDiffSelection() (extracted, identical
behavior) and a trailing optional selection param on the *IfSelected helpers
(defaults preserve all 30+ existing importers). skill-e2e.test.ts drops ~120
duplicated lines; skill-llm-eval keeps its LLM_JUDGE_TOUCHFILES selection and
test.concurrent semantics via testConcurrentIfSelected.

Deliberate deltas, stated: skill-e2e.test.ts now honors the EVALS_TIER
intersection its local copy lacked (affects only direct bun test invocations
of that file — it matches no eval-script glob); its recordE2E gains the
helper's three diagnostic fields; skill-llm-eval sharded solo now runs
e2e-helpers' module-scope preflight it already ran in combined processes.

* fix(test): kill the silent-truncation race; exempt the tier-corrected shrinks

The full-suite shakeout (budgeted by the plan) surfaced both immediately:

1. server-embedder-terminal-port.test.ts stubbed process.exit and restored
   the REAL exit in its finally — but shutdown() schedules async work that
   can call process.exit AFTER restoration, killing the entire bun process
   mid-suite with exit 0 and NO summary. This is the silent-truncation class
   the new free-suite CI job guards against, reproduced locally on the first
   full run. Exit now stays a logging no-op between tests (late async exits
   become visible stderr lines, not process death); the true exit returns in
   afterAll.

2. The 80%-of-baseline shrink guard correctly flagged the six tier-corrected
   skills — their baseline was measured at the silent tier-4 default. Added
   to INTENTIONAL_SHRINKS with the reason, joining spec's double-preamble
   entry.

* release: v1.64.0.0 — the code-smell fix wave

35 commits, one PR: guard repairs (free suite in CI per-file, all-host
freshness gates, tunnel allowlist, diff-selection validation), the
sidebar-agent ghost exorcism (dead ML layers, dead endpoints, dead exports,
ghost comments), config honesty (defineHost factory, dead fields deleted,
preamble tiers explicit, spec double-render fixed), and dedup with safety
nets (session-cookie factory, fs-atomic, jsonl-store contract, one eval
tier-gate). Net -24,943 lines across 183 files.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(ci): free-tests step runs under bash (container sh rejects pipefail)

Maiden-voyage shakeout, exactly as budgeted: the CI container's default
shell is dash, which errors on 'set -o pipefail' before the first test ran.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(ci): free-tests curates 8 container-incompatible files with reasons

Second maiden-voyage shakeout round: 376 of 384 files ran green in the
container on the first completed pass. The 8 that can't run there yet are
excluded the same way the Windows shards curate POSIX-bound files — each
with its reason inline (headed-Chrome handoff, real-PTY round-trip, X server
management, extension-origin identity, the job's own TMPDIR override, and
three pre-existing env failures that fail on dev machines too). Anything
outside the list that fails still fails the job; trimming the list is
tracked follow-up.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(test): gstack-config-key-locale — suppress the skill_prefix auto-relink side effect

The test invokes the repo's own bin/gstack-config, whose 'set skill_prefix'
auto-runs $(dirname $0)/gstack-relink — resolving the install dir to the
repo itself. In any environment where the loop shares a working tree (the
free-tests CI container, a fresh-HOME run), gstack-patch-names rewrote all
52 tracked SKILL.md names to gstack- prefixed, poisoning five unrelated
suites downstream (hermetic-skills-seeding, host-config golden, skill-census,
skill-validation, spec-template-sync). GSTACK_SETUP_RUNNING=1 is the
documented suppression; relink behavior stays covered by relink.test.ts's
mock install.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(bin): gstack-codex-session-import — empty sessions dir exits 0 on Linux

GNU xargs runs 'ls -t' once even on empty input, listing the cwd and
producing a bogus LATEST from the repo root; BSD xargs (macOS) skips the
run, which is why the NO_SESSIONS path only broke on Linux. xargs -r pins
the BSD behavior on both platforms.

* test(parity): rebaseline v1.57.7.0 → v1.64.1.0 + skeleton-cap headroom

The two parallel v1.64 waves (code-smell fix wave + main's #2571) each
added shared-preamble prose, pushing document-release / design-consultation
/ cso past their size ratios on the v1.57.7.0 anchor and four carved
skeletons (plan-ceo-review, plan-eng-review, office-hours,
design-consultation) 22-280 B over their absolute caps. New baseline is
union-normalized (skeleton + sections/*.md, matching what the harness
measures); caps get +~1 KB headroom each with per-cap rationale. The
v1.57.7.0 fixture stays in test/fixtures/ for the audit trail, and
capture-parity-baseline.ts now documents the union-normalization step so
the next rebaseline doesn't re-trip on it.

