Garry Tanandcapy-ai[bot] a889ef9393 v1.91.105.0 fix(tests): the free suite stops needing its retry pass; test:health tells the truth (#3154) (#3212)
* test: turn git auto-maintenance off in free fixtures, with a runner backstop

git 2.29+ starts a detached `git maintenance run --auto` after every commit;
it creates and deletes .git/objects/maintenance.lock after the commit returns.
Diagnosed on 47d639a (git 2.55.0):

- gstack-docs-candidate (ledger 37742176952/16): 4/150 replicated snapshots
saw the .git listing change, every one `-objects/maintenance.lock`; 0/150
with the keys. repo() now sets maintenance.auto=false and gc.auto=0.
- plan-design-sdk-fixture (ledger 37807608021/6): under commit load 18/150
runs failed the collector check; in every one Bun's rmSync(recursive, force)
returned without throwing and left the scratch repo behind. 0/100 with the
keys in the fixture's replacement git env, which never sees the runner's.
- qa-functional-observer (ledger 37800857251/4): under load 6/200 observer
setups threw ENOENT lstat .git/objects/maintenance.lock; 0/400 with the keys
in the inherited env. The observer and its fixture (22 paid cases) are not
edited; the runner backstop covers them.

runFreeShard appends the two keys after any inherited GIT_CONFIG_* pairs of
each shard child (a malformed inherited count is left for git to reject).
A GIT_TRACE commit shows 3 maintenance lines with defaults and 0 in the
runner child env; the new test pins that and the env merge cases.
Failure messages now name the changed .git entries and surviving fixture
directories; no assertion changed.

* test: synchronize the zombie publication-lock test on a framed PID line

Ledger 38153371796/3 and 38161906980/3: line 69 `existsSync(ready)` failed.
The test polled 300 x 10 ms for the Python parent's readiness file, then
another 300 x 10 ms of /proc for state Z; under shard load the fixed count
expired. existsSync also raced the write (an empty file read as pid 0).

The parent now waits for its child with os.waitid(P_PID, WEXITED|WNOWAIT),
which leaves it unreaped in state Z, and prints the PID with flush=True. The
test reads exactly one newline-framed line from the parent's stdout under the
test's own (unchanged) 10 s timeout, rejects a malformed or non-positive PID,
and makes one /proc read that must show Z. Early exit, a malformed line or a
timeout reports the parent's exit status and stderr tail; the owned parent is
always killed in cleanup.

Intentional assertion replacement: the readiness-file existence check and the
polling loop are replaced by the framed-line requirement (a positive PID) and
one /proc read; `expect(state).toBe('Z')` and every lock-recovery assertion
are unchanged.

* test: stop the next-version smoke test from calling live GitHub

Ledger 37804770972/9: "CLI runs against real repo and emits parseable JSON"
failed with `JSON Parse error: Unexpected EOF` after 30032 ms. The smoke test
ran the real CLI against live `gh`; bin/gstack-next-version's per-PR
`gh api` calls run one after another (spawnSync inside its async workers), so
27 open PRs passed the 30 s spawn timeout and stdout was empty. Measured with
a delayed fake gh: 8 PRs x 0.5 s = 4108 ms; 27 PRs x 1.2 s = 32637 ms. The
CLI defect is filed as a P1 follow-up, not fixed here.

The test keeps the real repository and the real CLI and puts a named fake gh
first on PATH. It logs every argv, answers exactly the calls the CLI makes
(auth status only when origin is not github.com, pr view exit 1, canned pr
list, repo view and contents with one same-owner PR claiming 1.6.4.0) and
exits 2 with a FAKE_GH_UNHANDLED marker for anything else; glab exits 1. The
test now checks the exit code with stderr before JSON.parse, the exact call
set, no marker, no glab call, host github, offline false, no fallback, no
warnings, the canned claim, and version 1.6.5.0. Every existing assertion is
kept; the spawn timeout is unchanged.

* fix(test-health): report the Windows wall over PR runs and name the seed's real prerequisite

free-wall read windows-free-tests.yml with `branch: 'main'`, but that
workflow runs only on pull requests and workflow_dispatch, so the "main" runs
it averaged were dispatch duration recordings. It now uses listWorkflowRuns'
existing `event: 'pull_request'` filter and prints the population inline:
`windows-free-tests (successful PR runs) 7.1 min (23 runs)`, or `no
successful PR runs in window`. The free-tests half is byte-identical.

The CEO-10 fix text stops offering a "CI recording" (none exists for the Linux
seed) and names UBICLOUD_API_KEY; test:ubicloud --record-durations copies the
seed back into the checkout, and contributors without the key leave the
refresh to a maintainer. docs/TESTING_INTERNALS.md drops "or a CI recording"
in the same change.

Intentional pin change: test/test-health-report.test.ts free-wall value
`windows-free-tests 0.0 min (1 runs)` -> `windows-free-tests (successful PR
runs) 0.0 min (1 runs)`; the fixture now carries a PR run and an excluded
dispatch run, plus a new case for the empty and mixed populations.

* fix(timeline-read): reject missing and unparseable --since/--branch values

`--since` and `--branch` read an unbound $2 under `set -u` and crashed with
exit 1 when given no value, and an unparseable --since ("yesterday", "2
weeks") silently returned unfiltered history because the Bun regex was
unanchored and fell through. Unknown arguments were silently ignored.

--since is now parsed once in bash (no `date -d`, no bash-4 features) into a
cutoff in epoch ms, and the Bun regex is deleted. Accepted forms: "<N>
minute|hour|day|week|month[s] ago" (case-insensitive, as before), "<N>h|d|w"
(the form gstack-global-discover documents) and ISO dates YYYY-MM-DD or
YYYY-MM-DDTHH:MM:SSZ (UTC). In #1723's shape, before any read:

--since must be a relative time like "7 days ago" (got no value); accepted: ...
--since must be a relative time like "7 days ago" (got 'yesterday'); accepted: ...
--branch must be a branch name (got flag '--limit')

with "got an empty value" for ''. Unknown arguments exit 2 with the usage
line; --help/-h prints it and exits 0. Tests extend the --limit table in
test/timeline.test.ts and run every case on empty and populated history,
plus each accepted --since form filtering old events; the quoted-branch and
no-filter tests are unchanged.

Behavior change: unrecognized arguments (previously ignored) now exit 2.

* docs: record the due audit follow-ups with verdicts, and file wave-2 follow-ups

docs/test-audit-2026-10.md gains a dated 2026-10-15 section from a no-spend
`bun run test:health --since-days 7`, with the 2026-10-12 figures from #3154
kept as history and a verdict per target: full-fallback 45.9% (open; the
audit's next step is touchfile mappings for bin/, lib/, hosts/, make-pdf/),
spend per merged PR $73.52 (verdict unavailable: the pre-change baseline,
recomputed with collectHealth at now=2026-10-04 behind an upper-bound client,
holds no trial-outcomes artifacts because the recorder shipped with the
audit), cancelled slice-minutes 1,545/week, push-to-verdict p50 11.4 min
(missed against 3-5 min), flaky-pass 6.1% (missed; this wave repairs it).
Census reliability is not due until 2026-10-26; the interim figure is
recorded and the repair stays with #3153/#3200.

TODOS.md: one appended dated section (P1 gstack-next-version serial fetch with
the 4160 ms measurement, P1 standing seed recorder, P1 full-fallback/spend,
P1 dated flake follow-ups, P2 createQAFunctionalFixture maintenance keys, P2
diagnostic-mode HOME/state-root artifacts). The timeline-read bullet was
deleted with its fix.

* chore(release): tentative stamp v1.91.105.0 with its CHANGELOG entry

Stamp-at-merge policy (.gstack/ship-policy.json): the branch carries a
tentative PATCH stamp and its CHANGELOG section directly under
`# Changelog`; the queue restamps at merge time. Base 47d639a is 1.91.101.0,
but the live slot scan shows 1.91.102.0 (#3190), 1.91.103.0 (#3200) and
1.91.104.0 (#3209) claimed, so this branch takes 1.91.105.0. Written with
`gstack-restamp --version 1.91.105.0 --base main --expect-base 47d639a
--allow-repo-commands` (VERSION, package.json's npm translation, the agents
digest via the policy's release tool, and the CHANGELOG header).

* test(seed): refresh the free-suite duration seed from a Ubicloud recording (#3154)

Recorded with `bun run test:ubicloud --record-durations` (standard-16,
eu-central-h1) on this branch, which equals origin/main 47d639a plus this
wave; the recorder exited 0 and wrote 1219 durations (1055 before, 164 new,
none removed; serial 2439 s -> 3202 s). `test:health` CEO-10 now reports
0 unseeded files.

Two earlier recordings did not count: one failed on an unrelated fixed-sleep
race in test/plan-seed-submission.test.ts (filed P2, not edited here), the
other failed free-seed-ratchet because it ran against the first attempt's
pulled seed. No file was excluded.

free-seed-ratchet: three files are over the 60 s ceiling in this recording
and are allowlisted with a split todo (setup-install-registry 82 s,
host-renders-prune 64 s, setup-codex-scope 60.4 s); the split follow-up and
the plan-seed-submission race are filed in the appended TODOS section.

The ratchet test now names scripts/free-test-seed-allowlist.json literally
(a pin on SEED_ALLOWLIST_FILE): pregate's lanes check maps data files to
tests through string literals, and the allowlist had no lane
(PREGATE_NO_LANE). Additive assertion; nothing weakened.

* test: give the git-maintenance backstop test a Windows-safe empty global config

windows-free-shard (2) on #3212 (run 38832498861) failed two cases in
test/test-free-shards-git-maintenance.test.ts, both serially reproduced by
the retry pass: Git for Windows rejects GIT_CONFIG_GLOBAL=os.devNull
(`fatal: unable to access '\\.\nul': Invalid argument`), so the malformed-count
case saw that error instead of git's GIT_CONFIG_COUNT rejection, and the traced
commit's `git init` exited nonzero. Point GIT_CONFIG_GLOBAL at a file that does
not exist in the scratch HOME, as test/ceo-plan-mode-fixture.test.ts does;
git reads a missing global config as empty on every platform. No assertion
changed.

Garry Tan 47d639abff v1.91.101.0 fix: close the field wave's residual gaps (#3173)
The field wave's residual gaps are closed. v1.91.96.0 through v1.91.100.0
shipped the install-honesty and unattended-`/autoplan` wave
(`docs/designs/GSTACK_FIELD_WAVE_2026_10_12.md`); running that release on a
Capy machine left six gaps, each reproduced here with a test that failed
first.

