Garry TanandOpenAI Codex 9f81911136 v1.86.0.0 feat: route outside reviews by harness (#2850)
* feat: add a restricted and supervised Claude Code runner

Preserve configured authentication and models while enforcing tool access, strict completion JSON, bounded output and process cleanup. Cover argv, failure handling, session metadata and Windows process containment.

* feat: route outside reviews by harness and migrate wrapper installs

Use Claude Code from Codex and Codex from other supported hosts, with shared invocation rendering, positive gate validation and per-phase provenance. Rename /claude to /claude-code, repair managed shared and copied installations safely, and generate native Kiro skills. Add installed-workflow, failure-injection and live cross-harness regression coverage.

* test: recognize CEO mode labels without terminal spacing

The paid workflow rendered SCOPEEXPANSION at option 4, but its driver required a literal space. Match the leading mode title without cursor-spacing artifacts and ignore adjacent preview text. Preserve missing-target failures and downstream posture assertions.

* test: isolate plan-count fixtures before starting review workflows

Seed the complete test plan in a private git repository before launching Claude, so a bare slash command cannot review the live workspace while a delayed fixture message remains queued. Preserve count thresholds, parsers and budgets. Add initial-context and installed-discovery tests, and retain startup/terminal diagnostics on failed evaluations.

* test: stabilize review fixtures and Claude eval startup

Preserve source boundaries in workflow judge inputs, isolate CEO mode plans, and wait for interactive trust input readiness. Keep startup failure evidence and retain existing models, budgets, and assertions.

* test: classify collapsed review modes and isolate seeded findings

Keep review questions out of the setup count when terminal cursor positioning removes spaces. State existing webhook safeguards so the five-finding control measures its seeded defects without accidental extra security and concurrency gaps. Preserve question bands and the paired control.

* test: isolate browser daemon state across free shards

Co-Authored-By: OpenAI Codex <noreply@openai.com>

* test: stabilize native review counting and interactive navigation

* chore: prepare v1.82.0.0 release

Co-Authored-By: OpenAI Codex <noreply@openai.com>

* fix: eliminate browser and process-cleanup test flakes

Pin every CI surface to Bun 1.4.0 to avoid extra-stdio finalizers closing
reused live sockets. Add an isolated GC/listener regression that fails on
Bun 1.3.13, and prevent coordinated rollback to an affected CI runtime.

Check renderer cleanup against the render's own staging directory so
concurrent renders cannot invalidate the assertion. Make the no-pgrep
process-tree walk tolerate disappearing /proc entries, and synchronize
its test fixture through child readiness and pipe EOF instead of sleeps.

Validation: 9,157 passed, 31 skipped, zero failures across 556 files with
retries disabled. Build, all-host generation freshness, and skill checks
passed. All three races have failing-before/passing-after regressions.

* fix: count completed native review questions in evals

* fix: drive review navigation from confirmed native choices

* fix: require complete section-loading eval reports

* test: isolate telemetry HTTP transport from local assertions

* fix: keep review input on the active native question

* test: let tunnel revocation daemon choose an available port

* test: allocate available ports for pairing and watchdog fixtures

* fix: stabilize planning eval navigation and phase reporting

* test: isolate installed runtime paths in planning evals

* test: stabilize review evidence and concurrent refresh fixtures

* fix: resolve design findings before editing the plan

* fix: honor and persist disabled outside plan reviews

* fix: preserve planning decisions and terminal evidence

Load installed host reviews at autoplan phase entry and wait for completed
reviewers and saved artifacts. Reuse approved remedies while preserving
individual finding decisions.

Drive interactive evals from the current terminal viewport, bind native
questions across scrolling, and require complete native report evidence.
Cover captured stale menus, permission lifecycles, setup classification,
and disabled-review tool availability with deterministic regressions.

Advance release metadata and the upgrade migration to the unclaimed
1.83.0.0 slot.

* fix: drive native review questions and preserve current plans

Use the native single-choice keyboard protocol and current terminal viewport,
with per-question navigation inside packets and completed-call coverage.
Keep permissions, multi-select menus, and Submit controls distinct.

Send Autoplan reviewers the amended implementation plan, keep its review record
separate, and supply retained application contracts in the chain fixture.
Clarify individual DevEx decisions and complete CEO fix options; use one active
plan destination for the section-loading report.

* fix: preserve complete plan-review decisions

* fix: recognize native plan dialogs and reviewer controls

* fix: preserve review decisions and phase completion

* fix: recognize completed reviews without losing findings

* fix: preserve review continuity and native eval completion

* test: fix native review completion and eval retry isolation

* test: handle native review menus and complete eval fixtures

* test: fix native review setup, completion, and isolation failures

* test: limit native skill discovery to runtime assets

* fix: bind Autoplan reviews to full ordered phase inputs

* test: fix planning eval routing, counting, and timeout handling

* chore: advance queued release to v1.84.0.0

* fix: preserve complete review inputs and planning decisions

* fix: reconcile review approvals and preserve phase obligations

* fix: preserve review obligations and unblock eval permissions

Carry recorded Autoplan requirements into blind phase inputs, require Eng
review approvals before exit, and exercise combined asynchronous flows in
CEO reviews. Correct native finding and handoff classification and unblock
repeated report edits using scoped request identities.

* fix: retain plan requirements and complete native review dialogs

* fix: complete native review prompts and retain plan references

* fix: preserve review inputs and classify native eval evidence

* fix: check competing completion orders in CEO reviews

* fix: recognize review decisions and require phase methodology

Require the current phase methodology before Autoplan snapshots. Correct
substantive decision, closed handoff, and cache-finding classification, and
honor the recommended implementation approach in native review dialogs.

Add captured-transcript regressions without changing review thresholds,
provider models, retries, or deadlines.

* test: bind native review decisions and close completed handoffs

* fix: complete review dialogs and verify methodology delivery

* fix: preserve review evidence and unblock native eval prompts

* fix: handle native review question completions

* fix: recognize native review narration and controls

* fix: recognize current review evidence and scoped edit confirmations

Clarify Design and engineering review entry instructions and Design scoring.
Recognize required legacy coverage and public Autoplan completion recaps.
Bind the pending Edit confirmation to its exact file, ordered digest, and
one-request approval when a preceding command display remains visible.
Keep reviews within their existing size limits and preserve scope gates
when extracting workflow fixtures from either supported preamble header.

Keep failure outcomes, review thresholds, provider choices, and eval budgets.

* fix: recover review workflow progress and eval evidence

* fix: recognize valid review evidence and scope selection

* test: fix review evidence parsing and repeated artifact prompts

* test: recognize valid review decisions and pending native cards

* fix(plan-eng-review): keep final navigation consistent with approved tasks

* test: recognize valid review evidence and bind legacy diff requests

* fix: stabilize review eval evidence and harness repair guidance

* docs: update project documentation for v1.85.0.0

Co-Authored-By: OpenAI Codex <noreply@openai.com>

* test: fix Windows CI fixtures and credential scan

Rebase captured JSON values and filesystem evidence using the appropriate
path convention. Compile native fake CLIs on Windows and synchronize pipe
holder readiness, with cleanup retained when assertions fail.

Assemble synthetic credential fixtures at runtime so the added-line scan
keeps enforcing the same gate without flagging its own rejection controls.

Discover generated skills directly for the empty-find regression check,
avoiding a recursive scan through saved evaluation artifacts and dependencies.

* fix: preserve source renders on Windows

Compare canonical generator paths using native separators so an output
sidecar pointing at the source cannot overwrite its skill or metadata.
Keep the regression fixture isolated from the real checkout and expose
freshness diagnostics before asserting subprocess status.

Detach Windows drain-test pipe holders from the fake provider's automatic
child cleanup while preserving the enclosing runner job and its assertions.

* fix: clarify outside review fallback and CEO decisions

Render one applicable own-harness fallback path and retain native review,
disabled policy, and missing-coverage semantics. Align report field names
and mode labels, and make the existing per-cut scope approval explicit.