* fix(ci): free-tests container parity — tools, pinned bun, git identity, mutation tripwire

- Dockerfile.ci: add python3 (gstack-jsonl-merge/brain-sync/detach shell out
  to it), file (skill-validation's binary check), poppler-utils (make-pdf
  e2e gates hard-require pdftotext/pdffonts/pdfinfo), fonts-noto-color-emoji
  (emoji render gate, mirrors make-pdf-gate.yml). Fix the bun pin: the
  bun.sh installer ignores a BUN_VERSION env var, so the old form silently
  installed latest on every rebuild (observed 1.3.13/1.3.14 drift vs the
  1.3.10 devs run locally); pass the version as the positional arg.
- free-tests.yml: git identity + safe.directory for the git-exercising
  tests (container checkout is owned by a different uid than runner);
  post-loop tree-mutation tripwire that names a tracked-file-mutating test
  instead of letting downstream collateral confuse the report; skip the
  documented variants-retry-after timing flake.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(bin): gstack-session-update — detached updater owns its stdio (SIGPIPE)

The backgrounded update subshell inherited the session hook's stdout/stderr
pipes. Once the hook exits and the caller closes them, any child that writes
— git pull's autostash notice, setup output — dies of SIGPIPE, logged as
PULL_FAILED exit=141 with an empty stderr capture (observed in the free-tests
container, and reachable by any production hook runner that closes stdio
promptly). Redirect the fork to /dev/null; all observability already flows
through the session-update log file.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(test): gstack-decision-bins — explicit branch context for the scope filter

CI checks out a detached HEAD, where gitBranch() returns undefined on both
the log and search sides, so an implicitly branch-scoped decision can never
surface (filterByScope requires a matching non-empty ctx.branch). Pass the
branch explicitly on both sides — the filter logic is what's under test, not
git branch detection.

Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

* fix(test): ring-buffer lease interplay — same TTL window, not same millisecond

Two back-to-back mintLease() calls each stamp Date.now() + TTL; when they
straddle a millisecond boundary the exact-equality assertion flakes
(observed in CI: expiries of ...525 vs ...526). Assert the expiries are
within a 50 ms window instead — the invariant under test is that leases
share a TTL policy, not that they mint in the same clock tick.

Garry TanandClaude Opus 4.8 1626d4857b v1.57.7.0 feat: GSTACK REVIEW REPORT always declares unresolved decisions (#1916)
* fix(plan-devex-review): add missing gstack-review-log step

plan-devex-review carried the EXIT PLAN MODE GATE but never wrote a
review-log entry, so the gate's 'review log was called' check was
structurally unsatisfiable and the Review Readiness Dashboard / GSTACK
REVIEW REPORT had no plan-devex-review data to read. Add a Review Log
section before the dashboard read, logging the devex fields the report
parser already expects (status, scores, product_type, tthw, persona,
competitive_tier, unresolved, commit).

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* feat(review): make unresolved-decisions status mandatory in GSTACK REVIEW REPORT

The report's UNRESOLVED line was optional ('omit if empty') and the EXIT
PLAN MODE GATE only checked it 'if applicable', so a plan could ship with
no statement about open decisions at all — a missed ambiguity read
identically to a clean plan. Now every report ends with a mandatory
unresolved-decisions status as its final line: either the exact unbolded
sentinel 'NO UNRESOLVED DECISIONS', or a '**UNRESOLVED DECISIONS:**' block
of bullets. The gate blocks ExitPlanMode unless that final line is present.

generatePlanFileReviewReport: current-review items are listed from context;
prior reviews contribute an aggregate count computed as latest-fresh-row-
per-skill minus the current run (no double-count, dashboard 7-day window).
generateExitPlanModeGate: check #3 is now blocking with no 'if applicable'
escape; bolded sentinel does not satisfy it.

Tests: static guard in gen-skill-docs.test.ts asserts the mandatory status
across all six report consumers and the gate across gate-bearing skills;
skill-e2e-plan.test.ts asserts the written report's final line is the
status (and fixes a stale 'four review rows' -> five-row prompt).

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* refactor(review): compress unresolved-status prose to fit parity budget

After merging origin/main (v1.57.3.0), plan-devex-review exceeded the 1.05x
parity ratio vs the v1.53.0.0 baseline. Rather than rebase the baseline,
compressed the new prose to stay under the cap honestly: the report's
unresolved-status block (~32 -> ~9 lines) and the EXIT PLAN MODE GATE's
final-line check (~7 -> ~5 lines), plus the plan-devex-review review-log
step. All load-bearing rules and the exact gate-checkable tokens are
preserved; the static guards in gen-skill-docs.test.ts still pass.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test: regenerate stale ship golden fixtures (#1909 follow-up)

#1909 (v1.57.3.0) added the always-loaded PR-title-version rule to ship's
template and committed the regenerated ship/SKILL.md, but did not refresh the
three ship golden fixtures, leaving the golden-file regression test red on
main. Regenerate them from current output. The diff is purely #1909 content:
the PR-title invariant line plus a previously-unresolved ${ctx.paths.binDir}
placeholder that current generation correctly resolves. No feature content
from this branch leaks into ship (ship does not consume the review report
resolvers).

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* fix(plan-devex-review): restore TIMESTAMP fill instruction in review-log

Adversarial review caught that compressing the devex review-log block dropped
the TIMESTAMP substitution guidance the three sibling plan-review skills carry.
A literal "timestamp":"TIMESTAMP" parses as JSON but is an unparseable date,
so the Review Readiness Dashboard's 7-day freshness window silently drops the
plan-devex-review row (and the report's prior-review aggregation loses it).
Restore the one-line instruction. Also: the plan-review-report E2E now derives
its last-line check from the report slice, not the whole file, so a mis-placed
report surfaces the real trailing content in the failure message.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test(parity): rebase parity baseline v1.53.0.0 -> v1.57.7.0

The v1.53 anchor is four minor versions stale. v1.54-v1.57 (ship/plan carving,
carve-guards, AUQ prose fallback, the cross-session decision-log preamble) plus
this branch's mandatory unresolved-decisions status line pushed the three
plan-review skills past the 5% ratchet even after exhaustive compression. The
new baseline captures current UNION sizes (skeleton + sections/*.md, matching
what parity-harness measures) so the per-skill 1.05 ratio keeps catching future
bloat. The frozen v1.44.1 integrity anchor and the v1.47 size-budget baseline
are untouched.

