* feat(autoplan): eng review always runs last — the gate reviews the final amended plan

Reorder the pipeline to CEO -> Design (if UI scope) -> DX (if developer-facing
scope) -> Eng. The old order (CEO -> Design -> Eng -> DX) let DX findings land
AFTER the required gate signed off, so eng validated a stale plan.

Accept-all semantics made explicit: every AskUserQuestion resolves to the
recommended option; premises no longer pause the pipeline mid-run (clearly-wrong
ones queue as User-Challenge items at the single Final Approval Gate). Eng's
Codex voice now sees the DX consensus summary. New free static test pins the
order; the chain E2E gains DX-between and Eng-terminal assertions.

specialist keeps the exact NO FINDINGS contract); with findings, the parent
prints 'net: -N lines possible' summed from lines_removable.

Tests: static pins for the carve-out + early-out contract (gen-skill-docs),
two periodic e2e cases with planted fixtures — activation (over-build traps:
hand-rolled Intl, one-impl abstract, dead config) and false-flag precision
(a lean ETHOS 'choose A' diff must yield NO FINDINGS).

Tier-3+ skills gain a per-edit reflex the section only stated as research
discipline: before writing new code, stop at the first rung that holds —

explicit reconciliation with Boil the Ocean: the ladder governs structure,
never coverage. Rungs 1/6/7 (YAGNI / one line / minimum that works) are

beats a guard in every caller.

After completing work, skills report in a few short lines — what changed,
what was skipped, what to watch — and cut any explanation that outgrows the
change. Explicit exemptions protect every mandated output: decision briefs,
completion-status blocks, user-requested explanations, and report-shaped
skills' report formats (the report IS the work in /qa-only, /plan-*-review,
/retro, /document-generate).

Rationale is signal-to-noise, not tokens: ponytail's own benchmark shows
terse prose alone doesn't cut cost (caveman arm: -20% LOC, +7% tokens), and
independent replications found its 'skipped on purpose' essays ate the code
savings. Includes a good/bad closer example pair per the model-overlay
guidance that a positive example beats a 'don't be verbose' instruction.

durable-scope call, the decision ledger entry (gstack-decision-log, ceiling +

each cut corner in code with gstack-shortcut(dec-<id>): <ceiling>, upgrade
when <trigger> — same edit, no follow-up question, never agent-initiated.

/retro Step 11.5 harvests markers into a debt ledger (grep || true — zero
matches is the healthy case; skill installs and docs excluded), joins on the
decision id so nothing double-counts, tags unlinked and no-trigger rot risks,
and closes with 'N markers, M with no trigger.'

/review suppressions: a marker with ceiling+trigger downgrades a would-be
Completeness Gaps finding to acknowledged debt. Redaction test pins that the
marker ships untouched (the ledger is the point) — it does not match the
TODO(owner) hygiene shape.

Format from dietrichgebert/ponytail's ponytail-debt; store inverted to gstack's
existing decision ledger.

gstack's ethos one-liners, the reuse ladder, and voice rules for hosts with

by scripts/gen-agents-digest.ts, auto-refreshed by gen:skill-docs, committed
like llms.txt so setup's explainer arms can point at it before any toolchain
exists. First line carries the gstack version as its own staleness nudge.

Delivery is print-path + user-performed copy ONLY: setup never writes or
overwrites a user's AGENTS.md (a test pins this — no cp/ln/mv/redirect into
AGENTS.md anywhere in setup). openclaw and hermes explainer arms print the
path; slate keeps routing to the full Claude install and gbrain ships from
its own repo. HostConfig gains the optional install.instructionTier slot,
declared by both instruction-tier hosts. README host table now matches what
setup actually does.

Inspired by dietrichgebert/ponytail's instruction-tier AGENTS.md fallback —
one generated source, never per-host hand copies.

verbosity floor and all 14 format pins stay (Layer 0 green).

The gate this decision rested on ran before landing (new periodic
skill-e2e-auq-repetition-cut-ab.test.ts, pre-cut ref 3263fffe vs this
render, same harness as auq-verbose-vs-carved-ab): POST 7/7 format elements,
substance 5 — identical to PRE. No degradation; the load-bearing-repetition
hypothesis did not hold for these duplicates.

Net: -236 bytes per tier-2+ skill (~9.7KB corpus). Golden ship baselines
re-captured for the deliberate change.

Ponytail's honest-benchmark method pointed at gstack itself: 3 build-shaped
tasks (native-platform over-build trap, CRUD endpoint, bug fix with planted
decoys) x 2 arms, real claude -p sessions, scored on the git diff left
behind. A research instrument, not a release gate — no assertion compares
arm scores.

