Drops 'the May 2026 transcript bug ... that's the bug, recognize it' and the
caps; keeps the rule: each non-trivial finding goes through AskUserQuestion
before it enters the plan, and only a zero-finding review skips asking. The
four skill-specific branches are unchanged.
W7a pin: test/skill-ceo-section-ordering.test.ts:868-873 pinned the SHA-256
of the default clause. It now checks, on every host, that the clause still
says the plan cannot replace the interactive review, routes findings through
AskUserQuestion, and lets only zero findings reach ExitPlanMode without
asking (case-insensitive), and that the clause is host-independent. The old
clause also passes; deleting the zero-findings rule turns it red. The DX
hash pin is unchanged.