Regenerate skill outputs and keep the workflow judge's model, thresholds,
and retry policy unchanged.

* chore: move release to free version slot (v1.86.0.0)

PR #2852 now claims v1.85.0.0. Align the release metadata and
rename migration so upgrades from that version still receive it.

* fix: include engineering review prerequisites and restore branch context

* fix: recognize coverage diagrams and clarify design review instructions

* fix: preserve file identities and join Windows test processes

Iteration-3 items 1-8, 10 and 11 were resolved in the plan's `autoplan-accepted:ceo` block (deep-link return-to preserved; 25% stage time-boxed at 10 business days; `dashboard_variant` event for control attribution; alert-vs-kill windows stated per condition; timeout/SLO/alert labels; permission-error rate defined; empty-notifications CTA = primary action or omitted; "remove the redirect fallback branch, keep the route"; bulk-read snapshot argument confirmed by contract; session floor gets a 10-day duration cap). Unresolved, carried to the /autoplan Final Gate as a taste decision:
- **Improvement gate as blocker vs reported outcome (raised in iterations 2 and 3).** The reviewer holds that the ≥20% median-improvement requirement should never block a stage; the record now applies it only at 25% → 100% with a 10-day time-box that advances anyway. The reviewer would remove it entirely. Primary keeps the time-boxed form because the user set the metric and the guardrails. User decides at the gate.

---
status: ACTIVE
---
# CEO Plan: User Dashboard Page
Generated by /plan-ceo-review (via /autoplan) on 2026-09-10
Branch: main | Mode: SELECTIVE EXPANSION
Repo: gstack-autoplan-chain-kVh2Sb (no remote)

This is the scope-decision record. Implementation detail (envelope type, rollout numbers, test list) lives in the active plan file's "Implementation plan" section inside the `autoplan-accepted:ceo` block; this document points at that block rather than repeating it.

## Baseline (the user's stated scope; one delivery change noted inline)
- `/dashboard` page with three panels: QuickActions, NotificationsPanel, ActivityFeed
- All authenticated members (this is a single-role member workspace; there are no other roles) land on `/dashboard` after login. Stated in the user's plan Context and kept. Delivery change: it ships behind the `dashboard_home` feature flag with a cohort rollout, because it touches the post-login auth redirect path, the one baseline item outside the page's own blast radius
- Loading skeleton, empty, and error state per panel; hover and focus-visible states
- Confirmation modal for "Mark all as read", built on the existing dialog primitive (kept as-is; not a new primitive)
- Toast feedback for actions (success and failure, including rollback)
- One aggregate `GET /api/dashboard` endpoint over existing tables
- Out of scope per user: dark mode, personalization

## Vision

### 10x Check
A home page that talks to you. When a teammate assigns you work, the resume card updates without a reload, the unread badge ticks, and the next action is already focused when the page opens. Concrete shape: SSE channel for notification and activity deltas, optimistic UI, keyboard focus on the primary action once data resolves. Effort: human ~2 weeks / CC ~1 day. Deferred: new transport infrastructure, outside this plan's blast radius. v1 gets the static, measured, accessible version and the shared primitives (per-panel envelope, PanelFrame, Toast) the live version would build on.

### Platonic Ideal
Not run (SELECTIVE EXPANSION mode).

## Definitions used below
- **Eligible:** the existing action registry's server-side predicate for an action returned true for this member in this workspace; the aggregate returns it as a boolean per action.
- **Primary action:** "resume assigned work" when eligible; otherwise the first eligible quick action; otherwise none (focus stays on the page heading).
- **Control cohort:** members who log in during the same window and are not redirected (flag off for them).
- **`serverTime`:** the timestamp in the `GET /api/dashboard` response that rendered the currently displayed notification list.

## Scope Decisions (expansions beyond the baseline)

| # | Proposal | Effort | Decision | Reasoning |
|---|----------|--------|----------|-----------|
| 1 | Relative timestamps ("2 min ago") with absolute-time tooltip, in NotificationsPanel and ActivityFeed | S | ACCEPTED | One helper, two call sites; in blast radius. Sub-decisions: injected clock for tests; labels re-render every 60s while the tab is visible (paused when hidden); future timestamps clamp to "just now" |
| 2 | Unread count badge in NotificationsPanel header, mirrored in the document title | S | ACCEPTED | Count is already in the response; both render from the same panel state so optimistic update and rollback move them together |
| 3 | Optimistic "Mark all as read" with rollback on failure | S | ACCEPTED | Same component; covers the slow-network path. Sub-decisions: snapshot time is `serverTime` (defined above), never the client clock; confirm control disabled while in flight; failure rolls back the list and surfaces an error toast with Retry |
| 4 | Refetch dashboard data on window focus / `visibilitychange` | S | ACCEPTED | Landing pages get left open; one hook. Sub-decision: a focus refetch is deferred until any in-flight mark-all-read request settles (success or rollback), so it cannot clobber optimistic state |
| 5 | State-specific empty copy: NotificationsPanel "You're all caught up" with one quick-action CTA; QuickActions "No actions available right now"; ActivityFeed "No recent changes yet" | S | ACCEPTED | Empty states are already baseline scope; this decides the copy and gives the empty notifications panel a next step |
| 6 | "View all" links from NotificationsPanel and ActivityFeed to the existing full pages (QuickActions has no list to page; no link) | S | ACCEPTED | Panels cap at 20 records; older navigation is owned by the existing pages |
| 7 | Skip link to the primary action, landmark regions, and keyboard focus on the primary action once the quickActions envelope resolves | S | ACCEPTED | Existing a11y policy requires named controls; landmarks and focus placement are the page-level equivalent. Focus rests on the page heading while skeletons show, moves once to the primary action when data resolves, and does not move again on later refetches |
| 8 | Real-time push (SSE/WebSocket) for notification and activity deltas | L | DEFERRED | New transport infra; not needed to hit the 45s login-to-first-completed-task target. Revisit after v1 reaches 100% and cohort data shows members keep the tab open |
| 9 | Smart next-action ranking / pinned or reorderable panels | M | SKIPPED | User out-of-scoped personalization; no ranking service exists. Revisit only inside the separate personalization plan |
| 10 | Server-side response cache for the aggregate endpoint | S | SKIPPED | 3 indexed 20-row queries; a cache adds staleness bugs before it adds speed. Revisit if the endpoint's production p95 exceeds the 300ms budget |

## Accepted Scope (added to this plan)
- Relative timestamps + absolute tooltip (injected clock, 60s tick, future clamp) — #1
- Unread badge in NotificationsPanel header and document title, from one state — #2
- Optimistic mark-all-read with rollback + error toast, serverTime snapshot, disabled-while-marking — #3
- Refetch on visibilitychange/focus, deferred while a mark-all-read is in flight — #4
- State-specific empty copy with a quick-action CTA on the empty notifications panel — #5
- "View all" links on NotificationsPanel and ActivityFeed — #6
- Skip link, landmark regions, focus on the primary action once data resolves — #7

## Baseline scope hardening (decided in review; not expansions)
These make the user's stated scope bulletproof. Exact values and verification are in the `autoplan-accepted:ceo` block.

