garrytan 6709bad3c9 fix(preamble): learnings review without incident archaeology (s01 M3) ...

Drops the issue number, the 43-of-44 statistic and the ALWAYS caps from the
Operational Self-Improvement step; keeps the rule that the review runs every
time and that an empty result is stated explicitly. No W7a pins on the
removed text.
