v1.91.78.0 feat(review): plan reality rows, sideways review and cso sweep, investigate recipes (multi-agent wave PR D) (#3119)

* v1.91.70.0 feat(unattended): artifact contract, unattended session kind, findings-derived review log, gate list

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* v1.91.72.0 feat(autoplan): unattended runner next|submit, pluggable outside voice, review checklists, spec-loop cap

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(b-runner): atomic lock creation in spend-ledger, judge bundle skips runner-only sections, fixture env spelling

- lib/spend-ledger.ts: create the lock by linking a complete temp file so a
  contender never reads an empty lock and breaks a live owner (CI admitted 20
  of a cap of 8 under contention); unreadable locks break only by mtime age.
- test/helpers/workflow-judge-input.ts: exclude manifest runner_only sections
  (the B4 checklists) from the judge bundle; the judge rates the interactive
  workflow. test/workflow-judge-input.test.ts expects the same set.
- test/helpers/autoplan-run-fixture.ts: spell the runner's session kind
  without the literal the free-suite environment scan rejects.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* v1.91.74.0 feat(review): plan reality rows, sideways review and cso sweep, investigate recipes

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test: register the investigate carve case and map Reality rows to real headings in the review checklists

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test: pin the corrected Reality rows obligation rows

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(investigate): use allowlisted placeholders in the flake and polluter fences; select the investigate carve case on PTY changes

# Changelog
## [1.91.78.0] - 2026-10-11
**Plan reviews check reality before they check taste, `/review` and `/cso` look sideways from every finding, and `/investigate` ships recipes for flakes, polluters, races, eval bisects and backlog triage.**
Multi-agent wave PR D (`docs/designs/MULTI_AGENT_WAVE_2026_10_10.md`, items D1–D3). The reviews of this wave's own plan found that the most expensive mistakes were premise errors (a file:line that did not say what the plan claimed, a thing already built, a number nobody re-derived), so those checks are now rows every plan review must fill.
### What this means for you
- **Reality rows in plan reviews.** `plan-ceo-review`, `plan-eng-review` and `/autoplan` render one `REALITY: <row> pass|finding|n/a <summary> <file:line>` line per row: premise table (every factual claim with a receipt; `unverified=<n>` is scored), already-done, surface check, binding decisions, numbers re-derived from raw data, deferred asks, PR count. Eleven tier-2 rows (db, perf, search, incident, dependency) load only under `--scope`. `bin/gstack-plan-reality rows|check` prints the applicable rows and the verdict line `PLAN_REALITY: phase=… rows=n/m missing=… verdict=complete|incomplete|repair`; an unattended run with a missing row is `incomplete`, never a warning (`REALITY_ROW_MISSING`, `REALITY_RECEIPT_MISSING`). Each tier-1 row ships with a positive fixture taken from this wave's own review record and a decoy.
- **Sideways review.** `/review` Step 4 greps every finding's pattern at every other ingress, command and call site, takes the worst sibling's severity, and forces CRITICAL for data loss, trust-boundary bypass and uncapped spend (vocabulary unchanged: CRITICAL/INFORMATIONAL, no lane downgrades). The checklist names the shapes to sweep: siblings, caches in front of guarded or metered calls, static→dynamic imports, parity claims, pool queries inside a caller's transaction, test-only flake fixes. The Scope Check gains `Diff read: full|partial` and `Undeclared behavior changes:`; CLEAN counts as approved only after a full read. The flake-fix rule (`P(fail | regression)` before and after, else RH-15) is shared with `/investigate`. `/cso` Phase 12 does the sibling sweep only.
- **Investigate recipes.** `/investigate` gains an `## Arguments` table and five carved sections: flake (reproduce on main in a worktree, failed/N, forced probe, never loosen, paid evals never retried, required `## Flake evidence`), polluter (CI-order bisect, `POLLUTER:` line), race (probes, forced interleaving, `Promise.allSettled` writer template), eval-bisect (distinct revisions, cached cheap scorer, detached runs with the sentinel) and backlog (`--backlog`: `still_open` needs a run repro, `already_fixed <commit>`, `partially_fixed`, `owned_elsewhere`, contributor PRs as evidence only). Phase 1 adds call-sites-before-duplicates and `git update-index --refresh` before timing git in a worktree (also in `/benchmark`).
### Itemized changes
#### Added
- `lib/plan-reality.ts`, `bin/gstack-plan-reality`, `scripts/resolvers/plan-reality.ts` (`{{PLAN_REALITY_ROWS}}`); `scripts/resolvers/sideways.ts` (`{{SIDEWAYS_SWEEP}}`, `{{FLAKE_FIX_NUMBER}}`); `investigate/sections/{manifest.json,flake,polluter,race,eval-bisect,backlog}.md`; fixtures `test/fixtures/multi-agent-wave/reality-rows.json` (+15 `INCIDENTS.md` rows); two result codes with troubleshooting anchors.
#### Changed
- `scripts/resolvers/plan-gates.ts` (review mode): `Diff read`, `Undeclared behavior changes`, full-read approval rule; ship's byte-pinned plan-completion untouched.
- Templates: plan-eng-review Scope Challenge + deep incident table, plan-ceo-review 0A/0B + Completion Summary, autoplan ceo/eng phases + Pre-Gate table, review Step 4 + checklist, cso Phase 12, investigate skeleton, benchmark Phase 1. `CARVE_GUARDS` gains `investigate`; caps refreshed with measured ratios (plan-ceo-review +960 B, plan-eng-review +3,794 B, autoplan +851 B, review +923 B replacing the enum paragraph, cso +51 B; qa and ship untouched).
#### For contributors
- New tests: `test/plan-reality.test.ts` (18), `test/sideways-review.test.ts` (8), `test/investigate-recipes.test.ts` (9); `test/skill-fixture.test.ts` pins the sideways paragraph as its extraction anchor.
- Not in this release: `gstack-autoplan submit` does not yet call `gstack-plan-reality check` itself (the runner landed in parallel); a parent loop runs the check before `submit`. Automatic tier-2 scope detection waits for the runner's scope set to grow beyond `ui`/`dx`.
## [1.91.77.0] - 2026-10-11
**`/ship` now runs a two-stage pre-gate before anything paid, regenerates generated files from a committed registry, and links issues only on explicit intent (multi-agent wave C-pregate, plan items C4, C6, C8 tier 1).**