**Hard obligations (block the 5% stage):**
- Aggregate endpoint returns HTTP 200 with a per-panel `{ ok: true, data } | { ok: false, error }` envelope, a 2s per-fetch budget, and a single `serverTime`; one failing dependency degrades one panel, never the page
- Redirect behind the `dashboard_home` flag with a kill rule at every stage: revert if cohort completed-task rate drops > 5% vs control or any panel failure rate > 5% for 30 minutes. The page is always reachable by URL and never redirects away on data failure
- Per-panel metrics (`dashboard_panel_result`, `dashboard_request_duration_ms`, `dashboard_exposure`, `dashboard_panel_interaction`, `mark_all_read_outcome`) and the two alerts that make the kill rule observable: `dashboard_panel_failure` (any panel > 5% over 5 min) and `dashboard_cohort_completion_regression` (−5% vs control over 1 hour)
- Shared PanelFrame (loading/empty/error/retry chrome) and shared Toast primitive (live region, queue of 3, reduced motion)
- Server-provided strings render as text nodes only (no innerHTML)
- Production telemetry baseline: before the 5% cohort starts, query existing login / action-start / action-completion events for the login-to-first-completed-task median, p75, p90 and replace the 75s walkthrough figure. Segmenting navigation vs post-arrival time needs a per-session page-arrival event; if none exists, report the unsegmented median and use the v1 exposure event as the arrival marker from the 25% stage onward

**Stage gates (decided; the improvement gate applies at 25%, not 5%):**
- 5% → 25%: at least 3 business days and at least 500 cohort sessions, with completed-task rate and permission-error rate within ±2% of control. No improvement requirement at this stage because a 5% cohort may not reach significance for a 20% median shift
- 25% → 100%: additionally, cohort login-to-first-completed-task median improved ≥ 20% vs control over at least 5 business days
- Flag removal: after 2 weeks at 100% with no kill-rule trigger, remove the flag and the old landing route regardless of whether the 45s target is met. The 45s target is the reported success measure; if it is missed, open a follow-up TODO for the hierarchy variant (single primary CTA above the fold). This keeps the advance gate (relative) and the removal gate (absolute) from diverging into a permanent flag

**Recommended operational deliverables (do not block the 5% stage):**
- Cohort-vs-control rollout dashboard (login-to-first-completion median/p75, completed-task rate, permission-error rate). Needed to evaluate the 25% gate, so it must exist before that stage
- Two more alerts: `dashboard_quickactions_failure` (> 2% over 5 min; this panel carries the metric) and `dashboard_latency_p95` (> 500ms over 10 min)
- Three runbook entries: "panel failure spike", "endpoint latency", "cohort metric regression"

Why an operations program at all: the redirect changes where every member lands after login and the feature is judged by a metric the user set. The kill switch and panel alerts are the minimum that makes the kill rule real; the dashboard and improvement gate are what make the 45s target checkable rather than asserted.

## Deferred to TODOS.md
- Real-time notification and activity deltas (P3) — after v1 at 100%; needs SSE/WebSocket transport and stream auth
",

Users land here after login.

## UI Scope
- New React page component `UserDashboard.tsx` at `src/pages/`
- Three new sub-components: `ActivityFeed`, `NotificationsPanel`, `QuickActions`
- Tailwind CSS for layout, mobile-first responsive (breakpoints: sm/md/lg)
- Empty state, loading skeleton, error state for each panel
- Hover states + focus-visible outlines on every interactive element
- Modal dialog for "Mark all as read" on notifications panel
- Toast notification system for action feedback

## Backend
- New REST endpoint `GET /api/dashboard` returns `{ activity, notifications, quickActions }`
- Backed by existing PostgreSQL tables; no schema changes

## Out of scope
- Dark mode (separate plan)
- Personalization / customization (separate plan)

## Existing product and application contracts

This is the existing single-role member workspace, not a new product or a new
onboarding flow. Members currently visit three separate pages after login to
resume work, check alerts, and inspect recent changes. In the team's last task
walkthrough, finding the next item took a median 75 seconds. The dashboard's
success measure is login-to-first-completed-task time, targeting 45 seconds,
with completed-task rate and permission-error rate as guardrails. Existing
analytics records login, action start, action completion, and permission errors;
the new page still needs its own exposure and interaction instrumentation.

Activity is the immutable audit history of workspace changes. Notifications are
member-specific alerts with persistent read state; acknowledging an alert does
not alter audit history. The existing action registry supplies three actions
(create an item, resume assigned work, invite a member), with stable IDs, labels,
route targets, and server-side eligibility predicates. These are links into
existing workflows; action ranking and a new configuration service do not exist.

The application already uses cookie sessions and workspace membership middleware.
Its request context supplies the authenticated member and workspace IDs. Existing
repository methods apply both IDs where appropriate; callers do not accept a
workspace ID from query parameters. Mutations already require CSRF tokens. The
new dashboard endpoint must compose these methods and follow the same boundaries;
its handler, authorization integration, and failure paths have not been written.

Existing list methods return the latest 20 records plus a cursor and have indexed
workspace/member and created-at access paths. The existing full activity and
notification pages own older-page navigation. The member-scoped bulk-read API is
idempotent and marks only notifications at or before the supplied snapshot time,
so later arrivals remain unread. Existing HTTP clients expose typed unauthenticated,
forbidden, validation, retryable-service, and network errors. Each dashboard panel
still needs to map these results to its loading, empty, error, retry, and success
states; the aggregate endpoint's response composition and partial-failure behavior
remain new implementation work. No schema migration or new mutation API is needed.

The app already has Tailwind spacing/color/type tokens, a responsive page shell,
buttons, links, and a dialog primitive with focus trapping, Escape dismissal, and
focus return. These primitives do not implement any dashboard panel, confirmation
flow, or toast system. The new modal and toast feedback must also work with keyboard
and screen readers; existing accessibility policy requires named controls, a live
region for nonblocking feedback, sufficient contrast, and reduced-motion support.
The dashboard still needs its own layout, content hierarchy, mobile behavior, and
state-specific copy at sm/md/lg breakpoints.

Vitest, React Testing Library, and Playwright already run in CI. Existing fixtures
cover authenticated members, another workspace, empty lists, and service failures;
there are no dashboard-specific tests yet. Existing staging feature flags and
request/error metrics support a member-cohort rollout and rollback to the current
landing page. The dashboard's rollout criteria, endpoint performance checks,
interaction tests, and accessibility verification must be specified and added.

All dashboard screen, panel, aggregate-endpoint, modal, and toast work listed above
is new. The existing contracts describe dependencies to reuse, not completed work
or prior approval of an implementation approach.

