* fix: pre-landing review fixes (GST-1322 B, ship-c-p1)

A start sweep that hit its bounds is a partial note, never a violation
(sweepResult). The sweep tests remove their fixture repos. A stage's rerun
dispatches no reader while a path its first attempt found still exists
(FETCH_RERUN_BLOCKED, the Captain's ruling C on #4011).

* fix: sweep strips remote userinfo; a spent or blocked rerun stops every later stage (GST-1322 B, grant a-7b63e4cad37c)

outside-voice#1: remote URLs are recorded and printed without user:token@.
outside-voice#2: resuming after FETCH_RERUN_SPENT stops before any later stage
and returns no reader output; while a kept path blocks one stage's rerun, no
stage's start (Step 11's voices included) dispatches a reader.
