A CR, U+2028 or U+2029 followed by 'diff --git ' inside a changed line cut the
file's section short, so the secret lines after it left the excerpt while the
file stayed excluded from the reader's own diff.

* fix(review-exclude): scan secret-excerpt lines with their context and overlapping chunks (GST-1447)
The scanner read changed lines alone, in chunks with no overlap, so a secret it
detects only beside its label (an AWS secret key under an unchanged
aws_secret_access_key line, or a label at the end of one 512 KiB chunk) left
every excerpt. Scan every hunk line, repeat 1 KiB between chunks, and count
findings on changed lines only.
