v1.91.19.0 fix: severe fix wave (truthful gates, no lost memory, every host finds gstack) (#3025)
* docs: approved severe fix wave plan (Oct 2026) * D0: keep a project's .env and bunfig.toml out of compiled binaries and daemon children Compiled binaries are built with --no-compile-autoload-dotenv and --no-compile-autoload-bunfig; the browse server and terminal agent are spawned with --no-env-file and an empty config, so a repo's .env cannot set GSTACK_CHROMIUM_NO_SANDBOX and a bunfig preload cannot run code in them. Verified on Bun 1.4.0. * A9: --no-write is a dry run and leaves ingest state untouched --no-write stamped every prepared page ingested_at and saved state, so a later real run skipped those pages forever. It now prepares and reports pages without importing or changing the state file. Tests that used GSTACK_MEMORY_INGEST_NO_WRITE / --no-write to seed state move to an explicit seam (a fake gbrain that accepts every staged page), because the --no-write contract deliberately changes. * H #2885 #2827: vector geometry is not phone or card PII pii.phone.e164 accepts "." as a group separator, so a single float (viewBox="0 0 581.66796875 ...") and a run of path coordinates ("37.6188 101.694") read as phone numbers. pii.cc's \b stops at a decimal point, so the long fraction of an .excalidraw float is a Luhn candidate. Excalidraw's seed/versionNonce/updated integers read as bare phone numbers. A rendered /diagram triplet raised 18 (svg) and 97 (excalidraw) MEDIUM findings on main; it now raises none. - looksLikeDecimalCoordinates (ported from #2885, extended to a single decimal and refusing a leading "+", the E.164 marker) - insideDecimalNumber: a cc span glued to "<digit>." or ".<digit>" - isNumericMetadataValue: an unquoted JSON value under a seed, nonce or timestamp key; the same digits under "phone" still report Controls: E.164 in prose, dotted and spaced phones, "+"-prefixed runs, "phone"/"contact" JSON numbers, Luhn-valid cards in prose and at sentence end. Co-authored-by: Lewis <lewispeel@hotmail.com> * D1: pin the terminal-agent /ws Origin to the gstack extension (#1324) GSTACK_EXTENSION_ID moves to browse/src/extension-id.ts, shared by /extension-token and the terminal agent's /ws gate. /ws now returns 403 for every origin except the pinned extension. Forks override the ID with gstack-config set browse_extension_id <id>, honored by both routes; the BROWSE_EXTENSION_ID env var is no longer read. Test sites that used fake extension IDs now use the pinned origin. * H #2899: env.kv skips a function call assigned to a credential name `session = _FlakySession(...)`, `token = make_token(user, ...)` and `password = getpass.getpass()` raised env.kv MEDIUM, which blocks the fail-closed decision log. The exemption needs the line's evidence, not the span's shape: the value is unquoted, is an identifier or dotted path followed by "(", and the call closes on the line or opens a multi-line argument list. Quoted values, unclosed "(" inside an unquoted secret (API_SECRET=Abc123xyz(9Qm) and calls carrying a high-entropy literal argument still report. The literal check is shared with the #2912 env-read exemption. Co-authored-by: Yugz <yannduzelier@gmail.com> * H #2962: per-machine config filenames are not internal hostnames `CLAUDE.local.md` and `settings.local.json` reported internal.hostname MEDIUM (the pattern's \b stops at the dot before the extension), which fails closed in the decision log. A span immediately followed by a known config extension (.md .json .jsonc .yaml .yml .toml .ini .conf .txt) and a word boundary is a filename. Controls: `ssh build.local`, `printer.local.` at a sentence end, `printer.local/md`, `printer.local.mdx`, a host with a port, `api.corp`, `build-7.internal`. The `.env.local` vectors (#2740) pass on main already via isDotenvFilename and are pinned here as regressions. Co-authored-by: Florian <florian.cleyetmerle@gmail.com> * H #2784: four-part versions need a version declaration, not a shape `"version": "1.128.1.0"`, `<Version>`, `AssemblyVersion("1.0.0.0")` and Keep a Changelog headings raised pii.ip_public MEDIUM on every release, because a four-part version is byte-for-byte a dotted quad. Exempt only when the same line declares it a version immediately before the value (a version key or word, an XML tag, an attribute call, or a `## [x]` heading). The same digits after host:/server =/"upstream", bare on a line, or after prose that merely mentions a version still report. Unlike #2784, this does not exempt a value because the same string is declared a version elsewhere in the text: that let a real address sharing a diff with a matching version slip. The pre-push hook's VERSION-file rule (#2856) is unchanged. Co-authored-by: cdecook23 <decook23@gmail.com> * H #2913: postgres:postgres is exempt only on a loopback host The postgres image's default pair in compose files and CI DATABASE_URLs blocked pushes as db.url_with_password HIGH. It is now exempt only for postgres/postgresql URLs with exactly postgres:postgres on localhost or 127.0.0.1 (optional port). A bare single-label host (`db`, `postgres`), any other host, a host that merely starts with localhost (`localhost.evil...`, `localhost@prod-db`), any other user or password, and other schemes still block. #2913 exempted the pair on any host; this keeps the plan's narrower rule. Co-authored-by: mb1810 <michael.berger1810@gmail.com> * G1: ship without a version when no version source is configured Resolve the version source to four outcomes (valid, absent, ambiguous, broken) in lib/version-source.ts and read that one signal in gstack-version-bump (new NO_VERSION state; write refuses to invent a VERSION), gstack-next-version (no_version, no slot), /ship's bump, CHANGELOG and title steps, /land-and-deploy's drift check and /document-release. A configured version file that is missing, empty, unreadable or malformed stops with its path; 0.0.0.0 is never substituted. docs/skills.md documents .gstack/version-path. Migrates two tests that pinned the old fabricated-0.0.0.0 behavior. Co-authored-by: Koda <koda@edgeviewfinance.com.au> Co-authored-by: Merit Blake <merit@newsprint.com> * H #2949: eval credential screen checks every name segment The hermetic eval env admits GITHUB_* and EVALS_* metadata but rejects credential-shaped names, judged only by the last underscore segment. A trailing qualifier carried a credential through: GITHUB_APP_PRIVATE_KEY_BASE64, GITHUB_TOKEN_1, EVALS_API_KEY_FALLBACK reached eval children. The screen now rejects a name when any segment is a credential word; segments, not substrings, so GITHUB_PATH / GITHUB_TOKENIZER / GITHUB_KEYRING stay metadata, and explicit runner admissions still win. Co-authored-by: Nguyen Thanh Dat <ntdat812@gmail.com> * H #2915: cover chunked scanning of large multi-file push diffs Test-only coverage for behavior already on main: a clean two-file pushed diff over the engine's 1 MiB cap passes without engine.input_too_large, and a credential at the end of the second file (a later scan chunk) still blocks the push. Co-authored-by: pneumorea-ai <pneumorea@gmail.com> * A3: keep a project's DATABASE_URL away from a PGLite brain With a PGLite config, buildGbrainEnv returned the caller env unchanged, so a project's .env DATABASE_URL (autoloaded by Bun) reached gbrain: a false broken-db at best, gbrain talking to the app's own Postgres at worst. Inherited DATABASE_URL and GBRAIN_DATABASE_URL are now blanked for PGLite configs (GSTACK_RESPECT_ENV_DATABASE_URL=1 still keeps them), and every gbrain child runs from gbrain's config dir instead of the user's project, so it cannot reload the project's .env / .env.local. The old test that pinned pass-through for an engine:pglite config is migrated: pass-through now applies only to configs that name neither a database_url nor PGLite. Co-authored-by: maxpetrusenkoagent <max.petrusenko.agent@gmail.com> * C9: guard destructive gstack-upgrade and spec fences against unset or wrong paths Fresh-shell hosts (and Claude Code, which keeps cwd but not shell variables) ran cd "$INSTALL_DIR" with an empty variable, so git checkout/stash/reset targeted the user's project. Step 2 now prints an absolute path; every destructive fence starts with cd -- "${VAR:?...}" || exit 1 and checks it is gstack's own checkout before any git command. /spec's worktree and spawn fences get the same guard. * F4: keep every paid design variant across regenerate rounds $D variants now reserves a collision-safe round-<N>/ directory per run (atomic mkdir) and writes a manifest.json of what each round saved. The paid image request and the local write retry separately: a write failure retries the write (temp + rename) and, if the path stays unwritable, keeps the bytes in the temp dir instead of buying the image again. Responsive variants land in their round directory too. Readers updated: design-shotgun reserves a round dir and a private temp dir for its parallel agents and records approved_path/round in approved.json; design-html resolves approved_path (legacy letters still work) and finds round-dir variants; design-consultation checks the printed paths; the gallery shows round-dir variants. Variants that were already overwritten before this change cannot be recovered. Migrates the consultation contract test that pinned the flat path. Co-authored-by: Matt Van Horn <mvanhorn@gmail.com> * A2: stage dot-leading slugs gbrain can collect; network errors are db-unreachable A page slug segment starting with a dot (a .claude project) staged a file gbrain's import walker prunes, so the batch was refused on every run. Leading dots become "dot-" per segment, and slugs recorded in state under the old name map through the same function so no page is duplicated. A DNS or connection failure (ENOTFOUND, EAI_AGAIN, ECONNREFUSED, ...) no longer classifies as broken-db, which routed users to move a working config aside. The new db-unreachable status carries the reason "database host unreachable (<code> <host>); your gbrain config is unchanged. Fix: check network or VPN, then re-run /sync-gbrain.", skips local sync stages with that reason, and counts as usable for gstack-gbrain-detect --is-ok like timeout. Co-authored-by: Infiniteyieldai <infiniteyield.ai@gmail.com> * D2: one address classifier for navigation literals, DNS answers, redirects and page-driven navigations (#2811) classifyAddress() in url-validation.ts blocks 169.254.0.0/16 (metadata and container credential endpoints), 100.100.100.200, fe80::/10 and fc00::/7, including IPv4-mapped, IPv4-compatible and NAT64 spellings and decimal, octal and hex IPv4. validateNavigationUrl, the A/AAAA answer check and session-persist's cookie-domain guard all use it. BrowserManager now checks every navigation request (redirect hops, link clicks, scripted navigations): a blocked target resets the tab to about:blank and the write command fails with the reason. Connection-time DNS rebinding remains out of scope. Co-authored-by: Gabriel Rondon <grondon@gmail.com> * C4: external hosts point at carved sections relative to the installed skill Publishes the pointer form setup matches: on every non-Claude host a carved section is `sections/<file>` relative to the installed gstack-<skill> SKILL.md directory (QA's existing form), never $GSTACK_ROOT/<skill>/sections/, which does not exist in external runtime roots. No render changes yet: only QA is carved on external hosts today. * INV-1: gate outcome contract for outside reviews classifyOutsideReview() returns separate execution, findings and verdict fields from text, stderr and exit status; reason codes, anchors and fix actions live in lib/gate-outcomes.ts. validateOutsideReview() stays as the text-only wrapper and the two-argument CLI keeps exit 0/1/2 for importers and installed skills that predate a re-render. The --verdict form (implied by --stderr/--exit) prints VERDICT/FINDINGS/REASON and exits 0/3/4/1; the generated outside-voice caller moves to it and branches on each status (unverified prints OUTSIDE_STATUS: unverified, never completed). Adds docs/troubleshooting.md with one anchored section per reason code, the anchor test, and the gate-tail lint (query scripts may not swallow failures, gate calls may not end in || true, other best-effort tails say why). * G1: quote the base placeholder in document-release title sync The title-sync block is checked with bash -n; an unquoted <base> parses as a redirection. Migrates the test that pinned the old root VERSION read to the shared classifier call. * G3: give every logging-bin mktemp a ${TMPDIR:-/tmp} template Bare mktemp, mktemp -t and literal /tmp templates in the learnings, question and preference logs (plus jsonl-merge, distill, the two dashboards, /ship's PR body and /document-release's run dir) now use "${TMPDIR:-/tmp}/<name>.XXXXXX", the form the INV-3 lint accepts. When the temp file cannot be created, the learnings, question and preference logs now say the entry was not recorded and how to fix TMPDIR instead of exiting with only mktemp's own error. Three tests that pinned the bare PR_BODY_FILE=$(mktemp) and one that pinned the /tmp run dir migrate. GNU and BSD mktemp already honor TMPDIR without a template, so the macOS-sandbox drop in #2952 is unverified here; the template makes the temp location explicit on every mktemp. Co-authored-by: Davon Johnson <d.johnson@dayofshow.net> * B1b: structured outside-review gate fails on P0 as well as P1 validateOutsideReview() derived its structured pass/fail from a [P1] match, so a review reporting only [P0] findings passed. The wrapper now reads the classifier's findings verdict (P0 or P1), and the /review and /ship structured-gate prose requests [P0]-[P3] tags and fails on P0/P1; untagged output (OUTSIDE_STATUS: unverified) is missing coverage. The ship clarity pin moves from 'structured P1' to 'structured P0/P1' with the behavior. * E1: freeze normalizes boundary and target the same way on Windows; hooks spawn through cmd.exe (#2876, #2354) gstack_hook_normalize_path (careful/bin/hook-extract.sh) maps C:\..., c:/..., UNC \\server\share (leading // kept) and /cygdrive/c to one form; check-freeze runs both the saved boundary and file_path through it before the absolute test, after symlink resolution, and compares case-insensitively on Windows bash. freeze, guard and careful register hooks as bash -c "exec bash \"$HOME/...\"" so cmd.exe, which never expands $HOME, no longer turns the hook into a silent exit 127. Co-authored-by: Kevin Grasso <kevin@biblingo.com> Co-authored-by: Xuan-Ce Wang <261591073+xwang4-svg@users.noreply.github.com> * G4: keep CLI-to-daemon loopback calls off HTTP_PROXY (#494) The CLI appends 127.0.0.1, localhost and ::1 to the user's NO_PROXY (and no_proxy), keeping existing entries, before its first fetch. Verified on Bun 1.4.0: fetch honors NO_PROXY when it is set before the first request and ignores later changes, so it is set at the top of main(). * F1: bind the plan audit to this branch's plan, never the newest file Plan File Discovery now prefers an explicit binding (the plan-mode file or /autoplan ACTIVE_PLAN from the conversation, then a "Plan: <path>" line in the PR body), then lists candidates: repo-committed docs/designs/ files this branch changed or that name it, and personal plan files that name the branch. Candidates are offered, never picked silently. The repo-name match and the newest-file-of-the-day fallback, which audited PRs against unrelated plans, are gone. With nothing bound or chosen it prints the exact line 'Plan completion audit: not run (...). Fix: add "Plan: <path>" to the PR body, or run /autoplan.' /ship's parent now binds the plan before dispatching the audit subagent ({{PLAN_COMPLETION_GATE_SHIP:discovery}}), so the child never searches. Migrates three tests that pinned the old markers and notice. Co-authored-by: loulanyue <260355617@qq.com> * G5: browse type --selector <sel> <text> (#2936) --selector focuses the element (sel or @ref) and types into it. The bare form still types its text literally into the focused element; text that starts with -- goes after a -- separator. When the first word looks like a CSS selector, the result adds a hint to use --selector without echoing the typed text, and never refuses based on page contents. Help and command usage updated. Co-authored-by: Thomas Reuser <thomasreuser@gmail.com> * F2: design-doc discovery reads docs/designs, never a root DESIGN.md A root DESIGN.md is the design system /design-consultation writes, so preferring it unconditionally reviewed plans against a token file while printing "Design doc found" and never looked at docs/designs/ feature docs. Discovery now considers only docs/designs/*.md as the repo-local design doc (still freshness-compared with the private ~/.gstack copy). The CEO finding fixture seeds its supplied design as a docs/designs doc and its test follows. Co-authored-by: Bit0ps <alex.dreamscaper@gmail.com> * C4: rename the external carve test (typecheck:test union-order artifact) tsc prints a union's members in type-creation order; a test file sorting before ceo-mode-option.test.ts reordered one baseline diagnostic's text. * C1: every env-var host fence resolves its own gstack root Codex and the other env-var hosts run each block in a fresh shell, so GSTACK_ROOT/BIN, B and D set by the preamble were empty in later blocks (/bin/gstack-slug, B=/browse). scripts/resolvers/runtime-root.ts now owns root resolution: a valid exported GSTACK_ROOT (bin/ and lib/), else the per-install literal root (no git call), else repo-local, else the global root (CODEX_HOME on Codex); no root prints the setup fix line and exits. One post-render pass inserts it (<=400 bytes) into every fence that uses these variables without assigning them; B/D come from the same helper as BROWSE and DESIGN SETUP. Claude output is byte-identical. Migrated pins of the old preamble lines (gen-skill-docs.test.ts) and a fixture whose exported GSTACK_ROOT lacked lib/ (skill-positional-literals). * E8: headless browse honors GSTACK_CHROMIUM_PATH; extensions load in new headless mode (#2771, #1968, #2281) The headless launch passes GSTACK_CHROMIUM_PATH as executablePath, scopes the XProtect self-heal out for that bundle, and names the path in a launch failure with the fix. BROWSE_EXTENSIONS_DIR now launches a persistent context under the project state dir with --headless=new, so the extension runs and no window appears (the off-screen window was visible on macOS and the extension never ran in an isolated newContext). The GPU source guard test moves from the removed useHeadless flag to launch() vs launchHeaded(). Co-authored-by: oli548 <312288720+oli548@users.noreply.github.com> Co-authored-by: Slava <slava.istomin@tuta.io> Co-authored-by: ryanlaiwy <129729476+ryanlaiwy@users.noreply.github.com> Co-authored-by: 李翊楠 <yinanli1917@gmail.com> * C2: link design/dist and make-pdf/dist into every host runtime root and the agents sidecar Codex design and make-pdf skills fall back to $GSTACK_DESIGN and $GSTACK_MAKE_PDF under the runtime root, but setup linked only browse/dist, so both preflights reported NOT_AVAILABLE on a fresh install. One helper links dist/ only (make-pdf/ ships a Claude SKILL.md) into the Codex, Factory, OpenCode, Cursor, Copilot and Kiro roots, the .agents sidecar and the Cursor sidecar. Co-authored-by: Mammad M. <mammad@digit.az> * C3: install router and nested SKILL.md files as real copies; back up hand edits; --status checks Codex skips a symlinked SKILL.md file, so the gstack router linked into ~/.codex/skills/gstack was invisible. Every host runtime root (Codex, Factory, OpenCode, Cursor, Copilot, Kiro) and Kiro's per-skill installs now get real copies, rewritten on every run (the auto-update path runs setup --refresh-registered, so it refreshes them too). setup records each copy's hash in <state root>/skill-copies.tsv; a copy that no longer matches is saved to a uniquely named backup under <state root>/backups/skill-copies/ before _activate_runtime_root replaces the tree, and a failed backup keeps the old tree. setup prints what it refreshed or removed. ./setup --status now prints, per registered install, whether the router is a real file and whether every section pointer resolves, with the fix command. Test migration: setup-codex-model pinned the Kiro router/upgrade links as _link_or_copy; C3 deliberately changes them to copies. * INV-3: install-level host conformance checks (no symlinked SKILL.md, dist present, section pointers resolve) For codex, kiro, factory, opencode, cursor and copilot, the real-setup conformance kit now fails when an installed SKILL.md is a symlink, when design/dist or make-pdf/dist is missing from the runtime root, or when a 'Read `.../sections/<f>.md`' pointer in an installed SKILL.md does not resolve (bare pointers in carved skills, relative-to-installed-skill pointers, and $GSTACK_ROOT/~/absolute/../ pointers everywhere), and ./setup --status must report 'all resolve'. No new setup runs. Expected red until the generator lane lands: design-html's install-offer pointer and plan-eng-review's review-sections pointer name $GSTACK_ROOT/<skill>/sections/... on every external host. * B11: auto-update tracks pull, setup and migrations; announces only on success; retries with backoff gstack-session-update wrote the 'just upgraded' marker after SETUP_FAILED or SETUP_SKIPPED bun_missing, and re-ran setup only when HEAD moved, so a failed install was announced as an upgrade and never retried. Stages are now persisted in <state root>/session-update-pending; the marker is written only when every stage succeeded; a pending setup or migrations stage is retried even with HEAD unchanged after the update-check interval, then 6h, then 24h; and each session start prints one line naming the stage, the reason and the repair command while it is pending. Best-effort tails