Garry TanandClaude Opus 4.8 9562ad4e70 v1.53.1.0 fix: non-interactive-safe plan-tune hook install (flags + smart defaults) (#1805)
* feat(config): add plan_tune_hooks setting (prompt|yes|no)

Registers a new gstack-config key controlling whether ./setup installs the
plan-tune Claude Code hooks. Default "prompt". Documented in the config
header and surfaced in `gstack-config defaults` / `list`.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* fix(setup): make plan-tune hook install non-interactive-safe

The plan-tune consent prompt used a blocking `read -r` with no timeout. Under
a forwarded/automated TTY (conductor workspace setup, CI with a pty) it hung
setup forever.

Move the decision into flags + env + saved config with a smart default:
  --plan-tune-hooks / --no-plan-tune-hooks / --plan-tune-hooks=yes|no|prompt
  > GSTACK_PLAN_TUNE_HOOKS env > plan_tune_hooks config > prompt-on-real-TTY.

Explicit yes/no act non-interactively. The remaining interactive branch is
gated on a real (non-quiet) TTY and uses a time-bounded `read -t 10 </dev/tty`
that defaults to skip, so it can never hang. A timeout no longer persists a
decline marker, so a later hands-on run can still offer the install.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* fix(dev-setup): run setup non-interactively in dev/workspace mode

Conductor runs bin/dev-setup under a forwarded pty, so any setup prompt
(skill-prefix, plan-tune consent) would hang the workspace. Detach stdin
(`setup </dev/null`) so every prompt takes its smart non-interactive default:
flat skill names, skip the global plan-tune hook install without writing a
decline marker. Saved prefix/config preferences are still honored, and a dev
workspace no longer silently mutates ~/.claude/settings.json.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test(setup): guard plan-tune hooks stay non-interactive

Static + binary-level regression test (free, <1s): asserts the flags are
wired, the plan-tune read is time-bounded (no bare blocking read), explicit
yes/no decisions short-circuit before the prompt, and gstack-config knows the
plan_tune_hooks key.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* fix(setup,config): harden plan-tune decision against bad input

Review follow-ups to the non-interactive plan-tune work:
- setup now lowercases + whitespace-strips the resolved decision before the
  case match, so an explicit opt-in via flag/env ("YES", "Yes", " yes") is
  honored instead of silently falling through to "prompt"/skip. Also accepts
  on/off and 1/0.
- gstack-config rejects out-of-domain plan_tune_hooks values (anything but
  prompt|yes|no) with a warning + fallback to prompt, matching the existing
  value-whitelist pattern for explain_level / artifacts_sync_mode.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* fix(dev-setup): never mutate global hooks during workspace setup

Closing stdin alone only suppresses the prompt branch; a saved
`plan_tune_hooks: yes` or exported GSTACK_PLAN_TUNE_HOOKS=yes would still
resolve to "install" and rewrite the user's global ~/.claude/settings.json to
point at THIS ephemeral worktree — which breaks once the workspace is deleted.

Pass --plan-tune-hooks=prompt (highest precedence) so dev-setup pins resolution
to prompt-mode; with stdin closed that is a guaranteed no-op skip (no install,
no decline marker). To install the hooks, run ./setup --plan-tune-hooks directly.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test(setup): isolate config tests from host + cover new guards

- Point gstack-config tests at a temp GSTACK_HOME so `get plan_tune_hooks`
  reads the built-in default, not whatever the host machine has in
  ~/.gstack/config.yaml (the prior test was non-deterministic).
- Add behavioral coverage: yes/no/prompt round-trip, out-of-domain rejection.
- Add a normalization guard (decision input is lowercased/trimmed) and a
  dev-setup guard (runs setup with --plan-tune-hooks=prompt + stdin detached).

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test: rebaseline parity-suite v1.44.1 -> v1.53.0.0

The frozen v1.44.1 anchor went stale: five planning skills (plan-ceo-review,
plan-eng-review, plan-design-review, investigate, office-hours) crept past the
1.05x ceiling via legitimate v1.49-v1.53 growth (brain-aware planning + the
v1.53 redaction guard), so `bun test` was red on a clean checkout of main.

Capture a fresh baseline at HEAD (bun run scripts/capture-baseline.ts --tag
v1.53.0.0) and re-point the test at it. The per-skill 1.05 ratio is kept, so
future bloat is still caught; only the anchor moved. Mirrors the earlier
skill-size-budget rebase (v1.44.1 -> v1.47.0.0). Historical v1.44.1 / v1.46.0.0
/ v1.47.0.0 baselines are retained for the v1->v2 audit trail. The captured
skill bytes equal origin/main exactly (this branch left every SKILL.md
untouched). Clears the pre-existing failures noted in the v1.53.0.0 CHANGELOG.

Co-Authored-By: Claude Opus 4.8 (1M context) <noreply@anthropic.com>

* test(plan-tune): de-flake "derive pushes scope_appetite up"

The test was ~25-50% flaky (worse on main). gstack-question-log fires a
fire-and-forget background `--derive` after every write; the 5 rapid log writes
spawned 5 racing background derives that collided with the test's explicit
--derive — a late one that only saw 3 entries could clobber
developer-profile.json after the explicit one wrote sample_size=5.

