One wall-clock deadline covers headers and body (stalled and dribbling bodies time out), the whole body is capped at 64 MiB (upstream_response_too_large), every terminal path clears its timer, removes listeners and destroys the request, and the daemon threads the deadline through both listeners. Ambiguous mutations are still never replayed after a timeout or abort. Ported from #2971's proxy-transport work plus the size bound.