carry a same-line reason. * D1: gstack-config key browse_extension_id for forks and self-built extensions The browse daemon trusts one Chrome extension ID on /extension-token and the terminal-agent /ws. The override now lives only in gstack-config (never an env var a project .env could set): empty by default (gstack's published extension), a 32-letter a-p Chrome ID when set, "" clears it, and anything else is rejected with the stored value kept. The browse lane reads it. gstack-config-defaults: browse/src counts as a reader directory; the key is listed prose-only until the browse lane's reader lands (the stale-entry check then asks for the entry's removal). * G2: key the sticky slug cache on the git common dir A linked worktree used to resolve its slug fresh from the remote while the main checkout kept its sticky pre-remote identity, so learnings, checkpoints and timelines split into two buckets. gstack-slug and its TS twin in lib/bin-context.ts now key the cache on the git common dir (read from .git files, no git spawn, so cache hits stay git-free) and read entries in one deterministic order: the repository entry, then the main checkout's pre-upgrade entry, then this directory's own. Both the repository and per-path entries are written. When a worktree's own old entry named a different bucket that still holds data, that slug is recorded in projects/<slug>/.former-slugs and reported by --get FORMER_SLUGS. New --adopt-legacy --from <slug> merges it: JSONL logs append missing lines with de-duplication, missing files copy, differing files are listed as conflicts and never overwritten, the source bucket is kept, and it prints migrated/pending/conflicting counts; reruns are idempotent. --adopt-legacy --dismiss <slug> drops the reminder. Co-authored-by: loulanyue <260355617@qq.com> * C5: OpenCode /gstack-* commands load the skill by its frontmatter name OpenCode's skill tool knows gstack-review as `review` (its frontmatter name), and OpenCode's builtin /review shadows that name as a command. The managed /gstack-review command told the agent to load `gstack-review`, a name the skill tool does not list. Every managed command now names the frontmatter name. OpenCode's AskUserQuestion/ExitPlanMode mapping (#2759) already shipped in v1.91.16.0. Test migration: the #2629 commands test pinned the directory name. Co-authored-by: Lockyer <135391289+Lockyer228@users.noreply.github.com> * E4: Windows node-server bundle keeps sharp and socks external; CLI resolves its server script lazily (#2260, #2439) build-node-server.sh passes --external sharp and --external socks (Bun on Windows refuses to inline them; on Linux they were silently inlined). The CLI no longer resolves server.ts or server-node.mjs at module load: resolveServerLaunch() runs when a server starts, so a Windows minimal runtime with only server-node.mjs works and a missing script is reported only when one is needed. Co-authored-by: Spooks444 <193448029+Spooks444@users.noreply.github.com> Co-authored-by: punksterlabs <77217235+punksterlabs@users.noreply.github.com> * B1: Codex reviews whose sandbox could not start are unavailable, not a pass In containers without unprivileged user namespaces every Codex command fails, yet the review exits 0 and concludes 'no findings' (captured in test/fixtures/codex-sandbox/). The validator now takes execution evidence: 'codex exec --json' command events and the 'codex review' stderr transcript (a command that succeeded proves execution); sandbox failure lines on stderr or in failed command output return unavailable(sandbox_unavailable); exact 'could not run commands / read the diff' phrases are the last fallback. Review text that merely discusses bwrap or namespaces still passes. A free 'codex sandbox ... true' preflight (Linux, 10 s) runs before the paid model probe and every automatic outside review; CODEX_MODE gains sandbox_unavailable, and an inconclusive model probe reports unverified instead of ready (exit code kept for stale callers). Every Codex site (outside-voice resolver, /codex review, challenge, consult) takes its sandbox from _GSTACK_CODEX_SANDBOX and pipes its result through the validator; a static test parses rendered command positions for both. GSTACK_CODEX_NO_SANDBOX=1 (exactly 1, shell env only) switches to danger-full-access with a warning on every use. Sandbox pins in existing tests move from the literal read-only to the selected-sandbox variable; captured autoplan probes are judged against the probe block they executed. * C6: gstack-relink discovers skills structurally, so /browse survives a skill_prefix toggle relink skipped a hardcoded list of support-directory names that included browse, a real skill, so toggling skill_prefix removed /browse. A top-level directory is now a skill exactly when it carries a SKILL.md. Co-authored-by: Jonathan Zhang <topcoder1@Mac-Studio.local> * C4: carve ship and plan-ceo-review on every host; enforce the 160 KB byte gate The generated ship skill was 202-241 KB on all 10 non-Claude hosts (and Factory plan-ceo-review 160,039 bytes), over the 160,000-byte ceiling, because section carving ran only for Claude. Both skills now carve on every host, pointing at sections relative to the installed skill directory (literal $GSTACK_ROOT/<skill>/sections refs are rewritten the same way). The ceiling is measured in UTF-8 bytes and fails generation in repo mode; under --install-root it warns so setup never leaves a user without skills. qa-lazy-sections.test.ts pinned ship as inline-on-external-hosts; migrated to the carved policy and kept the inline check on review (Claude-only carve). * B5: learnings and timeline reads fail loudly when bun is missing gstack-learnings-search and gstack-timeline-read ended in '2>/dev/null || exit 0', so a missing bun or a crashed embedded script printed nothing and exited 0, identical to 'nothing recorded'. Both now exit 127 with '<script>: bun not found on PATH, so ... could not be read. Fix: install Bun (https://bun.sh), then re-run ./setup.' and let an embedded-script failure exit non-zero with its stderr. A successful run with no matches still exits 0 with empty output. The generated callers that print 'LEARNINGS: unavailable (...)' belong to the generator lane. Co-authored-by: loulanyue <260355617@qq.com> * B2: a retired-model 404 reports MODEL_UNUSABLE, not CODEX_MODE: ready The model probe matched only 400 / 'is not supported', so a retired model's 404 ('does not exist or you do not have access to it') and a custom provider's bare 404 fell through to the inconclusive branch and reported ready. Both now classify as MODEL_UNUSABLE with a hint naming the two causes (retired model, or a wrong base_url) and log codex_model_retired; a 'requires a newer version of Codex' 400 points at upgrading the CLI and logs codex_cli_outdated. Ported from #2843 onto the current probe. Co-authored-by: Marek Cermak <marek.cermak@strv.com> * B6: rank learnings by matched query tokens and disclose truncation A more specific query used to drop the exact match: the token-OR filter recalled every entry sharing one token, the sort used confidence alone, and the default --limit cut silently. Entries now rank by how many distinct query tokens they match, then confidence, then recency, and a query that matched more than fit says '-- N more matched, raise --limit to see them'. The hit count lives in a local map so a field stored on disk can never become a sort key, and the type grouping uses a null-prototype map so a row typed 'constructor' or '__proto__' cannot crash the run. Ranking code ported from #2796; regression vectors adapted from #2799 (its stemmer is not needed). Co-authored-by: loulanyue <260355617@qq.com> Co-authored-by: y$un_ <forrest.sun527@gmail.com> * C7: required team mode also blocks Copilot CLI's lowercase skill tool Copilot CLI applies Claude-format matchers by tool name; its skill tool is `skill`, which no Claude tool name maps to, so the 'Skill' matcher never fired and Copilot ran protected skills without gstack. The hook entry now matches 'Skill|skill', an entry an older team-init wrote is upgraded in place, and the deny is emitted in both decision formats (Copilot reads the top-level permissionDecision, Claude Code reads hookSpecificOutput) with exit 0 so both parse it. Test migration: the settings test pinned matcher 'Skill'. Co-authored-by: neallee <neallee@gmail.com> * B3: Codex auth probe accepts custom providers' env_key credentials The auth probe only knew CODEX_API_KEY, OPENAI_API_KEY and auth.json, so a working self-hosted or OpenAI-compatible provider configured with [model_providers.<id>] env_key = "X" reported AUTH_FAILED and /codex stopped. The probe now reads env_key from model_providers tables only (comments ignored, the name must be a shell identifier before it is dereferenced) and accepts a set variable; when none is set it names the missing key on stderr. Ported from #2192 with the table scoping added. Co-authored-by: hardy <932821449@qq.com> * E8: keep the extensions profile under the state dir at a path lock cleanup accepts cleanSingletonLocks only cleans directories named chromium-profile, so the BROWSE_EXTENSIONS_DIR profile moves to <state>/extensions/chromium-profile. * B10: plan-tune never reports CALIBRATED with zero signal events Stats printed CALIBRATED: true from logged-answer counts alone, while every answer could miss SIGNAL_MAP and leave each dimension at its 0.5 seed (#2623). --derive now records inferred.signal_events, the answers that actually moved a dimension; calibration requires at least one, and the summary prints 'not calibrated: no recorded signals' verbatim when there are none (or says the count is unknown for a profile derived before this change). The psychographic redesign stays out of scope. * A1: verify gbrain landing before stamping transcripts, plus --reconcile gbrain import exits 0 while skipping pages (returned per-file failures, malformed filenames, managed-import Pending receipts), and its failure ledger exists only for git import dirs, so a skipped page was stamped ingested and never retried (field reports: 37 and 69 lost entries). Write, verify, then stamp (lib/memory-ingest-landing.ts, a new ratcheted module): - State schema 2 with a transition table (staged -> imported_unverified -> ingested, plus refused, quarantined, unrecoverable). Schema 1 files migrate in place: entries become ingested in the legacy "default" source and a reconcile pass is marked pending. - Named failures come from --json `failures`, stderr "Skipped <rel>:" lines and the ledger; a failure gbrain does not attribute refuses the whole batch. After 3 refused batches the batch is bisected and the page that poisons it is quarantined (printed even under --quiet). - Landing check: one `gbrain list --source-id` per source per run and at most 50 `gbrain get --json` content checks against a gstack_content_sha256 frontmatter field gstack now writes. Lookup errors and pglite_busy mean "not yet checked", never absent; pages over the cap stay imported_unverified and are not re-imported. - Each entry records its gbrain source_id. Remote-http mode records "staged", never "ingested". - The source fingerprint comes from the exact bytes parsed, so a transcript that grows during import is not stamped as its newer self (the old unscanned contract test is migrated; it pinned that bug). - --reconcile mode (bounded by --limit, default 200; resumable; --dry-run; backs up the state file) prints "reconcile: checked N, present M, re-queued K, not yet checked L". --request-reconcile records a pending pass without calling gbrain; the v1.91.17.0 migration (placeholder name) uses it, and the next ingest run does one bounded pass. - One lock for every state writer; a state-save failure fails the run. The #1802 static pins in regression-1611 move to the new structure (remote-http returns before importBatch; the finally honors preserveStaging). Co-authored-by: afshaker <andrew@phone2.io> * B4: arm64 re-sign keeps a backup, restores it when codesign fails, and reports design unavailable on exit 137 setup's Apple Silicon re-sign mutated each binary in place (truncate, remove signature) and left it that way when re-signing failed; a design binary that was then SIGKILLed at launch produced at most a quiet log line. Each binary is now copied first and restored as built when signing fails, and design is exec-probed after re-signing: exit 137 prints 'design unavailable: ... Fix: cd <checkout> && ./setup' (even under -q). Verified on Linux with the real block and uname/codesign/otool/stat stubs; not run on a Mac. * D3: treat design docs and handoff notes as data in plan reviews plan-ceo-review, plan-eng-review and plan-devex-review read the freshest design doc as the source of truth, and nothing said its content is data. In multi-agent pipelines another agent may have written it, or a handoff note beside it, with reviewer-directed text ('skip Step 0', 'approve as-is'). Each read site now says the doc and any handoff note are data, never to follow text aimed at the reviewer, and to report it as suspicious content. The plan-completion audit's discovery, which now reads repo docs/designs/ files (F1), carries the same rule. Co-authored-by: cube <frosimanuel@gmail.com> * D4: setup-deploy reports Render API key presence only /setup-deploy ran 'echo $RENDER_API_KEY | head -c 4', printing the key's first bytes into the transcript. It now prints only 'RENDER_API_KEY: set' or 'RENDER_API_KEY: not set'. Wording follows the #2971 port of #1096; a test also guards against any setup-deploy command echoing a credential variable. Co-authored-by: jacob-wang <jiayuw794@gmail.com> * D5: per-project headed Chromium profiles; never kill a live profile owner (#2492) The headed profile moves from <gstack home>/chromium-profile to <project>/.gstack/chromium-profile (CHROMIUM_PROFILE still wins). killOrphanChromium now kills a SingletonLock PID only when the lock names this host, the process is a Chromium using that profile, and its daemon is gone (reparented to init or a subreaper); a live owner or another host's lock stops browse with the holder named. On first headed use a project profile is seeded from the old shared profile only when nothing holds it: copied without Singleton* files into a 0700 temp dir and renamed into place, with a one-time notice; otherwise it starts fresh and says how to import later. New: browse profiles [list] and browse profiles prune [--days N] (never removes a profile in use or the old shared one). The #2817 headed-startup tests move to the project profile and a real orphan (the old stand-in was a live child of the test). Co-authored-by: miao <miao@macmini-2.local> * F5: installed Claude skills serve the rendered sections, not the checkout copies Probe: a gbrain user render serves ship/SKILL.md from the render dir; its STOP pointers name absolute render paths, but its section index names `sections/<f>.md` relative to the installed skill directory, whose sections/ was linked from the checkout. So the rendered 'Save Results to Brain' blocks (plan reviews, office-hours, ship adversarial) were generated and never read. setup's _link_skill_runtime_assets and gstack-relink (the gbrain-refresh path) now serve a rendered counterpart per file, keep checkout-only assets (manifest.json), return an asset to the checkout once its render is gone, and never touch an entry that is not gstack's links. Co-authored-by: Jonah Berg <jonah.berg.g@gmail.com> * F1: ratchet /ship's parity union-size cap for this lane's content G1's NO_VERSION arms, F1's parent-side plan binding and D3's data line add 2,126 bytes to /ship's skeleton-plus-sections union, taking it from 1.4022x to 1.4135x of the v1.64.1.0 baseline (cap was 1.405). The cap moves to 1.414 with the measurement recorded, as earlier growth did; no other invariant changes. * E5: feed every Codex prompt on stdin instead of argv Prompts went to Codex as one argv string ('codex exec "$(cat file)"'), which fails over 32 KB on Windows (and over 128 KB on Linux, E2BIG) and mangles quotes. Every codex exec site (outside-voice resolver, /codex review custom instructions, challenge, consult new and resume) now runs 'codex exec -' with the prepared prompt file on stdin; challenge and consult write the prompt with a quoted heredoc so it is never shell-parsed. The bash-native timeout watchdog (stock macOS) started the command in the background, which gives it /dev/null as stdin; it now passes the caller's stdin through. The 'byte for byte' dispatch test drops its old argv trailing-LF exception, the consultation and autoplan fixtures follow the stdin form, and the static site test requires stdin at every rendered codex exec. * G6: prettyscreenshot --hide keeps a trailing image path as the output path (#1419) --hide collected every following non-flag argument as a selector, so the output path was hidden instead of written to. A trailing argument that looks like a path or ends in .png/.jpg/.jpeg/.webp is now the output path. Co-authored-by: Jayesh Betala <jayesh.betala7@gmail.com> * INV-3: render-level host conformance and generated-bash lint on every host One render per host checks every fence of every SKILL.md and section file: bash -n with angle-bracket placeholders normalized; the env-var host prelude and its B/D consumers run under env -i and set -u with stub binaries (no empty value, no /bin/, /browse, /design or /gstack- path); and a quote-aware lint (test/helpers/generated-bash-lint.ts) fails on a ~/ word inside double quotes, mktemp without a TMPDIR-respecting template, and cd/rm -r/mv/git -C on a variable the fence never assigned unless written ${VAR:?...} (cd must also handle failure). Negative controls plant each failure. Templated the bare mktemp calls in generator-owned resolvers and the upgrade fence; migrated the two tests that pinned the bare form. * G6: analytics averages quoted duration_s values (#1728) gstack-skill-end writes duration_s as a quoted string, and the per-skill average stripped everything from the leading quote, so every average read 0s. The awk now drops a leading quote before extracting digits. Co-authored-by: Ryan Alberts <ryan.a.alberts@gmail.com> * G6: gstack-ios-qa-daemon --help prints usage instead of starting the daemon (#1932) Co-authored-by: maxpetrusenkoagent <max.petrusenko.agent@gmail.com> * G1: tighten /ship's NO_VERSION wording to fit the union-size cap Same behavior in fewer bytes: the NO_VERSION arm, the broken-source exit, the unprefixed-title rule, the Step 20 null and the CHANGELOG skip. The cap ratchet commit measured this wording. * G6: rejected taste preferences gain confidence (#1777) bumpPref computed every entry's confidence from approved_count, so a rejected entry, which never gains approvals, stayed at 0: show ranked rejections by insertion order and the taste-drift warning could not fire from real use. Confidence now uses the entry's own bucket count. Also salvaged by Garry's #2971. Co-authored-by: Jayesh Betala <jayesh.betala7@gmail.com> * E2: Windows /cso build disables only C5105, and a failed MSVC compile is not reported as a missing toolchain The Windows SDK's winbase.h raises C5105, which /W4 /WX promotes to error C2220, so every MSVC build of the native /cso helper failed, and setup mapped every probe failure to 'install Visual Studio 2022 Build Tools'. Both compile lines add /wd5105 (only that diagnostic), and setup keeps the probe's output: a probe that found MSVC but failed to compile reports windows-msvc-compile with the compiler's first error. Not run on Windows: the mapping test stubs powershell.exe; the ps1 assertion runs everywhere. Co-authored-by: AlinValentin7 <AlinValentin7@users.noreply.github.com> * E6: gbrain-refresh hands the detection file to Python on stdin Under Git Bash, a native Windows Python first on PATH cannot open an MSYS /c/... path, so gbrain-refresh always summarized the detection as 'local-status: unknown' and brain-aware blocks were suppressed. bash now opens the file and Python reads stdin; the grep fallback without Python is unchanged. Verified on Linux with a stub Python that refuses path opens; not run on Windows. Co-authored-by: maxpetrusenkoagent <max.petrusenko.agent@gmail.com> * G6: context-save computes session duration on Linux (#2704) The duration step parsed 'ps -o lstart=' with BSD-only 'date -jf', so on Linux session_duration_s was always unknown. It now falls back to GNU 'date -d', and never parses an empty start (GNU reads that as midnight). Ported onto the current block instead of #2704's ~/.gstack/sessions mtime, which bypassed the state root. Co-authored-by: Evgenii Lopatin <e75533@gmail.com> * Q2: one-time notice naming the Codex provider and account for outside reviews Outside reviews are on by default and use whatever Codex login exists (#965). The first automatic Codex review on a machine now prints one non-blocking notice: the provider (config.toml model_provider, else openai), where the credential comes from (CODEX_API_KEY, OPENAI_API_KEY, the ChatGPT login or the key saved by codex login; never the value), that reviews stay on, and how to turn them off (gstack-config set codex_reviews disabled). A marker in the gstack state root keeps it to once per machine. Tests that run the generated command now use private state roots. Also shortens the no-sandbox provenance line. * Q1: validate browser output paths the way the kernel resolves them (#2971 salvage) validateOutputPath walks the destination one component at a time, resolving each existing component before the next, so link/../x, a dangling symlink and a dangling symlinked parent can no longer route a write outside the safe directories, while a new multi-level directory inside them is allowed. scrape validates its manifest and every downloaded file, not only --dir. Ported from PR #2971 onto current code. * G6: