Ports this branch's changes onto the refactor: review.ts edits into plan-gates/outside-voice-steps,
PTY changes into test/helpers/pty/*, the paid runner onto the shared shard engine with case, plan
and report code split into scripts/lib/paid-{cases,plan,report}.ts under the size ratchet, touchfile
deltas applied per key, and the flake ranker's cache onto lib/state-root.