receipt. A receipt is `path:line`, `path:line-line` or a commit sha; it is what
makes the row a check rather than a claim.
**Fix.** Add where the check was made to the line, for example
`REALITY: already-done pass 0 hits since 2026-10-09 for bin/gstack-doctor 91bbd9e`
or `REALITY: surface-check finding bun upgrade --version absent on 1.3.14 bin/gstack-capy-install:52`.

A judged or scored metric regressed between two revisions. Bisect inputs, not

1. **Distinct revisions only.** `git log --oneline <good>..<bad> -- <prompt and

identical (build the prompt with the actual builder at each revision and
compare hashes; a template edit that renders the same bytes is not a
candidate).
2. **A cached cheap scorer first.** Score every distinct revision with the
deterministic or cheap check (the structural judge, a token count, the
quality judge at its lowest tier) and cache results per
`(revision, case)` under `.gstack/tmp/eval-bisect/`. Bisect on the cheap
score; spend the paid judge only on the two revisions where the cheap scorer
disagrees with the reported regression.
3. **Detached runs.** Launch paid cases through the documented detached runner
(`bun run eval:bg:pr`), poll the log for `### gstack-detach EXIT=<code> ###`,
and keep the eval lock. Never retry a paid case; a failed launch is a launch
failure in the report, not a second sample.