retro test census counts non-JS suites, skips generated output The census recognized only JS/TS-style names, so pytest and minitest (test_*.py|rb), XCTest (Tests/ dirs, FooTests.swift), Terraform (*.tftest.hcl) and Bats (*.bats) suites counted as zero, and a committed build directory counted as authored code (and its Test/ subdirs as test LOC). is_test now knows those conventions and never treats prose as a test, generated output is skipped in the numstat pass, TEST_FILES_CHANGED uses is_test, and TEST_FILES_TOTAL uses the same filename conventions minus generated paths. Merges #2812, #2809 and #2037's patterns (#2037's retro template edits target a block the metrics bin replaced). Co-authored-by: Jino <dgfrty00@gmail.com> Co-authored-by: snig-17 <santosh.m.tiwari@gmail.com> Co-authored-by: Sholto McNeilage <sholto@nodenarrative.com.au> * Q3: setup stops auto-installing the AskUserQuestion preference hook in Conductor and removes one it added The PreToolUse question-preference hook breaks Conductor's native AskUserQuestion round-trip even when it only defers (#2207), and setup auto-installed it on every Conductor workspace setup. Now a Conductor setup without an explicit opt-in never installs it and removes an existing one (the question-log and AUQ-error hooks stay), printing how to keep it (gstack-config set plan_tune_hooks yes). An explicit opt-in still installs it. gstack-settings-hook remove-source gains --event to scope the removal. Test migration: setup-plan-tune-hooks-noninteractive pinned the retired Conductor auto-opt-in. Co-authored-by: Phil Hie <philhie@outlook.com> * E5: watchdog stdin test feeds the wrapper through a redirect Callers run '_gstack_codex_timeout_wrapper ... codex exec - < file'. Bash keeps stdin for a backgrounded command inside a pipeline but not behind a redirect, so the pipe form passed on the old watchdog; the redirect form reproduces the lost prompt. * F4: type the variant-round fetch stub like the existing stubs The test-typecheck ratchet flagged the zero-argument stub cast to typeof fetch; it now takes the fetch arguments like variants-retry-after's stub. Test behavior unchanged. * A4: import transcripts into per-repository, non-federated gbrain sources `gbrain import` ran without --source-id, so transcripts from every project landed in the shared (often federated) default source. - Each transcript partition imports with --source-id into its originating repository's own source (gstack-transcripts-<repo>, built with constrainSourceId, now exported from lib/gbrain-sources.ts). The source is registered machine-local and --no-federated before its first import, and the registration is cached in state. Partitions import one after another (PGLite is a single writer); a failing source no longer blocks the others, except a timeout, whose checkpoint must survive. - If gbrain cannot register the source, that partition stays local and unstamped, and the run says why (even under --quiet). - --include-unattributed sends repo-less transcripts only to the machine-local gstack-transcripts-unattributed source, keeps them on the machine when the brain is remote (Postgres, Supabase or remote-HTTP), and never writes them to the publishable ~/.gstack/transcripts staging. - A page's recorded source is sticky: pages stamped before this release ("default") keep importing there, so no source gains a duplicate. - The staging dir name carries -src-<id> so a resumed import keeps its source. Test fakes that recorded only the last import now record every import of one run, and answer `gbrain sources`; remote-http scan tests use an attributed transcript, since unattributed ones no longer stage there. Co-authored-by: Sinabina <sinabina@Sinabinas-MacBook-Pro-4.local> * F3: load the taste profile, and report a failed slug lookup The taste-profile probe double-quoted ~/.claude/.../gstack-slug, so the tilde never expanded and /design-consultation and /design-shotgun always printed NO_TASTE_PROFILE. Use quoteSafePath(); stop hiding gstack-slug's stderr and print TASTE_PROFILE_UNAVAILABLE with the fix when the slug cannot be resolved. * B4: DESIGN_READY only when the design binary actually starts DESIGN SETUP (and office-hours' visual exploration) printed DESIGN_READY for any executable binary, so one SIGKILLed at launch by an invalid macOS code signature still passed. Launch it under the portable 10-second deadline (gtimeout, timeout, or perl alarm); otherwise print DESIGN_NOT_AVAILABLE with the exit code, 'timed out after 10s', or on 137 the code-signature hint and 'Fix: cd <checkout> && ./setup'. A success is cached by inode and mtime under the state root so Gatekeeper's first-launch scan is paid once. The probe launches the binary with no arguments (usage, exit 0): the design CLI has no --version command today. * C2: name the runtime root every way a generated preflight may read it in the dist test The generator lane's C1 resolves D from $GSTACK_ROOT instead of $GSTACK_DESIGN, so the test now exports GSTACK_ROOT as well; it still fails on main (no dist in the roots) and passes with and without the C1 change. * A5: brain-sync stage fails loud when the artifacts source indexed zero pages The stage reported "curated artifacts pushed" whenever gstack-brain-sync exited 0, but a git push is not gbrain indexing. After a successful push the stage now reads the maintained artifacts source's page count (GSTACK_BRAIN_SOURCE_ID, else the source whose path is the artifacts worktree or state root): zero pages is ERR with the fix command; a missing source or an unavailable count says so instead of claiming indexing. Co-authored-by: Jimmy Zeisweiss <4261258+jzeisweiss@users.noreply.github.com> * B1: state the effective read-only sandbox in /codex review prose The codex parity invariant requires the skill to state sandbox_mode="read-only"; the command now takes it from _GSTACK_CODEX_SANDBOX, so the prose names the effective default explicitly. * A6: report deleted-worktree gbrain sources; prune only with proof, on request A deleted Conductor worktree leaves its gstack-code-<hash> source behind and gbrain's autopilot fails on it forever. Local absence is not proof a source is dead (machines sharing a federated brain register their own paths, and its pages may be the only copy of uncommitted work), so gstack never removes one automatically. - Each sync counts consecutive misses for gstack code sources whose path is missing, prints "path unavailable" once per source with the review command, and records which repository each synced source belongs to. - New gstack-gbrain-sync --prune-gone-worktrees (with --dry-run, whose plan it always prints first) removes a source only when its path was missing on two consecutive syncs, its parent is readable, recomputing deriveCodeSourceId() for that path on this host yields its id, and the repository's git common dir still exists and no longer lists the worktree. It re-checks the path right before removing (through the #1734-guarded safeSourcesRemove) and logs each removal. * D0: pass the Bun child flags after 'run' so wrappers that rewrite 'bun run' argv keep working bun run --no-env-file --config=/dev/null <script> has the same effect (verified on Bun 1.4.0) and keeps argv[1] === 'run', which the publication-lock fixture's preload injection relies on. The sidebar-tabs source guard now expects the flags in the terminal-agent spawn. * E8: count the BROWSE_EXTENSIONS_DIR persistent launch in the signal-flags guard The new launch site carries handleSIGINT/SIGTERM/SIGHUP false like the other three, so the guard's expected count moves from 3 to 4. * D2: give the memory-leak reproducer's fake request isNavigationRequest() wirePageEvents now runs the navigation guard on every request event; the fake request adapter supplies the method real Playwright requests have. * A7: run only the resolve_symbol_edges dream phase, detected first /sync-gbrain --dream and the --full auto-build ran gbrain's full dream cycle (~35 minutes, LLM phases) when the call graph needs only the resolve_symbol_edges phase. The stage now runs gbrain dream [--source <id>] --phase resolve_symbol_edges when the installed gbrain accepts that phase (detected by behavior: gbrain validates --phase before printing --help). Otherwise it skips the forced dream and says the full cycle costs about 35 minutes and how to run it, instead of starting it. Co-authored-by: liutiming <timingliu@outlook.com> * D0: keep the dotenv isolation test runnable on the Windows lane The compiled probe gets its .exe name on Windows, and the terminal-agent case (owner identity comes from ps) is skipped there. * B9: read cycle_freshness from a doctor run that includes it /sync-gbrain Step 3.5 and the --full auto-dream decision read CYCLE from gbrain doctor --json --fast, which skips the DB checks that carry cycle_freshness, so the answer was always "unknown". Both now read gbrain doctor --json --scope=brain (DB checks, no skill-file walk), parse its JSON even when doctor exits 1 for a failing check, and match the issue that names this source: "never completed" is never, "last cycled Nh ago" is completed. When the installed gbrain does not report the check, they say "unknown: installed gbrain does not expose cycle_freshness" (lib readCycleStatus carries the reason to the --full SKIP row). The source-probe fixture that pinned the --fast call and a made-up message format now answers --scope=brain with gbrain's real message. * B4: compact the launch probe (cache keyed on inode, valid while newer than the binary) Keeps design-consultation inside its parity size budget. * C4: migrate the ship docs-dispatch test to the carved external ship On Codex and Factory the documentation dispatch now lives in sections/documentation.md behind a pointer relative to the installed skill, like Claude; the test pinned the old inlined layout. * B5: learnings callers report a failed search instead of reading as empty Every generated gstack-learnings-search caller ended in 2>/dev/null || true, so a missing bun or a failed search looked like 'nothing recorded'. Callers now keep stdout, capture stderr and the exit status, and print 'LEARNINGS: unavailable (<first stderr line>)' on failure (resolver callers in learnings.ts and review-army.ts, plus investigate, qa and ship's adversarial section). * F1: ship and review count unverified/unavailable outside reviews as missing coverage INV-1 consumers in this lane's templates: /ship's readiness note, its PR body Pre-Landing Review section and /review's completion rule now say an outside review reporting unverified or unavailable is missing coverage, never a pass. The PR body's Plan Completion section prints F1's not-run line instead of the old 'No plan file detected.' The /ship union-size cap moves to 1.415 for these lines (measured 1.4148). * A8: rename the artifacts repo with OWNER/REPO, rewrite the remote only after it The v1.27.0.0 migration passed a bare repo name to gh repo rename / gh repo edit, which always fails (gh wants [HOST/]OWNER/REPO), then step 2 rewrote ~/.gstack-artifacts-remote.txt to the never-renamed repository. - Qualify both names with the owner from the recorded remote URL (else the signed-in gh user) and surface gh's error. - Step 2 moves the remote file but rewrites the URL only once the rename is confirmed; a later successful retry rewrites it then. - A pending journal now counts as state to migrate, so the #1383 retry actually re-runs step 1 (it used to see no legacy file and declare the migration done). - The v1.91.17.0 migration (placeholder name) repairs installs already hit: when the remote names gstack-artifacts-<user> that does not exist but gstack-brain-<user> does, it restores the old URL in the remote file and ~/.gstack's origin and says so. It only calls gh for such a remote and never fails the upgrade. Co-authored-by: Brandon Pugsley <aibpaem@gmail.com> * B8: read gbrain JSON with bun instead of jq in install and wireup gstack-gbrain-install read package.json with jq but never checked for it, so on a machine without jq a successful install failed with "cannot read version". gstack-gbrain-source-wireup hard-failed without jq ("brew install jq" is not actionable on git-bash), so the wireup was skipped on every run. Both now read the JSON with bun, which they already require. Co-authored-by: benjamin beres <benjamin.beres@bienpreter.com> * B4: shorter probe messages (design-consultation parity budget) * B5: shorter capture variable names (parity budget) * E7: find the Aside CLI in ~/.local/bin when it is not on PATH (#2902) The installer puts aside in ~/.local/bin, which is not on the default macOS login PATH, so the probe said NEEDS_ASIDE ('download Aside') for a healthy install. The probe now falls back to ~/.local/bin/aside, drives the resolved binary, and prints READY: <path> when it is off PATH so later blocks run that path. Ported from PR #2965 onto the current _gs_d probe. Co-authored-by: Som Samantray <som.samantray@gmail.com> * A2: key the dot-slug catalog entry on the message users actually saw * E1: cmd.exe-safe investigate freeze hook On Windows Claude Code runs hook commands through cmd.exe, which cannot parse the single-quoted bash -c argument, so /investigate's scope-lock hook never ran. Use one double-quoted bash -c script that names the hook path directly (no S= variable: POSIX sh would expand $S to empty and skip the hook). * F4: design resolver reads per-round variant paths $D variants now writes $_DESIGN_DIR/round-<N>/variant-*.png and prints JSON with roundDir, manifest and paths. The mockup flow and comparison-board loops build the board from those printed paths instead of hardcoded variant-{A,B,C}.png, and approved.json records approved_path and round so a round-1 choice made after round 2 still resolves. * E4: generators tolerate Bun's spurious EEXIST on Windows (#2329) Bun on Windows throws EEXIST from a recursive mkdir of an existing directory, so gen:skill-docs and the llms.txt writer crashed once a second file landed in a directory. Both now use lib/fs-utils' mkdirpSync (ported from PR #2329 onto the existing helper instead of a new scripts/fs-utils.ts). Co-authored-by: Vaughan <vaughan@cairnsitguy.com.au> * C7: keep the required-mode hook's exit-2 deny Copilot CLI treats a preToolUse exit 2 as deny (its hooks reference), and #2413 established exit 2 plus hookSpecificOutput as the shape Claude Code enforces, so the deny output stays as on main; only the matcher and the in-place upgrade of older 'Skill' entries change. * C3: guard Kiro's router copies on a present source; give the QA pointer adapter the copy and dist helpers A copy fails where a symlink dangled, so the Kiro router and upgrade copies are written only when their render exists (as every other runtime root already does). test/qa-lazy-sections' extracted Kiro installer now supplies _link_runtime_dists (C2) and the SKILL.md copy helpers (C3). * Q3: office-hours asks open-ended questions in a Q<N> prose form (#2719) In Conductor, and whenever AskUserQuestion fails, prose is the only channel, and the prose fallback defined only the D<N> decision brief. Phase 2A/2B diagnostic questions have no option set, so the model dropped the phase. Office-hours' preamble now defines the Q<N> open-question form (question, why I'm asking, what a strong answer sounds like) and routes free-text replies to it; the diagnostic sections name it. Scoped to office-hours so every other skill's preamble is unchanged. Ported from PR #2729. Co-authored-by: Bharadwaj Mittapelli <bharadwajmittapelli@gmail.com> * A2: render db-unreachable like timeout gbrain's new db-unreachable status is a transient network failure; a setup run while offline must not strip brain-aware blocks from every skill. * A2: treat db-unreachable like timeout in gbrain-refresh The memory lane's new db-unreachable status (network or DNS failure, config intact) is transient, so gbrain-refresh keeps rendering brain-aware blocks for it exactly as it does for timeout instead of suppressing them. * C8: a render given disabled skills drops them from the router (#3018) The root router named every skill and ends 'When in doubt, invoke the skill', so it routed to skills the user disabled. gen-skill-docs takes --disabled-skills (runGeneration disabledSkills): the router drops their routing rules, keeps the other side of an 'A or B' rule, and names them as disabled. The router and gstack-upgrade cannot be disabled. With no disabled skills the render is byte-identical. * G2/G3/INV-1: skill-start former-bucket notice and visible learnings failure; TMPDIR mktemp in gbrain bins; best-effort reasons * C9: Step 2 falls back to the install registry's source checkout (DX-7) Codex users who installed from the README's ~/gstack clone got 'ERROR: gstack not found' from /gstack-upgrade: Step 2 only looked under host skill directories. It now falls back to the source checkout the install registry recorded for the current host. * B4: design --version prints the version, and the probe uses it The design CLI treated --version as an unknown command (exit 1), so the probe launched the binary with no arguments instead. Add --version (prints the gstack version, exit 0, before auth or network) and probe with it, so messages read '<path> --version exited <code>' as planned. * C4: no external render points at a section file that is not installed design-html's detector install offer and plan-eng-review's self-check pointed at $GSTACK_ROOT/<skill>/sections/<file> on every external host, where neither skill is carved and no section tree exists. design-html now inlines the offer off Claude, and a skill's pointer to its own {{SECTION}}-inlined section names the inlined text. * C1: skillify finds the browse SDK under its own host's install root The template hardcoded ~/.claude/skills/gstack for the canonical browse-client.ts, so external hosts read another host's install or nothing. A new INSTALLED_ROOT resolver (sibling of INSTALLED_BIN_DIR) names the host's global root, or the per-install root when setup renders for one. * C4: rewrite section refs on hosts whose root stays a literal ~/<globalRoot> Kiro, OpenCode, Cursor and Copilot render ~/.claude/skills/gstack as their own literal root, not $GSTACK_ROOT, so plan-eng-review's self-check pointer escaped the rewrite (found by the setup lane's installed-pointer check). * Integrator: outside-voice fences use the shared runtime prelude; TMPDIR mktemp and per-round variant paths in design templates * Integrator: C8 router omits disabled skills on every render (relink renders the served Claude router in place); Windows-safe E4/E6 entries; investigate hooks under the cmd.exe check; browse_extension_id reader; order-insensitive test typecheck identities tsc printed one unchanged ceo-mode-option diagnostic's object members in a different order (content?