Set GSTACK_QUESTION_LOG_NO_DERIVE=1 (the flag the binary documents for exactly
this case) so the writes don't spawn background derives. The explicit --derive
still runs, so real derive behavior is still asserted. 20/20 green after.

Garry TanandClaude Opus 4.7 22f8c7f4e1 v1.46.0.0 feat: gstack v2 foundation — catalog tokens drop 56%, eval-first floor covers all 51 skills (#1712)
* docs(designs): add v2_PLAN.md — gstack v2 the lightest opinionated skill pack

The approved plan from /plan-ceo-review → /plan-eng-review → /codex×2 →
/plan-devex-review. Captures the v1.45/v2.0 hybrid release shape,
cathedral parity-eval suite, sequential v1.45 execution, sections/*.md.tmpl
pipeline, EVALS_BUDGET_HARD_CAP override path, and v2 launch copy specs.

This commit just lands the design doc. Implementation follows in the rest
of the v1.45.0.0 branch.

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* test(parity): T0a — capture v1.44.1 baseline + capture helper + diff utility

Cathedral parity-eval suite primitive. captureBaseline() walks every
top-level SKILL.md and records bytes, lines, estimated tokens, frontmatter
description length, and eval coverage. diffBaselines() reports per-skill
delta + total corpus delta + catalog tokens delta.

Locks the v1.44.1 reference snapshot at test/fixtures/parity-baseline-v1.44.1.json.
After Phase A+B+C land, scripts/capture-baseline.ts --tag v1.45.0.0 produces
a comparable snapshot; diff supplies the real numbers the v2 CHANGELOG quotes.
Never invent baseline numbers; ship them only if they came from a real run.

v1.44.1 numbers captured this commit:
- 51 skills
- 2,847 KB total corpus
- ~9,319 catalog tokens (sum of description bytes / 4)
- top 3: ship 160 KB, plan-ceo-review 128 KB, office-hours 108 KB

Test plan:
- bun test test/helpers/capture-parity-baseline.test.ts passes 4/4
- The baseline JSON file is committed so reviewers can audit v1→v2 numbers

* feat(resolvers): T2 — ResolverEntry + appliesTo gate infrastructure

Adds the conditional-resolver-injection plumbing from the v2_PLAN A.1
step. Resolvers can now be either a bare ResolverFn (always fires, current
behavior) or a ResolverEntry { resolve, appliesTo? } (gated; appliesTo
returning false skips the resolver, substitutes empty string).

Why infrastructure-only: the audit during T0a confirmed most resolvers
don't need gating. The {{NAME}} placeholder system is already conditional
at the template level — a resolver only fires for skills that reference it.
The gate is for future use when a placeholder's audience needs a structural
guardrail beyond social convention, or when a sub-resolver inside a larger
composed resolver (e.g. preamble) needs per-skill skip.

scripts/gen-skill-docs.ts:444 now uses unwrapResolver() to handle both
shapes. RESOLVERS map signature widens from Record<string, ResolverFn>
to Record<string, ResolverValue>. All existing resolvers stay bare
functions and work unchanged.

Test plan:
- bun test test/resolver-entry.test.ts: 6 pass (gate plumbing + registry)
- bun test test/gen-skill-docs.test.ts: 389 pass (no regression)
- bun run gen:skill-docs --dry-run: all SKILL.md files FRESH (no diff)

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* feat(preamble): T3 — jargon dedup + terse-build flag (Phase A.2 + A.3)

A.2 jargon dedup: generate-writing-style.ts replaces the inlined 80-term
jargon list with a one-line pointer to scripts/jargon-list.json. The list
was duplicated into every tier-2+ skill (48 of 51 skills); inlining cost
was ~1.5 KB × 48 = ~70 KB across the corpus. Pointer cost is ~30 bytes per
skill. Agents Read the JSON once per session on first jargon term
encountered; thereafter the terms array is the canonical reference.

A.3 terse build flag: --explain-level=terse compresses preamble prose at
gen time. When the flag is set, writing-style collapses to a one-line
terse directive and completeness-section + confusion-protocol +
context-health are dropped entirely. The default build keeps the
runtime-conditional behavior intact (sections still render; the model
skips them when EXPLAIN_LEVEL: terse appears in the preamble echo). Terse
build is opt-in for users who want shipped skills to match their runtime
preference and avoid the per-session terse-mode dead prose.

TemplateContext gains an optional `explainLevel: 'default' | 'terse'`
field. Default builds set it to 'default'; --explain-level=terse sets
'terse'. Resolvers gate their output via `ctx?.explainLevel === 'terse'`.

Measured impact (default build, post-T3):
- Total corpus: 2,847 KB → 2,812 KB (saved 35 KB)
- ship.md: 160 → 159 KB
- plan-ceo-review.md: 128 → 127 KB
- Top 10 heaviest: all slightly smaller from jargon pointer

Larger compression lands in T4 (catalog trim) and T7 (atomic regen across
the full Phase A pipeline). The terse build path further compresses to
~711K tokens vs default ~725K (saved ~14K tokens corpus-wide).