- `GET /api/dashboard` is implemented by a `DashboardComposer` that runs the activity, notifications, and quick-action eligibility fetches concurrently with a per-fetch timeout budget (default 2s), returns HTTP 200 with a per-panel envelope `{ ok: true, data } | { ok: false, error: 'forbidden' | 'retryable' | 'timeout' | 'validation' }` for each of `activity`, `notifications`, `quickActions`, plus a single `serverTime` captured at handler entry. The handler reads member and workspace IDs only from the request context and ignores any query parameters. Whole-request 401 only when the session is invalid; 403 only when membership fails. Unknown exceptions propagate to the existing error middleware with a request id. Verification: Vitest cases for 0/1/2/3 sub-failures, per-fetch timeout, and predicate throw all return 200 and never 500; integration test asserts `?workspaceId=<other>` returns no other-workspace data.
- The post-login redirect to `/dashboard` applies to all authenticated members (single-role workspace), is behind the `dashboard_home` feature flag, uses history `replace`, and rolls out 5% → 25% → 100%. The redirect applies only when the login has no return-to destination; a protected deep link keeps its target. Every login emits a `dashboard_variant{variant: treatment|control}` event from the flag evaluation so cohort and control sessions are attributable; control = members logging in during the same window who are not redirected. Permission-error rate = existing permission-error events divided by action-start events per session cohort. Stage rules: 5% → 25% requires at least 3 business days and at least 500 cohort sessions, or 10 business days, whichever comes first, with completed-task rate and permission-error rate within ±2% of control (no-regression gate only); 25% → 100% requires the same no-regression gate over at least 5 business days and reports whether the cohort login-to-first-completed-task median improved ≥ 20% vs control; if the safety gates hold for 10 business days at 25% without the improvement, advance to 100% and record the miss. Alert vs kill thresholds: `dashboard_panel_failure` alerts at > 5% over 5 minutes and the flag is reverted if it stays > 5% for 30 minutes; `dashboard_cohort_completion_regression` alerts at −5% vs control over 1 hour and the flag is reverted if it persists for 3 hours. Flag removal: after 2 weeks at 100% with no kill-rule trigger, remove the flag and the redirect fallback branch (the previous landing page route itself stays reachable) regardless of whether the 45s target is met; the 45s target is the reported success measure, and if it is not met a follow-up TODO is opened for the hierarchy variant (single primary CTA above the fold). The dashboard route is always reachable by URL and never redirects away on data failure. The login `next` parameter accepts same-origin relative paths only. Verification: integration tests for flag on/off and for `next=//evil.com` rejection; manual check that all-three-panels-failed still renders the page with Retry.
- Before cohort stage 5% begins, the production login→first-completed-task distribution (median, p75, p90) is queried from existing analytics events (login, action start, action completion) and recorded in this plan's Context section as the real baseline replacing the 75s walkthrough figure. Segmentation into navigation time vs post-arrival time requires a page-arrival event joinable per session; if none exists, the unsegmented median is reported and the dashboard exposure event shipped with v1 provides the arrival marker for the 25% stage analysis.
- A shared `PanelFrame` component owns loading (skeletons matching final layout), empty, error, and retry chrome for all three panels; panels supply state-specific copy: QuickActions empty = "No actions available right now"; NotificationsPanel empty = "You're all caught up" with one CTA that is the primary action (omitted when no action is eligible or the quickActions envelope failed); ActivityFeed empty = "No recent changes yet". Each panel shows a Retry control on error and the other panels render normally on partial failure. Verification: RTL tests for all four states per panel and for a one-panel-failed response.
- QuickActions renders only eligible actions from the registry and gives "resume assigned work" primary visual weight and initial keyboard focus on load when eligible. NotificationsPanel shows an unread count badge and mirrors the count in the document title. NotificationsPanel and ActivityFeed render relative timestamps with an absolute-time tooltip, use an injected clock, re-render labels on a 60-second interval (paused while the document is hidden), clamp future timestamps to "just now", and link "View all" to the existing full activity and notifications pages (QuickActions has no list and no such link). Verification: RTL tests with a fake clock including a 60s advance, tooltip presence, future-timestamp clamp, and focus assertion.
- "Mark all as read" opens a confirmation dialog built on the existing dialog primitive (focus trap, Escape, focus return). On confirm the list updates optimistically, the confirm control is disabled while the request is in flight, the request calls the existing member-scoped bulk-read API with `serverTime` from the dashboard response as its snapshot-time argument (the contract states this API marks only notifications at or before the supplied snapshot time; never the client clock; if `serverTime` is absent the action is disabled with a reload toast), a stale-CSRF failure refreshes the token once and retries once, and any failure rolls back the optimistic state and shows an error toast with Retry (403 shows "You no longer have access"). Undo is not provided. Verification: RTL rollback test; double-click sends exactly one request; integration test inserting a notification between confirm and response asserts it remains unread.
- A shared `Toast` primitive lives in the shared UI layer next to the existing dialog primitive, is mounted once at the app root, exposes `useToast`, announces via an ARIA live region, queues at most 3 visible toasts, disables motion under `prefers-reduced-motion`, includes the request id in error toasts, and throws in development when used without its provider. Verification: RTL tests for queue cap, live-region text, and reduced-motion behavior.
- All notification and activity text renders as text nodes; no server-provided string is rendered as HTML. Verification: RTL test asserting a `` string renders escaped with no script element.
- The page includes a skip link to the primary action, landmark regions (`main`, `nav`, one `section` with a heading per panel), named controls, and refetches dashboard data on `visibilitychange`/window focus. A focus refetch is deferred until any in-flight mark-all-read request has settled (success or rollback), so it cannot overwrite optimistic state. Eligibility is the existing action registry's server-side predicate result per action, returned by the aggregate as a boolean. The primary action is "resume assigned work" when eligible, otherwise the first eligible quick action, otherwise the page heading; focus rests on the page heading while skeletons show and moves to the primary action once the quickActions envelope resolves (and does not move again on later refetches). The document-title unread count derives from the same NotificationsPanel state as the header badge, so optimistic update and rollback move both together. Verification: Playwright keyboard traversal test and axe scan with zero violations across loading, empty, error, and success states on desktop and `sm` viewports; RTL test that `visibilitychange` triggers a second fetch; RTL test that a `visibilitychange` dispatched during an in-flight mark-all-read does not refetch until it settles; RTL test that title and badge counts match after rollback.
- Observability: structured logs at handler entry/exit and per sub-fetch with request id, member id, workspace id, outcome, and duration; metrics `dashboard_request_duration_ms`, `dashboard_panel_result{panel,outcome}`, `dashboard_exposure{cohort}`, `dashboard_panel_interaction{panel,action}`, `mark_all_read_outcome{result}`, and DB pool wait; four alerts named `dashboard_quickactions_failure` (> 2% over 5 min), `dashboard_panel_failure` (any panel > 5% over 5 min), `dashboard_latency_p95` (> 500ms over 10 min), and `dashboard_cohort_completion_regression` (cohort completed-task rate −5% vs control over 1 hour); a rollout dashboard comparing cohort vs control on login-to-first-completion median/p75, completed-task rate, and permission-error rate; three runbook entries: "panel failure spike" (check that panel's repository health; keep the flag on), "endpoint latency" (check DB pool wait; hold the cohort stage), and "cohort metric regression" (flag off; members return to the old landing; page stays reachable). Analytics send failures increment an `analytics_drop` counter.
- Performance: three distinct numbers: per-dependency fetch timeout 2s inside the composer; endpoint p95 SLO 300ms at 20 records per panel, verified by a staging load test before cohort stage 5% and the trigger for revisiting a server-side cache; `dashboard_latency_p95` alert threshold 500ms over 10 minutes. No server-side response cache in v1.
- Tests ship with the feature: Vitest for the composer and helpers, RTL for every panel state and interaction, integration tests for auth scoping, redirect flag, and mark-all-read ordering, Playwright for the login → dashboard → resume flow, one-panel-failure resilience, mobile stacking, and modal behavior.
- Documentation: an ASCII architecture diagram (browser page → aggregate endpoint → composer → three repository/registry fetches → per-panel envelopes) is included as a comment in the composer file; `PanelFrame` and `Toast` have README usage snippets.
- Deferred to TODOS.md: real-time notification and activity deltas (SSE/WebSocket), P3, after v1 reaches 100%.

## Review record

## Decision Audit Trail

| # | Phase | Decision | Classification | Principle | Rationale | Rejected |
|---|-------|----------|-----------|-----------|----------|----------|
| 1 | CEO | Mode = SELECTIVE EXPANSION | Mechanical | P1+P2 | Autoplan override; also the context default for an enhancement to an existing system | EXPANSION, HOLD, REDUCTION |
| 2 | CEO | Approach B: keep the aggregate `GET /api/dashboard` endpoint, add per-panel result envelopes | Taste | P1, P5 | Quick-action eligibility needs a server call anyway; one auth boundary, one loading state, per-panel degradation. Files: `CLAUDE.md`, `README.md`, `.claude/plans/ui-heavy-feature.md`, `docs/designs/dashboard-context.md`. **No application source tree is present in this repository.** Every "read the code" step below is therefore grounded in the plan's "Existing product and application contracts" section and the design doc, which restate the same contracts. Where a section would normally cite `file:line`, it cites the contract paragraph instead and marks the item **VERIFY IN REPO** for the implementer.
- **In flight:** nothing (clean tree, no stash, no other branches).
- **TODOS.md:** absent. No prior deferred work to map.
- **FIXME/TODO comments:** none.
- **Design doc:** `docs/designs/dashboard-context.md` found. Content is identical to the plan's contracts section, so it is treated as the source of truth for constraints. No `Supersedes:` field.
- **Handoff note:** none. **Prior learnings:** none logged for this project. **Brain digests:** cold.
- **Retrospective check:** single commit, no prior review cycle, no problem-area history.
- **Frontend/UI scope:** DESIGN_SCOPE = yes (page, 3 panels, modal, toast, responsive breakpoints). Section 11 runs.
- **Taste calibration:** N/A. There is no code in this repository to hold up as a style reference or anti-pattern. The implementer should identify, in the real repo, the dialog primitive and the responsive page shell named in the contracts and match their conventions.

## Landscape check (Layer 1 / 2 / 3)

Aside not installed; WebSearch used (two read-only queries).