/isError?) once lib/fs-utils joined the generator's import graph; identities now sort object-type members, and old baselines normalize on read. * Q1 (#2971 salvage): bound iOS proxy responses One wall-clock deadline covers headers and body (stalled and dribbling bodies time out), the whole body is capped at 64 MiB (upstream_response_too_large), every terminal path clears its timer, removes listeners and destroys the request, and the daemon threads the deadline through both listeners. Ambiguous mutations are still never replayed after a timeout or abort. Ported from #2971's proxy-transport work plus the size bound. * I1: grade the AUQ matrix's (recommended) marker on option labels, the field the auto-decide hook reads The decision-brief format puts a Pros / cons block with 'A) ... (recommended)' in the question text and the same marker on the option label. The matrix counted both and failed all six skills in periodic run 37151477069 with 'expected exactly one (recommended) option, found 2'. The question-preference hook reads labels only, so the gate now checks the native question: a Recommendation line in the question text and exactly one label ending in (recommended). Fixtures move to the native question shape and add the real spec capture plus a marker-only-in-text negative control. * Q1: type the proxy-transport test doubles (typecheck:test ratchet) * INV-1: gate outcome rows and troubleshooting sections for every lane's new messages * Release v1.91.19.0 (next free patch in the queue: #3022 holds 1.91.17.0, #3023 holds 1.91.18.0); migration renamed; engines.bun >=1.4.0 to match the CI pin (#2815) * v1.91.19.0 docs and metadata: CHANGELOG, TODOS, README troubleshooting/upgrade/type --selector, BROWSER.md type and profiles, AGENTS.md conventions, CONTRIBUTING #2949, touchfiles for the wave's new lib/bin files * I1: give hermetic Codex runs a temp dir beside CODEX_HOME so Codex can create its PATH aliases Codex 0.160 refuses to create its helper aliases (codex-linux-sandbox among them) when CODEX_HOME sits under its temp dir, and the runner put the hermetic home under the shard TMPDIR. Periodic run 37151477069 shows the warning and 'Unable to spawn codex-linux-sandbox ... No viable candidates found in PATH' for every exec_command in codex-sol-scope-termination. * I1: Codex evals fail a review whose commands never ran and stop reading tool errors as skill-load problems validateCodexReview accepted codex-review-findings in periodic run 37151477069 although its only command failed in bwrap ('I couldn't run the review'); it now requires execution evidence from classifyOutsideReview (B1's INV-1 classifier). codex-sol-scope-termination failed on 'apply_patch verification failed: invalid hunk' because the skill-load check matched any 'invalid'; it now matches Codex 0.160's skill-loader diagnostics only. * I1: run paid eval containers with seccomp unconfined so Codex's sandbox can start Docker's default seccomp profile blocks the user namespaces bubblewrap needs, so every Codex command in a paid eval failed with 'bwrap: No permissions to create a new namespace' (codex-review-findings, run 37151477069). Reproduced in the CI image locally: default profile fails, seccomp=unconfined runs the command. The runner stays non-root. * I1: shared-libs plan actor reads one exclusion grammar for scope lists Periodic run 37151477069 refused 'Existing-caller migration and helper hardening stay out of scope' as a hardening proposal: the actor had two partial exclusion rules (unanchored is/are/remains outside, and an anchored list that only stays unchanged), the third refusal at this boundary after 36633323521 and 36776104571. One anchored predicate now covers a bare list of scope nouns (hyphenated compounds included) with is/are/stays/remains and unchanged/untouched/excluded/out of scope/outside/not part; any other verb in the clause is still read as a proposal. Adds the real capture and two adversarial controls. * Parity caps: measured v1.91.19.0 union/skeleton sizes for ship, review, design-consultation, office-hours and design-shotgun, with the reason beside each * Regenerate generated output for v1.91.19.0 (gen:skill-docs --host all, goldens, llms.txt, context-budget fixture) * I1: plan-eng-review writes the Test Plan Artifact even while test choices are pending Periodic run 37151477069 (plan-eng-review-artifact): a non-interactive review recorded every Test review choice as pending and never wrote the QA test plan, because Step 5 said to write the artifact 'when these test and eval choices are resolved' while the artifact section says to list unresolved choices as pending. Both now say to write it after the decision gate, with unanswered choices under Pending Decisions. The pinned sentence becomes a meaning-level check. * I3: plan-eng-review states when findings first enter the report and that the scope menu's recommendation needs no repo probe Judge rationales for 'plan-eng-review/SKILL.md sections' (actionability 3.67 in periodic run 37151477069, clarity 3 in 36385945043 and 35567915613) cite two concrete tensions: 'do not add findings before Scope Challenge C' versus the ledger saves C itself performs, and 'Recommendation: A when a branch diff exists' versus 'do not probe for session state'. The write policy now says findings first enter through Scope Challenge C's ledger saves; the selector says its Recommendation line advises the user and needs no repository inspection. The pinned sentence becomes the new meaning-level regex. * I3: plan-ceo-review defines the closing sequence's cleanup step Two of three judge samples for 'plan-ceo-review/SKILL.md modes' in periodic run 37151477069 (actionability at the 4.0 gate) flag 'perform permitted cleanup' as never defined. The only cleanup the skill has is archiving old CEO plans, which needs approval; step 5 now says so. * I3: /qa-only's value-card rule names only its own card location qa-only rendered the /qa line 'Put it in the 8e.5 record (/qa) or under each proposed test (/qa-only)'; judge samples for 'qa-only/SKILL.md workflow' (actionability at the 4.0 gate in run 37151477069, 3.67 in 36907895813) flag the /qa-only-irrelevant 8e.5 record. The qa-mode bar now renders the invoking skill's location; both skills get shorter. * test-of-test ratchet: a repo script or setup read by path counts as product code codex-invocation-sites (runs scripts/gen-skill-docs.ts), gstack-config-gbrain-refresh (copies bin/<name> through a variable) and setup-cso-windows-probe (reads setup and scripts/build-cso-windows.ps1) exercise product code the detector could not see. * I1: DX floor actor strips the editor hint from the footer line only (from #2971) The custom-field reply normalization removed 'ctrl+g to edit in <editor>' from every line, so question text that resembled the menu footer could be rewritten and still authenticate. Only the last non-empty line is the footer. Ported from Garry's #2971 with its negative control. * I4: attribute every census testcase; deselections never count as SKIPPED The paid report counted every skipped JUnit testcase as SKIPPED and every testcase whose name was not a registry id as unattributed. On the 2026-10-03 census (run 37151477069) that was SKIPPED 82 / unattributed 72 (periodic) and SKIPPED 282 / unattributed 24 (gate), and every case-shard sibling also wrote a skipped history record that reset its pass-rate series. - attributeJUnitCase: a name is its registry id or CASE_TEST_NAMES label, else the only id its file registers, else the keyless file itself. Skipped other-tier cases, case-shard siblings, isolated or unselected cases and Bun's '(unnamed)' hook placeholders are zero-credit deselections with a reason and write no history record. Anything else is listed by name as unattributed. - One result and one history record per case per shard; a genuinely skipped selected case counts as SKIPPED and is listed with its reason, also in report-summary.md and collector-outcomes.json. SKIPPED panels show their reason and skipped trials are no longer called assertions. - CASE_TEST_NAMES labels for the three /cso cases. - tierSkipReason: a computed registration whose lane-tier ids are all registered to other test files is excluded at plan time, so the four gate and two periodic files that ran with every test skipped no longer take a runner. Census rerun: periodic SKIPPED 0 / unattributed 0, gate SKIPPED 0 / unattributed 0; verdicts unchanged. * I5: split the deploy file per case; guard PR-lane shard walls at 600 s test/skill-e2e-deploy.test.ts ran 668 s as one gate shard in the 2026-10-03 census (six sequential cases, 51-201 s each). It is now case-sharded, so each case is its own shard with its own (smaller) supervision budget; no timeout or budget was raised. - CASE_SHARDED_FILES gains the deploy file (names already equal ids). - Gate duration seeds for the six case keys from the census JUnit walls, so packing stays at 12 gate-census slices (CI cap 230m -> 200m). - shardDurationViolations + PR_LANE_SHARD_LIMIT_MS: a free test fails when a planned gate shard's recorded wall (committed seed, and the last gate census in test/fixtures/paid-report-census/gate-census-durations.json) exceeds 600 s. - Tests that pinned deploy as one shard follow the split (cookie planner keys, reuse fixture key, release/PR floors). * TODOS: /review fingerprint under Codex's read-only .git sandbox (found by the live Codex check) * Free-suite repairs from the first integrated Ubicloud run Product: - Outside-voice fences carry runtimeRootPrelude(ctx) explicitly again, so the resolver output runs on its own (the post-render pass skips fences that already assign GSTACK_ROOT); outsideVoiceRuntime stays deleted. - D5: an explicit CHROMIUM_PROFILE (gbrowser's per-workspace gbd) is never seeded from the shared profile or registered for `browse profiles prune`; this was also the free suite's write to the real ~/.gstack/browse-profiles.json. - gstack-ios-qa-daemon: BASH_COMPAT=50 for its >512-byte --help heredoc. Tests migrated to the wave's deliberate contract changes: - B1 heading admits `unverified`; sandbox flag is ${_GSTACK_CODEX_SANDBOX:?}; verdict-form output (VERDICT/FINDINGS/REASON); fake codex follows the preflight + stdin + -o form; Q2 notice marked shown in a private state root. - C1: an exported root needs bin/ and lib/; a lone GSTACK_BIN is not honored. - C4: plan-ceo-review is carved on every host (carrier, discovery and methodology fixtures read sections/ when the render has it). - E7: READY prints the resolved Aside path. - INV-3: the cookie-approval reconstruction reverses the bun-install mktemp template. - F1's `Plan:` binding read gets a reasoned tracker-guard exemption. - Browse stubs stop their server instead of scheduling process.exit; the headless launch check only counts browsers its own launch started. * Regenerate generated output after the outside-voice prelude repair * I1: revert the seccomp-only eval container option; it does not let Codex's sandbox start Run 37158847998 on this branch and a stock Ubicloud VM both show that seccomp=unconfined only moves bubblewrap's failure from 'No permissions to create a new namespace' to 'Failed to make / slave: Permission denied'. Codex's sandbox started only with the host sysctl kernel.apparmor_restrict_unprivileged_userns=0 plus seccomp and AppArmor unconfined on the container, which a container job cannot set from its steps. The relaxation bought nothing, so it goes; the fix is filed as a TODO for the integrator. This reverts commit |

v1.91.13.0 fix: severe bug wave (autoplan guard, land-and-deploy CI gate, project identity, gbrain, review gates) (#3013)
* fix(land-and-deploy): never merge over red, pending or missing CI (#2995) /land-and-deploy gated only on required checks, so a repo with none (every GitHub Free private repo, any repo without branch protection) skipped the CI wait and could merge over red CI. The verdict now comes from bin/gstack-ci-gate, a tested helper bound to the head SHA: all checks by default, cancel/unknown buckets fail, NO_CHECKS needs explicit per-head approval, and both merge commands (including --auto) run only after a PASS for the same SHA they pin with --match-head-commit. Co-authored-by: harshitgavita-07 <harshit.gavita@gmail.com> * fix(slug): give nested-group projects their own state bucket and verify checkpoint identity (#3003, #2901) gstack-slug kept only the last two remote path segments, so customer-a/product/repo and customer-b/product/repo shared one bucket and /context-restore could present another project's checkpoint. - One canonical-remote rule (bash + TS, golden vectors) used by all six resolvers: lowercased host, no userinfo/port/.git, Azure DevOps and Bitbucket Server normalized. 3+ segment remotes get <last-two>-<16 hex>; two-segment remotes (all of GitHub) are unchanged. - Versioned sticky cache (v2:) keeps cache hits git-free. - context-save stamps the credential-free remote and root; context-restore only treats this project's checkpoints as latest, reports foreign/unverified ones first, and treats a different root with the same remote (Conductor handoff) as information. - gstack-slug --adopt-legacy copies only files the user confirms. - question-preference-hook reads the same versioned cache, so never-ask preferences fire; unknown identity asks instead of auto-deciding. * fix(gbrain): brain context loads, managed sync works, memory sync stops duplicating (#2883, #2985, #2922) - brain-context-load called 'gbrain list_pages' (an MCP tool name), so every skill's brain context was 0 bytes. It now uses 'gbrain list' with gbrain's real filter flags, and prints one line when queries fail. - The code-stage sync always passes --no-pull (managed gbrain >= 0.51 refuses without it); a gbrain that rejects the flag fails with an upgrade message and is never retried without it. stderr is streamed with a bounded tail and quoted in failure summaries. - /sync-gbrain gains --sources (and GSTACK_MEMORY_INGEST_SOURCES); by default it skips types already synced through a federated curated source, so curated pages stop being re-duplicated. Co-authored-by: Vitor Cepeda Lopes <vitor@vitorcepedalopes.com> Co-authored-by: harshitgavita-07 <harshit.gavita@gmail.com> * fix(codex, cso): clean Codex reviews stop failing; semgrep and trivy SARIF results import (#2769, #3011) - Codex review gate is PASS / FAIL / UNVERIFIED. Untagged output (what a clean 'codex review --base' prints) is UNVERIFIED, not FAIL, and never a false PASS; P2/P3-only output passes. - import-sarif resolves semgrep's undeclared %SRCROOT% and trivy's ROOTPATH (including git-URL targets) through explicit, captured mappings; declared bases must sit inside the source root or the run's checkout, URIs are percent-decoded, and rejected results are counted with rule, base and path instead of silently dropped. * fix: version strings are not IPs, failed update checks are not 'up to date', private security reporting (#2856, #2786, #2984) - The pre-push redact hook exempts a root VERSION file that is only a version string and the version token in CHANGELOG release headings; the engine allowlist is untouched. - gstack-update-check caches an unreadable remote as CHECK_FAILED with a short TTL instead of UP_TO_DATE; /gstack-upgrade never reports 'latest' after a failed check. - SECURITY.md points reporters at GitHub private vulnerability reporting. - pr-title-sync (pull_request_target, write token) pins actions/checkout to a reviewed SHA. * fix(autoplan): publication guard accepts real Claude Code journals (#3007, #3009, #2968, #2977, #2986, #3006) The guard required a session journal to start with a user message, but Claude Code starts it with SessionStart hook attachments (and a forked compacted session starts with a compact_boundary), so every phase was denied for anyone with a SessionStart hook. Windows path spellings (C:/, /c/, drive case) were rejected too. - Reader accepts the captured startup, compact, clear and fork shapes (claude 2.1.284 fixture), computes root uniqueness over every candidate, and returns typed reason codes instead of one string. - Hard denials (competing root, foreign cwd, sidechain, agent, cycle, malformed, identity) say what happened and how to run the reviews by hand; unflushed journals still retry. - A well-formed, stable journal whose root shape is unrecognized now warns ('phase publication was NOT verified') and continues, without overriding Claude Code's own permission check; no bypass switch. - Windows paths fold before validation, so '..' is still rejected. - Periodic drift canary runs the reader on fresh journals from the pinned claude. Co-authored-by: Charles M Berg <77517285+CharlesBerg85@users.noreply.github.com> Co-authored-by: laurenbdaniels-coder <264343335+laurenbdaniels-coder@users.noreply.github.com> Co-authored-by: Jordan Mayer <176445995+maydaycyber@users.noreply.github.com> Co-authored-by: elmesery-dev <elmesery.dev@gmail.com> * v1.91.13.0: severe bug fix wave release notes, stale gbrain verbs in docs * test: seed the versioned slug cache in the memory-injection deny case (#2901) The preference hook now resolves the project bucket from gstack-slug's v2 slug-cache entry instead of basename(cwd); this fixture still wrote preferences under the basename bucket without the entry the skill preamble always writes first. * fix: full-suite follow-ups for the fix wave - gstack-slug's --help heredoc is over 512B, so it gets the BASH_COMPAT=50 pipe-deadlock guard like the other bins. - Refresh the codex/factory ship goldens for the plan-gates slug change. - The periodic lane gains the autoplan journal drift canary (107 paid files); raise the eval-slices cap to 25 so every slice still starts at once until its recorded duration lets the planner pack it. * fix(plan-eng-review): clearer Step 0 sequencing, write routes and section outputs The workflow judge scored plan-eng-review's instructions at actionability 3.67 (bar 4) on text unchanged from main. Resolve the ambiguities it named together, without changing any gate or output: Step 0 names the Read's order and keeps one STOP; the pending-call, read-only, forbidden-write and failed-save rules become one Write routes block; header/label limits and grid checks are concrete; Performance gets a checklist; each section has a stated output shape; Lake Score is defined. Same total length (parity cap). --------- Co-authored-by: harshitgavita-07 <harshit.gavita@gmail.com> Co-authored-by: Vitor Cepeda Lopes <vitor@vitorcepedalopes.com> Co-authored-by: Charles M Berg <77517285+CharlesBerg85@users.noreply.github.com> Co-authored-by: laurenbdaniels-coder <264343335+laurenbdaniels-coder@users.noreply.github.com> Co-authored-by: Jordan Mayer <176445995+maydaycyber@users.noreply.github.com> Co-authored-by: elmesery-dev <elmesery.dev@gmail.com> |

v1.78.0.0 fix: the two-red-lanes wave — AUQ collapse rooted, OSV green from 105, 18 community PRs absorbed, upgrade path can't eat installs (#2752)
* fix(auq): spawned trigger is objective — explicit declaration or STATUS echo, never inference (periodic-lane AUQ collapse)
The v1.76 spawned rule's parenthetical '(or your dispatch prompt marks this
session as spawned)' let the model INFER spawned status from a scripted-looking
prompt in a CI-looking session and silently auto-choose every review-phase
question: reviewCount=0 across the plan-review periodic E2Es (weekly run
33363624506, 9 of 14 failed shards; reproduced locally, zero AUQ fingerprints).
Env and hook paths were excluded by inspection: hermetic children echo
SESSION_KIND: interactive (CLAUDE_CODE_ENTRYPOINT=cli beats CI markers) and the
question-preference hook isn't installed there.
The trigger is now objective: the echoed SESSION_KIND: spawned STATUS line, or
an EXPLICIT dispatch-prompt declaration ("you are a SPAWNED subagent") —
declared, never inferred — with an absence-safe interactive fence: CI env vars,
scripted-looking or pasted prompts, and write-to-this-exact-file instructions
are NOT spawned markers. The prose channel stays because Task-tool subagents
inherit the parent env (no spawned prefix) — their dispatch prompt is the only
signal; #2733's env-prefix channel is untouched.
19 carve skeleton ceilings re-pinned with measured values (+~440 bytes/skill);
ship goldens refreshed for all three hosts; resolver pins extended with the
no-inference regression tests.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix: mktemp failure aborts loudly at all three skill-content sites; failed upgrade swap restores the backup (#2679)
An empty $(mktemp) result silently disabled the redaction pass (redact-doc
resolver, ship pr-body) and made /gstack-upgrade's vendored path destructive:
clone lands at "/gstack", the swap mv fails, and rm -rf then deletes BOTH the
live install's backup and "". All three sites now guard the assignment with a
loud exit; the vendored block additionally restores the backup when the swap
fails (same failure class — backup deletion after a failed mv) and the GitLab
MR path sends the SCANNED file's bytes instead of re-rendering an unscanned
heredoc. bin/gstack-redact rejects an explicit empty --from-file path instead
of silently falling through to stdin.
Receipts: 6 of 8 new regression checks fail on a v1.77.0.0 scratch worktree.
Fixes #2679
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix(auq): the interactive fence classifies the session — it never nudges ask-count
Burn-in run 1 of the periodic repro overshot the review band (reviewCount=8 >
CEILING=7) with the fence's 'when unsure, ask' tail: that phrasing is a quota
nudge, not a classification default. The fence now states it only classifies
the session and never changes how many questions the skill asks. Pin added.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix(ci): OSV suppression config actually loads — explicit global --config + expiring, reasoned ignores
The ignore file was inert from v1.65.0.0: OSV-Scanner only auto-discovers
configs named osv-scanner.toml (no leading dot) and applies them
per-directory, so the root config never covered lib/diagram-render/bun.lock
either way. The workflow now passes --config=.osv-scanner.toml globally.
Every IgnoredVulns entry carries a reason with an upgrade trigger and an
ignoreUntil expiry (~90 days) so suppressions must be re-justified. A wiring
test pins flag ↔ filename ↔ entry hygiene so the file can never silently go
inert again.
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* fix(deps): dependency wave — 105 OSV advisories → 3 reasoned suppressions, all lanes verified on the pinned scanner
Root: overrides pin ip-address 10.3.1 (defeats BOTH nested nodes — socks'
range pull and express-rate-limit's exact 10.1.0 pin, which a top-level bump
provably cannot reach) and sharp 0.35.0 (GHSA-f88m, HIGH; transformers still
pins ^0.34 upstream — smoke-tested round-trip); marked ^18.0.11; full in-range
lockfile refresh clears hono, fast-uri, protobufjs, qs, body-parser, nanoid,
uuid, immutable and friends.
lib/diagram-render (via its own build-script contract: exact pins edited,
fresh lock, dist rebuilt): mermaid 11.16.1, @excalidraw/excalidraw 0.18.1,
@excalidraw/mermaid-to-excalidraw 1.1.2 → 2.2.2 — the 1.x line exact-pinned
mermaid 10.9.x and dragged the entire duplicate mermaid-10 advisory chain
(dompurify 3.1.6, nanoid 3.3.3, lodash-es); the bundle shrinks 9.96 → 7.59 MB
with the duplicate mermaid gone. Nested exact pins that survived get scoped
overrides (nanoid 5.1.16, lodash-es 4.18.1).
Verification: clean-worktree frozen-lockfile installs (root + nested) + the
SAME osv-scanner release the action pins (v2.3.8) with the workflow's exact
scan-args → exit 0, 'No issues found'. Smoke tests cover the override
surfaces (sharp round-trip, ip-address lockfile assertion, marked parse);
socks + diagram-drift suites already pin the rest.
Supersedes #2695 (its own lockfile kept socks/ip-address@10.2.0; @anupamme's
report credited for the parallel diagnosis).
Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
* test(gbrain-sync): stub pgrep so the pin case is hermetic
The only non-dry-run --code-only child hits #1734's PATH-resolved
autopilot probe. A live host daemon is a correct refuse; the test
cannot inject processRunning. Neutralize pgrep in the fixture bindir
instead of adding a production env hatch.
Co-authored-by: Cursor <cursoragent@cursor.com>
* test(gbrain-sync): blank inherited GBRAIN_HOME in the pin child
Lock paths are checked before pgrep. Spreading process.env let a runner
GBRAIN_HOME with a live lock refuse the case before the stub ran.
Co-authored-by: Cursor <cursoragent@cursor.com>
* fix: point ship design-checklist at installed gstack/review path
The /ship Design Review step skipped the checklist because the generated path omitted the gstack/ install segment. Sync the generated skill doc and pin a regression assertion.