Garry Tanandcapy-ai[bot] 7edaf4f50a v1.91.98.0 fix: plan obligations meet repo gates; priced paid gates; machine-proved restamps (field wave C, items 14-17) (#3166)
* docs(designs): gstack field wave plan (2026-10-12) and the shared simulated Capy host fixture

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(pregate): price_command config key for the non-executing call plan a paid gate is priced from

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(pregate): repo guards a lane must pass, verified launch Bun and per-file serial runs in lanes, unknown flags exit 2

The guards check runs each .gstack/pregate.json guard whose globs match a
touched file in its own process with its own timeout (PREGATE_GUARD_FAILED,
never downgradable, incomplete without --allow-repo-commands). The lanes
runner and guards get the first Bun in $BUN_INSTALL/bin or ~/.bun/bin that
satisfies gstack's and the project's engines.bun; serial_patterns files run
one process each. price_file replaces the unreleased price_command key: a
paid gate is priced from a call-plan data file, never by running a command.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(pricing): Sonnet 5.5, Gemini 3.x and gbrain embedding rows from fetched pages; Opus 4.7 at its current list price

Each new row carries its source URL and fetch date (2026-10-12). Embedding
models bill input only, so they live in EMBEDDING_PRICING beside PRICING
(eval arms never resolve to them); pricingRow also accepts gbrain's
provider:model ids (voyage:voyage-4). claude-opus-4-7 moves from 15/75 to the
5/25 the Anthropic pricing page lists; benchmark-runner's expected costs follow.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(eval-plan): price-gate prices a paid gate from a call-plan data file, never executing it

gstack-eval-plan price-gate --gate "<exact command>" reads a GSTACK_EVAL_CALLS
v1 file (--plan-file, else the base policy's price_file at HEAD), checks its
binding (gate command string equality, head with only the plan file changed
since, every input's sha256 at HEAD), prices each call purpose (aggregate
totals) and each one-call reservation, and writes a receipt binding the price
to the gate, head, inputs and plan bytes. Unpriced models print unknown, never
zero (EVAL_PRICE_MISSING); a missing plan or no call lines is
EVAL_GATE_UNPRICED unless --allow-unpriced records an owner override; over
--cap-usd / --smoke-cap-usd is EVAL_GATE_OVER_CAP; malformed or partial files
refuse EVAL_DRY_RUN_FAILED and stale ones EVAL_PLAN_STALE. dry-run --cap adds
the same reservation-fits-cap check. Wire format, receipt and the reference
emitter (formatEvalCalls): docs/eval-calls.md.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(eval-plan): the approval step prices every paid gate with price-gate

Step 4 runs gstack-eval-plan price-gate for each paid gate the plan names and
puts its GATE_PRICE line in the ask; the dry-run checklist documents
reservation-fits-cap under --cap.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(sealed): sealed-output guard over every published commit, in the pre-push hook and gstack-pregate sealed

A repo with a sealed key in .gstack/pregate.json (base branch union the
pushed commit, so a push can tighten but never relax) refuses publishing
sealed paths, transcripts, quoted answers, seeds or local home paths to a
public or unknown destination (SEALED_OUTPUT_REFUSED); private warns.
Path rules see every added, renamed or copied path in each published
commit (git rev-list <local> --not --remotes=<remote>), text rules each
commit's added lines; waivers only from the base branch allow file; a
malformed policy fails closed; visibility caches public 24 h, private 1 h.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(eval-plan): only an undeclared plan is unpriced; a missing price_file is stale, a bad base policy refuses

price-gate's EVAL_GATE_UNPRICED (and --allow-unpriced) now covers only no
plan declared at all or a plan with zero call lines. A declared price_file
missing from the HEAD commit is EVAL_PLAN_STALE (exit 1, not overridable); an
unresolvable origin/<base> is POLICY_SOURCE_UNAVAILABLE and an invalid
.gstack/pregate.json is PREGATE_CONFIG_INVALID (both exit 3).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(restamp): the repo's stamp rule (stamp_files, literal stamp_patterns) and restamp_command in the staging worktree

stamp_files is an allowlist applied before every stamp rule; stamp_patterns
are bounded literal {version} templates shared by the restamp writer and the
tree receipt. restamp_command runs first in the staging worktree with the
target version (argument and GSTACK_RESTAMP_VERSION), needs a clean caller
tree, skips gstack's version writer and keeps output confinement; a failure
or a wrong version writes nothing. migrations { dir, id } is the policy key
gstack-pregate regate reads.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(pregate): gstack-pregate regate --against <gated-head> with per-lane reuse and migration collisions

Reuse comes only from a tree receipt evaluated now, with a bundle produced
on the gated tree, the current execution identity and an unmoved, known
predecessor head. Each lane prints reusable/rerun/not-reusable; REGATE:
not-needed needs stamp-only, every bundle and --require lane reusable (a
stamp-sensitive lane reruns) and a finished migration lookup. Migration
numbers the branch adds are checked against the base tree and every other
open PR (gh pr list --limit 1000; 100-file lists refetched with gh api
--paginate); any lookup that cannot finish is unknown and blocks
not-needed (MIGRATION_COLLISION). Bare --against routes to the verb.
docs/pregate.md gains the regate section and the gbrain worked configs.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(contract): lanes run gstack-pregate (and the sealed guard where configured) before done; handoff carries the gstack trailer and PREGATE: line

The coordinator contract gains a 'Before done' paragraph and the handoff
order GSTACK_RESULT, gstack: ok|fail <rev>, PREGATE:, gate summary; the
spec section is regenerated and docs/unattended.md states the same rule.
A new test pins the terms and their order; the byte-identity pins stay.

Garry Tanandcapy-ai[bot] 945ac301a9 v1.91.97.0 fix: closable unattended /autoplan runs on Capy (field wave B, items 6-13) (#3165)

* fix(autoplan): scope keys on rendered surfaces, per-axis provenance, --no-dx and a read-only plan marker

UI scope reads the Implementation plan only and needs a rendered file path or a UI
term next to a visual noun; bare UI words stay weak evidence (sections load, no
Design phase). Scope records sources {ui,dx} (flag|marker|detected), --no-dx turns
DX off, a first-line <!-- gstack-autoplan-scope: ... --> marker is read and never
written, and the first next prints SCOPE: evidence plus a SCOPE_PIN hint. The
snapshot tool's scope output carries uiRequired from the same detector and
autoplan Step 2 uses it instead of a prose grep (field wave item 11).

Intentional pin change: test/review-checklists.test.ts Scope literal and the
source= line shape (source=ui:detected,dx:detected).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(plan-reality): a receipt per measured number, the rule text for a rule claim, dot-path receipts

Each clause stating a measured quantity (a unit, or a number after measured/costs/
takes/took) needs its own receipt; misses print unreceipted_numbers=<n> and one
NUMBER_UNRECEIPTED: <clause> line each. A line claiming a decision rule must quote
it or link its file:line (unquoted=<n>). Either miss maps to the existing verdict
(incomplete unattended, repair otherwise). RECEIPT_RE accepts dot paths and needs
a hex letter or a commit/backtick context for a sha. New pure checkPlanReality(planText)
runs the same checks over plan prose for gstack-autoplan submit (field wave item 12).
The unreceipted= text token is now pinned as well as the JSON.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(autoplan): generated DRIVER.md, the compact entry for a parent driving gstack-autoplan

autoplan/DRIVER.md (from autoplan/DRIVER.md.tmpl) is what a parent agent on any host
reads instead of autoplan/SKILL.md: phase order and scope flags, the next/submit/
answer/revise/export loop with the plan-specified submit --record/--attest/
--unavailable, revise --incorporates/--withdraw and next --scaffold, the reviewer
RESULT FORMAT, the gate grammar including all as-is, the run files, the result codes
and the coordinator contract, every list rendered from code (117 lines). gen:skill-docs
emits it as a Claude asset, so normal, out-dir and dry-run inventories and the
freshness check cover it. gstack-autoplan --help and the first next print
DRIVER: <root>/autoplan/DRIVER.md, resolved through an install root's recorded
.source-path (new lib/source-root.ts) (field wave item 13).

Intentional pin change: test/gen-skill-docs-checks.test.ts asset inventory gains
autoplan/DRIVER.md.

* feat(doctor): --min-revision, a launch Bun found off PATH, planning-only pins SKIP

gstack-doctor --check gains --min-revision <v>|unknown: below it the revision
row FAILs with REVISION_BELOW_MINIMUM naming the Capy wrapper or
bin/gstack-upgrade; unknown says so and never claims the minimum. The runtime
and pins rows name a supported Bun found in $BUN_INSTALL/bin (not on PATH)
with the export/GSTACK_LAUNCH fix (gstack_bun_launch_dir, one helper in
bin/gstack-bun-version.sh). --for a planning-only workflow (autoplan, the plan
reviews, office-hours) reports a repo test-pin mismatch as SKIP pins
(planning-only; ...). New result codes UPGRADE_REFUSED, UPGRADE_FETCH_FAILED,
REVISION_BELOW_MINIMUM, CAPY_WRAPPER_STALE, INSTALL_UNVERIFIED with
troubleshooting anchors; REVISION_UNMET's fix names bin/gstack-upgrade.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(install): install verdict and install admission at skill start

bin/gstack-install-verdict.sh (Bash) and lib/install-verdict.ts (TypeScript)
are the one reader of the install verdict gstack-capy-install writes to
<state root>/install-verdicts/<sha256(checkout realpath)>.json (checkout,
HEAD, workflow and required components, planning-only pins, trailer). The
source checkout and capy/support.env resolve through gstack_source_root /
sourceRoot, which follow an installed root's .source-path.

gstack-skill-start, after SESSION_KIND: an unattended session (any host)
refuses on a PATH Bun below the supported minimum; with GSTACK_CAPY=1 an
unattended or explicit spawned session also refuses without a passing
verdict for this checkout, HEAD and workflow (INSTALL_UNVERIFIED). A refusal
prints the INSTALL: row, an install-refused instruction block bound to its
SESSION_ID, GSTACK_RESULT ... status=refused run=- and exits 3. Ambient
spawned sessions warn; GSTACK_ALLOW_UNSUPPORTED_RUNTIME=1 admits and says so.
capy/support.env declares GSTACK_MIN_REVISION and GSTACK_CAPY_MIN_WRAPPER.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(capy-install): launch PATH, Bun reuse and CI pins, setup skip on HEAD, verdict, --check

The GSTACK_LAUNCH line carries PATH=<dir>:$PATH when the certified Bun is not
first on PATH, plus GSTACK_CAPY=1 and GSTACK_INSTALL_VERDICT, and
GSTACK_LAUNCH_PATH: <dir>; the doctor runs with that PATH. A supported Bun in
$BUN_INSTALL/bin is reused instead of downloaded again. Project pins are read
with bin/gstack-runtime-pins.ts (CI lanes included): when every exact pin is
gstack's verified version the archive is installed alongside a different PATH
Bun; any other exact pin is RUNTIME_PIN_CONFLICT. Both ./setup runs are
skipped when this HEAD was set up with the same hosts, model and browser flag
and the check passes (never after a fast-forward); a failed setup ends with
gstack: fail setup and is retried next run. --check certifies only;
GSTACK_CAPY_LOCK_HELD=1 accepts the bootstrapper's held lock; result anchors
come from the generated components table; REV_NOTE names bin/gstack-upgrade.