- **Layer 1 (tried and true):** Post-login "home" pages in workspace tools converge on one primary next action above the fold with secondary streams below. Linear's default view is the canonical example of restraint: an active list, nothing else on the landing view. Aggregation gateways (Azure "Gateway Aggregation" pattern) are standard for composing several back-end reads into one client call, with the aggregator responsible for partial-response handling.
- **Layer 2 (what the search says):** Independent-widget dashboards use `Promise.allSettled` semantics so one failed panel does not cascade. Gateways signal partial results explicitly (KrakenD's completed-flag header is one example). Notification UX guidance: batch same-type alerts, reserve urgency for real events, put mark-as-done inline.
- **Layer 3 (first principles):** The plan's metric is *time to first completed task*, not *time to see three panels*. Whatever the layout, the thing that moves the metric is how fast the "resume assigned work" action is under the cursor. This does not require ranking infrastructure; it is an information-hierarchy decision inside the existing three-panel scope. Feeds into 0A and Section 11.

Sources: [Gateway Aggregation pattern](https://learn.microsoft.com/en-us/azure/architecture/patterns/gateway-aggregation), [KrakenD response composition](https://www.krakend.io/docs/endpoints/response-manipulation/), [Notification UX best practices](https://www.eleken.co/blog-posts/notification-ux), [Dashboard design principles 2026](https://www.designrush.com/agency/ui-ux-design/dashboard/trends/dashboard-design-principles), [Activity feeds vs in-app notifications](https://getstream.io/blog/activity-feeds-app-notifications/).

## Step 0A — Premise challenge

Premises named and evaluated:

| # | Premise (as stated or implied) | Status | Notes |
|---|---|---|---|
| P1 | Members lose time visiting three pages after login; median 75s to find the next item | **Challenged, accepted provisionally** | Source is a team walkthrough, not production telemetry. The contracts say analytics already record login, action start, and completion, so the real distribution is a query away. Queued for the Final Gate as a challenged premise (see decision 3). |
| P2 | One page showing activity + notifications + quick actions is the most direct path to "first completed task" | **Accepted with a condition** | Direct enough if hierarchy puts "resume assigned work" first. If the 75s is decision time rather than navigation time, a flat three-equal-panels layout will under-deliver. Condition carried into Section 11 and the design phase. |
| P3 | Login-to-first-completed-task, target 45s, with completed-task rate and permission-error rate as guardrails, is the right metric set | **Accepted** | Outcome metric with guardrails against gaming. Proxy skepticism: the guardrails catch "faster but fewer completions." Good. |
| P4 | A new aggregate endpoint is needed | **Accepted** | Quick-action eligibility predicates are server-side and have no stated HTTP surface, so at least one new read endpoint is needed regardless. Composing all three there is cheaper than adding one and calling three. |
| P5 | No schema changes and no new mutation API are needed | **Accepted** | Bulk-read is existing, idempotent, snapshot-scoped. Nothing in this plan writes new data. |
| P6 | Members land on `/dashboard` after login | **Accepted as a flagged experiment, not a fact** | This is the highest-blast-radius line in the plan and is stated as a fact. Decision 5 converts it to a cohort-gated redirect with a written ship/kill rule. |
| P7 | Dark mode and personalization are separate plans | **Accepted** | Focus as subtraction. The dashboard must not paint itself into a corner on either (tokens already exist; panel layout stays static). |

What happens if we do nothing: members keep doing the three-page tour. Real, measured (even if by walkthrough) pain. Not hypothetical.

## Step 0B — Existing code leverage map

Sub-problem → existing code (per contracts; **VERIFY IN REPO**):

| Sub-problem | Existing code | Plan reuses? |
|---|---|---|
| Auth + workspace scoping | Cookie sessions, workspace membership middleware, request context with member + workspace IDs | Yes. New handler composes; never reads workspace ID from query |
| CSRF on the one mutation | Existing CSRF token requirement on mutations | Yes, via the existing bulk-read client |
| Activity list | Existing repository list method, latest 20 + cursor, indexed | Yes |
| Notifications list + read state | Existing list method + member-scoped idempotent bulk-read API with snapshot time | Yes |
| Quick actions | Existing action registry (3 actions, stable IDs, labels, routes, server-side eligibility) | Yes; the aggregate exposes eligibility results |
| Typed HTTP errors | Existing clients expose unauthenticated / forbidden / validation / retryable-service / network | Yes; each panel maps these to states |
| Layout + tokens | Tailwind tokens, responsive page shell, buttons, links | Yes |
| Modal | Dialog primitive with focus trap, Escape, focus return | Yes; "Mark all as read" confirmation wraps it |
| Toast | **Does not exist** | New, built as a shared primitive (decision 6) |
| Panel chrome (loading/empty/error) | **Does not exist** | New shared `PanelFrame` (decision 21) |
| Older-page navigation | Existing full activity and notifications pages | Yes, via "View all" links (decision 15) |
| Feature flags + rollout metrics | Existing staging flags, request/error metrics | Yes |
| Test harness | Vitest, RTL, Playwright in CI; fixtures for members, other workspace, empty lists, service failures | Yes; dashboard-specific tests are new |

Rebuilding anything? No. The plan builds only what the contracts say does not exist.

## Step 0C — Dream state mapping

```
CURRENT STATE THIS PLAN 12-MONTH IDEAL
───────────────────────── ────────────────────────────── ──────────────────────────────
Login → generic landing Login → /dashboard (flagged) Login → "your next thing" home
Member visits 3 pages to One page: quick actions first, Same page, plus: real-time
resume / check alerts / unread alerts, recent activity updates, per-member layout,
inspect changes All states designed; a11y done dark mode, action ranking fed
Median 75s (walkthrough) Target ≤45s, measured per cohort by completion telemetry
No shared toast primitive Shared Toast + PanelFrame Design-system primitives reused
No dashboard telemetry Exposure + interaction events Telemetry drives ranking
```

Direction: toward the ideal. The two shared primitives (Toast, PanelFrame) and the per-panel envelope are the pieces the 12-month version builds on. Nothing here has to be undone for personalization or real-time.

## Step 0C-bis — Implementation alternatives

```
APPROACH A: Three parallel calls to existing endpoints (minimal viable)
Summary: Dashboard page calls existing activity + notifications list endpoints in
parallel; quick actions need a new small endpoint for eligibility.
Effort: M (human ~4 days / CC ~2h)
Risk: Low
Pros: Least new backend; each panel already has typed errors; no composition logic
Cons: Still needs a new endpoint for quick actions; 3 auth checks + 3 round trips;
no single place to enforce a page latency budget; panels load at different times
Reuses: All existing list endpoints and clients
Completeness: 7/10
APPROACH B: Aggregate endpoint with per-panel result envelopes (ideal architecture, as planned + hardened)
Summary: GET /api/dashboard composes the three repository reads concurrently with a
per-fetch timeout, returns 200 with { activity, notifications, quickActions }
each as { ok, data } | { ok:false, error }, plus serverTime.
Effort: M (human ~5 days / CC ~2.5h)
Risk: Low-Med (new composition + partial-failure logic must be tested)
Pros: One auth boundary; one loading state; one latency budget; partial failure
degrades one panel; serverTime solves the mark-all-read clock problem
Cons: New backend surface with its own failure modes; envelope type must be shared FE/BE
Reuses: Repository methods, request context, action registry, typed errors
Completeness: 10/10
APPROACH C: Ladder — telemetry pull → redirect-to-existing-route spike → single-CTA page → full dashboard
Summary: Do not build the three-panel page until cheaper rungs miss the 45s target.
Effort: S per rung, but calendar time is weeks of sequential experiments
Risk: Med (organizational: stalls a decided feature behind experiments)
Pros: Might capture most of the win with zero UI; establishes real baseline first
Cons: Contradicts the user's stated direction; sequential experiments cost weeks
of calendar time to save days of build; the dashboard is still wanted for
alerts + activity regardless of the redirect result
Reuses: Everything; builds nothing at first
Completeness: 5/10 (as a path to the stated deliverable)
```

**RECOMMENDATION (auto-decided, decision 2): B**, because it is the complete option (P1) and the per-panel envelope is the explicit, readable failure design (P5). C's one good idea (pull the real baseline) is absorbed as a P1 task that runs in parallel with build, not as a gate before it. **Taste decision** surfaced at the Final Gate: the adversarial subagent prefers A or C.

