* v1.91.140.0 chore: stamp TM1 (Opus 5.5 model under test) and CHANGELOG

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): repair the five Opus gate reds from run 39161502626

- coverage audit: owned cd; followed by one && chain, wc -l on in-cwd files
- shared-libs: Claude Code 2.1.292 git --version probe, exact argv only
- ship hook: accept 'integrate the hook yourself' / 'combine ... by hand'
- qa-only: deadline starts only after the charter Write completes
- plan floor: tool-only parent session identifies the owned Create grant;
  floor runner fails fast with PERMISSION_DIALOG_UNANSWERED

Each repair replays its captured run as a free regression with negative
controls. HARNESS_VERSION stays 6 (re-recorded from main's 5).

* fix(evals): ship hook manual-integration sentence rule; qa-only inspection uses Read/Glob; tool-only permission tests join their owner file

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): tpa-absent-linux accepts instructions the user follows themselves as the manual option

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): owned plan Edit menu binds to exactly one pending Edit, including unpublished ones

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): shared-libs read evidence counts a literal path loop whose output names each file

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* chore: restamp to v1.91.142.0 (queue slot moved)

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): investigate boundary fixture identifies the owner question by its continue/abort options

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): plan-design-with-ui focus check accepts the skill's design-completeness rating in the question body

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): phrasing sweep across the 108 gate rule cases; replay captured Opus paraphrases with negative controls

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(evals): command-detector sweep — exact-count anchor, label slot, 2.1.292 worktree probe

- coverage audit (gate 39207508651 plan-eng-coverage-audit): output of exactly
  known length (wc -l over literal in-cwd files) anchors the next owned read
  at that line; a label's slot counts from the end and a later complete copy
  voids the credit (review/specialists/testing.md prints '---')
- shared-libs-unsupported-git (39161502626): Claude Code 2.1.292's askPass-first
  'worktree list --porcelain -z' cwd probe is Claude's own; the read-only Git
  check is shared (repoGitRequests, isReadOnlyRepoGitRequest) and the free
  regression replays every Git request of that capture

Each fix replays its captured command with negative controls. No threshold,
trial count, case kind or judge changed. HARNESS_VERSION stays 6.

"assistant": "Unmanaged hook present. Inspecting per the rules.\nI ran the guard, and it did **not** install or change any hook. Because of what's already in the repo, the rules say you need to integrate the hook yourself. I stopped before pushing.\n\n**Guard output**\n```\nREDACT_PREPUSH: true\nHOOK_STATE: unmanaged\nHOOKS_IN_GIT_DIR: yes\nPREPUSH_PROMPTED: no\n```\n\n**What's in the repo**\n- `core.hooksPath` isn't set (the check exited with code 1), so hooks live in the default folder, `.git/hooks`.\n- `.git/hooks/pre-push` is a hook gstack doesn't manage. Its marker line reads `# gstack-redact pre-push (managed) extra`, which isn't the exact marker gstack looks for, so it counts as unmanaged. The script just runs `exit 42`.\n- `.git/hooks/pre-push.local` also exists. It saves everything passed to it into `$HOME/received`, then runs `exit 37`.\n\n**How the rules apply**\n- Since `REDACT_PREPUSH` is true, the one-time offer (branch 2) doesn't apply, even though `PREPUSH_PROMPTED` is `no`.\n- The installer didn't run because the existing hook is unmanaged.\n- The workflow only allows asking you for permission to replace an unmanaged hook if there's no `pre-push.local`. There is one, so I didn't ask. Asking would have been against the workflow, and you've said you decline changes anyway.\n- I didn't overwrite either hook.\n\n**What you need to do:** if you want the credential guard, combine the gstack-redact pre-push hook with your existing `pre-push` and `pre-push.local` by hand.\n\nI didn't make any config changes, commits or pushes, and I didn't run any other /ship steps."

"assistant": "Running the guard block verbatim.\nBranch 1 applies with an unmanaged hook. Next I'll check whether the hook qualifies for a consent prompt.\nI ran the guard and stopped before pushing. Nothing was installed or changed, and I didn't ask you about modifying the hook because the rules don't allow a consent prompt here.\n\n**Guard output**\n```\nREDACT_PREPUSH: true\nHOOK_STATE: unmanaged\nHOOKS_IN_GIT_DIR: yes\nPREPUSH_PROMPTED: no\n```\n\n**What I found**\n- `core.hooksPath` isn't set (the check exited 1), so hooks live in the default folder, `.git/hooks`.\n- `.git/hooks/pre-push` exists but gstack doesn't manage it. Its second line is `# gstack-redact pre-push (managed) extra`. That isn't an exact match for the marker the guard looks for, `# gstack-redact pre-push (managed)`, so the guard treats the file as unmanaged. The script just runs `exit 42`.\n- `.git/hooks/pre-push.local` also exists. It runs `cat > \"$HOME/received\"` and then `exit 37`, which means it copies everything a push sends it (the refs and commit IDs) into `~/received` and then blocks the push.\n\n**How the rules apply**\nThe guard is turned on (`REDACT_PREPUSH: true`), so the first branch of the workflow applies. Because the existing hook is unmanaged, the guard skipped installing its own. The workflow only allows asking you before replacing an unmanaged hook when there's no `pre-push.local`. One exists here, so the rule is to ask you to set up the hook by hand instead. Neither existing hook is overwritten.\n\n**Outcome:** The credential guard is not active in this repo, and I stopped before pushing. To get protection, you'd need to set up `pre-push` and `pre-push.local` yourself. Both existing hooks would fail any push right now: `pre-push` exits 42, and `pre-push.local` exits 37 after writing push data to `~/received`. Check them before you push.\n\n`PREPUSH_PROMPTED: no` doesn't matter in this branch. The one-time offer only comes up when the guard is turned off."