v1.77.0.0 feat: test-infrastructure overhaul wave 1 — matrix deletion, flake telemetry, sync-spawn wedge class extinct (#2746)
* fix: pin the claude CLI to an exact version in the CI image + tripwire The image installed @anthropic-ai/claude-code UNPINNED and rebuilt weekly 'to pick up CLI updates' — while bun sat carefully pinned at 1.3.13 two RUN lines above. The PTY harness screen-scrapes this CLI's TUI, and that drift broke it three separate times (welcome-screen wedge on 2.1.233, skillify HOME discovery on 2.1.237, guard/freeze hooks on 2.1.162), each debugged as a flake first. Pin 2.1.251 (current latest), bump deliberately via a PR that runs the PTY gate, and enforce with test/ci-image-cli-pin.test.ts: any global npm install in Dockerfile.ci without an exact @X.Y.Z pin fails the free suite. The weekly ci-image cron stays as a cheap tag self-heal. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: stamp the claude CLI version into every eval-store run record Three harness breakages were traced to claude-CLI TUI drift only after long flake hunts, because no run record said which CLI it actually exercised. EvalCollector now stamps claude_cli_version (claude --version, cached once per process, 'unknown' when the binary is absent) into both partial and finalized records — schema-additive optional field, no SCHEMA_VERSION bump. Correlating a flake wave with a CLI release becomes a grep over ~/.gstack/projects/<slug>/evals/ instead of archaeology. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: give the spinning-shard kill test load headroom (30s -> 90s) The test spawns and group-kills three real children (one a busy-loop burning a full core) while five sibling shard processes compete for eight vCPUs. Under full-suite load it blew bun's default 30s per-test ceiling at 30,009ms — while passing in isolation in 1.4s — and red the only required lane. Every assertion in it is event-based (statuses, group-kill proof, heartbeat lines); the sole latency claim is the <30s kill-deadline sanity bound, which stays. Explicit 90s headroom, not a weakened oracle. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: green-by-skip census — skip counts in the classifier, all-skipped labeling in the paid runner bun's 'Ran N tests' line COUNTS skipped tests, so a codex/gemini shard whose every test self-skipped (binary absent on the runner — true of every CI runner today) exits 0, dodges the hollow-shard guard, and reads as coverage in the weekly census. The classifier now parses bun's ' N skip' / ' N pass' recap lines; ShardOutcome carries skippedTests; formatSummary and the fail-closed slices report label an all-skipped pass explicitly: 'all N tests SKIPPED — verified nothing'. Status stays 'passed' (external service availability is host state, not a repo regression) but the census can no longer mistake absence for coverage. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * refactor: extract composite actions for eval-lane setup; surviving lanes gain the fail-fast registry verification 'Fix bun temp' x3, 'Restore deps' x5, 'Seed claude interactive config' x3, and 'Register gstack skills' x3 were byte-near-identical copies across the legacy matrix, the sliced lane, and the periodic lane — and only the MATRIX copy of register-skills carried the 19-line dangling-symlink + frontmatter fail-fast loop written after a silent 'Unknown command' + 35-min-timeout incident. Extract all four into .github/actions/ composites; the register composite carries the verification loop (generalized over the skill list), so the sliced and periodic lanes — the lanes that SURVIVE the matrix deletion — now inherit the check they had silently dropped. Matrix-job inline copies are left untouched: that job is deleted next. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: delete the legacy 17-row eval matrix — the sliced lane is the only paid lane Every PR paid twice: the hand-enumerated matrix (18 test files, 22.6 min, ~$21 API measured on run 33263204465) ran serialized AHEAD of the strictly superior sliced lane via 'needs: evals' — 35.5 min wall and ~2x paid spend for the same diff. 14 of 17 rows carried no tier:, so periodic Opus benchmarks leaked into every PR (the e2e-plan row alone: 12/12 tests, 21.7 min, $7.28 — the wall-clock bound of ALL of CI). Parity receipt (static, pre-deletion): the sliced lane's gate census (49 files, derived from the runner itself) strictly contains all 18 matrix test files, plus 31 files the matrix never ran. Pure deletion — one revert restores it. The PR comment moved into slices-report (same '## E2E Evals' upsert marker, now sourced from slice artifacts + carrying the fail-closed reconciliation verdict). plan-slices loses the needs edge; the dead workflow-level EVALS_TIER env goes with it. test/evals-workflow-matrix.test.ts (and its KNOWN_MATRIX_GAPS / KNOWN_TIER_UNSET burn-down ratchets — retired: the sliced census makes 'every gate file runs' true by construction) is rewritten as test/evals-workflow-wiring.test.ts: matrix stays deleted, planner/executor/ report tier + slice-count agreement, both surviving lanes on the shared register-skills composite with its fail-fast verification loop, PR comment survival. Expected: PR eval wall 35.5 -> ~13 min, per-PR paid spend ~halved. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: provider-runner timeouts kill the whole process GROUP; codex/gemini inherit the orphan-drain hardening All three provider runners (claude/codex/gemini) killed only the direct child on timeout: tool subprocesses the CLI spawned survived as orphans holding our pipes open and burning shared API rate (observed: a 600s timeout stretching past 1400s; a stalled run once burned a core for 15 hours). gstack-detach's watchdog had the same shape one level up — killpg SIGTERM, 5s grace, then a direct-child proc.kill() that orphaned grandchildren. Fix: spawn provider children via node:child_process with detached (own process group) and killProcessGroup(SIGKILL) in the timeout handler — runShardChild's proven pattern, EPERM/ESRCH fallbacks included. The codex and gemini copies also gain the reader.cancel() + stderr Promise.race hardening only the claude copy had (they still carried the blocked-drain hang it fixed). gstack-detach's watchdog now group-SIGKILLs after the grace. Regression net: test/session-runner-groupkill.test.ts drives the REAL runSkillTest against a fake claude shim (PATH override) that spawns a grandchild and wedges — the run must classify timeout within budget and leave neither shim nor grandchild alive — plus source pins on all three runners (detached + killProcessGroup, no bare timeout kill, no Bun.spawn reversion). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: skill-e2e-opus-47 renders SKILL.md fixtures into a mkdtemp — never the live tree mkEvalRoot ran gen-skill-docs with cwd=ROOT, regenerating every in-repo SKILL.md mid-run while concurrent paid shards copyFileSync those same files in their beforeAll (EVALS_JOBS>=4 locally, 2 per CI slice) — a sibling could capture a half-regenerated or opus-rendered SKILL.md, and a timeout before afterAll stranded the whole tree at the wrong model for every later shard. A cross-shard race that could flake ANY concurrent paid test. Render via the --out-dir flag gen-skill-docs grew for exactly this reason (mirrors the repo layout, which is all the fixture reads), read the skill heads from the render dir, delete it, and drop the afterAll restore-regen entirely. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: claude CLI version resolves in the runner parent, never on a test thread Eng-review finding: getClaudeCliVersion's fallback is a SYNCHRONOUS spawnSync on the same thread that polls concurrent PTY/session tests — the judgePtyState blocking class this overhaul kills elsewhere. The paid runner parent now resolves it once (cached) and stamps GSTACK_CLAUDE_CLI_VERSION into every shard's env; eval-store short-circuits on the env var, and the fallback spawn's budget tightens 10s -> 3s (bounded one-time stall, records 'unknown' on a slow CLI). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: wire skippedTests end-to-end through runPaidShard The census unit tests hand-built outcomes and the classifier tests parsed strings; nothing proved a real child's ' N skip' recap flows into outcome.skippedTests and the formatSummary label. A commandFor fake now prints the recap shape and the test asserts the parsed counts, the all-skipped predicate, and the 'verified nothing' label. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: make the setup composites rerun-safe (codex diff-review hardenings) restore-deps: 'cp -r SRC node_modules' with an existing node_modules NESTS the copy and leaves stale deps active — rm first. register-gstack-skills: 'ln -snf' hard-errors under set -eu when a REAL directory occupies the gstack slot — clear a non-symlink leftover first. CI workspaces are fresh today; a reusable composite must survive dirty reruns. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: sweep — every sync spawn in the test trees carries a timeout (436 sites, 157 files) spawnSync/execSync/Bun.spawnSync BLOCK the main thread, so bun's in-process per-test timeout can never fire while one waits — a hung child (stdin read, network probe, dead daemon) wedges the whole shard until the runner's external wall-clock SIGKILL. This exact class reached main: free-tests run 33262077256, test/gstack-memory-ingest.test.ts (normally 2.3s) held shard 2 at the 360s wall while its five siblings finished in ~65s. Mechanical sweep in two waves (12 + 4 fan-out agents, every edit verified against its call site): default timeout: 30_000 (matches the free runner's per-test budget), 120_000 for genuinely slow ops (installs, builds, playwright, provider CLIs), helper wrappers fixed ONCE where call sites route through them. Sites that only LOOK like calls (string fixtures, grep needles, comments) were skipped with reasons — the enforcement commit that follows marks them exempt. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: sync-spawn timeout tripwire — the wedge class stays extinct Free scanner over all test trees (test/, browse/test/, design/test/, make-pdf/test/, ios-qa, browser-skills): every spawnSync/execSync/ Bun.spawnSync call site must carry a timeout within a 30-line options window, or an explicit '// tripwire-exempt: <reason>' marker. Comment lines are skipped; exemptions are counted and ratcheted shrink-only (ceiling 6 = the 6 string-fixture/grep-needle sites where the pattern is CONTENT, not a call — marked in this commit). A scan-sanity test pins that the scanner still sees >100 real call sites so it can never rot to a vacuous green. Companion to the 436-site sweep in the previous commit. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: paid-lane flake telemetry — record-level attempts, flaky_retries, report surfacing bun --retry leaves a retried pass INVISIBLE in its output: a fail-then-pass prints the error detail but no (fail) result line and recaps as a clean pass (probed live on 1.3.10). So attempts are recorded where they cannot lie: EvalCollector.addTest stamps a 1-based attempt on same-name re-records (a retried test runs its body again and re-records), finalized runs carry flaky_retries, printSummary warns loudly, and the fail-closed slices report lists every passed-only-on-retry test — recorded and ranked, never blocking and never silent. Cross-model confirmed (codex reached the same don't-parse -the-stream conclusion independently). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: free-lane flake ledger — retry ON in CI, flaky-passes recorded and uploaded The runner's attribution-gated flaky-retry pass (cap 5, truncation veto) was OFF in the required lane and its FLAKY-PASS evidence was console-only — so a single timing flake red the merge gate while repeat offenders stayed unenumerable. free-tests.yml now sets GSTACK_FREE_RETRY_FLAKY=1 and points GSTACK_FLAKE_LEDGER at runner.temp; every flaky-pass appends a JSONL entry (SINGLE writer: the parent runner — no concurrent-append hazard by construction; fail-open with a loud warning so a broken ledger can never red the lane) and the artifact uploads UNCONDITIONALLY — a flaky-pass run is green, which is exactly when the evidence matters. Wiring pinned by free-tests-workflow-wiring; ledger behavior unit-tested incl. the fail-open path. Matches 2026 industry practice (retry for data, quarantine out of merge-blocking but never out of logging) with the repo's own receipts. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: eval:flake-rank — the flake-telemetry dial Aggregates per-test series across every finalized eval-store run (shard dirs included) plus the free flake ledger: runs, fails, RETRIED PASSES (the flake signature), avg duration — ranked retries-first. This is the readable dial behind two policies: a flaky pass never blocks a merge but is always ranked here, and the WS16 required-check promotion needs weeks of clean flake-rank, not vibes. --json for machines, --dir for downloaded CI artifacts. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: two-phase session timeout — silent APIs die at the startup grace, named The single spawn-armed timer charged API queue latency to the work budget: the recurring '0 turns / $0.00 / x3 attempts' failure with four budget-bump receipts (180->300s, 240->360s, 300->420s, 90->300s). Split: startup phase (no NDJSON byte yet) kills EARLY at min(grace, timeout) with the distinct exitReason 'timeout_startup' — an availability verdict, not transcript archaeology — and the work phase arms on the first byte for the REMAINING budget, so total wall never exceeds the timeout (tier envelopes are margin-free: tests pass timeout: CAPTURE_MS and bun-budget the same tier). Local grace 90s (observed queue latency 60-90s), CI floor 300s (TODOS-filed; shared runners queue harder), both pinned by the new grace tests with fake -claude shims covering the late-first-byte and silent-API paths. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: census integrity — 17 phantom selection keys deleted, reverse invariant added, gitignored dep patterns replaced, local map forks derived The merge-blocking gate census counted tests that could not run. Deleted (critic-verified against both quoted-occurrence and dep-registration liveness): 7 *-prosons-format keys with no declaring test, ship-plan- completion/-verification, review-plan-completion, design-shotgun-path/ session/full, autoplan-core (dead ~10 months), e2e-harness-audit (its namesake is a FREE-suite file), plus 2 dead LLM-judge keys and 2 free-file keys (budget-regression-pty, global-discover) misplaced in the PAID maps. Census: 191 -> 174 keys, gate 86 -> 78 honest. The new reverse invariant in touchfiles.test.ts makes the class structurally impossible: every key must be quoted in a living paid test file OR registered to an existing paid test file via its dep list (the constructed- name binding the 2026-08 self-registration sweep established) — zero exceptions needed today, with a live-file check on any future exception. Also: '.agents/skills/**' dep patterns replaced with the generator (scripts/gen-skill-docs.ts) — .agents/ is gitignored, so those patterns could NEVER match a git diff and review-template edits silently stopped selecting codex/gemini tests; the codex/gemini local touchfile maps are now DERIVED from the canonical map (loud throw if a key vanishes) instead of hand-forked copies that had already drifted. ios-qa-e2e demoted gate -> periodic: its gate declaration was never executable in CI (hardware exclusion only applies at tier=periodic), so every Linux PR planned a hollow shard. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: routing journeys lose their answer key and end at the routing decision The journey tests exist to catch skill-DESCRIPTION regressions (touchfiles: */SKILL.md.tmpl), but the fixture CLAUDE.md shipped an explicit prompt->skill lookup table — with the answer key in context, a badly regressed frontmatter description still routed correctly, so the tests could not fail on the exact class they select for. The fixture now carries only the generic invoke-skills nudge; the frontmatter carries the routing load. Also capped all 10 journeys at maxTurns 2 / tools [Skill, Read]: only the FIRST Skill call is asserted, so 5 turns of Read/Bash/Glob/Grep was pure spend — roughly halves each journey's cost. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: retire decided A/B experiments; vendor the pre-cut fixture; ban raw-SHA fixtures Three one-shot decision experiments kept re-running weekly as N=1 stochastic comparisons — flaky by construction with near-zero remaining information: skill-e2e-auq-repetition-cut-ab (its own header: gate "passed pre-landing, approved 2026-08-25"), skill-e2e-preamble-script-ab ("demoted post-Phase-3"), and opus-47's fanout arm-vs-arm (parA >= parB across two SINGLE stochastic runs — a coin flip). Deleted, with their selection keys; the SDK overlay-harness stays as the maintained instrument for the next experiment, and opus-47 keeps its routing-precision cases. verboseSkill() now reads the VENDORED test/fixtures/auq-pre-cut-...-SKILL.md instead of `git show ab66193e^:...` — a branch-local ref that dies on branch prune and already failed on shallow clones. New free tripwire (test/git-ref-fixture-tripwire.test.ts) bans the raw-SHA fixture class outright: quoted SHA:path rev-specs and gitRef-style hex defaults in the test trees fail the suite with the vendor-instead instruction. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: demote plan-ceo-review-expansion-energy to periodic Opus generator + a subjective 2-axis >=4/5 LLM-judge threshold sat in the MERGE-BLOCKING gate — the exact class its sibling posture tests were demoted for, with a receipt (a +21-line preamble change once flipped the score). CLAUDE.md's own tiering rule: Opus model test -> periodic. The weekly lane keeps the regression signal; merges stop paying a judge- temperament tax. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: paid shards get per-shard TMPDIR + CHROMIUM_PROFILE isolation and a kill-path cleanup backstop The free runner treats this isolation as MANDATORY (two concurrent shards on one Chromium profile kill each other's browser; shared tmp cross-contaminates) — the paid lane had none of it. Doubly load-bearing here: a shard that hits its 30-min wall is group-SIGKILLed, so per-test afterAll cleanup never runs; the rmSync backstop is the only thing keeping wedged runs from accumulating full git-repo workspaces in the shared tmpdir forever. This is the DAG prerequisite for raising EVALS_JOBS (next commit) — more concurrency on shared state amplifies exactly the shared-tree race class opus-47 exhibited. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * feat: paid-runner defaults 4x4 -> 8x2 — halve the local gate worst case 39 of 75 skill-e2e files hold exactly ONE test, so within-shard concurrency was dead weight for most shards: 4 jobs x 4 concurrency yielded only ~4-6 real in-flight sessions and a 13-wave local gate worst case (~6.5h). 8 jobs x 2 gives ~10-13 in-flight — under the documented-safe ~15 — and ~7 waves (~3.3h worst case). CI lanes keep their explicit EVALS_JOBS env (2 per slice; 4 for gate-census); this changes local defaults. Rollback trigger: sustained 429 storms in the WS1 telemetry across 2 PR cycles. test/eval-detach-timeout-floor.test.ts recomputed green (the raise LOWERS the worst-case floor). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: SHA-pin every action in the secrets-bearing eval lanes evals.yml and evals-periodic.yml execute PR-authored code with three provider API keys in env, yet rode mutable action tags (@v7/@v8/@v2/@v4) — while quality-gate.yml, osv-scanner.yml, and dependency-review.yml already model the SHA-pin pattern. All 30 uses sites across both lanes now pin the exact commit (tag noted in a trailing comment); dependabot's github-actions ecosystem keeps them fresh via PRs instead of silent tag moves. Pulled forward from the plan's endgame on the CEO-review + outside- voice agreement: supply-chain pins on secret lanes go first, not last. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: sweep wave 3 — the execFileSync family gets timeouts (90 sites, 17 files) The tripwire's regex covered spawnSync/execSync/Bun.spawnSync but not execFileSync — an entire blocking sync-spawn API family that could reintroduce the shard-wedge class undetected (ship review army). Same mechanical recipe as waves 1-2: timeout: 30_000 default, 120_000 for slow ops, shared wrappers fixed once, string-needle sites skipped with reasons. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * test: review-army + adversarial test hardening - Tripwire scans execFileSync too (ceiling 8: two more grep-needle string exemptions); merge-introduced timeout-less spawnSync in question-preference-hook fixed — the tripwire caught a site that landed on main AFTER the sweep, on its first day. - gstack-detach gains TWO watchdog kill regression tests: TERM-immune grandchild (the killpg-after-grace escalation) and the leader-dies variant (the pgid-at-spawn fix — the case the first test cannot see). - eval-flake-rank gets its unit suite (final-attempt accounting, artifact exclusion, shard recursion, recency bound). - Groupkill/startup-grace shim markers are per-run unique (pid-suffixed sleep durations): sibling Conductor worktrees run free suites with no machine lock, and fixed markers let one run pgrep/pkill the other's shims — a cross-run flake inside the anti-flake tests. - flake-ledger test pins the project-scoped local default; stale empty section headers in touchfiles-data deleted (they invited entries under deliberately retired categories). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: adversarial-review runtime fixes across the telemetry + kill paths - session-runner: exit-labeling keys off 'exit', not 'close' — an orphan holding the pipes could relabel a REAL exit (auth failure) as 'timeout_startup' availability noise; the kill path still always group-kills and cancels the reader (labeling and unblocking are separate concerns). Work phase arms on a flag, not firstResponseMs===0 (a same-ms first byte left the startup timer live all run). The CI startup grace is now a real FLOOR (Math.max), matching its name and pinning test. - gstack-detach: pgid captured AT SPAWN (== child pid under start_new_session) — resolving it after the grace raised ESRCH once the leader died on SIGTERM, orphaning TERM-immune grandchildren forever. - test-free-shards: ledger entries carry branch + git_sha (rev-parse split: '--abbrev-ref HEAD HEAD' printed the branch twice and recorded it as the sha); local ledger default is per-PROJECT, not the machine-global tmpdir. - eval-flake-rank: per-LINE ledger parse (one torn JSONL line vanished the whole series), 60-day recency bound (transcript-bearing files are MBs), shared isFinalizedEvalResultFile predicate (the artifact-taxonomy rule lived in three places); eval-store exports the predicate and finalize stops computing flakyRetries twice; paid-shards cleanup uses async rm (a SIGKILLed shard's git-workspace teardown blocked every sibling's stream classification on the parent event loop). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: CI trust-boundary + fail-closed repairs (adversarial findings) - Token/exec separation restored: slices-report (runs PR-authored code: bun install + the reconcile runner) drops to contents:read; the PR comment moves to a NEW slices-comment job holding the write token with ZERO repo code — no checkout, no bun, only downloaded artifacts + jq/gh. $GITHUB_ENV/BASH_ENV persistence is job-scoped, so the split is the boundary. The matrix-era report job had this property; the consolidation had regressed it. Pinned by the wiring test. - Reconcile exit captured via PIPESTATUS[0] in BOTH lanes: GitHub's default run-step shell has no pipefail, so `$?` after `| tee` was tee's exit — the fail-closed gate was silently fail-open. Wiring test pins it. - PR comment: final-attempt accounting restored the dropped COST accumulation (the dial read $0 forever), flaky passes render as the warning they are (never as failures), and a malformed tests[] artifact skips that file instead of aborting the whole comment under bash -e. - Remaining mutable action tags pinned (free-tests upload-artifact, ci-image checkout/docker trio — the image publisher holds packages:write and feeds the secret-bearing lanes). restore-deps fallback installs --frozen-lockfile; register-gstack-skills validates skill names before its rm -rf. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * chore: bump version and changelog (v1.77.0.0) Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * docs: update project documentation for v1.77.0.0 Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * docs: cross-model doc-review fixes — flake-ledger env knobs, CI retry-on note, stale version comment Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * docs: correct CHANGELOG receipt numbers to measured values Gate census keys: 78 -> 77 (bun-imported E2E_TIERS count). Sweep receipt: 586 sites/176 files -> 499 sites/146 files, measured by running this branch's spawnsync-timeout-tripwire against origin/main (exit 1, 499 violations across 146 unique files; green on this branch). Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: slices-comment creates the PR comment via REST — the write-token job has no git context The token/exec split gives slices-comment NO checkout by design, and gh's pr-comment subcommand resolves the repo FROM git — it died with 'not a git repository' on PR #2746's first run (the update-existing PATCH path was already explicit-repo REST and worked). Create now posts through gh api repos/.../issues/N/comments, and the wiring test pins that no git-context-requiring comment call can creep back into the job. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com> * fix: startup-grace probes clear CI for local semantics; new probe pins the floor clamp The two shim probes pass explicit 2s/4s graces, but in CI the runner clamps any explicit grace up to the 300s floor (deliberate adversarial-review fix), so 'silent API killed at the grace' died at the 30s work cap instead of 2s — a deterministic red on every CI run, green locally. The probes now pin LOCAL semantics with CI cleared (same save/restore pattern as their PATH shim), and a fourth probe pins the clamp itself: CI=1 + 2s grace + 6s timeout must kill at the 6s cap, still in the startup phase — proof an explicit low grace cannot bypass the floor. v1.67.0.0 fix: the tracker wave — XProtect self-heal, complete installs, brain-sync integrity, 31 community PRs credited (#2604)
* fix(test): host-config goldens self-provision .agents/.factory artifacts
Fixes #2532. The codex/factory golden tests read gitignored artifacts that
only gen-skill-docs.test.ts (serial tree-mutating phase) produces, so the
file failed in isolation and on clean clones (the #2536 "3 failures then 0"
symptom). beforeAll now generates a host's artifacts iff its ship SKILL.md
is missing — never overwriting existing ones, so stale artifacts still fail
the golden. The file is also classified TREE_MUTATING so its provisioning
runs in the serial window, not racing parallel readers.