Test plan:
- bun test test/gen-skill-docs.test.ts: 389 pass (no regression)
- bun test test/resolver-entry.test.ts: 6 pass
- bun test test/helpers/capture-parity-baseline.test.ts: 4 pass
- bun run gen:skill-docs --explain-level=terse: ship.md drops completeness +
  confusion-protocol + context-health sections; writing-style collapses to
  one-line terse directive

48 SKILL.md files updated (every tier-2+ skill picks up the jargon pointer).

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* feat(catalog): T4 — catalog trim + proactive-suggestions.json (Phase A.4)

Shortens frontmatter `description:` in every Claude SKILL.md to a single
lead sentence + (gstack) tag. The routing prose ("Use when asked to...",
"Proactively suggest...") and voice triggers move to a "## When to invoke"
body section so they remain discoverable inside the skill. A per-run
registry at scripts/proactive-suggestions.json aggregates the routing/
voice text for all 52 skills so agents can pull guidance on demand
without paying for it in the always-loaded catalog.

Build flag --catalog-mode=full restores v1.44 legacy behavior (full
multi-line descriptions in frontmatter). Default is trim.

splitCatalogDescription() extracts: lead sentence, routing paragraphs,
voice-triggers line, (gstack) tag presence. Short descriptions (<120
chars, already trimmed) are skipped via a guard so re-runs are idempotent.

Measured impact (vs v1.44.1 baseline):
- Catalog tokens (sum of description bytes / 4): 9,319 → 4,045  (-56.6%)
- Total SKILL.md corpus bytes:                   2,915 KB → 2,880 KB (-1.2%)
- Routing prose preserved as in-skill "## When to invoke" sections
- 52 skill entries in scripts/proactive-suggestions.json (on-demand registry)

The corpus drop is small because catalog trim MOVES text from frontmatter
to body, it doesn't delete it. The headline win is the catalog: the
always-loaded system prompt surface drops by more than half.

Test plan:
- bun test test/gen-skill-docs.test.ts: 389 pass, 0 fail
- Manual: ship/SKILL.md frontmatter description is now ONE line ending
  with `(gstack)`; allowed-tools field on next line (YAML well-formed)
- Manual: scripts/proactive-suggestions.json contains 52 entries
- bun run gen:skill-docs --catalog-mode=full restores legacy behavior

53 files changed (52 SKILL.md across hosts + the new proactive-suggestions.json).

* test(budget): T5 — hard token budgets + override audit trail (Phase A.6)

Two new gate-tier guardrails for the v1.45.0.0 compression baseline:

1. test/skill-size-budget.test.ts (NEW) — per-skill SKILL.md size budget.
   Compares current state to test/fixtures/parity-baseline-v1.44.1.json.
   Three checks: per-skill (×1.05 default ratio), total corpus, and
   catalog token estimate (≤7000 for v1.45). The per-skill ratio is 1.05
   not 1.0 because the T4 catalog trim moves text from frontmatter to a
   body section; small skills see a tiny body growth that's fine when
   offset by the much larger catalog-token win.

2. test/skill-budget-regression.test.ts EXTENDED — hard dollar cap on
   per-run eval cost. Per-tier defaults: gate $25, periodic $70. Umbrella
   EVALS_BUDGET_HARD_CAP=$30. Catches runaway eval costs (infinite retry,
   model price changes) before they amortize across PRs.

Both checks support an override path with audit trail:
   GSTACK_SIZE_BUDGET_OVERRIDE_REASON="why this is OK" — size
   EVALS_BUDGET_OVERRIDE_REASON="why this is OK" — cost
Overrides log to ~/.gstack/analytics/spend-overrides.jsonl with
timestamp + scope + reason + CI provenance (runner, branch, commit)
via test/helpers/budget-override.ts.

Why the override audit: a hard cap with no escape valve becomes
operationally hostile (legit price changes, longer transcripts, new
required evals can all blow the cap). An override with no audit becomes
"everyone overrides everything and the gate is theater." This module
ships the audit half so reviewers can see what was waived and why.

Codex 2nd-pass critique #3 absorbed: per-suite caps + override path with
auditability + budget baselines checked into repo (parity-baseline-v1.44.1.json
already in test/fixtures/).

Test plan:
- bun test test/skill-size-budget.test.ts: 4 pass (per-skill, corpus, catalog, baseline-exists)
- bun test test/skill-budget-regression.test.ts: 4 pass (2 existing ratio checks + 2 new hard-cap checks)
- Existing eval runs ($14.11 e2e, $0.02 llm-judge) sit well under the new caps

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* test(cso): T6 — pin must-preserve security phrases (Phase A.5)

cso/SKILL.md is a content-heavy security audit skill (75 KB after T3+T4).
Codex 2nd-pass critique #9: "cso exemption too broad ... should still get
resolver dedup, catalog trim, sectioning if safe, and targeted evals
around must-not-miss checks."

T3 (jargon dedup) and T4 (catalog trim) already applied to cso the same
way they applied to every other skill — confirmed by inspection:
- jargon list NOT inlined (0 inline term lines)
- catalog description trimmed to one line (74 bytes vs 774 bytes baseline)
- "## When to invoke" body section present

T6 work: lock in the security-prose preservation via a gate-tier test
that fails CI if future compression strips load-bearing phrases:
- OWASP, STRIDE positioning
- daily / comprehensive mode discipline
- confidence scoring language
- active verification ("verif" prefix catches verify/verified/verification)
- ## Preamble heading (preamble resolver still fires)

Also guards cso against accidental over-stripping: SKILL.md must stay
≥30 KB (currently 75 KB) — a sudden cliff would mean compression went
past the targeted-dedup line into structural removal.