Intentional pin change: test/gstack-capy-install.test.ts --revision trailer
now names <root>/bin/gstack-upgrade (the old text named a command that never
existed).

* result codes: PLAN_SECTIONS_INVALID, PLAN_AMEND_PENDING, GSTACK_VERSION_CHANGED, INSTALL_VERDICT_FAILED, ARTIFACT_GATE_UNSETTLED

Troubleshooting anchors for each and the regenerated doctor components table (field wave lane B-core).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* artifact contract: changes, recommended_basis, carried decisions, attested count, settled-gate check

Findings and decisions carry the user challenge's `changes` value; decisions record recommended_basis, origin,
carried_from and chosen_text; validate refuses a complete run with an unsettled gate (ARTIFACT_GATE_UNSETTLED) and
prints attested=<n>. The gate reply grammar gains `all as-is` and resolves partial replies against prior answers
(field wave B2a/B2b).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* autoplan: run-state fields, plan-text hashing, version migration and admission modules

RunState gains gstack_version, plan_input/heading, scope_flags, plan_revision, amendments and plan_reality; voices
record receipt kind and reviewed hash. New pure modules: Implementation-plan review hash, scaffold and drift lines
(B1c); gstack_version stamp and legacy decision-id migration (B1a); install-verdict and Bun-minimum admission
with a local verdict shim until lane A's lib/install-verdict.ts lands.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

bin/gstack-autoplan-snapshot.ts exports its section scanner and review projection, and initializes only a plan with
no section headings by wrapping it.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* autoplan: closable runs - submit --record/--attest/--unavailable, revise, amendment interlock, agreeing-UC defaults, admission

B1a: an older run directory prints GSTACK_VERSION_CHANGED once and migrates legacy decision ids; a newer one refuses.
B1b: submit binds hand-run records (fence, table, severity list, No findings.), attested binds need --attempt,
--attempt is optional with one open attempt, --unavailable/--failed close an outside voice native_only, and submit
caches the plan-reality report by Implementation-plan hash.
B1c: revise reviews a revised plan in the same run as one recoverable transaction (reopen on hash drift plus Eng,
add newly in-scope phases, .r<rev>/.v<k> archives, gate_rev+1 at the gate); PLAN_SECTIONS_INVALID and next --scaffold;
PLAN_CHANGED / PLAN_SOURCE_CHANGED drift hints.
B2a/B2b: user challenges carry changes; agreeing voices with the same changes on a confirmed finding recommend b with
both remedies shown (or the reconcile proposal); accepted changes print APPLY and become pending amendments that next
and export refuse past until revise --incorporates names them or --withdraw drops one; all as-is; partial replies,
DECIDED block, carry-forward and rerun demotion.
Mutating verbs pass admission (GSTACK_CAPY install verdict; unattended Bun minimum; override recorded as a deviation).
Intentional pin rewrite: test/autoplan-answer-invalidation.test.ts reopen tests now revise --incorporates before rerun.

* feat(upgrade): bin/gstack-upgrade, the unattended fast-forward every fix line names

--check fetches and prints current|behind <installed> -> <remote> (<n>
commits); the default fast-forwards to the remote default branch tip and
reinstalls (bin/gstack-capy-install on a Capy install, else ./setup
--refresh-registered), completing a pending reinstall on the next run. It
refuses (UPGRADE_REFUSED, exit 3, nothing changed) a dirty tracked byte, a
diverged commit, a pinned checkout, the incoming Bun floor and a held
.setup-lock; never stashes or resets; an unreachable remote is
UPGRADE_FETCH_FAILED. Fetches are receipted (fail-open). The unattended
skill-end UPGRADE_AVAILABLE line is followed by UPGRADE_COMMAND naming it.

* feat(capy): vendored Capy skill, frozen bootstrapper and full wrapper bin/gstack-capy-session

capy/skill/gstack/ is the Drive skill's source of truth. scripts/install.sh
is a frozen bootstrapper: it prints gstack-capy-skill: wrapper <n> (<path>),
clones or fetches under .gstack-capy-install.lock, reads the remote
capy/support.env (FETCH_HEAD), fast-forwards a clean unpinned checkout below
GSTACK_MIN_REVISION (refusing dirty, diverged or pinned checkouts with the
exact commands; GSTACK_CAPY_AUTO_UPGRADE=0 disables), and execs the
checkout's own bin/gstack-capy-session with GSTACK_CAPY_LOCK_HELD=1 so the
held lock is never taken twice. Offline it says the remote minimum is
unknown. A Drive copy older than GSTACK_CAPY_MIN_WRAPPER prints an owner
action (CAPY_WRAPPER_STALE). SKILL.md: check at session start, install
lazily before the first workflow, the GSTACK_LAUNCH line, the trailer rule,
the subagent first-report line, gstack-pregate sealed before any push, and
the autoplan/DRIVER.md pointer. The context-budget fixture gains the skill's
entry.

* docs: Capy install layers, lazy install, snapshot bake, forward-only recovery, raising the minimum

docs/capy-install.md describes the three layers (Drive bootstrapper, full
wrapper, installer), the lazy install, the GSTACK_LAUNCH line, install
admission, baking gstack into a Capy snapshot, and that recovery after an
interrupted fast-forward completes or refuses the pending install stages
(checking out an older commit is not a rollback). docs/CHANGELOG_STYLE.md
(the release doc; there is no docs/RELEASING.md) says when
GSTACK_MIN_REVISION may be raised.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* refactor(capy): the stale-copy owner action moves from the frozen bootstrapper to bin/gstack-capy-session

The checkout-owned full wrapper reads GSTACK_CAPY_MIN_WRAPPER through
gstack_support_env_get(gstack_source_root) and compares it with the
bootstrapper revision it was handed, so the rule can change without
recopying the Drive skill. The bootstrapper keeps only what a stale checkout
cannot do: clone, fetch, read the remote minimum, fast-forward, exec.

* feat(doctor): --min-revision, a launch Bun found off PATH, planning-only pins SKIP

gstack-doctor --check gains --min-revision <v>|unknown: below it the revision
row FAILs with REVISION_BELOW_MINIMUM naming the Capy wrapper or
bin/gstack-upgrade; unknown says so and never claims the minimum. The runtime
and pins rows name a supported Bun found in $BUN_INSTALL/bin (not on PATH)
with the export/GSTACK_LAUNCH fix (gstack_bun_launch_dir, one helper in
bin/gstack-bun-version.sh). --for a planning-only workflow (autoplan, the plan
reviews, office-hours) reports a repo test-pin mismatch as SKIP pins
(planning-only; ...). New result codes UPGRADE_REFUSED, UPGRADE_FETCH_FAILED,
REVISION_BELOW_MINIMUM, CAPY_WRAPPER_STALE, INSTALL_UNVERIFIED with
troubleshooting anchors; REVISION_UNMET's fix names bin/gstack-upgrade.

Garry Tanandcapy-ai[bot] dbf400ec3f fix(autoplan): decision ids stay unique across reopened gate revisions (#3160)
Gate item labels restart at every gate revision, but decision rows were minted
as <run>-<label>, so a second gate reused the first gate's ids and
gstack-artifact validate failed the export (ARTIFACT_DUPLICATE_ID). Revisions
after the first mint <run>-r<rev>-<label> through one decisionId helper, and
exported tasks block on the current gate's plan approval.

70fab3e246 v1.91.94.0 feat(codex): --effort <level> overrides reasoning effort for one run (#3146) (#3151)
* feat(codex): --effort <level> sets GSTACK_CODEX_EFFORT for one run

/codex review --effort medium (or --effort=medium) prefixes each
run-with-timeout dispatch with GSTACK_CODEX_EFFORT=<level>, which outranks
codex_effort and the per-mode default; --xhigh becomes shorthand for
--effort xhigh. The probe keeps owning validation. A missing value, a value
that is not one lowercase word, or conflicting flags stop instead of falling
back.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* test(codex): per-run effort prefix reaches the rendered /codex review dispatch

Runs the rendered review fence against the fake codex: no prefix uses
codex_effort, GSTACK_CODEX_EFFORT=medium|xhigh wins over it, an unknown level
never reaches codex. Step 1 check pins --effort tokens and the stop rule.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* docs(codex): document codex_effort and --effort; Challenge runs at high, not xhigh

* docs(changelog): [Unreleased] entry for /codex --effort

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

* fix(codex): --effort reaches the probe as an allowlisted request level, parsed before the mode and checked before the paid probe

The level now travels as run-with-timeout --request-effort '<effort-level>'
(placeholder grammar ^[a-z]*$) instead of an agent-built env prefix, and the
probe names /codex --effort when it rejects one. Effort flags count only right
after /codex or its mode word and are removed before mode detection. Step 0.5
runs check-effort before probe-model, so a bad level makes no Codex call even
on a cold cache. Tests cover all five dispatch fences and the zero-call cases.

8fc9f2a58d v1.91.91.0 fix(review-gates): fail closed on unparsed severities, survive auto-compaction copies, read-only plan-eng-review reports back (Wave 9 Lane B) (#3143)
Every outside-review gate, execution included, reads P0-P3 labels in label
position, table cells and severity words; an unreadable label is unverified
(unparsed_severity) and "Verdict: not ready" with no severity blocks with
FINDINGS: unspecified. The owned journal reader collapses Claude Code
2.1.293's post-compaction copies (equal apart from uuid, parentUuid and
message.usage), so /autoplan's guard no longer denies with tool_identity,
init_own or a false disarm. /plan-eng-review with writes forbidden ends DONE
with PERSISTED: no and a final PERSIST: line; /ship names it when no Eng
Review is saved.

6e250405fe v1.91.90.0 fix(windows): LF for nested scripts with a setup CRLF repair, Stop hook slug fast path, setup-gbrain smoke test via temp file (Wave 9 Lane G) (#3142)
.gitattributes pins nested bin/, hook shims, lib/cso/images and scripts/app
to LF, and setup rewrites copies an older checkout left with CRLF, leaving
locally edited files alone with a warning (#3110). The timeline Stop hook
reads the slug from gstack-slug's cache and hands a miss to a detached,
locked, negatively cached repair child (#3103). The /setup-gbrain smoke test
writes its page to a temp file instead of piping into gbrain put (#1675).

e6edff0265 v1.91.89.0 fix(windows,setup): Git Bash hook false-positive recovery, incoming-checker auto-update, setup never deletes its own checkout (Wave 9 Lane A0) (#3140)
Git Bash hands native Bun C:/ paths while gstack-hook-check looked them up as
/c/, so every TypeScript hook was refused and team-mode auto-update held every
release since 1.91.31. The checker maps Bun's paths back to the shell spelling,
the auto-updater lets the incoming release's own checker decide (installed
checker only as a fallback, hook-check-unavailable when neither verifies), and
docs/troubleshooting.md gives stuck installs one recovery command.
/gstack-upgrade and /spec guards use the --show-prefix root check, gstack-wtree
treats drive-letter --git-path answers as absolute, and setup refuses to
replace a runtime root that is or contains its own checkout, Kiro's in-place
root included (#3084), with --status suggesting the move. New sourced helper
bin/gstack-path-spelling.sh and TS twin lib/path-spelling.ts.