## Step 0F — Mode selection

SELECTIVE EXPANSION (autoplan override; also the context default for an enhancement to an existing system). Baseline scope is held and made bulletproof; expansions surfaced individually below and auto-decided by the blast-radius rule.

## Step 0D — Mode-specific analysis (SELECTIVE EXPANSION)

**Complexity check.** Estimated files touched (VERIFY IN REPO for exact paths): `src/pages/UserDashboard.tsx`, `ActivityFeed.tsx`, `NotificationsPanel.tsx`, `QuickActions.tsx`, `PanelFrame.tsx`, `MarkAllReadDialog.tsx`, shared `Toast` (provider + component + hook, 2-3 files), dashboard API client + envelope types, route registration, post-login redirect flag check, backend route + handler + composition service, analytics event definitions, Vitest/RTL specs (~6), Playwright spec (1-2), feature-flag config. That is 20+ files and 3 new components plus 2 primitives. **Smell acknowledged.** Challenge: can fewer moving parts achieve the goal? The panels are the product; PanelFrame and Toast *reduce* parts by centralizing chrome. The redirect flag and telemetry are non-negotiable for a landing-page change. No cuts.

**Minimum set of changes for the stated goal:** page + 3 panels + aggregate endpoint + redirect flag + telemetry + tests. Modal and toast are required by the "Mark all as read" interaction. Nothing in the baseline is deferrable without breaking a stated requirement.

**Expansion scan.**

*10x check:* A home page that talks to you. The moment a teammate assigns you work, the resume card updates without a reload, the unread badge ticks, and the next action is already focused when the page opens. Concrete shape: SSE channel for notification deltas + optimistic UI + keyboard focus on the primary action at load. Effort: human ~2 weeks / CC ~1 day. S. **Accepted** (decision 14).
6. "View all" from each panel into the existing full pages. S. **Accepted** (decision 15).
7. Skip link + landmark regions so a screen-reader user lands on quick actions in one keystroke. S. **Accepted** (decision 16).
8. Server-side cache for the aggregate. S but adds staleness. **Skipped** (decision 19).
9. Pinned/reorderable panels. M. **Skipped** (user out-of-scope: personalization, decision 18).

*Platform potential:* Toast and PanelFrame become design-system primitives; the per-panel envelope type is a reusable composite-response shape for any future multi-source page.

**Cherry-pick ceremony (auto-decided, neutral posture applied via blast-radius rule):** 7 accepted, 1 deferred, 2 skipped. Accepted items are plan scope for every remaining section.

## Step 0D-POST — CEO plan persisted

Written to `~/.gstack/projects/gstack-autoplan-chain-kVh2Sb/ceo-plans/2026-09-10-user-dashboard.md`. Spec review loop result recorded below under "CEO plan spec review".

## Step 0E — Temporal interrogation (human hours; CC compresses ~10-20x)

```
HOUR 1 (foundations): Envelope type shape and where it lives (shared FE/BE types module).
Exact names of the repository methods and the action registry
API (VERIFY IN REPO). Flag name for the redirect.
HOUR 2-3 (core logic): Per-fetch timeout value (2s default) and what a timeout maps to
in the envelope (retryable-service error). Mitigation: cap predicate evaluation at the 3 registered actions; measure per-fetch duration.
**Single points of failure:** the aggregate handler itself (mitigated by flag → old landing page); PG (already an SPOF for everything).
**Security architecture:** endpoint callable by any authenticated member; returns only that member's notifications and their workspace's activity; nothing writable through it. Mutation reuses the existing CSRF-protected bulk-read API. See Section 3.
**Production failure scenario per integration point:** (a) notifRepo timeout → one panel errors, page usable, metric fires; (b) actionRegistry predicate throws → quick actions panel errors; the resume CTA is missing, which hurts the metric, so alert threshold is tighter for this panel (Section 8); (c) bulk-read 403 (membership revoked mid-session) → rollback + toast "You no longer have access"; (d) redirect flag misconfigured → members loop between login and dashboard: prevented by rendering the dashboard even on 3× panel failure, never redirecting away from it.
**Rollback posture:** flag off returns members to the current landing page instantly; the endpoint and route remain deployed and harmless. Git revert not needed for rollback. Time: seconds.
**SELECTIVE EXPANSION additions:** what makes this elegant is that the page has exactly one data hook, one envelope type, and one chrome component; a new panel in 6 months is one repo call in the composer plus one component. Platform: the envelope + PanelFrame pattern.
**Findings:** (1) per-fetch timeout budget was unspecified → decision 20. (2) partial-failure contract unspecified → decision 9. (3) redirect loop risk → handled by "never redirect away from dashboard on data failure" (folded into decision 5). All auto-decided under P1.
## Section 3 — Security & threat model
| Threat | Likelihood | Impact | Mitigated? |
|---|---|---|---|
| IDOR: member A reads member B's notifications via crafted params | Low (endpoint accepts no IDs) | High | **Yes.** Handler takes member + workspace only from request context; no query params. Test asserts a `?workspaceId=` param is ignored. |
| Cross-workspace activity leak | Low | High | **Yes.** activityRepo scoped by ctx workspace ID; fixture "another workspace" test asserts zero leakage. |
| CSRF on mark-all-read | Low | Med | **Yes.** Existing mutation API requires CSRF token; stale token path handled in Section 2. |
| Stored XSS via notification/activity text | Med (text originates from other members' actions) | High | **Yes** (decision 23). Render as text nodes; never `dangerouslySetInnerHTML`. RTL test asserts a `` string renders escaped. |
| Open redirect via `?next=` after login | Med | Med | **Yes.** `next` allow-list: same-origin relative paths only (VERIFY existing login already does this; if not, it is in scope for the redirect flag work). |
| Aggregate as a cheap DoS amplifier (3 queries per hit) | Low | Med | **Partly.** Inherits existing per-session rate limits (VERIFY IN REPO). 20-row cap and 2s budget bound cost. No new mitigation needed at 1x-10x. |
| Eligibility predicate leaks existence of restricted resources | Low | Low | **Yes.** Response returns only `eligible: boolean` per stable action ID; no reasons. |
| New secrets | None | — | N/A. No new credentials or dependencies. |
| Audit logging | — | — | Mark-all-read is member-scoped read state and by contract does not touch audit history. No new audit requirement. Existing request logging covers the endpoint. |
Input validation: the endpoint has **no** user inputs beyond the session. The mutation sends `snapshotTime` from `serverTime` (decision 22); the server already validates it. Data classification: notification and activity text may contain member names (PII-lite); handled the same as the existing pages.
**Findings:** XSS render rule (decision 23), `next` allow-list verification (added to Section 6 tests). Auto-decided.
## Section 4 — Data flow & interaction edge cases
**Data flow trace (aggregate read):**
```
INPUT(ctx) ──▶ VALIDATION(middleware) ──▶ TRANSFORM(compose) ──▶ PERSIST(none) ──▶ OUTPUT(200 envelope)
│ │ │ │
[nil ctx?] [no session?] [fetch throws?] [stale?] → refetch on focus
→ 401 → 401 → Env error [partial?] → per-panel error
[ws missing?] [not member?] [timeout?] → Env timeout [encoding?] → JSON, UTF-8
→ 401 → 403 [predicate throws?] → Env error
```
Tested: each shadow path has a Vitest case on the composer (fixtures: service failure, empty lists, another workspace) and an RTL case on the panel.
**Async ordering — mark-all-read vs new notification arrival (shared state: notification read flags):**
```
t | Client (NotificationsPanel) | Server bulk-read(snapshot=S) | New notification N2 | Read state
---|-----------------------------------------|---------------------------------------|---------------------|---------------------------
1 | GET /api/dashboard → serverTime S | | | N1 unread
2 | confirm → optimistic: all grey | | | (client) N1 read
3 | | | N2 created at S+ε | N1 unread, N2 unread
4 | | mark read WHERE created_at ≤ S | | N1 read, N2 unread ✔
5 | 200 → refetch → shows N2 unread | | |
Alt order: step 4 before step 3 → same end state (N2 created after S is untouched). ✔
Alt: request fails at 4 → client rollback → N1 unread again; refetch confirms. ✔
Mechanism preventing violation: snapshot-scoped idempotent bulk-read (existing contract) + serverTime
from the same response the list came from (decision 22). Findings (double-submit guard, serverTime, replace-not-push redirect, toast queue cap) auto-decided under P1.