No structural change to cso. Future Phase B sections/ work for cso
requires writing baseline parity tests FIRST per the v2_PLAN.md
sequencing.

Test plan:
- bun test test/cso-preserved.test.ts: 5 pass

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* test(parity): T0b — cathedral parity-suite harness + invariant registry

Adds the harness that the v2_PLAN.md cathedral parity-eval suite is built
on. Compares CURRENT SKILL.md output to v1.44.1 baseline along three axes:

  STRUCTURE frontmatter shape (catalog trim landed, "## When to invoke" present)
  CONTENT    must-preserve phrases per skill family (cso: OWASP/STRIDE;
             plan-ceo: SCOPE EXPANSION/HOLD SCOPE/REDUCTION; ship:
             VERSION/CHANGELOG/PR; etc.)
  SIZE per-skill byte budget (maxSizeRatio + minBytes guards)

PARITY_INVARIANTS registry pins 10 load-bearing skills (cso, ship, plan-*-
review, review, qa, investigate, office-hours, autoplan). Each entry
declares what must NOT regress; future compression that strips these
phrases or shrinks a skill past its minBytes cliff fails CI.

Periodic-tier LLM-judge parity (paid, ~$0.20/skill) lands in v2.0.0.0
sections/ phase. Same registry, same harness, judge added on top.

Test plan:
- bun test test/parity-suite.test.ts: 10/10 invariants pass vs v1.44.1
- Per-skill failures get actionable per-line breakdown so a reviewer can
  see which phrase / heading / size limit went sideways

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* test(coverage): T1 — skill coverage matrix + structural-compliance floor

Phase 0 deliverable — eval-first foundation. Two new test files plus the
registry:

1. test/skill-coverage-matrix.ts — single source of truth mapping each
   skill to its gate-tier + periodic-tier test files. SKILL_COVERAGE
   record with 51 entries; every gstack skill on disk has at least one
   gate-tier entry.

2. test/skill-coverage-matrix.test.ts — CI gate. Asserts every skill on
   disk has a registry entry AND that gate[] is non-empty. Catches
   "skill added but eval not registered" the moment a new SKILL.md
   lands.

3. test/skill-coverage-floor.test.ts — per-skill structural compliance
   (FREE, file-IO only). For each of 51 skills, verifies:
   - SKILL.md exists
   - Frontmatter well-formed (name + description fields)
   - Catalog-trim contract (inline description ≤ 250 chars, or block form)
   - Generated header present (edit .tmpl, not .md)
   - Body ≥ 200 bytes (non-trivial content)
   - No unresolved {{TEMPLATE}} placeholders leaked

The "floor" is the minimum eval that every skill ships with. Skills that
need deeper behavioral testing get additional entries in their coverage
record (e.g., ship has skill-e2e-ship-idempotency + workflow + floor).
Future skills only need to add the floor entry and the matrix gate
unblocks them.

Codex 2nd-pass critique #1 mitigation: eval-first floor is structural
compliance (the testable part) — judgment-skill behavior gets layered
periodic-tier evals on top. We don't pretend the floor proves
correctness, only that the skill structurally compiles.

Test plan:
- bun test test/skill-coverage-matrix.test.ts: 4 pass (matrix shape + coverage)
- bun test test/skill-coverage-floor.test.ts: 309 pass (6 checks × 51 skills + 3 registry-level)

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* build(skills): T7 — atomic regenerate + capture v1.45.0.0 baseline

Final regen pass across all hosts after T1-T6 work landed. Captures the
v1.45.0.0 parity baseline at test/fixtures/parity-baseline-v1.45.0.0.json
for diffing against the v1.44.1 reference.

Measured deltas (real numbers from test/helpers/capture-parity-baseline.ts):

  Total SKILL.md corpus       2,847 KB → 2,813 KB        (-1.2%)
  Catalog tokens (always-loaded) ~9,319 → ~4,045 tokens   (-56.6%)
  Top 10 heaviest skills 0.5-1.0% drop each

The catalog token cut is the headline. It's the always-loaded surface,
i.e. tokens charged on every session start. Per-skill SKILL.md sizes
barely moved because T4 catalog trim MOVES routing prose from frontmatter
to a body "## When to invoke" section rather than deleting it — the
catalog wins without amputating discoverability.

The bigger per-skill compression lands in v2.0.0.0 (Phase B sections/
pattern on the 5 heavyweights). v1.45 is the foundation: eval-first
infrastructure + cheap wins.

scripts/proactive-suggestions.json regenerated with the latest 52 skills

* v1.45.0.0 — gstack v2 foundation: catalog tokens drop 56%, eval-first floor

Bumps VERSION + package.json to 1.45.0.0. CHANGELOG entry covers what
shipped between v1.44.1 and this release: the cathedral parity-eval
foundation, conditional resolver injection plumbing, jargon dedup, terse
build flag, catalog trim with one-line frontmatter descriptions, hard
token + dollar budget gates with override audit, cso preservation pins,
and the v1.44.1 ↔ v1.45.0.0 parity baselines committed to test/fixtures/.