3d52b26905 v1.91.88.0 fix(memory): transcript ingest lands on managed gbrain 0.60, /retro global counts every session (Wave 9 Lane F) (#3136)
#3097: on a managed gbrain 0.60 brain every transcript source was path-less,
so every managed import into it was refused (owner_unavailable). Each
transcript source is now bound to its own checkout under
<state root>/gbrain-transcript-sources/<id>/; path-less sources are migrated
on the next run and resumed checkpoints are admitted first; the checkout holds
one amended snapshot commit once every page has its file. Sources held by
another owner are left alone with the owner and commands named; nothing is
bound under an ephemeral state root. Classic brains are unchanged.

#1315: global-discover reads each Claude Code session's recorded cwd (whole
lines up to 1 MiB, longer lines skipped and counted) and resolves it through
git to its worktree root instead of decoding the project directory name.

Garry Tanandningfeemic-dev 47cd4a9f14 v1.91.86.0 fix(browse): proxy from environment, Windows 8.3 temp paths, no NUL bunfig (Wave 9 Lane E) (#3135)
browse resolves its proxy from --proxy, BROWSE_PROXY_URL, then HTTPS_PROXY,
HTTP_PROXY and ALL_PROXY (BROWSE_PROXY_URL=direct opts out), translates
NO_PROXY to Chromium bypass rules with loopback always kept, and hashes the
proxy and NO_PROXY rules into the daemon config so a differing environment is
refused with the variable named. Unparseable standard variables warn and go
direct; browse stop always stops (#915, supersedes #916).

Safe directories and targets resolve through one realpath
(fs.realpathSync.native), fixing Windows 8.3 temp paths (#3104). Bun children
get an empty private bunfig instead of --config=NUL, which panicked Bun
1.3.10 on Windows (#3085).

50d020ad9d v1.91.85.0 fix(cso,deploy): /cso on Windows Git 2.56 and macOS Docker, zsh-safe loops, honest deploy stops (Wave 9 Lane D) (#3134)
/cso sets GIT_CONFIG_GLOBAL=/dev/null on every platform (Git for Windows 2.56
rejects NUL), shared by childEnvironment and the eval and producer Git
environments, with a Windows-safe test that `git config --global --list` is
empty. On macOS the local Docker socket resolves without opening it when Bun's
realpathSync throws EOPNOTSUPP; Linux admission and the local-Unix-only rule
are unchanged. /test-audit and /ship loop over file lists zsh does not
word-split, with a static test against `for x in $VAR` in generated bash.
/setup-deploy stops and writes nothing when nothing is deployed yet;
/land-and-deploy runs a recorded command trigger only with explicit approval
before claiming revision proof. TODOS.md records the open P2/P3 bugs by area,
the Wave 9 parked items and the feature themes.

Closes #3082. Adopts #3094, #3095, #3096 and #3109.

* test(pregate): keep the bin-driving pregate tests off the Windows subset; pin files are free-only for eval selection

The three C-pregate tests that spawn bin/gstack-regen, bin/gstack-pregate,
bin/gstack-issue-links and bin/gstack-ship-receipt do so through
test/helpers/pregate-fixture.ts runBin, so the Windows curation's source
scan of the test file never saw a bin/ spawn and shipped them to
windows-latest, where CreateProcess cannot exec a shebang (14 failures in
windows-free-shard 2 and 5). Each is now a KNOWN_WINDOWS_INCOMPATIBLE entry
with the precise reason. .gstack/generated.json and .gstack/pregate.json
join FREE_ONLY_PR_FILES: no paid case reads gstack's own pin files (the
tools read origin/<base> of the repo they run in, and paid cases run in
fixture repos), so touching them no longer restores every gate case.

* test(harness-version): record the FREE_ONLY_PR_FILES pin-file entries as non-behavioral

scripts/test-pr-profile.ts is a pinned shared-harness file; the two

* fix(ci): result-codes is selection data, receipt counts come from files, C-stamp tests leave the Windows subset

- scripts/pr-dependencies.ts: lib/result-codes.ts joins SELECTION_DATA_MODULES; its fix: prose names commands a person reruns, not files the module consumes (test/pr-dependencies.test.ts traced review cases through it to bin/gstack-next-version)
- ship/sections/pr-body.md.tmpl: gstack-ship-receipt write reads --gate/--ci from agent-written .gstack/tmp/ship-*.json files and --spend-usd from the allowlisted <spend-usd> placeholder (generated-bash lint: no free text in commands)
- scripts/lib/windows-curation.ts: the five C-stamp test files spawn bin/ shebang scripts through test/helpers/restamp-fixture.ts, which the source scan misses

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(ship): the receipt step names where each input comes from

The workflow judge (eval-slices 3, median 3/4) found pr-body's gstack-ship-receipt command demanding a tree receipt, CI counts and a spend figure no earlier step produces, with RECEIPT_INVALID halting publication. The command now takes only Step 16 stage 4's lane counts (file-written JSON); --receipt is added only when Step 16 wrote .gstack/tmp/tree-receipt.json; RECEIPT_INVALID says to fix the named input file and rerun. The unused <spend-usd> allowlist entry is removed.

Garry Tan 91bbd9e6c4 v1.91.69.0 fix(cso-eval): producers keep usage on timeout, give the agent reporting time, and accept the helper's own artifact names (#3111)
- every helper artifact path tripped the secret redactor: the run ID's
13-digit epoch reads as a phone number, some 32-hex IDs as wallets.
Validate paths against the helper layout; schema-shaped ID components
are accepted by shape, every other component still goes through redact
- the provider was killed at exactly --budget after cell start, before
the helper deadline (start + budget) and before finish; give it the
budget plus a 300 s reporting grace (evaluator graceSeconds)
- Claude producers stream events (stream-json --verbose
--include-partial-messages) and add usage per message, so a timed-out
receipt keeps tokens, cost, turns and the last text
- failed post-run checks name the check in the receipt reason

Garry Tan 20eb6202fa v1.91.68.0 fix(cso-eval): Claude producers keep the source sealed, run the launcher outside the sandbox, and keep usage on failed checks (#3093)
* v1.91.68.0 fix(cso-eval): failed receipts with usage for post-run integrity failures; Claude producers stop mutating the sealed source

Claude Code adds <dir>/.mcp.json as a sandbox deny path for every --add-dir.
The producer passed the sealed source as one, so bubblewrap left an empty
0444 .mcp.json mount point there that Claude Code cannot unlink from the 0555
directory, and the post-run source check failed with no receipt.

- claudeExecArgs no longer adds the source directory
- post-run integrity failures write status:failed receipts carrying the
check code/reason plus usage, tokens and estimated cost (no output or
  artifacts)
- INVALID_PRODUCER_SOURCE / PRODUCER_CHANGED_SOURCE_MODE name the first
offending relative path and the rule it broke

* fix(cso-eval): run the trusted launcher outside Claude's sandbox; keep every other command sandboxed

Claude Code 2.1.263 forces every Bash command into its bubblewrap sandbox
while CLAUDE_CODE_SUBPROCESS_ENV_SCRUB is set, excludedCommands
notwithstanding. Inside it the launcher's owner check sees root as nobody
and the Docker socket is unreachable, so comprehensive cells degraded to
static-only results.

- drop CLAUDE_CODE_SUBPROCESS_ENV_SCRUB for producers; pass --settings with
sandbox enabled, failIfUnavailable, no unsandboxed fallback, no
auto-allow, and excludedCommands naming only the exact launcher path
- ClaudeAdapter.run refuses to start the agent
(CLAUDE_PRODUCER_SANDBOX_POLICY_MISMATCH) unless args and env carry
  exactly that policy
- README containment note and CHANGELOG updated

* test(doctor): split the two multi-run doctor tests so each fits Windows' 30s per-test budget

On windows-latest each bin/gstack-doctor run costs ~4-5s. The cached/live probe test made 7 runs and the artifacts-sync test 5; both hit the 30s per-test timeout, which killed doctor mid-output ("no row codex probe") and then EBUSY on cleanup. Same failure on #3093 and the fix/runtime-root-source-guard PR. Every assertion is kept; each test now makes at most 4 runs.

54efba6dd5 v1.91.67.0 feat(hosts): Antigravity CLI (agy) host (#3089)
* feat(hosts): Antigravity CLI (agy) host

Adds Google's Antigravity CLI as an experimental host through defineHost():
skills in ~/.gemini/antigravity-cli/skills/gstack-*, a runtime root with
.source-path for /gstack-upgrade, names equal to their gstack-<skill>
directories, name+description frontmatter with the 1024-char limit, the
Gemini overlay, and a preamble glossary mapping Claude tool names to
Antigravity's documented tools (run_command, view_file, write_to_file,
replace_file_content, grep_search, find_by_name, invoke_subagent,
ask_question).

Setup: ./setup --host agy (alias antigravity), --host auto on the agy
binary or ~/.gemini/antigravity-cli, never a Gemini-CLI-only ~/.gemini.
Registry tier, --status known roots, uninstall and team-init cover it.
Gemini CLI's ~/.gemini/skills and settings are never touched.

* ci: refresh GitHub Actions SHA pins (setup-buildx 4.4.1, build-push 7.4.0, attest 4.2.2, setup-node 7.0.0)

Rewrite of dependabot #2988. Every bumped `uses:` stays pinned to the full
commit SHA (verified against the upstream tags) and now carries its exact
version comment, including the lines that had none. setup-node 6.5.0 -> 7.0.0
is a major bump; its notes list an ESM migration, cache outputs and removal of
a dummy NODE_AUTH_TOKEN export, none of which affect our node-version-only use.
actionlint 1.7.11 (CI image) and 1.7.12 are clean.

* docs(openclaw): stop advising a per-spawn env for spawned sessions

OpenClaw's sessions_spawn has no env argument (openclaw/openclaw#51654

ACP harness or Gateway environment instead and guard the old advice.

* fix(benchmark): scope /benchmark routing to web page performance

/benchmark claimed the bare words "benchmark" and "performance", so model
and eval benchmark requests could route to the web performance skill
instead of /benchmark-models. Narrow the description, Use-when list and
triggers to web-specific phrases, point model/skill benchmarks at
/benchmark-models, and split the root router line the same way. The
catalog lead stays within the discovery budget ("Web performance ...").

Adopts #2302 and closes duplicate #2083 (rewritten on current main).

* fix(llms.txt): link skills by generated path, drop host-skipped skills

gen-llms-txt built every skill link as <name>/SKILL.md, so the router
pointed at a nonexistent gstack/SKILL.md (it is the root SKILL.md) and
/claude-code pointed at a SKILL.md the Claude host never generates.
Links now use the template's generated output path, and skills the
Claude host skips are left out of the index. Adopts #2279.