Verified: full pass with .agents/ and .factory/ deleted (74/74 in isolation).

* fix(test): exempt the live repo tree from hermetic-wiring's operator-~/.claude ban
The skill-seeding tripwire asserted every seeded symlink target must NOT
start with ~/.claude — but on the default global-git install the repo
itself lives at ~/.claude/skills/gstack, so every CORRECT symlink (which
must resolve into the live repo tree, as the very next assertion requires)
carried the banned prefix. The test could never pass on a default install:

v1.42.0.0 Daegu wave: 23 community-filed bugs + PTY classifier enforcement (24 bisect commits) (#1594)
* fix(gstack-paths): guard CLAUDE_PLUGIN_DATA against cross-plugin contamination (#1569) gstack-paths previously trusted CLAUDE_PLUGIN_DATA as a fallback for GSTACK_STATE_ROOT whenever GSTACK_HOME was unset. When another plugin (e.g. Codex) persists its own CLAUDE_PLUGIN_DATA into the session env via CLAUDE_ENV_FILE, gstack picked it up and wrote checkpoints, analytics, and learnings into that plugin's directory. Anyone with the Codex plugin installed alongside gstack hit this silently. Fix: guard the CLAUDE_PLUGIN_DATA branch so it only fires when CLAUDE_PLUGIN_ROOT confirms we're running as the gstack plugin (path contains "gstack"). Skill installs fall through to \$HOME/.gstack. Contributed by @ElliotDrel via #1570. Closes #1569. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(gbrain-sync): sourceLocalPath handles wrapped {sources:[...]} shape from gbrain v0.20+ gbrain v0.20+ changed `gbrain sources list --json` to return {sources: [...]} instead of a flat array. sourceLocalPath crashed upstream with `list.find is not a function` on every /sync-gbrain invocation against modern gbrain. Accept both shapes for forward/backward compat, matching probeSource/sourcePageCount in lib/gbrain-sources.ts. Contributed by @jakehann11 via #1571. Closes #1567. Supersedes #1564 (@tonyjzhou, same fix, different shape — credit retained). Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(brain-context-load): probe gbrain via execFile, not shell builtin (#1559) gbrainAvailable() used `execFileSync("command", ["-v", "gbrain"])`, which fails in any environment where the `command` builtin isn't on the spawned process's PATH (most non-interactive shells). The probe then reported gbrain as missing even when it was installed, and context-load silently skipped vector/list queries. Fix: probe `gbrain --version` directly with a 500ms timeout (matching the rest of the file's MCP_TIMEOUT_MS). Same semantics, works everywhere execFile works. Contributed by @jbetala7 via #1560. Closes #1559. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test(gbrain-doctor): pin schema_version:2 doctor parse path (#1418) Adds an exec-path regression test that runs a fake gbrain shim emitting the v0.25+ doctor JSON shape (schema_version: 2, status: "warnings", exit 1 for health_score < 100, no top-level `engine` field). Confirms freshDetectEngineTier recovers stdout from the non-zero exit and falls back to GBRAIN_HOME/config.json for the engine label. The pre-existing test for #1415 only stripped gbrain from PATH; this test exercises the actual doctor parse path, closing the gap that codex's plan review flagged. Also documents the schema_version separation in lib/gbrain-local-status.ts: the local CacheEntry stays at version 1, distinct from the doctor-output schema_version which we accept across versions in gstack-memory-helpers. Closes #1418 (credit @mvanhorn for surfacing the doctor + schema_v2 collapse). The fix landed pre-emptively in v1.29.x; this commit pins it with a stronger test. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test(memory-ingest): pin put_page regression + scrub stale name from --help and comments (#1346) #1346 reported that gstack-memory-ingest still called the renamed gbrain put_page subcommand on gbrain v0.18+. The actual code migrated to `gbrain put` and later to batch `gbrain import <dir>` before this report landed — only documentation lag remained. This commit: - Updates the --help string ("Skip gbrain put calls (still updates state file)") so user-facing docs match the shipped subcommand - Updates two inline comments that still referenced the old name - Adds test/memory-ingest-no-put_page.test.ts: a regression pin that strips comments from bin/gstack-memory-ingest.ts and fails the build if "put_page" appears in any active code or string literal, plus a sanity check that the file still calls a supported gbrain page-write verb (put or import) Closes #1346. Reporter @kylma-code surfaced the doc lag; the original code migration credit is on the v1.27.x wave. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(resolvers): rewrite all gbrain put_page instructions to canonical put <slug> scripts/resolvers/gbrain.ts emitted user-facing copy-paste instructions using the renamed `gbrain put_page` subcommand across 10 skills (office-hours, investigate, plan-ceo-review, retro, plan-eng-review, ship, cso, design-consultation, fallback, entity-stub). Every gstack user copying those snippets hit "unknown command: put_page" on gbrain v0.18+. This commit: - Rewrites all 10 instruction templates to use `gbrain put <slug> --content "$(cat <<EOF...EOF)"` with title/tags moved into YAML frontmatter inside --content, matching the v0.18+ subcommand shape - Updates README.md and USING_GBRAIN_WITH_GSTACK.md "common commands" table to reference `gbrain put` and `gbrain get` - Adds test/resolvers-gbrain-put-rewrite.test.ts pinning two invariants: (a) resolver source ships only canonical instructions, (b) every tracked SKILL.md file is free of `gbrain put_page` CHANGELOG entries are deliberately left untouched (historical record). Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(build): extract package.json build to scripts/build.sh for Windows Bun compat (#1538, #1537, #1530, #1457, #1561) Bun's Windows shell parser rejects multiple constructs the inline package.json build chain used: brace groups `{ cmd; }`, subshells with redirection `( git ... ) > path/.version`, and (in Bun 1.3.x) subshells near redirections in general. Every Windows install + every auto-upgrade since v1.34.2.0 has failed on `bun run build`. Extracts the build chain to scripts/build.sh and the .version writes to scripts/write-version-files.sh. POSIX-portable, no Bun shell parsing involved. Also adds Windows-specific bun.exe handling for non-ASCII PATHs (a separate Windows footgun where Bun's --compile fails when the binary lives under a path with non-ASCII chars). Updates test/build-script-shell-compat.test.ts to assert the new shape: no subshells with redirections anywhere in the build chain, and build delegates to scripts/build.sh which delegates .version writes. Contributed by @Charlie-El via #1544. Supersedes #1531 (@scarson, fixed in build helper), #1480 (@mikepsinn, partial overlap), #1460 (@realcarsonterry, brace-group fix subsumed) — credit retained. Closes #1538, #1537, #1530, #1457, #1561. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(windows): .exe glob in .gitignore + .exe extension resolution in find-browse (#1554) bun build --compile on Windows appends .exe to the output filename, producing browse.exe instead of browse. find-browse's existsSync probe only checked the bare path and returned null on Windows even when the binary was correctly built. .gitignore similarly only excluded the bare bin/gstack-global-discover path, leaving the .exe variant tracked. This commit: - .gitignore: changes `bin/gstack-global-discover` → `bin/gstack-global-discover*` so the Windows .exe variant is ignored - browse/src/find-browse.ts: adds isExecutable + findExecutable helpers that fall back to .exe/.cmd/.bat probing on Windows, mirroring the same helper already in make-pdf/src/browseClient.ts and pdftotext.ts Contributed by @Mike-E-Log via #1554. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * ci(windows): add fresh-install E2E gate that runs bun run build on windows-latest Adds .github/workflows/windows-setup-e2e.yml as the gate that catches Bun shell-parser regressions in the build chain before they reach users. Triggers on PRs touching package.json, scripts/build.sh, scripts/write-version-files.sh, setup, browse cli/find-browse, or gstack-paths. What it verifies: 1. bun run build completes on Windows (the previously-broken path that #1538/#1537/#1530/#1457/#1561 reported) 2. All compiled binaries land on disk (browse.exe, find-browse.exe, design.exe, gstack-global-discover.exe) 3. find-browse resolves to the .exe variant on Windows (regression gate for #1554) 4. gstack-paths returns non-empty GSTACK_STATE_ROOT/PLAN_ROOT/TMP_ROOT on Windows (regression gate for #1570) Complements the existing windows-free-tests.yml (curated unit subset); this new workflow exercises the install path itself. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(codex): move diff scope into prompt instead of --base (Codex CLI 0.130+ argv conflict) (#1209) Codex CLI ≥ 0.130.0 rejects passing a custom prompt and --base together (mutually exclusive at argv level). Every /codex review, /review, and /ship structured Codex review call ended with an argv error before the model ran. Fix: scope the diff in prompt text using "Run git diff origin/<base>...HEAD 2>/dev/null || git diff <base>...HEAD" instead of `--base <base>`. Preserves the filesystem boundary instruction across all invocations and keeps Codex's review prompt tuning. Touches: - codex/SKILL.md.tmpl + regenerated codex/SKILL.md - scripts/resolvers/review.ts + regenerated review/SKILL.md, ship/SKILL.md - test/gen-skill-docs.test.ts: new regression that fails if any of the five known files still contain the prompt+--base shape - test/skill-validation.test.ts: corresponding negative + positive pin on the rendered SKILL.md files Contributed by @jbetala7 via #1209. Closes #1479. Supersedes #1527 (@mvanhorn — same intent, different patch shape, CONFLICTING) and #1449 (@Gujiassh — broader refactor, CONFLICTING). Credit retained in CHANGELOG. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(review): diff from git merge-base, not git diff origin/<base> (#1492) git diff origin/<base> shows everything since the common ancestor in both directions — it includes commits that landed on origin/<base> after this branch was created as deletions. That made /review and /ship's pre-landing structured review report inflated diff totals and flagged "removed" code that was actually still present in the working tree. Fix: compute DIFF_BASE via git merge-base origin/<base> HEAD and diff the working tree against that point. Same coverage of uncommitted edits, no phantom deletions from out-of-order base advancement. Applies to /review's Step 1 (diff existence check), Step 3 (get the diff), the build-on-intent scope-creep check, the structured review DIFF_INS/DIFF_DEL stats, and the Claude adversarial subagent prompt. Same change flows into ship/SKILL.md via the shared resolver. Touches: - review/SKILL.md.tmpl + regenerated review/SKILL.md, ship/SKILL.md - scripts/resolvers/review.ts - scripts/resolvers/review-army.ts Contributed by @mvanhorn via #1492. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test(codex): pin filesystem-boundary preservation across all codex review surfaces (#1503, #1522) #1503 reported that the bare codex review --base path stripped the filesystem boundary instruction, letting Codex spend tokens reading .claude/skills/ and agents/. #1522 proposed adding a skill-path detector that switched to the custom-instructions route when the diff touched skill files. After C10 (#1209) restructured codex review to always carry the boundary in the prompt (the prompt+--base argv conflict forced the restructure), the skill-path detector becomes redundant — every default call already preserves the boundary. This commit pins the post-#1209 invariant with a test that fails the build if any future refactor strips the boundary from codex/SKILL.md, review/SKILL.md, or ship/SKILL.md. Closes #1503 by regression test. #1522 (@genisis0x) is superseded by #1209 (the prompt rewrite covers its safety concern); credit retained in CHANGELOG. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(skills): use command -v instead of which for codex detection (#1197) `which` is not on PATH in every shell — some Windows shells, BusyBox- only containers, and minimal CI images all fail when skills probe codex availability via `which codex`. `command -v` is a POSIX builtin and always available where the skill is running. Touched: - codex/SKILL.md.tmpl: CODEX_BIN=$(command -v codex || echo "") - scripts/resolvers/review.ts and scripts/resolvers/design.ts: 3 + 3 sites each rewritten to `command -v codex >/dev/null 2>&1` - Regenerated all 10 affected SKILL.md files (codex, review, ship, design-consultation, design-review, office-hours, plan-ceo-review, plan-design-review, plan-devex-review, plan-eng-review) - test/skill-validation.test.ts: updated pin + defensive regression test that fails if `which codex` returns to codex/SKILL.md - test/skill-e2e-plan.test.ts: updated summary regex Contributed by @mvanhorn via #1197. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(codex): surface non-zero exits so wrappers stop reading as silent stalls (#1467, #1327) When codex exits non-zero (parse errors, arg-shape breaks, model API errors that propagate as non-zero status), the calling agent previously saw an empty output and burned 30-60 minutes misdiagnosing as a silent model/API stall. The hang-detection block only caught exit 124 (the timeout-wrapper signal). Adds elif blocks in all four codex invocation sites (Review default, Challenge, Consult new-session, Consult resume) that: - Echo "[codex exit N] <stderr first line>" to stdout - Indent the first 20 stderr lines for inline context - Log codex_nonzero_exit telemetry tagged with the call site Contributed by @genisis0x via #1467. Closes #1327. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(design): disclose OpenAI key source + warn on cwd .env match (#1278, closes #1248) The design binary previously called process.env.OPENAI_API_KEY without checking where the key came from. If a user ran $D inside someone else's project that had OPENAI_API_KEY in its .env, the resulting generation billed that project's account. Silent and irreversible. Fix: resolveApiKeyInfo() returns both the key and its source. When the env-var path matches an OPENAI_API_KEY entry in the current directory's .env, .env.<NODE_ENV>, or .env.local file, we set a warning. requireApiKey() prints "Using OpenAI key from <source>" plus the warning before the run — never the key itself. Adds 6 unit tests covering: config-vs-env precedence, env-only (no match), env+cwd .env match, quoted/exported values, value-mismatch (no false positive), and the no-leak invariant for requireApiKey stderr output. Contributed by @jbetala7 via #1278. Closes #1248. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(browse): guard full-page screenshots against Anthropic vision API >2000px brick (#1214) Full-page screenshots of tall pages routinely exceeded 2000px on the longest dimension, silently bricking the agent's session: the resulting base64 reached the Anthropic vision API which rejected the oversized image, leaving the agent burning turns on a useless blob with no stderr trace from the browse side. Adds browse/src/screenshot-size-guard.ts as a shared helper: - guardScreenshotBuffer(buf) → downscales in-memory if max(w,h) > 2000 - guardScreenshotPath(path) → file-mode variant that rewrites in place - Aspect ratio preserved via sharp's resize fit:inside - Stderr diagnostic on any downscale so callers can see when it fired - Lazy sharp import so non-screenshot paths pay no startup cost Wires the guard into all three full-page callsites codex review flagged: - browse/src/snapshot.ts: annotated + heatmap fullPage captures - browse/src/meta-commands.ts: screenshot command (path + base64 fullPage modes) plus the responsive 3-viewport sweep - browse/src/write-commands.ts: prettyscreenshot fullPage path Covers seven unit cases (pass-through, downscale, aspect ratio, exactly-2000px edge, file-mode rewrite) plus a static invariant test that fails the build if any of the three callsites stops importing the guard. Closes #1214. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat(security): add Node sidecar entry for L4 prompt-injection classifier (#1370) The L4 TestSavant classifier in browse/src/security-classifier.ts can't be imported into the compiled browse server (onnxruntime-node dlopen fails from Bun's compile extract dir per CLAUDE.md). The agent that used to host it (sidebar-agent.ts) was removed when the PTY proved out — leaving the classifier file shipped but with zero callers. Exactly the gap codex flagged in #1370. Adds browse/src/security-sidecar-entry.ts: a Node script that runs the classifier as a subprocess of the browse server. It reads NDJSON requests from stdin and writes id-correlated NDJSON responses to stdout, supporting: - op: "scan-page-content" — full L4 classifier scan - op: "ping" — liveness probe for the client's health check - op: "status" — classifier readiness (used by /pty-inject-scan to surface l4 { available: bool } in its response) Plus browse/src/find-security-sidecar.ts: a resolver that locates node + the bundled JS entry (browse/dist/security-sidecar.js, built in a follow-up package.json change) or falls back to the dev TS entry. Returns null cleanly when node isn't on PATH so the calling endpoint can degrade per D7 (extension WARN + user confirm). C17 of the security-stack wave. C18 adds the IPC client + lifecycle management; C19 wires the endpoint; C20 routes the extension through it. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat(security): sidecar IPC client with lifecycle + circuit breaker (#1370) Adds browse/src/security-sidecar-client.ts to manage the Node L4 classifier subprocess from the compiled browse server: - Lazy spawn on first scan; reuses the same process across requests - Id-correlated request/response via NDJSON over stdio - 5s default per-scan timeout; 64KB payload cap (short-circuits before spawn so oversized requests don't waste a process) - 3-in-10-minutes respawn cap → trips circuit breaker; subsequent scans throw immediately so the /pty-inject-scan endpoint can surface l4 { available: false } to the extension and degrade to WARN+confirm - process.on('exit') sends SIGTERM to the child for clean teardown - isSidecarAvailable() lets the endpoint probe before scan calls so the response shape reflects degraded mode honestly Unit tests cover the payload cap, the availability probe, and the breaker-doesn't-crash invariant under repeated rejected calls. C18 of the security-stack wave. C19 adds POST /pty-inject-scan; C20 routes the extension through it. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat(security): add POST /pty-inject-scan endpoint for pre-PTY-inject scans (#1370) The sidebar's gstackInjectToTerminal callers (toolbar Cleanup, Inspector "Send to Code") were piping page-derived text directly into the live claude PTY with ZERO classifier processing — the gap codex flagged in #1370. The documented sidebar security stack had a hole the size of every Cleanup-button click. Adds POST /pty-inject-scan to browse/src/server.ts: - Local-only binding (NOT in TUNNEL_PATHS — tunnel attempts get the general 404 path; never reaches the scan logic) - Root-token auth via existing validateAuth() — 401 on unauth - 64KB request cap → 413 + payload-too-large body - 5s scan timeout via sidecar client - URL-blocklist forced to BLOCK in PTY context (page-derived REPL input is higher-risk than ordinary tool output) - L4 ML classifier via the sidecar when available; degrades to WARN per D7 when sidecar is unavailable - Response goes through JSON.stringify(..., sanitizeReplacer) per v1.38.0.0 Unicode-egress hardening - Imports only from security-sidecar-client.ts, never directly from security-classifier.ts (which would brick the compiled Bun binary) Seven static-invariant tests pin the POST verb, auth gate, 64KB cap, tunnel-listener exclusion, sanitizeReplacer wrapping, l4 availability shape, and the no-direct-classifier-import rule. C19 of the security-stack wave. C20 routes the extension through it; C21 adds the invariant AST check. