Fires when Claude encounters architectural decisions, data model changes, destructive operations, or contradictory requirements. Does NOT fire on routine coding. Addresses Karpathy failure mode #1 (wrong assumptions) with an inline STOP gate instead of relying on workflow skill invocation.