* fix(ci): install Bun from the SHA-256-verified release archive in GitLab CI and Ubicloud setup

Rewrite of #1713. .gitlab-ci.yml's .setup-bun anchor and
scripts/ubicloud/setup-free-suite.sh still piped https://bun.sh/install into
bash. Both now download the bun-v$BUN_VERSION linux-x64 release zip, check it
against the same SHA-256 Dockerfile.ci uses (#1706) before unzipping, install
it to ~/.bun/bin and assert `bun --version`. test/ci-image-cli-pin.test.ts runs
curlPipedToShell over both files and binds their hash and BUN_VERSION to
Dockerfile.ci's amd64 arm and package.json's engines floor, so a Bun bump has
to move all of them together. Verified by running the GitLab anchor in
debian:stable-slim (bash and sh) and the Ubicloud block in a temp HOME.

* fix(browse): viewport auto unpins a pinned viewport without a restart

`viewport WxH` pinned the page with no way back short of `browse restart`,
which also drops a headed /connect-chrome session. `viewport auto` (aliases
`reset`, `unpin`) now undoes the pin:

- headed: the context already launches with `viewport: null`, but Playwright
cannot drop a page's emulated size (and re-applies it on new frame
sessions), so the active tab gets a fresh page in the same context at the
same URL or load-html content. Cookies and storage persist, the tab id and
ownership stay, refs reset as on navigation, and the page follows the real
  window again.
- headless: back to the 1280x720 launch default, tracked viewport included.

`auto` with --scale is rejected. Fixes #1059. Adopts #2782 and #1881.

* fix(browse): screenshot accepts --clip=x,y,w,h and --selector=<css>

Agent-generated commands often use the GNU `--flag=value` form, which threw
"Unknown screenshot flag". Both value flags now accept it, through the same
validation and conflict checks as the space-separated form. Adopts #1563.

* fix(benchmark): price Codex cached input at the cache rate, stop calling Gemini OAuth ready

Codex reports cached_input_tokens as a subset of input_tokens, but the
adapter billed all of input_tokens at the full input rate. Split them
into disjoint uncached and cached counts (what estimateCostUsd expects)
and show new+cached input in the table and Markdown reports. A stored
Gemini OAuth file alone is now reported as not ready, with a reason
naming GEMINI_API_KEY; key-based auth stays ready. Adopts the focused
subset of #2283; the capacity error code and stdin change are left out
(no captured failure or reproduced hang).

* fix(review): catch loosened-input consumers and stale user-facing strings

The Enum & Value Completeness check fired only on newly added values, so a
diff that widens what a validator, parser or guard accepts, and breaks an
unchanged downstream consumer outside the diff, was a blind spot. Widen the
check's trigger (heading unchanged) and add a consumer-grep bullet. Teach
the maintainability specialist to flag user-facing strings whose guarding
condition changed while the text did not.

Adopts #2141 (rewritten on current main; the section heading is kept).

* fix(global-discover): bucket Codex sessions by originator

Every Codex rollout counted as one undifferentiated codex bucket, so
/retro global could not tell interactive Codex Desktop work from
codex_exec runs or Claude Code-driven calls. Normalize
session_meta.payload.originator into desktop, exec, claude_code or
other (missing, unknown and non-string values), report the split per
repo as codex_originators and globally as tools.codex.originators, and
print it in the summary. Existing totals are unchanged. Adopts #1488.

* fix(hook-check): cover skill frontmatter hooks, sourced helpers, conflict markers and the caller's tsconfig

Rewrite of #3066. bin/gstack-hook-check only listed setup's hooks plus
/autoplan's by name, so the /careful and /freeze PreToolUse checks (also run
by /guard and /investigate), /plan-ceo-review's hand-off hook and the helpers
they source were never parse-checked before Claude Code ran them.

- The hook list is setup's _hook_command_path arguments plus every gstack path
on a `command:` line in a generated */SKILL.md frontmatter (9 hooks, up from
6). The generated files are read rather than the .tmpl sources because the
/autoplan and /plan-ceo-review commands come from resolvers.
- Each shell hook's gstack-tree helpers (`.`/`source` lines, assignments and
shellcheck source= directives, followed transitively) are parsed too:
  careful/bin/hook-extract.sh and bin/gstack-state-root.sh for /careful and
  /freeze.
- The shim, its helpers, each TypeScript entry and every local module the
bundle used (bun build --metafile; entry only when bun lacks it) are scanned
for conflict-marker lines, which still parse inside heredocs, strings and
  template literals.
- bun build runs from the gstack root, so the caller's broken tsconfig.json or
bunfig.toml no longer fails every TypeScript hook. Lists are newline
separated, so a root with spaces works.

Tests in test/setup-hook-syntax-gate.test.ts cover each case; the gstack-doctor
fixture now copies autoplan/SKILL.md, whose frontmatter registers the hook its
hooks-row test breaks. docs/troubleshooting.md describes the wider check.

* fix(review): extend defensive-security framing to the security specialist and Red Team

The "authorized defensive-security review" framing and fixture summary mode
lived only in the adversarial subagent prompt. The security specialist and
the Red Team also reason adversarially over the full diff, so repos whose
test corpus is attack payloads could still get those passes refused.

Move both paragraphs into scripts/resolvers/defensive-review.ts (the
adversarial render stays byte-identical), state them once in the specialist
dispatch for the security specialist and the Red Team, and have the Red
Team prompt start from them. Testing and the other specialists keep full
fixture reads; the codex host still strips Review Army. Parity caps for
ship and review rise by the measured 1,380 bytes each (carve-guards notes).

Adopts #1921 (rewritten on current main).

* fix(team-init): make the required-mode hook shell-neutral (node CommonJS, no shell variables)

Rewrite of #2217. Required team mode wrote a bash check-gstack.sh and
registered it as "$CLAUDE_PROJECT_DIR/.claude/hooks/check-gstack.sh", which
only works when the hook runner is a POSIX shell. Under cmd.exe or PowerShell
(Windows, Copilot CLI) the variable is not expanded and the .sh cannot run, so
an intended allow or deny became a hook error.

- The hook is now .claude/hooks/check-gstack.cjs: the same root search
(GSTACK_ROOT, every host's global root, the migrated repo), with the home
directory from HOME, then USERPROFILE, then os.homedir(). It prints {} when
gstack resolves and the PreToolUse deny JSON with exit 2 when not.
- The registered command is `node -e "require(require('path').resolve(
  process.env.CLAUDE_PROJECT_DIR || '.', '.claude', 'hooks',
'check-gstack.cjs'))"`: no `$` or `%`, so sh, cmd.exe and PowerShell all
  pass it through unchanged.
- An existing check-gstack registration (matcher Skill or Skill|skill) is
replaced in place; duplicates are dropped and unrelated hooks in the same
entry kept. The legacy .sh is deleted only after the settings write
succeeds (a tracked one joins the suggested git add), and is kept when bun
is missing or the write fails, which now reports instead of claiming success.

Tests: team-mode and the hook-schema test run the command as registered;
test/team-init-hook-shell-neutral.test.ts runs it through the platform's
default shell (cmd.exe on the Windows lane, which curates it in) and through
PowerShell when installed (verified here in the pwsh 7 container). The
routing-probe check for the bash `_GSTACK_ROOT` gate now pins the same
contract in the .cjs: every host root probed, block only when none resolves.

* fix(setup-gbrain): register the gbrain MCP through bun.exe on Windows so no console window opens

Rewrite of #2007, folding #1963's MCP notes. On Windows the local stdio
registration stored the bun `gbrain` binstub; gbrain.exe starts bun in a new
console, so every Claude Code session opened a visible terminal for the MCP
(the contributor's process trees show it; bun.exe launched directly attaches
to Claude Code's headless ConPTY).

Under MINGW/MSYS/Cygwin the skill now resolves BUN_ROOT from BUN_INSTALL or
~/.bun and registers `$BUN_ROOT/bin/bun.exe <global node_modules
gbrain/src/cli.ts> serve`. That path also covers a `bun link`ed clone, whose
global node_modules entry links to it; gstack-gbrain-install records no other
clone location. When bun.exe or the entry is missing it registers
`$BUN_ROOT/bin/gbrain.exe` by its full name, because Claude Code does not
apply PATHEXT to a stored path (#1963). POSIX keeps the binstub line. The
skill also notes that `claude mcp list` can show "Failed to connect" while a
live session holds a local PGLite brain's single-writer lock (#1963).

test/setup-gbrain-windows-mcp.test.ts runs the generated fence under bash with
stub uname/claude and a temp HOME for MINGW64, MSYS with BUN_INSTALL, a
missing entry, and Linux. It is listed as Windows-incompatible (POSIX stubs);
the console behavior needs a manual Windows check.

* fix(hosts): rewrite every "Agent tool" phrasing for agent-runtime hosts

The Agent-tool rewrite for hermes, gbrain, openclaw and factory keyed only
on "use the Agent tool", but templates mostly say "via the Agent tool",
"with the Agent tool", "multiple Agent tool calls" or "Use the Agent tool",
so the Claude tool name leaked into those hosts' skills and sections. Add
ordered rules after the existing key (possessive, "the Agent tool", then
bare "Agent tool") mapping to sessions_spawn (gbrain/openclaw),
delegate_task (hermes) and delegation (factory). No goldens change.

* fix(setup-deploy): Vercel auto-deploys only when connected to the Git repo

/setup-deploy stated that Vercel always deploys on push, which is false for
CLI-only projects with no Git integration, and its nothing-detected path did
not say why such a project is invisible to detection. Qualify the Vercel
trigger (ask for and record the CLI deploy command otherwise) and note that
`vercel link` creates .vercel/ so a re-run detects the project.

Adopts the Vercel accuracy part of #2992 (rewritten on current main); the
third-party bring-up tool pointer is not adopted.

* fix(retro): persist recommendations so the next retro detects follow-through

/retro produced "3 Things to Improve" but never saved them in its JSON
snapshot, so the next retro could not tell whether last week's
recommendations were acted on. Step 13 now drafts the items before saving
and always writes a `recommendations` array ({category, text}, [] when
empty); Step 12 scores each prior recommendation addressed/partial/open
against this window's commits, files and metrics and renders a
follow-through line, skipping silently for snapshots without the key. The
report format ties the items to the saved array. Global mode is unchanged.

* fix(global-discover): count the interactive Codex CLI (codex_cli_rs) in its own cli bucket

* feat(developer-profile): --reconcile backfills office-hours tenure from timeline.jsonl

sessions[] is written only at office-hours Phase 4.5, so runs that ended
earlier never counted toward tenure and the tier undercounted. Add a manual
`--reconcile` that scans projects/*/timeline.jsonl for office-hours
`completed` events and appends a count-only row ({date, mode, project_slug,
signal_count: 0, signals: [], backfilled: true}) for each run with no
sessions[] entry for the same project within 60 minutes, extended back by a
duration_s under 8h. It is idempotent, skips malformed lines, and leaves the
file untouched on a no-op. `--read` counts backfilled rows for SESSION_COUNT
and TIER but takes LAST_*, DESIGN_*, CROSS_PROJECT and the nudge from logged
sessions only. Not wired into the preamble yet; the office-hours greeting
guard for backfill-only profiles is left to the office-hours owner.