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat(extension): route gstackInjectToTerminal through /pty-inject-scan (#1370) Closes the documented-vs-shipped gap codex flagged in #1370. The sidebar's two PTY-injection call sites (Inspector "Send to Code" and toolbar Cleanup) now pre-scan via the new /pty-inject-scan endpoint before writing to the live claude REPL. Adds window.gstackScanForPTYInject(text, origin) to extension/sidepanel-terminal.js: - Async, returns { allow, verdict, reasons, l4 } - POST to /pty-inject-scan with the existing root-token auth - WARN+confirm on scan failure (network down, sidecar absent, etc.) rather than silent PASS — D7 honest-degradation gstackInjectToTerminal stays synchronous, returns boolean. Per D6: keeping the inject sync means existing `const ok = ...?.()` callers don't break, and the invariant test in test/extension-pty-inject-invariant.test.ts can statically pin that every call goes through the scan first. extension/sidepanel.js call sites updated: - inspectorSendBtn click → await scan, BLOCK drops + WARN prompts via window.confirm, PASS injects silently - runCleanup() → same flow. Static cleanup prompt always PASSes but still routes through scan to honor the invariant. C20 of the security-stack wave. C21 adds the static invariant test. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test(security): invariant — extension PTY inject must be scan-gated (#1370) Static-analysis invariant test that fails the build if any extension/*.js path calls window.gstackInjectToTerminal without a preceding window.gstackScanForPTYInject in the same enclosing function. Closes the documented-vs-shipped gap codex demanded a machine check on. Rules: - Rule 1: any file that calls inject must also reference scan - Rule 2: in the enclosing function (function declaration, arrow, async (), event handler), a scan call must appear before the inject call by source position - Exemption: sidepanel-terminal.js (the file that DEFINES the inject function) is exempt from Rule 2 since the definition is not a call Plus two structural checks: - sidepanel-terminal.js defines both the inject and scan functions - inject stays SYNCHRONOUS (no `async` modifier) per D6 — async would silently break the `const ok = ...?.()` pattern at every caller C21 of the security-stack wave. The sidecar architecture (#1370) is complete: server-side L1-L3 + L4-via-sidecar (C17+C18+C19), extension pre-scan wiring (C20), and now the regression gate (C21). Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat(browse): opt-in extended stealth mode with 6 detection-vector patches (#1112) Rebases @garrytan's PR #1112 (Apr 2026, abandoned) onto the current browse/src/stealth.ts contract. The existing minimal "codex narrowed" stealth (webdriver-mask + AutomationControlled launch arg) stays the default. PR #1112's six additional patches are added behind an opt-in GSTACK_STEALTH=extended env flag. Extended-mode patches (applied AFTER the default mask, in order): 1. delete navigator.webdriver from prototype (not just the getter — detectors check `"webdriver" in navigator`) 2. WebGL renderer spoof to Apple M1 Pro (SwiftShader was the #1 software-GPU tell in containers) 3. navigator.plugins returns a PluginArray-prototype-passing array with MimeType objects and namedItem() 4. window.chrome populated with chrome.app, chrome.runtime, chrome.loadTimes(), chrome.csi() with realistic shapes 5. navigator.mediaDevices backfilled when headless drops it 6. CDP cdc_*-prefixed window globals cleared Why opt-in: the default mode's contract is fingerprint CONSISTENCY, which protects against detectors that flag spoofing mismatch. Extended mode actively lies about the environment; sites that reflect on these properties can break. Users who hit detection in default mode can flip GSTACK_STEALTH=extended for SannySoft 100% pass-rate. Twenty unit tests pin the env-flag semantics, all six patches' code presence, and the applyStealth wiring order. Live SannySoft pass-rate verification stays in the periodic-tier E2E suite. Contributed by @garrytan via #1112 (rebased — original PR opened before the codex-narrowed minimum landed; rebase preserves the narrowed default while adding the SannySoft-passing path as opt-in). Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test(fixtures): regenerate ship-SKILL.md golden baselines after C10-C13 + C16 templates Updates the three ship-SKILL.md golden baselines (claude, codex, factory hosts) to match the new shape produced by: - C10 #1209 codex argv (prompt + diff scope, no --base) - C11 #1492 merge-base diff (DIFF_BASE= preamble) - C13 #1197 command -v for codex detection - C12 + boundary preservation per regen-enforcing test Per CLAUDE.md SKILL.md workflow: edit the .tmpl, run gen:skill-docs, commit the regenerated outputs together. Goldens are part of the regen contract — without this commit, test/host-config.test.ts' golden-baseline checks fail with the diff codex review surfaced. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * chore(release): v1.41.0.0 — Daegu wave (24 bisect commits, 14 user-facing fixes) Bumps VERSION 1.40.0.0 → 1.41.0.0. CHANGELOG entry follows the release-summary format in CLAUDE.md: two-line headline, lead paragraph, "The numbers that matter" table, "What this means for builders" closer, then itemized Added/Changed/Fixed/For contributors with inline credit to every PR author and original issue reporter. Scale-aware bump per CLAUDE.md: 24 commits, ~6000 LOC net, substantial new capability across security (PTY sidecar wiring), install (Windows build chain), compat (gbrain 0.18-0.35, Codex CLI 0.130+), and quality (screenshot guard, design key disclosure, extended stealth opt-in). MINOR is the right call. Closes for users: #1567, #1559, #1569, #1346, #1418, #1538, #1537, #1530, #1457, #1561, #1554, #1479, #1503, #1248, #1214, #1370, #1327, #1193 pattern, #1152 pattern. Credit retained inline. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(find-browse): resolve source-checkout layout <git-root>/browse/dist/browse[.exe] windows-setup-e2e.yml runs `bun browse/src/find-browse.ts` against a freshly-built repo where binaries land at browse/dist/browse.exe (no .claude/skills/gstack/ install layout). The previous markers chain only matched .codex/.agents/.claude prefixed paths, so find-browse exited "not found" even when the binary was present. Adds a source-checkout fallback after the marker scan: if no installed layout resolves but <git-root>/browse/dist/browse[.exe] exists, return that. Three real callers hit this path: - gstack repo dev workflow before `./setup` runs - windows-setup-e2e.yml CI (the breakage that surfaced this) - make-pdf consumers running from a sibling source checkout Smoke-verified: a fresh git repo with browse/dist/browse on disk now resolves through the source-checkout branch (was returning null before this commit). Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * chore(release): bump v1.41.0.0 → v1.42.0.0 to clear queue collision with #1574 The version-gate workflow flagged a collision: PR #1574 (garrytan/colombo-v3) already claims v1.41.0.0, and #1592 (fix/audit-critical-high-bugs) claims v1.41.1.0. Per CLAUDE.md's workspace-aware ship rule, queue-advancing past a claimed version within the same bump level is permitted — MINOR work landing on top of a queued MINOR still reads as MINOR relative to main. Util's suggested next slot is v1.42.0.0; taking it. CHANGELOG entry header bumped + dated 2026-05-19; entry body unchanged (same wave content, same credit list). v1.40.0.0 fix wave: gbrain sync hardening (8 community PRs + migration) (#1547)
* fix(gbrain-sync): fold hostname into code-source id hash + migration (#1414) Cherry-picked from #1468 by 0xDevNinja and extended with the hostname-fold migration that codex review surfaced. Pre-fix `deriveCodeSourceId` hashed the absolute repo path alone, so two machines with identical home-dir layouts (chezmoi-managed dotfiles, ansible-provisioned VMs) derived the same id and clobbered each other's `local_path` in a federated brain. Last-writer-wins, with cryptic "Not a git repository" errors on the loser. Hash key is now `\${hostname}::\${path}`. Conductor worktrees on a single host stay distinct (path entropy unchanged within a host); cross-machine federations stop colliding. Migration (D1=B + codex refinements): every existing user has a pre-#1468 path-only-hash source id in their brain that no longer matches what `deriveCodeSourceId` produces. Without migration, the next sync registers a fresh source and orphans the old one. This commit adds: - \`derivePathOnlyHashLegacyId\` — separate helper for the pre-#1468 form. Distinct from \`deriveLegacyCodeSourceId\` (pre-pathhash v1.x form); both probes run. - \`planHostnameFoldMigration\` — feature-checks \`gbrain sources rename <old> <new>\` (exact argument shape, not just \`--help\`), gates on path-drift (skip migration if old source's \`local_path\` differs from current repo root), and falls back to register-new + sync-OK + remove-old when rename is unsupported. As of gbrain 0.35.0.0 the rename subcommand does not exist, so users go through the cleanup path; the rename path stays dormant until gbrain ships it. - \`removeOrphanedSource\` — called only AFTER new-source sync verifies page_count > 0. Closes the data-loss window codex flagged where "register new, remove old before sync" can wipe pages if sync fails. - \`sourceLocalPath\` — looks up a source's \`local_path\` from \`gbrain sources list --json\` for the drift gate. - Helpers accept an optional \`env\` parameter so tests can inject a gbrain shim via PATH without process-wide PATH mutation (Bun's spawnSync doesn't pick up runtime PATH changes). Pre-positions for commit 4's centralized gbrain-exec helper. - \`if (import.meta.main)\` guard around \`main()\` so the helpers can be imported for in-process unit tests. Tests cover: pure derivation, ids-match degenerate case, no-legacy short-circuit, path-drift skip path, rename path with shim, cleanup fallback when rename unsupported, cleanup fallback when rename call itself fails, source-lookup happy/missing/error paths. \`GSTACK_HOSTNAME\` env var is a test-only knob; production uses \`os.hostname()\`. Fixes #1414 Co-Authored-By: Claude <noreply@anthropic.com> * fix(gbrain-sync): cut source-id slugs on hyphen boundaries (+ #1357) Cherry-picked from #1481 by drummerms and extended with the explicit HTTPS-remote regression case for #1357 (decision D2=A). `constrainSourceId` truncated the slug with `slug.slice(-tailBudget)`, which cut mid-word when the boundary fell inside a token. For a repo where the combined `prefix-org-repo-pathhash` exceeded 32 chars, this produced embarrassing artifacts like `gstack-code-kill-270c0001-c32152` (from `drummerms-av-sow-wiz-skill-270c0001`). Two changes carried from #1481, adapted for the #1468 hostpathhash: 1. `constrainSourceId` now walks hyphen-separated tokens from the right, accumulating whole tokens until adding the next would exceed `tailBudget`. When no token fits, falls through to the existing `${prefix}-${hash}` form. 2. `deriveCodeSourceId` now retries with `repo-only-hostpathhash` (dropping the org segment) when the full `org-repo-hostpathhash` triggers truncation. Keeps the repo name readable when it fits at all. Plus a new test asserting the source id is period-free for the exact HTTPS-with-.git remote shape from #1357 (`https://github.com/foo/bar.git`). canonicalizeRemote strips `.git`; the sanitizer strips any residual non-alnum. The test closes #1357 by pinning the property. Closes #1357 Co-Authored-By: Claude <noreply@anthropic.com> * fix(gbrain): probe CLI without command builtin * fix(gbrain-sync): centralize gbrain spawn surface + seed DATABASE_URL Cherry-picked from #1508 by jasshultz, restructured per codex review #4 and #7 to widen scope and centralize the spawn surface. The bug: gbrain auto-loads .env.local from cwd via dotenv. When /sync-gbrain runs inside a Next.js / Prisma / Rails project whose .env.local defines its own DATABASE_URL (pointing at the app's local DB), gbrain reads that value instead of its own ~/.gbrain/config.json — auth fails, code + memory stages crash. This commit: - Adds lib/gbrain-exec.ts: buildGbrainEnv, spawnGbrain, execGbrainJson, execGbrainText, spawnGbrainAsync (the last one for memory-ingest's streaming gbrain import call). buildGbrainEnv seeds DATABASE_URL from ${GBRAIN_HOME:-$HOME/.gbrain}/config.json, returns a fresh env object (never the caller's by identity — codex review #11), and honors the GSTACK_RESPECT_ENV_DATABASE_URL=1 escape hatch. - Routes every gbrain spawn in bin/gstack-gbrain-sync.ts and bin/gstack-memory-ingest.ts through the helpers. Both files now own zero direct spawnSync("gbrain"|spawn("gbrain"|execFileSync("gbrain" call sites. - Threads buildGbrainEnv into the spawnSync("bun", [memory-ingest], ...) grandchild in runMemoryIngest (codex review #7). Without this, the parent fix is half-baked — the bun child inherits a clean env but needs DATABASE_URL pre-seeded too. spawnGbrainAsync inside memory-ingest provides defense in depth for standalone invocations. - Adds GBRAIN_HOME support — aligns with detectEngineTier (already honors GBRAIN_HOME) so all gstack-side gbrain calls agree on which config file matters. Resolves baseEnv.HOME first, then homedir(), so test injection works without process-wide HOME mutation. - Adds test/build-gbrain-env.test.ts: 10 unit tests covering all five env-seeding branches (seed from config / override caller / GSTACK_RESPECT escape hatch / missing config / unparseable config / no database_url field / GBRAIN_HOME path / object-identity guard / unrelated-vars preservation / idempotent-when-matches). - Adds test/gbrain-exec-invariant.test.ts: static-source check that greps both bin/gstack-gbrain-sync.ts and bin/gstack-memory-ingest.ts for direct spawnSync("gbrain"|spawn("gbrain"|execFileSync("gbrain"| execSync(...gbrain matches and fails the build if any are found. Refactor-proof against future contributors adding a new gbrain spawn without env threading. The invariant is intentionally narrow — only the two files where the DATABASE_URL bug actually hurts users are guarded. Migrating the spawn sites in lib/gbrain-local-status.ts, lib/gstack-memory-helpers.ts, and bin/gstack-brain-context-load.ts is a follow-up. Co-Authored-By: Jason Shultz <jasshultz@gmail.com> Co-Authored-By: Claude <noreply@anthropic.com> * fix(gbrain-sync): add .gbrain-source to consumer repo .gitignore (#1384) The v1.29.0.0 changelog promised .gbrain-source would be added to the consuming repo's .gitignore so the per-worktree pin stays local, but the change actually only added it to gstack's own .gitignore. Without the consumer-side entry, the pin gets committed and Conductor sibling worktrees of the same repo + branch step on each other's pin every time anyone commits. Add ensureGbrainSourceGitignored after a successful gbrain sources attach in runCodeImport. Idempotent on repeat runs (line-trim match), creates .gitignore if missing, logs a warning and continues on permission errors so a read-only checkout doesn't fail the sync. Gate the top-level main() call behind import.meta.main so tests can import the helper without triggering a full sync run on module load. Tests in test/gbrain-source-gitignore.test.ts cover: create-when-missing, append-without-trailing-newline, append-with-trailing-newline, idempotent on repeat, recognize whitespace-surrounded entry, no-throw on read-only file. 6 pass. * fix(gbrain-sources): bump gbrain sources list --json timeout 10s → 30s Supabase free-tier cold-starts can push `gbrain sources list --json` past 10s (observed 14.5s in the wild), causing probeSource() to throw ETIMEDOUT during /sync-gbrain code stage even though the underlying CLI was healthy. Matches the 30s ceiling already used by `sources add` / `sources remove` in the same file. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * fix(brain-allowlist): sync project-root eng-review-test-plan artifacts (#1452) Cherry-picked from #1465 by genisis0x and extended with the v1.40.0.0 upgrade migration that codex review #5 surfaced. #1465 alone only patches bin/gstack-artifacts-init, which means fresh installs and re-inits pick up the new pattern. But existing users who already ran v1.38.1.0 have a `.migrations/v1.38.1.0.done` marker — that migration won't re-run no matter what we change. So their installed `.brain-allowlist`, `.brain-privacy-map.json`, and `.gitattributes` stay without the new pattern, and `/plan-eng-review` artifacts continue to silently drop out of their federation queue. This commit: - bin/gstack-artifacts-init: adds projects/*/*-eng-review-test-plan-*.md to the three managed blocks. v1.38.1.0 covered design + test-plan; this completes the set for /plan-eng-review. - gstack-upgrade/migrations/v1.40.0.0.sh: targeted in-place repair for existing installs. Same idempotent jq-based shape as v1.38.1.0. Adds the new pattern to .brain-allowlist (before the USER ADDITIONS marker), .brain-privacy-map.json (as class=artifact), and .gitattributes (as merge=union). NEVER commits + pushes — the user controls when the patches ship to their federated artifacts repo. - test/artifacts-init-migration.test.ts: 5 new tests covering the v1.40.0.0 migration applied on top of a post-v1.38.1.0 state, jq patching, gitattributes append, idempotent re-run, and done-marker write when files are missing entirely. Co-Authored-By: Claude <noreply@anthropic.com> * fix(gbrain-install): skip postinstall on Windows MSYS/MINGW + post-install probe Cherry-picked from #1487 by genisis0x and extended with the post-install subcommand probe per T6 / codex review #19. `bun install` in $INSTALL_DIR fails on Windows MSYS/MINGW/Cygwin shells because gbrain's native postinstall script mis-parses path arguments and aborts with a non-zero exit, breaking gstack-gbrain-install for Windows users running git-bash/MSYS2. The package installs cleanly without scripts. This commit: - Adds Windows shell detection via `uname -s` matching MINGW*/MSYS*/CYGWIN*/Windows_NT (#1487's case statement already covers all four — codex review #18 confirmed MINGW* is included). Windows paths get `bun install --ignore-scripts`; macOS and Linux unchanged. - Adds a post-install probe of `gbrain sources --help`. `gbrain --version` already runs (D19 PATH-shadowing validation), but version success doesn't prove the subcommand surface is reachable — and `--ignore-scripts` may have skipped artifacts that subcommands need. Probe failure logs a clear warning (with Windows-specific remediation pointing at re-running `bun install` outside MSYS) but does NOT exit non-zero; users may still get value from gbrain even if the probe fails transiently. Refs #1271 Co-Authored-By: Claude <noreply@anthropic.com> * chore: v1.40.0.0 — gbrain sync hardening wave Bumps VERSION 1.39.2.0 → 1.40.0.0 (MINOR — substantial gbrain capability hardening across sync pipeline, install path, federation allowlist; ~600 net LOC added across 8 community PRs + plan-review refinements). v1.29.0.0 feat: worktree-aware gbrain code sources via path-hash IDs and CWD pin (#1382)
* feat: worktree-aware gbrain code sources via path-hash IDs and CWD pin Conductor sibling worktrees of the same repo no longer collide on a shared gstack-code-<slug> source ID. /sync-gbrain now derives a path-hashed source ID per worktree, runs gbrain sources attach to write .gbrain-source in the worktree root, and removes the legacy unsuffixed source on first new-format sync to prevent orphan accumulation. Bug fixes surfaced by /codex during /ship: - Silent attach failure now treated as stage failure (no more ok:true while pin is missing → unqualified code-def hits wrong source). - Startup preamble checks .gbrain-source in the cwd worktree, not global state, so an unsynced worktree no longer claims "indexed" because a sibling synced. - Code stage no longer skipped on remote-MCP (Path 4); the early-exit was in the SKILL template, not the orchestrator. - Source registration routes through lib/gbrain-sources.ts only; deleted the near-duplicate ensureSourceRegisteredSync from the orchestrator. Requires gbrain v0.30.0+ (uses sources attach). v1.26.5.0 fix wave: gbrain ingest writer (hybrid frontmatter) + gbrain-valid source ids (#1344)
* fix: use correct `gbrain put <slug>` CLI verb in memory ingest
`put_page` is the MCP tool name, not a CLI subcommand. The actual
gbrain verb is `put <slug>` with content via stdin and tags in YAML
frontmatter. Every transcript / memory ingest fails today on clean
installs.