## Section 5 — Code quality review

- **Organization:** page under `src/pages/`, panels colocated under `src/pages/dashboard/`, primitives (`Toast`, `PanelFrame`) in the shared UI directory next to the existing dialog primitive (VERIFY exact path). Backend: composer next to existing services; handler in the existing router.
- **DRY:** three panels × (loading/empty/error/retry) is the violation → `PanelFrame` (decision 21). Envelope type declared once in a shared types module imported by both sides.
- **Naming:** `DashboardComposer` (composes; does not own domain logic), `PanelEnvelope<T>`, `useDashboard`, `MarkAllReadDialog`, `useToast`. Avoid `DashboardService` (implies domain logic).
- **Error handling pattern:** per-fetch wrap + typed envelope; no `catch (e) {}` anywhere; unknown → propagate.
- **Missing edge cases explicitly listed:** what if `quickActions.data` is `[]` (all ineligible)? → panel shows "No actions available right now" rather than empty card. What if `serverTime` is missing (older server)? → client falls back to disabling mark-all-read with a toast asking to reload; never uses client clock.
- **Over-engineering check:** no generic "widget framework"; PanelFrame is props-in, children-out. No global state library for one fetch.
- **Under-engineering check:** relative-time helper must handle future timestamps (clock skew) by clamping to "just now".
- **Cyclomatic complexity:** the composer's per-fetch mapping is the only branchy function; keep it a table lookup `errorClass → envelope error`, not an if-chain.

Findings: PanelFrame (decision 21), missing-serverTime fallback, future-timestamp clamp. Correct shape. **Flakiness:** relative time (injected clock, decision 25); toast auto-dismiss (fake timers); focus refetch (dispatch `visibilitychange` manually). **Load:** k6/Autocannon run against staging `GET /api/dashboard` asserting p95 < 300ms (decision 24). **LLM/prompt changes:** none.

Findings: clock injection, XSS test, ordering test with controlled fixture, redirect `next` test. Auto-decided.

## Section 7 — Performance review

- **N+1:** three list reads are single queries each (indexed on workspace/member + created_at per contract). Risk is eligibility predicates: if each runs a query, that is 3 extra queries per load, bounded and acceptable; if any predicate is unindexed, it is the first thing to break (flag in staging load test).
- **Memory:** 3 × 20 rows; negligible.
- **Indexes:** existing (contract). No new queries need new indexes.
- **Caching:** rejected (decision 19). Revisit only if p95 misses budget.
- **Slow paths (estimated p99):** compose with predicates ~150ms; page hydration ~200ms; mark-all-read ~100ms. Budget p95 < 300ms for the endpoint.
- **Connection pool:** 3 concurrent queries per request instead of 1. At 10x login bursts (Monday 9am) this triples pool pressure from the landing page. Mitigation: the 2s budget prevents pile-up; monitor pool wait time during cohort ramp.

Finding: pool pressure under login bursts → add pool-wait metric to the rollout dashboard (Section 8). Auto-decided.

## Section 8 — Observability & debuggability

- **Logs (structured):** handler entry/exit with request id, member id, workspace id, per-panel outcome and duration; each Env error logs error class + which repo + duration; mark-all-read logs snapshot time and outcome.
- **Metrics:** `dashboard_request_duration_ms` (histogram), `dashboard_panel_result{panel, outcome}`, `dashboard_exposure` (page rendered, flag cohort), `dashboard_panel_interaction{panel, action}`, `mark_all_read_outcome{result}`, `db_pool_wait_ms` (existing or new), `login_to_first_completion_s` (derived from existing events, by cohort).
- **Tracing:** request id propagated into all three fetches and into the client error toast text ("Reference: abc123").
- **Alerts:** quickActions panel failure rate > 2% over 5 min (page: this panel carries the metric); any panel > 5%; endpoint p95 > 500ms; cohort completed-task rate drops > 5% vs control.
- **Dashboards (day 1):** rollout panel: cohort vs control for login-to-first-completion median/p75, completed-task rate, permission-error rate; endpoint latency + per-panel failure; exposure vs interaction per panel.
- **Debuggability:** a bug report 3 weeks later reconstructs from request id → per-panel outcomes → client event stream. Yes.
- **Admin tooling:** none needed; flag console already exists.
- **Runbooks:** (1) panel failure spike → check that repo's health, flag stays on; (2) endpoint latency → check pool wait, consider cohort hold; (3) metric regression → flag off, members return to old landing, page stays reachable.
- **Joy to operate:** one Grafana row per panel with outcome + duration, plus the cohort comparison in the same view.

Findings: request id in toast text, tighter alert on quickActions, cohort dashboard as a deliverable. Auto-decided (P5 observability is scope).

## Section 9 — Deployment & rollout

- **Migrations:** none.
- **Feature flags:** `dashboard_home` (redirect) — required. The route and endpoint ship dark (reachable by URL) regardless.
- **Rollout order:** (1) deploy backend endpoint + frontend route, flag off; (2) internal cohort 5% for 3 business days; (3) 25% for 5 business days; (4) 100%. **Ship rule:** proceed a stage only if cohort login-to-first-completion median improves ≥ 20% vs control AND completed-task rate and permission-error rate are within ±2% of control. **Kill rule:** revert flag if completed-task rate drops > 5% or any panel failure rate > 5% for 30 min. **Final rule:** at 100% for 2 weeks with the median ≤ 45s → remove the flag and the old landing route; otherwise hold and revisit hierarchy (design phase's single-CTA variant).
- **Rollback:** flag off (seconds). Endpoint stays; no data to unwind.
- **Deploy-time risk window:** new frontend against old backend → `/api/dashboard` 404 → page-level error with Retry (acceptable, flag is off during first deploy). Old frontend against new backend → nothing calls it.
- **Environment parity:** staging load test (Section 7) and axe pass gate stage (2).
- **Post-deploy checklist:** first 5 min: `GET /api/dashboard` 200 for a test member, zero 5xx; first hour: per-panel failure < 1%, p95 within budget, exposure events flowing.
- **Smoke tests:** synthetic login → dashboard → one quick action, run post-deploy.

Findings: gate numbers (decision 4), replace-not-push, dark ship first. Auto-decided.

## Section 10 — Long-term trajectory

- **Debt introduced:** one new backend surface; two new primitives (positive debt if kept in the shared layer). Testing debt: none if the Section 6 list ships. Documentation debt: component-level README for PanelFrame and Toast usage.
- **Path dependency:** envelope shape is additive-friendly; a fourth panel is one composer entry + one component. Personalization later can reorder panels without touching data.
- **Knowledge concentration:** the composer and envelope are the only non-obvious pieces; a 20-line ASCII diagram in the composer file (Section 1's) is required.
- **Reversibility:** 4/5. Flag off reverts behavior; deleting the code is a clean removal with no data migration.
- **Ecosystem fit:** React + typed fetch + Tailwind; standard.
- **1-year question:** a new engineer reads "dashboard = composer + envelope + PanelFrame + 3 panels" and gets it in 5 minutes. Yes.
- **What comes after:** real-time deltas (deferred), personalization (separate plan), dark mode (separate plan). Architecture supports all three.
- **Retrospective on cherry-picks:** optimistic mark-all-read depends on serverTime, which the aggregate provides; the aggregate choice (decision 2) is load-bearing for decision 12. Consistent.

