* feat: /ship finish and train ready/land check the Plan line (GST-1751)

finish --plan resolves the brief's Plan line before stage 1; train ready,
base-red ready, train land and stamp land read and write the READY note's
plan record like direct ready and land. Review fixes: grant and exception
rows never pass as plan approvals, render --plan stays short when redacted,
no explicit changelog-only exemption, off mode still records a resolved line.

* fix: the Plan line stands for the gate's real state, and a broken store never throws (GST-1751)

A row id from an autoplan run resolves to that run's newest gate revision; a
run id needs the plan approval answered and no decision left unanswered.
readStore skips malformed rows and unreadable files; with the key off the
check never throws or prints. Train land rechecks the Plan line before its
merge on every path. /ship's Step 19-C renders Approvals with --plan;
DRIVER.md back under its size limit.

* fix: pre-landing review fixes

Step 8 plan discovery (lib/ship-audit.ts and the plan-completion sections) skips the Plan record line, so a later Plan: <path> binding is still found; render --plan refuses a line it cannot parse. The review skill's pins drop by 4 bytes.

**Crew PRs now show whether they had their plan step, and a repo can require it (GST-1751).** In qm, a Captain's brief replaced /autoplan with "light plan … no gate to Garry" for a new UI feature (qm#3186), and nothing noticed. A PR now carries its brief's `Plan:` line under `## Approvals` (`gstack-approvals render --plan "<line>"`): `approved: <autoplan run id or approval row id>`, `exempt: <reason>` from docs/captain.md's list, or `required`. /ship's finish step (`gstack-ship-release finish --plan "<line>"`) and `gstack-ship-queue ready` resolve it against every branch's approval rows and print `PLAN:`; ready writes the result into the READY note, and `land` (direct, stamp and train) checks the note still matches the PR body. A gate the lane answered in its own words, a newer gate revision that isn't approved, and a ruling or grant row never count as the owner's plan approval; imports now record `words_by` for that. gstack never guesses from the diff or the title whether a change is a feature. No line reads as `required`, and a diff of changelog fragments only needs none. The new `plan_record` ship-policy key is `off` by default; Capy crew sessions warn, and `require` (policy or `GSTACK_PLAN_RECORD=require`) refuses with one of six codes, each with a fix. `gstack-approvals plan-check --plan "<line>"` checks a line by hand. Replayed on the last 60 merged gstack PRs with each PR's brief line, 57 pass and 3 would be refused: two features Garry approved where no approval row was ever written (#4100, #4055), and one whose gate the lane answered in its own words (#4049).

**Meaning.** The line cites an approval, but this machine has no approval store: no
state-root `approvals/` directory and no Drive mirror. That is reported apart from a
missing row, because nothing could be read.
**Fix.** Run `gstack-ship-queue ready` on the lane machine that imported the approval, or
`gstack-approvals sync` there. `land` checks the plan record `ready` wrote into the READY
note and does not need the store.
<a id="queue-plan-mismatch"></a>
### `gstack-ship-queue land: ... (QUEUE_PLAN_MISMATCH)`
**Meaning.** Under `plan_record: require`, land found a READY note with no plan record
(written before the key, or by a `ready` that refused nothing in warn mode), or a note
whose Plan line differs from the PR body's current line.
**Fix.** The lane fixes the PR body's Plan line if needed and reruns
`gstack-ship-queue ready --pr <n>` at the pin.
## /ship token budget and fresh-context handoffs (gstack-ship-budget)
The flow, the BUDGET line field by field and the meters are in [ship-budget.md](ship-budget.md).

* plan-record — whether a crew PR had its plan step (GST-1751, docs/captain.md#plan-before-build). The brief's
* `Plan:` line travels in the PR body (gstack-approvals render --plan writes it) and says which case applies:
*
* Plan: approved: <token> an owner-approved plan: `a-<12 hex>` is an approval row id; a path is an autoplan
* run directory (its run.json names the run); anything else is a run id matched
* against row sources `autoplan:<id>@gate_rev=<n>`
* Plan: exempt: <reason> one of the exemptions docs/captain.md lists, or `crew:<rule>` for a crew's own
* Plan: required the plan step has not finished
*
* The resolver reads every branch's file in the repository's approval store (the state root and each mirror this
* machine can see), so a row imported under another branch or another lane still counts. A row answered in an
* agent's own words, a newer gate revision that is not approved, and a ruling or grant row never count as the
* owner's plan approval. gstack never infers the class from the diff, the title or the changelog: a PR with no line
* reads as required. A run id, or a row id from an autoplan run, resolves to the newest gate revision imported into the
* store (gateState), so a revised gate must be imported again before it counts. Callers: gstack-ship-queue ready (writes the record into the READY note), land (checks