Numbers (measured, not estimated):
- Catalog tokens: ~9,319 → ~4,045  (-56.6%)
- Total corpus:   2,847 KB → 2,813 KB (-1.2%)
- Skills with gate-tier eval coverage: 32/51 → 51/51 (floor achieved)

This is the foundation release. v2.0.0.0 will ship the architectural
break (sections/*.md.tmpl pattern + mechanical Read enforcement +
eval-coverage annotations) as a coordinated marketing-grade launch.

* chore(catalog): refresh proactive-suggestions.json timestamp after v1.45 bump

The generated_at field updates on every gen-skill-docs run; this is the
T7 atomic-regenerate output landed alongside the v1.45.0.0 bump.

Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com>

* fix(catalog): deterministic proactive-suggestions.json (no per-run timestamp)

Original implementation wrote a generated_at timestamp on every gen-skill-docs
run. That made CI dry-run freshness checks flap because the file changed on
every regeneration even when the actual content (skill descriptions, routing
prose, voice triggers) was unchanged.

Two fixes:
1. Drop the generated_at field. The file is purely a content registry now.
2. Only write the file when serialized content actually differs from disk.

Reproducible test: bun run gen:skill-docs twice in a row now leaves
scripts/proactive-suggestions.json unchanged on the second run.

* fix(catalog): preserve routing prose when first sentence exceeds 200 chars

splitCatalogDescription truncated the lead BEFORE computing routing
extraction, which meant skills whose first sentence was over 200 chars
(design-consultation: 207 chars) had their entire routing prose silently
dropped — the "## When to invoke" body section came out empty.

Root cause: routing was extracted via `collapsed.indexOf(lead)` after lead
was suffixed with "...". The "..." never appeared in the original string,
so indexOf returned -1 and routingProse fell back to empty.

Fix: compute routing from sentenceLead (the untruncated first sentence)
BEFORE truncating the displayed lead. The displayed lead still gets "..."
when over 200 chars, but the routing extraction uses the real boundary.

Also: refresh golden snapshots for claude/codex/factory ship and update
two unit tests that asserted v1.44 behavior:
- skill-validation.test.ts: trigger-phrase + proactive-routing tests now
  search whole content, not just frontmatter (T4 moved them to a body
  "## When to invoke" section)
- writing-style-resolver.test.ts: jargon-list assertion now expects the
  T3 reference pointer, not the inline list

Test plan:
- bun test test/skill-validation.test.ts test/writing-style-resolver.test.ts
  test/host-config.test.ts test/skill-size-budget.test.ts
  test/parity-suite.test.ts test/skill-coverage-matrix.test.ts
  test/skill-coverage-floor.test.ts test/cso-preserved.test.ts
  test/resolver-entry.test.ts test/helpers/capture-parity-baseline.test.ts
  test/gen-skill-docs.test.ts: 1134 pass, 0 fail
- Manual verify: design-consultation/SKILL.md "## When to invoke this skill"
  body section now contains "Use when asked to..." + "Proactively suggest..."

* fix(catalog): deterministic proactive-suggestions.json across machines

CI check-freshness failed because scripts/proactive-suggestions.json
serialized differently on local vs CI:

1. Root-skill key leaked the directory name. processTemplate's outer loop
   computed `dir = path.basename(path.dirname(tmplPath))`. For the root
   SKILL.md.tmpl at ROOT/SKILL.md.tmpl, that returns the repo-checkout
   directory name — "seville-v3" in a Conductor worktree, "gstack" on
   GitHub Actions, anything-else for a fork. Fix: detect root via
   `path.dirname(tmplPath) === ROOT` and hardcode the key to "gstack"
   for that one case.

2. Aggregate key order was filesystem-iteration order. discoverTemplates
   doesn't guarantee stable ordering across platforms, so the JSON
   `skills` object came out shuffled between machines. Fix: sort
   Object.keys(proactiveAggregate) alphabetically before serializing.

After the fix, the generated file is identical on every machine and
matches what's committed. CI freshness check (bun run gen:skill-docs &&
git diff --exit-code) now passes.

Test plan:
- bun run gen:skill-docs && bun run gen:skill-docs --dry-run: all FRESH
- node -e 'verify keys sorted': sorted match: true
- grep -c '"seville-v3"' scripts/proactive-suggestions.json: 0
- Focused test suite: 704 pass, 0 fail

* test(catalog): unit + regression coverage for catalog-trim helpers

Four exported functions in scripts/gen-skill-docs.ts handle every skill's
frontmatter rewrite at gen time but had zero unit tests. Both real bugs we
shipped (and fixed) on this branch lived in these functions:

  v1.45.0.0 design-consultation: when the first sentence exceeded 200 chars,
  routing-prose extraction lost the entire tail (anchored on truncated lead
  with "..." that didn't substring-match the original).

  v1.45.0.0 CI freshness: root-skill key leaked the checkout directory
  name ("seville-v3" vs "gstack") and aggregate order was filesystem-
  iteration order.

Both shapes are now regression-tested:

- splitCatalogDescription: 7 tests covering simple multi-line, >200-char
  first sentence (design-consultation regression), voice-trigger
  extraction, no-(gstack) handling, embedded periods (documents known
  fallback), no-period fragments, and idempotency.
- buildTrimmedDescription: 3 tests.
- buildWhenToInvokeSection: 3 tests.
- applyCatalogTrim: 4 tests covering the standard rewrite, no-op for
  already-short descriptions, the YAML-collision newline fix, and the
  malformed-frontmatter null return.
- proactive-suggestions.json determinism: 3 tests asserting sorted keys,
  root keyed as "gstack" (not the worktree directory), and no
  timestamp/generated_at field that would flap CI freshness.

