fix(codex): prompts and review focus travel as agent-written files
Co-authored-by: dgrant <dgrant@users.noreply.github.com> Challenge and consult prompts, and the /codex review focus text, are written by the agent into private .gstack/tmp files and fed to Codex on stdin or read back with cat; no heredoc carries them. Consult's new and resumed runs share one block (and one parser) so no placeholder stands in for the parser; a resumed run reads its session id from .context/codex-session-id and checks it. Adds the A3 group 2 placeholder allowlist block.

**Session-cost reality (measured):** every `codex exec` call — resumed
or fresh — pays Codex's ~21K-token session prelude (its skill catalogue +
instructions); `resume` does NOT amortize it (a measured resume came in
slightly ABOVE a fresh call). Resume buys conversational continuity, never
token savings. So: prefer ONE codex call per skill where the workflow allows,
batch questions into that call, and reach for resume only when the follow-up
genuinely needs the prior session's context.
5. Run codex exec with **JSONL output** to capture reasoning traces. Set `_CODEX_MODE`
to `resume` if the user chose "Continue" in step 1, else `new`; a resumed run reads the
session id from `.context/codex-session-id`. Substitute the printed name for
`<prompt-file-name>` (letters, digits, `.`, `_` and `-` only). Use `timeout: 600000` on

the Bash call (the tool's maximum) — the gate sits ABOVE the 540s wrapper so the wrapper
fires first, ends Codex, and prints its explicit stall message:

If the user passed `--xhigh`, use `"xhigh"` instead of `"medium"`.