scores per revision (cached hits marked), the pre-registered panel size and

## Flake recipe
A flake is a test whose verdict depends on something other than the code it
guards. The Iron Law holds here: no rerun, no wider timeout, no retry and no
skip until the cause is named and probed.
1. **Reproduce on main.** Check out `origin/<base>` in a worktree
(`git worktree add <dir> origin/<base>`) and run the failing file alone N
times (N ≥ 10). On gstack:
`bun run scripts/ship-measure.ts free --files <file> --reruns N`; elsewhere a
loop over the project's single-file test command. Record `failed/N` on main,
then the same on the branch. A failure that reproduces on main is outside the
PR; one that appears only on the branch is inside it.
2. **Cause inside or outside the PR.** Diff the two runs' logs at the first
divergence and name the dependency: timing, file order, shared state, network,
clock, a resource limit. "Flaky" is a symptom, never a cause.
3. **Compare the metric with the last N green runs.** Once for `<base>` and once
for the branch (`--branch <branch-name>`), against the workflow that ran the
failing shard; read the duration or retry token next to the test:
```bash
gh run list --branch <base> --workflow free-tests.yml --status success --limit 20 --json databaseId,headSha,createdAt,updatedAt
gh run view <run-id> --log | grep -F -- "<failing-test-file>"
```
The run where the duration, retry count or failing test first moved dates the
regression; cite its `headSha`.
4. **Forced probe.** Build the smallest change that makes the test fail
deterministically for the stated cause (inject the delay, pin the file order,
freeze the clock, exhaust the resource). Show the probe output failing before
the fix and passing after. No probe, no cause.
5. **Never loosen.** A wider timeout, a retry, a looser assertion or a skip is a
gate relaxation (review tag RH-15); the fix lives in the code or in the test's
isolation, not in its threshold.
**The flake-fix number.** A fix that touches only the test (a wider timeout, a retry,
a looser assertion, a skip) must state `P(fail | regression)`: the probability the test
still fails when the behavior it guards regresses, before and after the change. Derive
it from the forced probe (break the behavior on purpose, run the test N times, count
failures) and write both numbers in `## Flake evidence`. A number that drops is a gate
relaxation (review tag RH-15) and needs the owner's decision, not a merge.
6. **A rerun only when the cause is known and outside the PR** (infrastructure,
a polluter already fixed on main, a provider outage), stated in the PR body.
Reruns follow `ship-measure`'s discipline: bounded, counted, logged,
`flaky retry off`; a free rerun is diagnostic, never the green. Paid evals
are never retried (`EVAL_POLICY` in `test/helpers/periodic-exclude-data.ts`).
7. **`## Flake evidence` in the PR body (required for any flake fix).** The
probe's before and after output, `failed/N` on main and on the branch, the
`gh run list` comparison with the first-moved `headSha`, and a routing note
for the owner: `fix in this PR` | `rerun; cause outside the PR: <what>` |
`owner decision needed: <the relaxation and its P(fail | regression) before/after>`.

## Polluter recipe
A polluter is an earlier test that leaks state the victim depends on: the victim
passes alone and fails in the suite or in one shard.
1. **File order from the CI log.** The shard runner prints its file list in
execution order. Take the files that ran before the victim in the failing
shard (`gh run view <run-id> --log`, or the local shard listing
`bun run scripts/ship-measure.ts free --shard <I>` which reproduces the exact
packing `bun run test` used).
2. **Confirm the dependency.** The victim alone passes; the victim after the
earlier files fails (`<affected-files>` is the earlier files then the victim,
comma-joined, in shard order):
```bash
bun run scripts/ship-measure.ts free --files <failing-test-file> --reruns 3
bun run scripts/ship-measure.ts free --files <affected-files> --reruns 3
```
3. **Bisect the earlier files.** Halve the earlier list, keep the half that
still fails the victim, repeat until one file remains. Each step is one
`--files` run; log the list at every step.
4. **Name what leaked.** Read the polluter for state that outlives it: an env
var, `process.cwd()`, `process.exitCode`, a module-level cache or singleton,
a mock or spy left installed, a server or watcher left listening, a temp
file under a shared path, a learnings or state file under the home
directory (the free lane's home guard reports these), a global timer.
5. **Fix the polluter, not the victim.** Restore or isolate the state where it
is created (`afterEach`, `mkdtemp`, a private env); an order-dependent
`beforeEach` added to the victim hides the leak for the next victim.
6. **Report:** `POLLUTER: <file> leaked <what> into <victim>; fixed by <change>`,
with the bisect log. If the polluter is on main, the victim's failure is
outside the PR (route it with the flake recipe's evidence section).