* fix(autoplan): the publication hook approves /autoplan's own Reads after the skill turn

Claude Code applies a skill's allowed-tools only to the turn that invoked it.
On 2.1.292 the default fork-subagent mode runs reviewers in the background, and
the completion notice starts a new turn, so later Reads outside the working
directory (the next phase's section file, the close packet) raised permission
cards. Skill hooks stay registered for the session, and a PreToolUse "allow"
skips the card (reproduced in a PTY on 2.1.292).

autoplan/bin/owned-read.ts approves a parent Read only when the guard itself
returned no output, and only for this installation's autoplan/sections/*.md
(canonical path or the gbrain :user render) or an immutable phase artifact in
<repo>/.gstack/tmp/autoplan/autoplan-<phase>-*/ whose repo is the session's
project directory or contains it. Denials, unverified allows, child calls and
every other path are unchanged. renderSectionBase and userRenderRoot move
there with it.

Tests that asserted a verified driver Read as {} now expect the approval;
autoplan-owned-read covers the shipped shim, the close packet without a
journal, verified artifacts through the guard, look-alikes (foreign install,
writable copy, symlink alias, other repository, other names) and a guard
denial staying a denial.

* fix(autoplan): restore point and phase artifacts live in the project's .gstack/tmp/autoplan/

Skill hooks do not run inside subagents, so a reviewer launched after a
background completion notice raised a card for its own input (native-prompt.md
under the state root). Step 1 now puts RESTORE_PATH in the repository's
git-excluded .gstack/tmp/autoplan/ (added to .git/info/exclude, mode 700), and
the snapshot tool writes every phase artifact beside it, so /autoplan and its
reviewers read them from the working directory without a card. The guard's
bindings are unchanged: they anchor on the restore point's directory. Step 1
still prints the slug, which the old RESTORE_PATH carried.

A free test runs the generated Step 1 block twice from a repository
subdirectory: the restore point lands in <repo>/.gstack/tmp/autoplan/, the
exclude line is written once and git status stays clean. The autoplan parity

* test(evals): autoplan-guard-pty fails on any permission card after the reviewer's notice turn

The scripted boundary now mirrors Step 1's layout (restore point and artifacts
in the project's .gstack/tmp/autoplan/) and continues past the Phase 2 entry to
dispatch the Phase 2 reviewer after the CEO reviewer's completion notice. The
case asserts four verified guard allows, that the close packet Read, the Phase 2
section Read and the Phase 2 dispatch came after a task-notification turn, that
the Phase 2 reviewer read its input, and that no permission card appeared at
all. Still one trial on haiku, about 75-85 seconds.

* fix(outside-review): read a trailing '— Medium.' or '(High)' severity on a finding line, the shape Codex now writes

* feat(design): honor OPENAI_BASE_URL for every design API call

One resolver, openaiUrl() in design/src/auth.ts, now builds every OpenAI
URL the design binary, its prototype and its live model check use. Setting
OPENAI_BASE_URL sends image generation and screenshot analysis to an
OpenAI-compatible gateway; unset or blank keeps api.openai.com. The
prototype script now goes through receiptedFetch, so its egress is
receipted like the rest, and receipts already record the gateway host.

A source scan keeps any design file outside auth.ts from hardcoding an
OpenAI API URL again, and a request-shape test proves the gateway reaches
both the vision and image endpoints.

* review: testing specialist treats negative assertions as decisions

A new assertion that something is absent records a behavior removal and
needs cited intent. A rename or removal in the diff can empty a standing
negative assertion so it passes forever; the specialist now greps the suite
for negative assertions naming the old value.

Adopted from the testing-specialist part of a community PR; the scope-check
hunk targeted text that no longer exists.

* review: security specialist checks authorization a route-guard read misses

Four access-control classes where the guard is correct and the object still
leaks: error pages composed outside the guard, ownership surviving a role
downgrade, list/detail/download paths checked as one, and a session mutated
before the identity-switch callback is validated.

Adopted to review/specialists/security.md only; the /cso hunk targeted a
bullet list that is now an OWASP/ASVS table.

* review: zero findings is valid; suppress six common false positives

The checklist now says a clean diff may come back clean, and suppresses
framework-owned error handling, long exhaustive switches, null derefs a
type guard already narrows, N+1 on fixed-cardinality loops, hardcoded test
fixture values and non-cryptographic Math.random().

The contributor's "missing input validation on an internal function" and
"missing await on fire-and-forget" suppressions are left out: both can hide
real bugs.

* docs(todos): parked contributor ideas by theme; /autoplan permission follow-ups

* design-review: capture code tokens before universal principles

/design-review read only DESIGN.md and otherwise fell back to universal
principles, so a project whose system lives in CSS custom properties,
tailwind.config or a theme file got audited and mocked up against an
invented aesthetic. The skill now reads code tokens before falling back,
and the target-mockup brief reuses the captured colors, fonts, radii and
spacing, changing layout only.

register) are not adopted here.

* feat(sidebar): add a terminal text-size picker to the toolbar

The sidebar terminal toolbar gets an XS-XL text-size picker beside
Restart. SM (13px) stays the default, so nothing changes until a user
picks a size; the choice persists in chrome.storage.local and applies
to the live terminal, re-fitting xterm and re-sending the PTY size two
frames later so the new character cell is measured first.

A VM test runs the real sidepanel-terminal.js through one auto-connect
and checks the creation size, the stored-size fallback, persistence and
the live resize.

* feat(design-shotgun): generate with Codex's built-in $imagegen on Codex

On the Codex host, /design-shotgun now generates and edits mockups with
Codex's documented $imagegen skill in its default built-in mode (the
image_gen tool), so users signed in with ChatGPT need no OPENAI_API_KEY.
The design binary stays for the local comparison board only, images are
published with gstack-design-claim as before, and screenshot evolution
loads the captured page with view_image before an $imagegen edit.

Step 3c-3d moved into a DESIGN_SHOTGUN_GENERATION resolver that returns
the existing design-binary text unchanged for every other host; the
Claude output differs only in two host-neutral rewordings of the
evolve-from-screenshot pointers. DESIGN_SETUP and the regenerate step of
the board loop gain matching Codex-only text for design-shotgun.

* voice: ban the "load-bearing" stock phrase

Models overuse "load-bearing"; it joins the AI-vocabulary ban line in both
voice tiers. The tier-2 voice directive is also tightened (the meta
"compressed for runtime" tag and the bounded closer's restated exemption
clause) so the shared preamble shrinks by 163 bytes per tier-2+ skill,
which also pays for the reply-language rule that follows. The ship goldens
are refreshed for the new render.

* voice: reply in the language of the user's latest message

Every skill's voice directive now tells the agent to reply in the language
of the user's latest message unless asked otherwise, so long English skill
text no longer drags a Chinese (or any non-English) session into English.
Mixed-language sessions follow the latest message. Code, commands, paths,
identifiers and quoted output stay verbatim, and at tier 2+ so do the
machine-read question markers (D<N>, option letters, (recommended)).

The rule costs 205 bytes per tier-2+ skill and 144 per tier-1 skill. A
further 25-byte cut to the bounded closer's exemption list leaves the net
for this wave at +17 bytes per tier-2+ skill and +158 per tier-1 skill.

* feat(redact-prepush): committed exact-span allowlist at .gstack-redact-allowlist

The pre-push hook never fed the engine's allowlist option, so a reviewed,
known-benign value (a public dev-compose URL, a documented sample key) warned
or blocked every push, and the only ways out were a bypass or reshaping code
until the detector stopped matching.

The hook now reads the repo-root .gstack-redact-allowlist from each pushed
commit (never the working tree), one exact matched span per line with #
comments. Only a finding whose matched text equals an entry is suppressed: a password alone, a
substring, or a different key still reports and still blocks, and engine.*
unscanned findings are untouched. Every push that carries entries prints the
entry count and each suppressed finding's rule and file line. Oversized files
(over 64 KiB) are ignored. gstack.redact.allowEmail keeps working as before.

* feat(brain-sync): opt-in removal publishing with a bulk-removal valve

gstack-brain-sync never published a removal: a deleted synced file, or the
old directory after a project slug migration, stayed on the remote forever
and gbrain indexed both copies.

New setting artifacts_sync_removals (off by default). When on, each drain
commits and pushes, receipted like every push, the removal of tracked files
that are gone from disk and match the sync allowlist. Transcript pages are
never removed here, and a decision log goes only together with its active
snapshot. One valve: a drain that would remove more than 20 files at once
publishes none and says so. `gstack-brain-sync --publish-removals` lists the
pending removals and `--publish-removals --yes` publishes them on demand,
whatever the setting.

* feat(windows): setup.ps1 checks prerequisites and runs ./setup in Git Bash

A PowerShell entry point for Windows users. It checks that Git for Windows,
Bun and Node.js are on PATH (printing the winget command for each missing one
and changing nothing), finds Git Bash from git.exe rather than a PATH bash
that may be WSL's, and runs ./setup from the checkout with every argument
passed as its own argv entry to a constant bash script. No argument is
interpolated into shell source. ./setup itself is unchanged.

* fix(ci): link bunx after the SHA-verified Bun install in GitLab CI and the Ubicloud setup

* fix(redact-prepush): strip every parent column from combined-diff lines, so a merged release bump is exempt like any other

* fix(hook-check): treat a Windows drive path as absolute (cygpath when present), so --list finds the skill hooks from a native caller

* fix(windows): owned-read requires the read-only attribute on Windows; setup.ps1 passes arguments NUL-separated through a private file; Windows-only tests expect the owned-read approval

* perf(hook-check): bundle every hook's TypeScript entries in one bun process instead of two bun starts per entry (seconds each on Windows)

* perf(hook-check): read all skill frontmatter in one awk and scan conflict markers with one grep per hook (half the process starts)

b357b50378 v1.91.65.0 fix(install): skills only for the agents you installed, every runtime-root file their skills read (#3080)
* Fix spaced-out terminal text on Windows: put a Latin monospace before the CJK fallbacks

On Windows none of the Mac/Linux monospace fonts in the stack are
installed, so the first font that resolves is Malgun Gothic (the Windows
Korean UI font). xterm.js sizes its character cell from that font's
advance width, and Malgun Gothic advances Latin glyphs in a full-width
CJK cell — doubling every cell and rendering the whole terminal (and the
sidepanel mono UI) as spaced-out text.

Insert Consolas (present on every Windows install) and the Cascadia
faces ahead of the CJK fonts in both the xterm fontFamily and the
--font-mono CSS var. The CJK fonts stay last for actual CJK coverage.
No bundled font, no user install needed.