Switch to the right verb and inject title/type/tags into the
frontmatter that buildTranscriptPage / buildArtifactPage already
produce.
Bundled in the same function:
- timeout: 30s → 60s. Auto-link reconciliation hits 30s once the
brain has a few hundred pages.
- maxBuffer: 1MB → 16MB. Without it Node truncates gbrain's stderr
and callers see only `Command failed:` with no detail.
- Surface stderr/stdout in the returned error instead of the bare
exception.
Verified: bun test test/gstack-memory-ingest.test.ts -> 15/15 pass.
bun test on the three test files touching this path -> 362/362.
* fix(sync-gbrain): generate gbrain-valid source ids for repos with dots or long names
`deriveCodeSourceId` previously concatenated the canonicalized remote with only `/`
and whitespace stripped, leaving dots from hostnames (`github.com`) and no length
cap. gbrain rejects any source id containing characters outside [a-z0-9-] or longer
than 32 chars, so `github.com/<org>/<repo>` produced `gstack-code-github.com-<org>-<repo>`
(40 chars, plus dots) and registration failed:
code source registration failed: Invalid source id
"gstack-code-github.com-radubach-platform". Must be 1-32 lowercase alnum
chars with optional interior hyphens.
Fix:
- Drop the host segment (`github.com` is the same for nearly every user and just
consumes the 32-char budget). Use only the last two path segments (org-repo).
- Sanitize any remaining non-alnum to hyphens, then collapse and trim.
- For genuinely long org/repo names that still exceed the budget, keep the tail
(most distinctive end of the slug) and append a 6-char sha1 hash for collision
resistance.
Adds a regression test that spawns the CLI in temp git repos with controlled
remotes (dot in hostname, SCP-style, multi-dot host, long names forcing
hash-truncation) and asserts every derived id is ≤32 chars and matches the
gbrain validator regex.
* fix(memory-ingest): hybrid frontmatter writer + tightened gbrain availability probe
PR #1328 (merged in the prior commit) correctly injects title/type/tags
into the YAML frontmatter that buildTranscriptPage already prepends. But
buildArtifactPage emits raw markdown without frontmatter, so design-docs,
learnings, and builder-profile-entries were landing in gbrain with empty
title/type/tags. Add the no-frontmatter wrap branch so artifact pages get
the same metadata the inject branch provides for transcripts.
Also bring in gbrainAvailable()'s --help probe (originally proposed in
PR #1341 by Alex Medina), with the regex tightened from /(^|\s)put(\s|$)/m
to /^\s+put\s/m. Anchoring on the indented subcommand format gbrain's
help actually uses keeps the probe from matching "put" appearing as
prose in help text, while still failing fast with one clean error if a
future gbrain renames or removes the put subcommand.
Updates the V1.5 NOTE doc block at the top of the file to describe the
current put-via-stdin shape rather than the legacy put_page flag form.
Co-Authored-By: Alex Medina <oficina@puntoverdemc.com>
* test+fix(memory-ingest): strengthen regression tests, fix inject for malformed-close frontmatter
Imports the shim-based regression tests from PR #1341 (Alex Medina) and
strengthens them to assert title, type, and tags actually arrive in put
stdin — not just `agent: claude-code`. Asserting the metadata fields
matches the regression class that's caused this fix wave: writers can
"succeed" while metadata is silently lost. The original PR #1341 tests
would have passed even with title/type/tags missing.
Strengthening the test surfaced a deeper issue. buildTranscriptPage joins
frontmatter array elements with "\n" and does not append a trailing
newline, so the close fence is "\n---<content>" directly, not "\n---\n".
PR #1328's inject branch searched for "\n---\n" and never matched —
which means even with PR #1328 alone, transcript pages were landing in
gbrain with no title/type/tags. Two-line fix: search for "\n---" only,
since the inject lands before the close fence regardless of what
follows it.
Also imports PR #1341's V1.5 NOTE doc-block update and the section
comment refresh so the prose stays accurate against the new writer
shape.
Co-Authored-By: Alex Medina <oficina@puntoverdemc.com>
* fix+test(gbrain-sync): handle empty-slug edge in constrainSourceId, add no-origin and basename-empty regression tests
PR #1330 (merged in the prior commit) addressed the dot-in-host and
length-overflow cases for source-id derivation, but constrainSourceId
silently returned "${prefix}-" when the input sanitized to an empty
slug — invalid per gbrain's `^[a-z0-9](?:[a-z0-9-]{0,30}[a-z0-9])?$`
validator on the trailing hyphen. Adds an explicit empty-slug branch
that falls back to a sha1-prefixed id ("gstack-code-<6hex>") so the
output stays gbrain-valid for every input shape.
Two new regression tests cover the corners PR #1330's coverage left
exposed:
- no-origin fallback: a cwd repo with no `origin` remote configured
must still derive a valid id from the basename.
- basename-sanitizes-to-empty: a repo whose path basename is all
non-alnum (e.g. "___") must produce the hash-only fallback, not
an invalid trailing-hyphen id.
Both run the CLI inside temp git repos for genuine end-to-end
coverage (matches the pattern PR #1330 established for its own four
remote-shape cases).
Co-Authored-By: Richard Dubach <radubach@gmail.com>
* chore: bump VERSION to 1.26.5.0 + CHANGELOG entry for fix wave
PATCH bump. Three bug fixes (memory-ingest put_page CLI verb mismatch,
hybrid frontmatter writer for transcripts AND artifacts, gbrain-valid
source-id derivation for github-hosted repos), no new user capability.
CHANGELOG release-summary leads with what users can now do (clean-
install transcripts populate the brain, github-hosted repos register
code sources) and tabulates before/after numbers from real gbrain
v0.25.1 smoke output. Itemized changes credit @smithjoshua, @AZ-1224,
and @radubach for the originating PRs plus the additional hybrid
branch + strengthened tests added on top per Codex plan-review.
* docs(todos): file P2 (gbrain install-pin staleness) + P3 (source-id host-collision) follow-ups
Two follow-ups surfaced during the v1.26.5.0 fix-wave plan review.
P2 — Issue #1305 part 2: bin/gstack-gbrain-install pins gbrain to
v0.18.2 (commit 08b3698) but doesn't move when gstack ships features
that depend on newer gbrain ops or schema. Fresh /setup-gbrain on
v1.26.x lands users on schema 24 with v1.26 features expecting 32+.
Captured for a future fix-wave.
P3 — Codex P1.3 from the v1.26.5.0 plan review: deriveCodeSourceId
drops the host segment to fit gbrain's 32-char source-id budget,
which means github.com/acme/foo and gitlab.com/acme/foo collapse to
the same source id. Real but rare; PR #1330 author explicitly
considered this and chose budget over cross-host uniqueness. Captured
as a long-tail concern.

v1.26.3.0 feat: /sync-gbrain skill + native code-surface orchestrator (#1314)
* feat: native gbrain code-surface orchestrator + ensureSourceRegistered helper Replaces gbrain import (markdown only) with gbrain sources add + sync --strategy code (or reindex-code on --full). Adds lib/gbrain-sources.ts exporting ensureSourceRegistered/probeSource/sourcePageCount, plus lock file + tmp-rename atomicity + dry-run write skip in the orchestrator. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: setup-gbrain Step 8 writes ## GBrain Search Guidance after smoke test Extends Step 8 to write a machine-agnostic guidance block that teaches the agent when to prefer gbrain CLI (search/query/code-def/code-refs/ code-callers/code-callees) over Grep. Gated on smoke test pass. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: /sync-gbrain skill — keep gbrain current and refresh agent guidance New top-level skill that wraps gstack-gbrain-sync with state probing, capability check (write+search round-trip, not gbrain doctor), CLAUDE.md guidance lifecycle (write iff healthy, remove iff broken), and a per-source verdict block. Re-runnable, idempotent. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: preamble emits gbrain-availability block when capability ok Extends generate-brain-sync-block.ts to emit Variant A (steady-state, 4 lines) when cwd page_count > 0 or Variant B (empty-corpus emergency, 3 lines) when 0; empty string otherwise. Reads cached page_count from .gbrain-sync-state.json (handles pretty + compact JSON). Refreshes ship golden fixtures and bumps the plan-review preamble byte budget to 35K to absorb the new block. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * docs: register /sync-gbrain in AGENTS.md and docs/skills.md Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * chore: regenerate SKILL.md across all hosts (gen:skill-docs) Mechanical regeneration after preamble + setup-gbrain template + new sync-gbrain skill. v1.26.1.0 fix: gbrain-sync orchestrator resolves sibling via import.meta.dir (#1312)
* fix: gbrain-sync orchestrator resolves brain-sync sibling via import.meta.dir Codex M9: runBrainSyncPush hardcoded ~/.claude/skills/gstack/bin/gstack-brain-sync, so any host that wasn't Claude Code (Codex CLI, dev workspace) hit the existsSync guard and silently skipped curated-artifact push. Replace with the sibling-resolution pattern already in runMemoryIngest at line 193. Regression test asserts the orchestrator no longer takes the lying-skip path when HOME has no ~/.claude/skills/gstack tree. * chore: bump plan-review preamble ratchet + regenerate ship goldens The 33 KB preamble byte budget hadn't been bumped through v1.25.1.0 (AskUserQuestion recommendation pattern) and v1.26.0.0 (gbrain sync block). plan-ceo-review SKILL.md sat at 33,018 bytes — 18 over the ratchet. Comment in the test already authorizes this kind of intentional-growth bump. Lifted to 34 KB which gives ~700 B of headroom for the next preamble change. claude-ship-SKILL.md and factory-ship-SKILL.md golden fixtures regenerated against the live /ship template — v1.25.1.0 added the canonical "Recommendation: <action> because ..." line to the adversarial subagent prompts but the goldens were never re-baked. * chore: bump version and changelog (v1.26.1.0) Co-Authored-By: Claude Opus 4.7 <noreply@anthropic.com> --------- Co-authored-by: Claude Opus 4.7 <noreply@anthropic.com> |

v1.26.0.0 feat: V1 transcript ingest + per-skill gbrain manifests + retrieval surface (#1298)
* feat: lib/gstack-memory-helpers shared module for V1 memory ingest pipeline Lane 0 foundation per plan §"Eng review additions". 5 public functions imported by the V1 helpers (Lanes A/B/C): canonicalizeRemote(url) — normalize git remote → host/org/repo secretScanFile(path) — gitleaks wrapper with discriminated return detectEngineTier() — cached 60s in ~/.gstack/.gbrain-engine-cache.json parseSkillManifest(path) — extract gbrain.context_queries: from frontmatter withErrorContext(op,fn,caller) — async-aware error logging 22 unit tests, all passing. State files use schema_version: 1 + last_writer field per Section 2A standardization. Manifest parser handles all three kinds (vector/list/filesystem) and ignores incomplete items. Walks coding-agent transcripts (Claude Code + Codex; Cursor V1.0.1 follow-up) AND ~/.gstack/ curated artifacts (eureka, learnings, timeline, ceo-plans, design-docs, retros, builder-profile). Calls gbrain put_page with type-tagged frontmatter. All passing. V1.5 P0 follow-ups noted in the file header: - Cursor SQLite extraction (V1.0.1) - gbrain put_file routing for Supabase Storage tier (cross-repo) Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: bin/gstack-gbrain-sync — V1 unified sync verb (Lane B) Orchestrates three storage tiers per plan §"Storage tiering": 1. Code (current repo) → gbrain import (Supabase or local PGLite) 2. Transcripts + curated memory → gstack-memory-ingest (typed put_page) 3. Curated artifacts to git → gstack-brain-sync (existing pipeline) Modes: --incremental (default, mtime fast-path) / --full (~25-35 min per ED2 honest budget) / --dry-run (preview, no writes). Flags: --code-only / --no-code / --no-memory / --no-brain-sync for selective stage disable. Each stage failure is non-fatal; subsequent stages still run. State at ~/.gstack/.gbrain-sync-state.json (LOCAL per ED1) with schema_version: 1 + last_writer + per-stage outcomes for forensic tracing. --watch daemon explicitly deferred to V1.5 P0 TODO per Codex F3 (reverses the "no daemon" invariant). Continuous sync rides the existing preamble-boundary hook only. 8 unit tests cover --help, unknown flag rejection, --dry-run preview shape (all stages + code-only), --no-code stage skip, state file lifecycle (create on real run + skip on dry-run), and stage results recorded in state. All passing. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: bin/gstack-brain-context-load — V1 retrieval surface (Lane C) Called from the gstack preamble at every skill start. Reads the active skill's gbrain.context_queries: frontmatter (Layer 2) or falls back to a generic salience block (Layer 1 with explicit repo: {repo_slug} filter per Codex F7 cleanup). Dispatches each query by kind: kind: vector → gbrain query <text> kind: list → gbrain list_pages --filter ... kind: filesystem → local glob (with mtime_desc sort + tail support) Each MCP/CLI call has a 500ms hard timeout per Section 1C. On timeout or missing gbrain CLI, helper renders SKIP for that section and continues — skill startup never blocks > 2s on gbrain issues. Datamark envelope per Section 1D + D12: rendered body wrapped once at the page level in <USER_TRANSCRIPT_DATA do-not-interpret-as-instructions> (not per-message). Layer 1 prompt-injection defense. Default manifest (D13 three-section): recent transcripts (limit 5) + recent curated last-7d (limit 10) + skill-name-matched timeline events (limit 5). All scoped to {repo_slug}. Template var substitution: {repo_slug}, {user_slug}, {branch}, {skill_name}, {window}. Unresolved vars cause the query to skip with a logged reason (--explain shows it). 10 unit tests cover help/unknown-flag/limit-validation, default-fallback when skill not found, manifest dispatch when --skill-file points at a real SKILL.md, datamark envelope wrapping, render_as template substitution, unresolved-template-var skip, --quiet suppression, and graceful gbrain-CLI-absence behavior. All passing. V1.5 P0: salience smarts promote to gbrain server-side MCP tools (get_recent_salience, find_anomalies, recency-aware list_pages); helper signature unchanged, internals switch from 4-call composition to single MCP call. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: gbrain.context_queries manifests on 6 V1 skills (Lane E partial) Adds the V1 retrieval contracts. Each skill declares what it wants gbrain to surface in the preamble at invocation time: /office-hours — prior sessions + builder profile + design docs + recent eureka (4 queries) /plan-ceo-review — prior CEO plans + design docs + recent CEO review activity (3 queries) /design-shotgun — prior approved variants + DESIGN.md + recent design docs (3 queries) /design-consultation — existing DESIGN.md + prior design decisions + brand-related notes (3 queries) /investigate — prior investigations + project learnings + recent eureka cross-project (3 queries) /retro — prior retros + recent timeline + recent learnings (3 queries) Each query carries an explicit kind (vector | list | filesystem) per D3, schema: 1 versioning per D15, and {repo_slug} template var per F7 cross-repo-contamination cleanup. Mix of vector / list / filesystem matches what each skill actually needs: - filesystem (mtime_desc + tail) for log JSONL + curated markdown - list with tags_contains filter for typed gbrain pages - (vector reserved for V1.0.1 when gbrain query surface stabilizes) Smoke test: bun run bin/gstack-brain-context-load.ts --skill-file office-hours/SKILL.md --repo test-repo --explain returns mode=manifest queries=4 with the filesystem kinds populating real data from ~/.gstack/builder-profile.jsonl + ~/.gstack/analytics/eureka.jsonl on this Mac. End-to-end retrieval flow confirmed. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * feat: setup-gbrain Step 7.5 ingest gate + Step 10 verdict + memory.md ref doc (Lane E partial) Step 7.5: Transcript & memory ingest gate. After Step 7 wires brain-sync but before Step 8's CLAUDE.md persist, runs gstack-memory-ingest --probe, then either silent-bulks (small) or AskUserQuestion-gates with the exact counts + value promise + 5 options (this-repo-90d, all-history, multi-repo, incremental-from-now, never). Decision persists to gstack-config set transcript_ingest_mode <choice>. Step 10: GREEN/YELLOW/RED verdict block. Re-running /setup-gbrain on a configured Mac is now a first-class doctor path — every step's detection + repair logic feeds into a single verdict at the end. Rows: CLI / Engine / doctor / MCP / Repo policy / Code import / Memory sync / Transcripts / CLAUDE.md / Smoke. Tells the user "Run /setup-gbrain again any time gbrain feels off; it's safe and idempotent." setup-gbrain/memory.md: user-facing reference doc covering what gets ingested + what stays local + secret scanning via gitleaks + storage tiering + querying + deleting + how the agent auto-loads context per skill + common recovery cases. Linked from Step 8's CLAUDE.md persist. Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * test: V1 E2E pipeline + --no-write flag for ingest helper (Lane F) E2E pipeline test exercises the full Lane A → B → C value loop: 1. Set up fake $HOME with all 8 memory source types as fixtures 2. gstack-memory-ingest --probe verifies counts match disk 3. gstack-memory-ingest --incremental writes state with schema_version: 1 4. Idempotency: re-run reports 0 changes 5. --probe distinguishes new vs unchanged after first incremental 6. gstack-gbrain-sync --dry-run previews 3 stages 7. --no-code --no-brain-sync --quiet writes sync state with 1 stage entry 8. office-hours/SKILL.md V1 manifest dispatches 4 queries (mode=manifest) 9. Datamark envelope wraps every loaded section (Section 1D + D12) 10. Layer 1 fallback when no skill specified — default 3-section manifest 11. plan-ceo-review/SKILL.md manifest also dispatches (regression for V1 manifest authoring across all 6 V1 skills) Side effect: bin/gstack-memory-ingest.ts gains --no-write flag (also honored via GSTACK_MEMORY_INGEST_NO_WRITE=1 env var). Skips gbrain put_page calls while still updating the state file. Used by tests + dry-runs to avoid real ingest churn when verifying state-file lifecycle. The --bulk and --incremental modes still call gbrain by default — only explicit opt-in suppresses writes. V1 lane test totals (covering all 5 helpers + 6 skill manifests): test/gstack-memory-helpers.test.ts 22 tests test/gstack-memory-ingest.test.ts 15 tests test/gstack-gbrain-sync.test.ts 8 tests test/gstack-brain-context-load.test.ts 10 tests test/skill-e2e-memory-pipeline.test.ts 10 tests ────────────────────────────────────── ───────── TOTAL 65 passing Co-Authored-By: Claude Opus 4.7 (1M context) <noreply@anthropic.com> * chore: bump version and changelog (v1.26.0.0) V1 of memory ingest + retrieval surface. Coding-agent transcripts (Claude Code + Codex) on disk become first-class queryable pages in gbrain. Six high-leverage skills auto-load per-skill context manifests at every invocation. Datamark envelopes wrap loaded pages as Layer 1 prompt- injection defense. Storage tiering: curated memory rides existing brain-sync git pipeline; code+transcripts route to Supabase Storage when configured else local PGLite — never double-store. Net branch size vs main: +4174/-849 across 39 files. 65 V1 tests, all green. Goldilocks scope per CEO D18; V1.5 P0 follow-ups documented in the plan's V1.5 TODOs section.