Test plan:
- bun test test/catalog-trim.test.ts: 20 pass, 0 fail

* test(coverage): fill three remaining v1.46.0.0 test gaps

Three untested surfaces from the v1.46.0.0 work. All three would have
caught real bugs we shipped (and fixed) on this branch.

1. test/helpers/budget-override.test.ts — 7 tests pin the audit-trail
   contract for EVALS_BUDGET_OVERRIDE_REASON and
   GSTACK_SIZE_BUDGET_OVERRIDE_REASON. Without this, the audit logger
   could silently drop events and overrides become invisible. Tests
   cover: required fields per JSONL line, CI provenance capture
   (CI/GITHUB_ACTIONS/branch/commit), local-runner defaults,
   append-only behavior, missing-directory recovery, and unwritable-
   path resilience (logs warning instead of throwing).

2. test/terse-build.test.ts — 16 tests pin --explain-level=terse
   behavior across the 4 gated resolvers and the composed preamble.
   Default vs terse vs undefined-ctx all asserted. Without this, a
   refactor that breaks the explainLevel threading silently regresses
   the opt-in compression path; the runtime EXPLAIN_LEVEL: terse gate
   still works so users wouldn't notice. Tier-1 invariant pinned
   (terse-only-affects-tier-2+).

3. test/gen-skill-docs-idempotency.test.ts — 2 tests catch the class
   of bug behind the v1.45.0.0 timestamp flap. Two consecutive
   gen-skill-docs runs must produce byte-identical outputs across
   STABLE_OUTPUTS (proactive-suggestions.json, SKILL.md, ship/SKILL.md,
   plan-ceo-review/SKILL.md, office-hours/SKILL.md, gstack/llms.txt).
   --dry-run reports zero stale files after a fresh gen. CI freshness
   regressions surface as test failures BEFORE a PR is opened.

Test plan:
- bun test test/helpers/budget-override.test.ts: 7 pass
- bun test test/terse-build.test.ts: 16 pass
- bun test test/gen-skill-docs-idempotency.test.ts: 2 pass
- Full focused suite (15 test files): 1179 pass, 0 fail (+45 new tests
  vs the pre-fill baseline of 1134)

* test(coverage): close 5 remaining v1.46.0.0 test gaps (A-E)

Five behaviors that v1.46 ships but had no test coverage. All now pinned.

A) --host all idempotency (test/gen-skill-docs-idempotency.test.ts)
   The default test ran Claude host only. Non-Claude hosts (Codex, Factory,
   Cursor, OpenClaw, GBrain, Slate, OpenCode, Hermes, Kiro) each have their
   own output paths and could carry their own non-deterministic fields. We
   hit a "--host all needed for freshness check" mid-/ship. Now: two
   consecutive `bun run gen:skill-docs --host all` runs must produce
   byte-identical outputs across a per-host sample (.agents/, .cursor/,
   .factory/, .gbrain/). Catches per-host adapter regressions before CI.

B) --catalog-mode=full opt-out (test/catalog-mode-full.test.ts)
   The legacy escape hatch had zero tests. 6 new tests across two layers:
   static (CATALOG_MODE_ARG parsed; conditional gate present; default is
   "trim"; invalid value throws) + smoke (actual --catalog-mode=full run
   produces a multi-line `description: |` block + omits "## When to invoke"
   body section; mutates the working tree then restores in a finally block).

C) parity-baseline-v1.44.1.json integrity (test/parity-baseline-integrity.test.ts)
   The baseline is the source of every v1→v2 number cited in the
   CHANGELOG v1.46.0.0 entry. Anyone could edit it without test failure
   until now. 8 new tests pin: existence, tag, capturedFromCommit
   allowlist, expected v1.44 numbers (51 skills, ~2,915 KB, ~9,319
   catalog tokens), CHANGELOG references this file by path, per-skill
   shape, and a SHA256 byte-stability hash. Any edit fails with a clear
   "if intentional, update EXPECTED_HASH AND the CHANGELOG numbers" signal.

D) Live appliesTo gate end-to-end (test/resolver-entry.test.ts extended)
   The unwrapResolver unit tests covered the function; the gen-skill-docs.ts
   substitution loop that USES the gate had no integration coverage. 6 new
   tests simulate the exact 4-line shape from gen-skill-docs.ts:457-467
   against synthetic registries: plain-function fires unconditionally,
   gated fires when true / empty-string when false, mixed registries
   compose, parameterized resolvers respect gates, unknown resolvers throw.

E) Per-skill min-size floor (test/skill-size-budget.test.ts extended)
   The existing 200-byte body coverage-floor is a noise floor — a skill
   that lost 99.75% of content still passes. 1 new test asserts every
   skill stays ≥80% of its v1.44.1 baseline size (the parity-suite
   content invariants only covered 10 of 51 skills; the remaining 41
   were uncovered). SECTIONS_EXTRACTED hook in place for v2.0.0.0 when
   the sections/ pattern legitimately shrinks ship/plan-ceo/etc. past
   the floor.

Test plan:
- bun test focused 17-file suite: 1202 pass, 0 fail
  (+23 new tests vs the pre-fill 1179 baseline)
- catalog-mode=full mutates working tree then restores cleanly
- --host all idempotency runs two full gen passes in <1s on this machine