* fix(windows): every compiled binary answers --version before any side effect

browse --version started a server and exited 1, pdf printed usage and
exited 1, gstack-global-discover rejected the flag, and find-browse ran
discovery. A launch probe for Smart App Control (#2595) needs one
side-effect-free command that exits 0 on a healthy build, so browse,
find-browse, make-pdf and gstack-global-discover now answer --version
first (design already did).

* test(extension): pin Consolas ahead of the CJK fallbacks in both mono stacks (#2287)

Regression test for @tomfluff's fix: the xterm fontFamily and --font-mono
must list Consolas before Noto Sans Mono CJK KR and Malgun Gothic, or
Windows renders the sidebar terminal double-width.

* fix(cso): record each CSO build's outcome and stop arming the parent trap across the native publisher exec (#3071)

scripts/build-cso.sh now writes bin/.gstack-cso-build-result: result,
failed stage (build or publish), reason, this revision, the revision of
the generation still installed, whether its launcher is usable, and the
diagnostic log. An 'interrupted' entry is written before each stage so a
killed run is visible. A failed publish that restores the old generation
records it as still installed.

Before exec'ing the native publisher, the parent re-checks the locker,
clears its EXIT trap and enables execfail: the locked child's own trap
owns the stage. On Git Bash an exec of a native program may not replace
the shell, so the parent's trap could otherwise delete the stage under
the running child (the vanishing files in #3071; root cause unproven).
If the exec itself fails the parent cleans the stage and records it.

The Windows build prefers PowerShell 7 (pwsh) over Windows PowerShell
5.1, which crashed outright on the reporter's machine.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(setup): an optional CSO build or publish failure no longer aborts setup (#3071)

CSO was optional at probe time but fatal at build time: scripts/build.sh
ran build-cso.sh under set -e before the browse server bundle and the
completion stamp, and setup exited when the CSO artifact set was
incomplete. A failed CSO step now prints 'CSO unavailable: the <stage>
step failed' with its log (bin/.gstack-cso-build.log) and retry, and the
build and setup finish. GSTACK_STRICT_BUILD=1 keeps it fatal; the free-tests,
make-pdf-gate and windows-setup-e2e builds set it, so regressions still
fail CI.

setup's summary reads the build-result record instead of the pre-build
availability flag: probe failures keep their prerequisite instruction;
build and publish failures name the stage, the diagnostic and
'bun run build:cso && ./setup', and a restored earlier helper is
reported as 'previous CSO kept: <revision>'. setup records probe
failures in the same file.

The Windows prerequisite probe prefers pwsh, and a probe that fails for
any reason other than a missing toolchain or a compile error keeps its
exit status and first output line (windows-toolchain-probe) instead of
being labeled windows-msvc-toolchain. Reported and diagnosed by
@dviolante.

* feat(windows): detect Smart App Control-blocked binaries in setup and gstack-doctor (#2595, #2124)

Smart App Control refuses the five unsigned binaries gstack compiles
locally, and Git Bash reports it as 'Permission denied', so setup's
[ -x ] gate passed and skills failed later. bin/gstack-launch-probe.sh
runs each binary's side-effect-free --version and classifies it:
native, blocked (spawn error, exit 126/127, or an application-control
message), broken (launched, exited non-zero) or missing. It shows the
raw first output line, since the exact Windows text is unverified.

On Windows, setup names each blocked binary and the skills that need it,
cites #2595, says which shell ran setup and whether WSL is installed,
gives today's workarounds (WSL, or turning Smart App Control off), how to
undo, and the new troubleshooting section. gstack-doctor's browse row now
says 'blocked at launch' instead of passing, a binaries row covers the
other four, and a cso row reads the build-result record.

windows-setup-e2e builds with GSTACK_STRICT_BUILD=1, checks all five
binaries answer --version, and adds two forced CSO failures: fatal under
strict mode, and a completed setup that reports the failure and a doctor
cso warning under GSTACK_STRICT_BUILD=0.

No shim or launcher: it could not be called from PowerShell and would
load the caller's project .env/bunfig. Signing is the follow-up in
TODOS.md. Diagnosis credit: @knetics9000 (#2595, #2596), @smartjelic-sys
(#2595), @pstilwell90 (#2124), @cko32002 (#2265), @salluexez (#2127).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* docs: Windows Smart App Control known issue, optional CSO failures, doctor rows

README's Windows section says /cso failures no longer stop setup
(GSTACK_STRICT_BUILD=1 keeps them fatal), documents Smart App Control as
a known issue with today's workarounds (WSL, or turning it off) and the
Consolas fix. troubleshooting.md gains a Windows section for blocked
binaries and an entry for CSO build/publish failures. TODOS.md replaces
the Node CLI lane item with Windows code signing (needs a signing
account), the #3071 root-cause follow-up and SAC text verification.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(setup): never write the CSO build record into a source checkout that needed no build

Setup wrote bin/.gstack-cso-build-result for every probe-stage outcome,
so a run that built nothing still changed the source checkout's bin/
(caught by setup-codex-scope's unchanged-source checks). A missing
prerequisite now comes from the run's own probe, as before, and setup
only removes a stale record alongside the removed helper.
scripts/build-cso.sh remains the only writer. The doctor's cso row
reports 'native helper not built' with no record.

* test(extension): move the Consolas font-stack check into sidebar-ux (#2287)

The standalone file read extension sources without importing product
code, which the test-of-test ratchet rejects; sidebar-ux already owns
the sidepanel.css structural checks.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* v1.91.43.0 fix(windows): setup degrades instead of dying on a CSO failure, detects Smart App Control, Consolas terminal font

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(setup): every env-var runtime root carries the files its skills read as $GSTACK_ROOT/<path> (#1077 residue)

Codex, Factory, Kiro, OpenCode, Cursor and Copilot runtime roots linked
bin/, lib/ and the compiled tools but not the files the generated skills
read on demand: the preamble's jargon list and question registry, the
AskUserQuestion split and CJK docs, the DX Hall of Fame, the test-value
bar, the review checklists and specialists, design-html's vendored
pretext.js, VERSION and plan-design-review/SKILL.md. The reads failed
silently on every non-Claude host.

_link_runtime_dists now links the shared list for every root, and
_copy_runtime_skill_refs copies the host-rendered office-hours and
plan-design-review SKILL.md (the #2449 pattern) into each root. The
per-root review loops it replaces had drifted (Cursor shipped two of
four checklists, only OpenCode had specialists). hosts/define-host.ts
mirrors the list in sharedRuntimeRoot().

test/runtime-root-assets.test.ts renders every host, builds each root
with setup's own functions and checks every $GSTACK_ROOT literal on
disk; it fails on the previous setup for all five roots.
scripts/ship-measure.ts stays deferred (it needs the eval harness).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(build): an install renders skills only for the hosts installed from it (#1694)

./setup ran gen:skill-docs --host all inside the install, so a global
Claude install at ~/.claude/skills/gstack carried 632 SKILL.md files
(34.7 MB) where 56 are its own; Claude Code and Cursor-agent scan that
tree, and Cursor-agent freezes above roughly 11 MB.

setup now keeps an additive record of the hosts it installs from a
checkout (.gstack-installed-hosts, owned by bin/gstack-host-renders.sh),
seeded from installs that predate it: registry rows, install roots that
resolve to the checkout, host skills that link into its renders, and
legacy skill copies with no runtime root. scripts/build.sh renders claude
plus the recorded hosts and prints which hosts and why. A checkout with no
record (development) or GSTACK_RENDER_HOSTS=all renders every host. The
Codex .agents/ render runs only when Codex is installed or recorded;
bin/dev-setup records Codex because it serves the repo's .agents/.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(setup): prune skill renders for hosts a checkout does not serve, without losing a file (#1694)

Every ./setup now removes the render dirs of hosts missing from the
installed-hosts record, so an install that rendered every host (632
SKILL.md, 34.7 MB under ~/.claude/skills/gstack) converges on its own
hosts. It runs per install, so two installs sharing one state root are
both cleaned, and never touches a recorded host, so ./setup --host codex
keeps a --host factory render.

The rules follow bin/gstack-relink: a symlink into the checkout (STRONG)
is removed; a file proven generated (WEAK: gen-skill-docs' banner, the
generator's exact openai.yaml shape, byte identity with the checkout, or
setup's runtime pointers naming it) is moved to
$GSTACK_STATE_ROOT/backups/host-renders/<ts>-<id>/; any other file,
including a link outside the checkout or bin/dev-setup's link to the
checkout itself, stays where it is. Only directories left empty are
removed. setup prints each kept path and a per-host summary, and
prune.log in the backup lists every path. A tree with nothing left to
remove is silent, so reruns print nothing.

gstack-upgrade/migrations/v1.91.50.0.sh records the hosts an existing
install serves and runs the same prune; a checkout no install points at
is left alone. The Hermes, OpenClaw and GBrain hints say to add the host
to the record before rendering it by hand.

* fix(build): name the installed-hosts record in the build's host line

'Rendering skills for: claude codex (recorded in <checkout>/.gstack-installed-hosts;
GSTACK_RENDER_HOSTS=all renders every host)' instead of a nested parenthesis.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* v1.91.50.0 fix(install): skills only for the agents you installed, every runtime-root file their skills read (#1694, #1077)

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(team-mode): copy bin/gstack-host-renders.sh into the setup fixture

setup sources the new helper (#1694); team-mode's hand-picked fixture file
list lacked it, so its two setup runs exited 1 ('gstack-host-renders.sh is
missing'). Found by the final full free suite on 9b39cc6; product code is
unchanged.

Garry Tan 9e9e674b1e v1.91.62.0 fix(cso): Rails verification fits two arm64 offline preparations (#3086)
* fix(cso): Rails verification fits two arm64 offline preparations

Offline Rails preparation compiles native gems in 6.8-8.5 min on 4-vCPU arm64 (3.3-3.5 min on amd64),
so a two-phase attempt outran the 900 s bound and its watchdogs' scheduled cleanup surfaced as
ISOLATION_FAILED. MAX_VERIFICATION_ATTEMPT_MS is now 1800 s, the longest comprehensive run budget;
attempts stay cut to the run's reporting deadline.

v1.91.58.0

* Take v1.91.62.0 (1.91.60.0 and 1.91.61.0 are claimed)

4dfd83bf8c v1.91.60.0 fix(windows): setup degrades instead of dying on a CSO failure, detects Smart App Control, Consolas terminal font (#3079)

GSTACK_STRICT_BUILD=1 keeps it fatal; the free-tests,
make-pdf-gate and windows-setup-e2e builds set it, so regressions still
fail CI.

setup's summary reads the build-result record instead of the pre-build
availability flag: probe failures keep their prerequisite instruction;
build and publish failures name the stage, the diagnostic and
'bun run build:cso && ./setup', and a restored earlier helper is
reported as 'previous CSO kept: <revision>'. setup records probe
failures in the same file.

