garrytan 69a71f5da2 fix(office-hours): only blocking findings send a design back for another review round ...

Review findings carry severity blocking|minor (verdict schema v2). PASS when
no blocking findings remain; minor findings are recorded in Reviewer
Concerns and the Spec Review table, never a reason for another round.
CONVERGENCE needs a persisting blocking prior. A persisting or unverified
blocking prior cannot be relabeled minor. The 3-round cap is unchanged.