Arms use the PROVEN project-scope pattern: the with-arm installs a
build-discipline skill (extracted reuse-ladder + bounded-closer content, not
whole-file copies) into the fixture's .claude/skills/ with a CLAUDE.md
routing line and an explicit invocation; a live spike confirmed claude -p
discovers and invokes project-scope skills via the Skill tool (3 turns,
exact-output probe). Fixtures are git init + local bare origin; diff capture
is three lines of git, no worktree machinery.

Failure taxonomy: zero-diff arms are VALID scored cells (deterministic
0/none, no API call), harvest failures record harvest:null, judge_error
cells are excluded from aggregates but named in the report — nothing drops
silently. armJudge: fixed sonnet judge, 0-3 unrequested-structure rubric,
must name the construct or say none, bounded retry-on-malformed; callJudge
gains optional temperature/max_tokens (defaults unchanged). recordE2E now
populates tokens_used for every E2E. Eval schema v2: harvest gains
{insertions, deletions, net}, tolerant reads keep v1 runs comparable.

Registered periodic in E2E_TIERS + touchfiles (with the auq-repetition-cut
A/B); periodic detach timeout raised to the new shard-census floor. Free
selftest (8 tests, zero API) pins fixtures, extraction, arm asymmetry, diff
capture, judge plumbing, and the retry bound.

GSTACK_FREE_JOBS overrides the computed shard count (the free runner's
analogue of the paid runner's EVALS_JOBS). On Vercel sandboxes, PID 1
installs a seccomp filter whose supervisor spuriously fails access(2) for
busy processes — measured: 200/200 git-init probes fail 'Cannot access work
tree: Permission denied' while the suite runs at 6 shards, 0/200 idle;
statx succeeds while access fails on the same path in the same process.
One serial mega-shard maximizes per-process pressure and fails too; 2
shards is the measured sweet spot.

GSTACK_FREE_RETRY_FLAKY=1 (default OFF — dev boxes should see flakes)
re-runs attributed failures once, serially, capped at 5 files; a clean
retry downgrades to a loud FLAKY-PASS naming the offenders, a repeat
failure stays red, timeouts and unattributed failures never retry.

Local path validation now accepts os.tmpdir() alongside the classic /tmp
(new TEMP_DIRS in platform.ts): on macOS os.tmpdir() is /var/folders/...,

both are legitimate scratch space. Remote file serving (TEMP_ONLY) stays
pinned to TEMP_DIR alone; no change to the exfil boundary.

commands.test.ts drops 41 hardcoded /tmp literals for a tmpp() helper on
os.tmpdir() (two message assertions now reference the same variable), and
path-validation's symlink-escape test targets /etc/hosts instead of
/etc/crontab — the target must EXIST for realpath to resolve the link (a
dangling target falls back to the link's own path and passes vacuously),
and /etc/crontab is absent on Amazon Linux.

beside the binary would false-red the freshness gate on version drift), but

again. Also: uniform output shape (agentsDigest: null on the JSON-manifest
branch), a REAL generator round-trip test replacing the misnamed lockstep

as ship so the two grading surfaces agree.

remote always advertises at least its default branch, so an exit-0 zero-head

sibling no-toc test saw 3 — per-render image-promotion timing makes any
count assertion here a coin flip. The sibling test owns exact promotion
counts; this test's actual invariant is that --toc does not break the
promotion machinery: >=1 landscape page + the TOC rendered. Also drops
the second render (halves the test's runtime).

Flaky per-render image promotion itself is worth its own look — noted

once the skill loaded. Sibling disk assertions sweep both $HOME/.gstack
and cwd .gstack roots (positives and negatives).

Verified paid: refusal 2x consecutive green with the skill's EXACT
message rendered ('Launching skill: skillify' in-transcript), then the
full file 5/5 green (~$1.35) with both siblings driving real Skill
calls (25-27 turns each).

behaved CORRECTLY; the test's fixture set was open to concurrent

local-green/CI-red split explained.

The restore test now uses its own .gstack-restore-home (the whole home
moves, not just the handed path — an agent deriving the dir from
GSTACK_HOME/projects/<slug> must land in the closed set too). Full file
4/4 paid green with all evidence flags firing.

Also: the on-failure shard-log artifact glob uploaded nothing — the
Fix-bun-temp step points TMPDIR at /home/runner/.cache, so the spool
lands there, not /tmp. Both eval workflows now glob both locations
(this gap is why diagnosing THIS failure required digging transcripts
out of the slice-results artifact).