No new findings beyond the composer diagram requirement (auto-decided, P5).

## Section 11 — Design & UX review (CEO calling in the designer)

**Information architecture:** First: quick actions, with "Resume assigned work" visually primary when eligible (this is what moves the metric). Second: notifications with unread badge. Third: recent activity. On `sm`, that is the stacking order; on `lg`, quick actions span the top, notifications left, activity right.

**Interaction state coverage map:**

```
FEATURE | LOADING | EMPTY | ERROR | SUCCESS | PARTIAL
-------------------|----------------|--------------------------------|--------------------------|--------------------|------------------
QuickActions | 3 skeleton | "No actions available right | message + Retry | 1-3 buttons, | n/a (single fetch)
| buttons | now" | | primary = resume |
NotificationsPanel | 5 skeleton rows| "You're all caught up" + CTA | message + Retry | list + badge + | other panels ok
| | | | Mark all as read |
ActivityFeed | 5 skeleton rows| "No recent changes yet" | message + Retry | list + View all | other panels ok
Mark-all-read | button spinner | n/a | rollback + error toast | greyed list, toast | n/a
Page | shell + 3 skel | n/a (panels own empties) | 401→login; 403→message | rendered | 1-2 panels errored
```

**User journey (emotional arc):** login → immediate shell (no blank) → the resume button is the first focusable thing → click → back in the work. Break points: all-panels-error (must still show the page chrome and Retry, not a dead end) and slow network (skeletons, not spinners, so layout does not jump).

**AI slop risk:** "three equal cards" is the generic pattern. Mitigation: the primary action gets visual weight; panels are not identical cards. This is the design phase's job to make concrete.

**DESIGN.md alignment:** no DESIGN.md; the contracts name Tailwind tokens and existing primitives. Plan must use them, no new colors.

**Responsive intention:** stated (sm/md/lg, mobile-first). Modal full-width on sm.

**Accessibility:** skip link, landmarks, named controls, live region for toasts, focus trap/return in modal, contrast via existing tokens, reduced motion. Axe in E2E.

**User flow (required diagram):**

```
[Login] ──flag on──▶ [/dashboard: shell + skeletons] ──data──▶ [Dashboard ready]
│ ├─ click Resume ──▶ [work route]
└──flag off──▶ [current landing] ├─ click Mark all ─▶ [Modal] ─confirm─▶ [greyed + toast]
│ └─cancel/Esc─▶ [Dashboard ready]
├─ View all ─▶ [existing full page]
└─ panel error ─▶ [Retry] ─▶ [skeleton] ─▶ ready
```

**Inevitable-feeling touches (30-min):** focus lands on the primary action on load for keyboard users; unread badge in the document title; skeleton shapes match final layout.

Findings: hierarchy rule (primary action), skeleton-not-spinner, focus-on-load. Auto-decided (P5, P1). Phase 2 (design review) runs next since UI scope is detected.

---

## Step 0.5 — Dual voices

**Codex SAYS (CEO — strategy challenge):** *disabled* (`codex_reviews=disabled`). No outside process started. Outside coverage for this phase: **disabled**.

**Claude SUBAGENT (CEO — strategic independence):** completed, INPUT hash matched. 12 findings: 1 critical (no ship/kill rule), 2 high (aggregation page not the shortest path to first completed task; baseline is a walkthrough number while production telemetry exists), 8 medium (redirect stated as fact; navigation-bottleneck assumption; ActivityFeed value; toast as platform primitive inside a feature; partial-failure deferred; aggregate vs three calls; redirect-to-existing-page alternative; opportunity cost), 1 low-medium (modal vs undo reasoning). Bounded by the flag + kill rule (decision 4), which is why the primary recommends building now and measuring in parallel rather than pausing.

---

## Required outputs (Phase 1)

### NOT in scope
- Real-time notification push (SSE/WebSocket) — new transport infra; deferred to TODOS.md (decision 17).
- Smart next-action ranking / pinned or reorderable panels — user-declared separate plan (personalization); skipped (decision 18).
- Server-side cache for the aggregate — premature; revisit only if p95 misses 300ms (decision 19).
- Undo for "Mark all as read" — needs a reverse mutation API the plan excludes (decision 7).
- Dark mode — user-declared separate plan.
- Pausing build behind a redirect-to-existing-route spike — rejected as sequencing (decision 2/3); the telemetry query itself is in scope.
- Pagination inside panels — existing full pages own older navigation; panels link out (decision 15).

### What already exists
See Step 0B table. Reused: session/membership middleware, request context, CSRF, activity/notification list methods, bulk-read API, action registry, typed HTTP errors, Tailwind tokens, page shell, buttons/links, dialog primitive, feature flags, request/error metrics, analytics events, Vitest/RTL/Playwright + fixtures. New: page, three panels, PanelFrame, Toast, MarkAllReadDialog, aggregate endpoint + composer + envelope type, redirect flag check, dashboard analytics events, tests. Nothing existing is rebuilt.

### Dream state delta
This plan lands the page, the envelope pattern, and two shared primitives; the 12-month ideal adds real-time deltas, personalization, dark mode, and telemetry-fed ranking. All four sit on top of what this plan builds; none require undoing it. Delta closed by this plan: roughly the first half of the ideal (static, measured, accessible home page).

### Error & Rescue Registry
See Section 2 tables (14 codepaths, 13 rescue rows, 0 remaining GAPs; 3 GAPs found and closed in-plan).

### Failure Modes Registry

```
CODEPATH | FAILURE MODE | RESCUED? | TEST? | USER SEES? | LOGGED?
----------------------------|---------------------------------|----------|-------|----------------------------|--------
middleware | no session | Y | Y | login page | Y
middleware | not a member | Y | Y | no-access message | Y
composer/activity fetch | throws / timeout | Y | Y | panel error + Retry | Y
composer/notifications | throws / timeout | Y | Y | panel error + Retry | Y
composer/quickActions | predicate throws / timeout | Y | Y | panel error + Retry | Y (alert 2%)
composer | all three fail | Y | Y | 3 panel errors, page usable| Y
handler | unknown exception | Y | Y | full-page error + ref id | Y
client read | network / parse | Y | Y | page error + Retry | Y (client)
mark-all-read | stale CSRF | Y | Y | nothing / retry toast | Y
mark-all-read | 403 revoked | Y | Y | rollback + toast | Y
mark-all-read | network / 5xx | Y | Y | rollback + toast + Retry | Y
mark-all-read | N2 arrives mid-request | Y | Y | N2 stays unread | Y
redirect | flag misconfig / loop | Y | Y | never redirected off page | Y
redirect | open `next` | Y | Y | rejected, default route | Y
relative time | future timestamp | Y | Y | "just now" | n/a
toast | >3 queued | Y | Y | 3 visible | n/a
analytics | endpoint down | Y | N | nothing | Y (drop counter)
```
**CRITICAL GAPS: 0.** (analytics row is RESCUED=Y, USER SEES=nothing by design, LOGGED=Y; not a silent failure.)

### TODOS.md updates (proposed; TODOS.md does not exist and is not written in plan mode — created on approval)

```markdown
# TODOS
## Dashboard
### Real-time notification deltas on the dashboard
**What:** Push new notifications and activity to an open dashboard without reload (SSE or WebSocket).
**Why:** The dashboard is a landing page people leave open; refetch-on-focus covers the stale-tab case, but live updates make the page feel alive and shave the "did that go through?" moment after quick actions.
**Context:** v1 ships refetch on `visibilitychange`. The aggregate envelope and PanelFrame are in place; a delta channel would feed the same panel state. Start from the notifications repo's create path; requires a new transport and auth on the stream.
**Effort:** L (human) → M with CC
**Priority:** P3
**Depends on:** Dashboard v1 at 100% rollout; cohort data showing members keep the tab open
```

Decision: **A) Add to TODOS.md** (auto-decided, P3 outside blast radius).

",