## Race recipe
A race is two actors touching shared state with an ordering nobody enforces.
Make it deterministic before fixing it.
1. **Timing probes.** Log a monotonic timestamp (`performance.now()`) and the
actor id at every read and write of the shared state (file, row, map, lock).
A race shows as interleaved writes or a read between another actor's
check and its write. Keep the probe output as evidence.
2. **Forced interleaving.** Inject an `await` or sleep at the suspected point
in one actor (behind a test-only hook or env var) so the other actor runs in
the window. The bug must now appear every time; if it does not, the window is
elsewhere.
3. **Concurrent-writer template.** Drive N writers at once and assert the
invariant, not the absence of an error:
```ts
const results = await Promise.allSettled(Array.from({ length: N }, (_, i) => writer(i)));
// exactly one winner | all N applied | file or row intact | lock released and owner recorded
```
Run with N ≥ 8 under the forced interleaving; the template becomes the
regression test with the interleaving still injected.
4. **Fix shape.** An atomic primitive: create with `wx`, compare-and-swap,
`UPDATE … WHERE old_value = ?`, a lock with an owner token and a stale-owner
rule. A check-then-act with a smaller window is not a fix. Rerun step 3
after the change and paste both outputs.

```ts
const results = await Promise.allSettled(Array.from({ length: N }, (_, i) => writer(i)));
// exactly one winner | all N applied | file or row intact | lock released and owner recorded
```

* plan-reality — the reality rows every plan review must carry (multi-agent
* wave D1, release promise P5). One table owns the rows: the resolver
* scripts/resolvers/plan-reality.ts renders them into the plan-review
* templates, bin/gstack-plan-reality prints them at run time and checks a
* review for them, and the tests pin the row ids and the line grammar.
*
* A review emits one grep-able line per row:
*
* REALITY: <row-id> <pass|finding|n/a> <summary carrying a receipt>
*
* A receipt is `path:line`, `path:line-line` or a commit sha; a row without one
* is not a row. Tier 1 rows are required in every review. Tier 2 rows are
* required only when the review's detected scope (the manifest `scope` field
* from plan B4, passed to the checker as `--scope`) names theirs; otherwise
* they may be omitted or marked `n/a`. A tier 1 row marked `n/a` is missing.
*
* `checkRealityRows` is deterministic and returns the missing rows; the
* session kind decides what a miss means (unattended: the phase is
* `incomplete`; interactive: repair twice, then warn at the gate). The row
* table carries each row's incident, so the deep section can say why the row

## Sideways sweep (every finding)
A finding is a defect class, not a line. Before rating any finding from either pass or a specialist:
- **Siblings.** Grep the same pattern at every other ingress, command and call site (routes, CLI subcommands, workers, webhooks, cron, hooks); Read each match. Rate severity on the worst sibling and list the siblings under the finding as one decision.
- **Forced-CRITICAL.** Data loss, a trust-boundary bypass (auth, tenancy, sandbox, signature, redaction or consent skipped) and uncapped spend (a paid call, retry or loop with no cap) are CRITICAL on sight. No lane, specialist, dedup step or saved decision downgrades them; a prior Skip does not apply.
- **A new cache in front of a guarded or metered call.** Name every policy check a hit skips (auth, quota, consent, rate limit, freshness); a hit that skips one is a trust-boundary bypass.
- **Imports moved from static to dynamic.** List the top-level side effects no longer loaded (registrations, schema setup, env validation, route tables) and who reads each registry; a reader that now sees an empty registry is the finding.
- **A changed function with parity claims.** Find modules that claim parity with it (docstrings, parity tests, mirrored helpers, a bash twin of a TS module) and flag each one left unchanged.
- **Pool queries inside a caller's transaction.** A query awaited on a pool connection inside a caller's transaction on a hot read path holds two connections and reads state the transaction cannot see; flag it.
- **Test-only flake fix.** A fix that touches only the test (timeout, retry, looser assertion, skip) states `P(fail | regression)` before and after, from a forced probe; without both numbers it is a Gate Integrity finding (RH-15).