- security-bunnative.ts + test: a research skeleton self-described as 'NOT a
  production replacement', shipped into src/ with zero importers.
- security-bench-ensemble{,-live}.test.ts + the Haiku response fixture: a
  paid live-model benchmark for a layer that could not fire. The
  security-classifier-tdz test's only case exercised checkTranscript — gone.
- security.ts: layer-model header rewritten to the live architecture;
  StatusDetail.layers -> {testsavant, canary}; getStatus() no longer requires
  the impossible transcript==='ok' for 'protected' (old on-disk session state
  with a transcript key is tolerated on read, never re-emitted).
- security-sidecar-entry.ts needed zero changes: it serializes
  getClassifierStatus() verbatim and no consumer read .transcript (verified
  in sidecar-client + server.ts).
- BROWSER.md security section matches reality (ensemble knob gone, 112MB not

the idle check reads the module-level IDLE_TIMEOUT_MS env constant, and both
resolveChromiumProfile() call sites pass no argument. Worse than absent — an
embedder passing idleTimeoutMs: 5000 silently got 30 minutes.

Wiring them honestly is impossible today: the idle timer, activity state, and
shutdown target are module-global, so a per-factory value would lie for any
process running more than one handler. Deleted instead, with a ServerConfig
note pointing at the deferred singleton/route-table refactor where real

and prefers same-slug priors, so each shard baselines against its own

subdirs; eval:compare's no-arg mode also stops picking an in-progress
accumulator as the after-run.

eval-watch stays flat (documented follow-up): it tails a single dir for
live progress and gains nothing from per-shard baselines until the
runner emits a merged stream.

scripts/test-paid-shards.ts runs the gate/periodic tier one Bun process
per test file, with an EXTERNAL wall-clock timeout that SIGKILLs the
shard's detached process group and an aggregate that distinguishes

longer read as a pass. Bun's native --shard/--isolate covers none of
this: no process-group kill (hung claude/codex PTY grandchildren
survive in-process isolation), no never-started taxonomy, no per-shard
env. Each shard child gets GSTACK_EVAL_DIR=<evalDir>/shards/<slug>/
(slug = test filename sans extension, stable across runs) so shard
baselines compare against their own prior runs.

Output classification lives in scripts/test-strict-output.ts (strict
exit-code derivation, incremental fail-line classifier, child signal
forwarding) so the runner and any future strict bun-test wrapper share
one implementation. Enumeration derives from the shared paid-test-set
module; tier exclusion fires only on an explicit whole-file
EVALS_TIER === '<other>' guard.

package.json gains test:gate:sharded / test:periodic:sharded, and
eval:bg:gate / eval:bg:periodic now run the sharded scripts with detach
timeouts sized to the worst case (gate: 49 shards x 30min / 4 jobs ~
6.2h -> 25200s; periodic: 59 -> 28800s).

test/paid-shards.test.ts pins enumeration, tier classification, and the
kill-and-continue property with a real busy-loop shard.

write|outcome subcommands, exit 3 + EGRESS_RECEIPT_FAILED on stderr on

- gstack-brain-sync: fail-closed git-class receipts. The push receipt
  is written BEFORE the commit consumes the queue, so a refused receipt
  leaves the queue intact and the next run retries the whole drain
  (pinned by a new queue-intact-on-refusal test, including the
  problem/cause/fix refusal message shape). The retry-path fetch and
  retry push carry their own fail-closed receipts.

gstack-team-init needs no wiring: every git clone in it is inside an
echoed instruction string, not an executed command.

The lib now self-locates with shell builtins only (no dirname), so

Three upstream fixes over the fork:
(a) findSkillDirs treats the walk ROOT as a container — the repo root's
    router SKILL.md is billed AND its children are walked (the fork
    short-circuited and billed one skill); walkMd skips node_modules
    and dot-directories.
(b) installed-tree layout: subdirs that are their own repo checkout
    (a gstack/ clone inside ~/.claude/skills, detected by .git) are
    skipped, and directory symlinks (connect-chrome) are followed with
    a container-recursion cycle guard.
(c) ROUTER_KEYS widened to the upstream frontmatter contract {name,
    description, version, allowed-tools, triggers, preamble-tier}.

--exact writes an egress receipt (sink context-bill-exact, host
api.anthropic.com) BEFORE any count_tokens POST; if the receipt cannot
be written the run degrades to the offline estimate with a warning —
nothing is sent unrecorded. bin/gstack-context-bill is the bun shim.

Tests: fixture-tree ledgers, the three fixes, --diff/--budget exit
codes, --exact with injected fetch (envelope subtraction, receipt
ordering, fail-open degradation), CLI smoke test, and ground truth

Every host loads every skill's frontmatter name + description at
discovery, every session. applyCatalogTrim in scripts/gen-skill-docs.ts
shapes each description and the 160KB per-file warn covers body size,
but nothing capped the aggregate frontmatter — the catalog could grow
one reasonable-looking description at a time. This test is that
enforcement layer.

Measures the catalog via test/helpers/skill-census.ts authoredSkills
(symlink-deduped, root router counted separately as the _gstack-command
alias line item): 53 skills + router = 4,420 bytes = 1,105
token-equivalents today, asserted <= 1,150 (~4% headroom). Per-skill
sub-cap of 260 bytes (largest today: design-consultation at 229), plus
a non-empty-description check.

Failure messages are self-service ratchets: they print the new total,
the delta, and the update protocol (bump the constant AND the
derivation comment in the same commit; trim instead of grow for
existing descriptions). Parser handles folded block scalars
(description: >-) for fork parity; import-free by design so it
survives generator refactors.

local listener: the Origin header must be exactly
chrome-extension://<GSTACK_EXTENSION_ID> and the Host header's hostname
must parse to 127.0.0.1 or localhost (parsed via new URL, never literal
equality — Host arrives as '127.0.0.1:34567'). Wrong origin/host → 403
with no detail. The tunnel surface 404s the endpoint (not in
TUNNEL_PATHS, verified by test).

The extension ID is pinned by a new "key" field (RSA public key) in
extension/manifest.json; browse/scripts/extension-id.ts reproduces the
ID derivation (first 16 bytes of SHA-256 of the DER public key, hex
mapped 0-9a-f → a-p). The private key is not committed anywhere —
unpacked/baked-in loads only need the public key.

Extension side: background.js bootstraps and refreshes the token via
POST /extension-token (403 → disconnected state); sidepanel.js direct
connect path does the same; sidepanel-terminal.js's dead /health token
fallback (read AUTH_TOKEN/authToken keys the server never sent,
hardcoded port) is replaced with the window.gstackAuthToken path.

MIGRATION NOTE: the manifest key pins the extension ID, so existing
installs' side-panel local state (saved port, snoozes) resets once —
explained in-product via a one-time notice (flag
gstack_id_migrated_v162). After upgrading the server, restart the
browser so the old service worker stops polling for a token GET /health
no longer serves.

skill-e2e-ship-idempotency.test.ts (~$3/run, 5-10 min):
- Builds a real git fixture with VERSION 0.0.2 already bumped, matching
  package.json, CHANGELOG entry, pushed to a local bare remote. Runs
  /ship in plan mode and asserts STATE: ALREADY_BUMPED echoes from the
  Step 12 idempotency check, OR plan_ready terminates without mutation.
- Snapshots VERSION + package.json + CHANGELOG entry count + commit
  count + branch HEAD before/after; fails if any changed.

skill-e2e-autoplan-chain.test.ts (~$8/run, 12-18 min):
- Asserts /autoplan phases run sequentially: tees timestamps as each
  "**Phase N complete.**" marker first appears. Phase 1 (CEO) must
  precede Phase 3 (Eng); Phase 2 (Design) is optional but if it
  appears, must sit between 1 and 3.
- Auto-grants permission dialogs that fire during phase transitions.

All three auto-handle permission dialogs (preamble side-effects on
fresh user envs without .feature-prompted-* markers).

Per user feedback: don't shorten AskUserQuestion to AUQ — the
abbreviation reads as cryptic. Apply across all the new code from this

- Touchfile entry auq-format-pty → ask-user-question-format-pty
  (touchfiles.ts + matching assertion in touchfiles.test.ts)
- Function rename navigateToModeAuq → navigateToModeAskUserQuestion
- Variable auqVisible → askUserQuestionVisible
- Outcome literal 'real_auq' → 'real_question'

No behavior change. 49/49 free tests still pass.

Per Garry: write the entry assuming a critic will screencap one line
and try to use it as ammunition.

Reframed the v1.15.0.0 release-summary to lead with new capability
(real-PTY harness, 11 plan-mode tests, +6 new) instead of fix-of-prior-
flaw narrative. Removed phrases that critics could weaponize:

- "0/5 → 5/5 passing", "finally pass", "∞ (never green)" — drop
- "Skill prompts get a 25% haircut" — implied self-inflicted bloat
- "770K → 574K tokens" — absolute number lets critics quote "still 574K
  of bloat"; replaced with relative "−196K tokens per invocation"
- "5 plan-mode E2E tests turned out to have never actually passed" —
  literal admission of long-term breakage; cut entirely
- Itemized "Fixed: tests finally pass" entry — moved to Changed with
  neutral "rewritten on the new harness" framing
- "Removed: harness with the runPlanModeSkillTest API that never
  worked" — replaced with "superseded by claude-pty-runner.ts"

Added concrete code receipts to pre-empt "it's just markdown":

- Net branch size: −11,609 lines (89 files, +7,240 / −18,849)
- 654 lines of TypeScript in test/helpers/claude-pty-runner.ts
- 8 new test files, ~1,453 lines of new TS code
- 23 helper unit tests + 6 new gate/periodic E2E tests

The deletion-heavy net diff (−11.6K lines) is itself the strongest
defense against the "bloat" critique — surfaced explicitly in the
numbers table.