The Windows prerequisite probe prefers pwsh, and a probe that fails for
any reason other than a missing toolchain or a compile error keeps its
exit status and first output line (windows-toolchain-probe) instead of
being labeled windows-msvc-toolchain. Reported and diagnosed by
@dviolante.

* feat(windows): detect Smart App Control-blocked binaries in setup and gstack-doctor (#2595, #2124)

* test(extension): move the Consolas font-stack check into sidebar-ux (#2287)

The standalone file read extension sources without importing product
code, which the test-of-test ratchet rejects; sidebar-ux already owns
the sidepanel.css structural checks.

4e5ffbb857 v1.91.59.0 fix(autoplan): bind init through symlinked plan directories and superseded failed inits (#3083)
* fix(autoplan): bind init through symlinked plan directories and superseded failed inits

The publication guard compared snapshot init's JSON against the literal argv
spelling, but init reports canonical destinations, so a plan under macOS /tmp
or /var (realpath /private/...) always denied init_mismatch. The guard also
hard-failed on the first errored init, so a corrected re-run never bound.

- bin/gstack-autoplan-snapshot.ts: export canonicalDestination (the inner
destination() path math); initializePlan output is unchanged.
- autoplan/bin/phase-publication-hook.ts: compare activePlan/restorePath
against the canonicalized argv; a mistyped tool path is unbindable, not a
crash; skip errored inits and deny init_failed only when the most recent
init errored; Write/Edit to the active plan match by canonical path.
- autoplan/bin/guard-reasons.ts: init_mismatch never says re-run; init_failed
  does.
- tests: PAX-3638 Fix A/A2/B/C cases (bun test -t "Fix B" runs one fix); the
three autoplan-init assertions that failed on macOS now compare canonical
paths. 9 of the 11 new guard cases fail against the previous hook.
- docs: init_mismatch, init_failed, Symlinked plan directories.

Co-Authored-By: Claude Fable 5.1 <noreply@anthropic.com>

* docs(autoplan): canonicalRequested comment matches realpath behavior

realpathSync resolves a leaf link as well as directory links; only a
missing target or dangling link keeps the literal spelling. Note why
resolving a leaf link cannot bind an aliased active plan, and drop Read
from the callers list (only Write/Edit use it).

Garry Tanandcapy-ai[bot] 9a1dc81a2b v1.91.54.0 fix: office-hours first sessions get the first-session closing, design doc shown before approval, strict timeline --limit (#3075)
* fix(timeline): reject a missing or malformed --limit with exit 2 (#1723)

gstack-timeline-read parsed --limit with parseInt and fell back to 20, so
'--limit 1abc' became 1, 'nope', 0 and -5 silently became the default, and a
bare --limit crashed on an unbound $2. Validate the argument as a positive
integer before the no-timeline early exit and exit 2 with an example. No flag
still means 20.

* fix(skill-start): echo SESSIONS, the live session-marker count (#1651)

The sessions directory is touched and expired at skill start, but the count
was never printed, so CLAUDE.md rules that read the preamble's session count
had nothing to read. Echo SESSIONS: N (markers fresher than 120 minutes,
this session included). Idea from @0xDevNinja's PR #1747, redone in the
consolidated skill-start script.

* fix(office-hours): pick the closing tier from the profile read before this session is logged (#2801)

Phase 4.5 appended this session to the builder profile and Phase 6 then read
the profile to choose its closing, so every run saw itself as history: a
first-time user got welcome_back and was asked how the assignment issued
minutes earlier went. Phase 4.5 now reads the profile before --log-session and
records the prior tier, count and last-session fields in the transcript;
Phase 6 uses those values (this session is prior + 1) and re-reads only the
running totals. A failed read proceeds as a first session and says so. Tier
headers now name the session numbers each tier actually covers.

* fix(office-hours): print the full design doc inline before asking for approval (#879)

Tool output collapses in Claude Code, so users were asked to approve a doc
they could not see. Port of @RyanAlberts's PR #1116 into the carved
design-and-handoff section.

* fix(office-hours): report success only when this run wrote its design doc (#1049)

Before telemetry, check the design doc this run wrote, by its exact path under
the state root, never the newest file in the folder. Success needs a
non-empty doc there; otherwise the outcome is abort or error with failed step
design_doc. Rewrite of @walton-chris's PR #1049, which matched the newest
global session marker and hardcoded ~/.gstack.

* fix(plan reviews): skip the /office-hours prerequisite offer in spawned and headless sessions (#1958)

The BENEFITS_FROM offer had no recommended option, so a spawned run had
nothing defined to auto-pick and a headless run could only block. Skip it
there and continue with the standard review.

* chore: regenerate SKILL.md files

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test: office-hours tier order, failed-read fallback, inline doc, design-doc check and offer skip

Rendered-block tests run the Phase 4.5 read and log on a temp state root
(first session reads introduction, failed read is marked) and the
pre-telemetry check against this run's doc, an empty doc, another session's
doc and a path outside the state root. Parity caps take the measured growth.

* chore(release): v1.91.37.0 changelog and TODOS

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(browse): run watchdog test servers under a private HOME

The headed-mode case starts a real server with BROWSE_HEADED=1, and its
headed launch writes ~/.gstack/.auth.json for the extension. With the
caller's HOME, a slow kill let that write reach the real home and failed the
free runner's home-containment check (free-suite shard 5 on #3075). Give each
spawned server a private HOME and GSTACK_HOME, keeping Playwright's browser
cache path.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(evals): name the office-hours-auto-mode case so the PR lane can select it

The PR lane runs each short probe with a Bun name pattern ending in its case
id. The office-hours AskUserQuestion-blocked smoke had a descriptive test
name and no CASE_TEST_NAMES label, so whenever office-hours changed its shard
matched 0 tests and failed in 0 s (E2E Evals slice 6 on #3075). Name the
test after its id, keep the description on the describe, and check that
every literal-named PR probe has a test the pattern selects.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(office-hours): skip a last-session greeting clause whose value is empty

A profile backfilled from the timeline can carry a welcome_back or regular
tier with empty LAST_ASSIGNMENT and LAST_PROJECT. The welcome_back and
regular greetings now drop the "last time" clause and its question when the
value it names is empty, and say only "Welcome back."

Garry Tanandcapy-ai[bot] d082eccc50 v1.91.52.0 fix(qa): annotations limits shape; qa-quick does less work in the same 300 s budget (#3081)
* test(qa-quick): run the fixture in a committed git repository

/qa runs inside a repository: its clean-tree check and the evidence
recorder's revision come from git. The qa-quick working directory was a
bare temp dir, so all 15 CI transcripts read (runs 37374409928 to
37744583272) hit 'revision is required when git rev-parse HEAD is
unavailable' on the first materialize and spent a retry (6-46 s) plus a
learnings-log of the pitfall. qa-only-no-fix already commits its fixture.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(qa-quick): scope Quick to the fixture page, not its 404 links

basic.html's /page1 and /page2 links 404 on the test server. With the
links in scope, the 15 CI transcripts spent 55-75 s of the 30 s caller
probe window and its wrap-up navigating, screenshotting and checkpointing
those 404s, then triaging and reporting them as issues. The case proves
/qa Quick completes end to end (fallback browser, deadline guard,
checkpoint, evidence materialize, report); link-following and defect
detection are covered by the qa-b6/b7/b8 and qa-fix-loop cases. The
links are now listed as untested coverage.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(qa): say that annotations.json limits is a non-empty string array

exploratory.md §4 gave the annotations shape as {evidence: [...], limits}
with 'checkpoints in limits', but never said what type limits is. The
materialize validator (lib/qa-evidence.ts) wants a non-empty array of
strings. In 11 of 15 qa-quick CI transcripts the agent's first
annotations.json wrote limits as an object, failed with 'need limits
(non-empty string array)', and spent a retry. The section now shows the
shape with a one-line example and says it is never an object. Shared by
/qa and /qa-only through the QA_EXPLORATORY resolver.

Garry Tanandcapy-ai[bot] c7edf46bcf v1.91.47.0 fix(careful,freeze): safety hooks cover PowerShell and NotebookEdit (#3077)
* fix(careful): check PowerShell tool commands and nested pwsh/cmd (#3067)

/careful and /guard matched only the Bash tool, so on Windows, where
Claude Code's PowerShell tool is the primary shell, the guard never ran.
check-careful.sh also had no PowerShell or cmd patterns.

- careful/guard register a PowerShell matcher beside Bash.
- hook-extract.sh gains gstack_hook_extract_tool: one parser call reads
tool_name with the field, so dispatch adds no fork.
- check-careful.sh runs one PowerShell/cmd pattern table on PowerShell
tool commands and on the tail of any Bash command that launches
  pwsh/powershell/cmd: Remove-Item and aliases with -Recurse/-Force
  prefixes, rd/rmdir/del/erase /s, Format-Volume, Clear-Disk,
Clear-Content, [IO.Directory]/[IO.File]::Delete, and asks with an
explanation for -EncodedCommand, iex, Start-Process of a shell and
& $cmd. Command-position anchors keep Bash negative controls allowed.
- /careful documents the coverage as best-effort and points to Claude
  Code permission deny rules.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(freeze): NotebookEdit respects the freeze boundary (#3067)

NotebookEdit was in no freeze/guard/investigate matcher, and
check-freeze.sh read only file_path, so notebook edits ignored /freeze on
every platform. NotebookEdit carries tool_input.notebook_path.

- freeze, guard and investigate register a NotebookEdit matcher beside
  Edit and Write.
- check-freeze.sh reads file_path, then notebook_path, through the shared
extractor; denials name the tool, the field, the boundary and
/unfreeze. A helper without the combined reader denies as out of date.

* test: run hook-scripts on the Windows lane (#3067)

The careful/freeze hooks are Windows coverage, but test/hook-scripts.test.ts
was excluded from windows-free-tests by the raw /tmp/ pattern; its /tmp/
literals are payload strings that are never opened. Admit it by name and
gate the three cases Windows runners cannot host: a symlink (no Developer
Mode), a newline in a directory name, and a POSIX ':' PATH prefix.

* chore(evals): check-careful.sh is free-only for PR selection

No paid fixture copies or runs careful/bin/check-careful.sh (the
investigate boundary fixture copies hook-extract.sh and check-freeze.sh
only); test/hook-scripts.test.ts covers it. As an unknown dependency it
restored the full paid gate on every /careful hook change.

* fix(careful): hook payload reader survives Windows text-mode Python and backslash source paths

windows-free-tests (run 37739845723) showed every PowerShell-tool case
falling through: Python's text-mode stdout on Windows wrote tool_name as
"PowerShell\r", so dispatch never matched, and freeze denials carried
stray CRs. gstack_hook_extract_tool now reads and writes bytes.

hook-extract.sh also located gstack-state-root.sh with ${BASH_SOURCE%/*},
which strips nothing when the file is sourced through a C:\... path, so
gstack_hook_state_root vanished (state-root parity case on the same run).

* fix(evals): PR lane selects office-hours-auto-mode by its real test name

The PR paid lane filters audited files with -t built from
CASE_TEST_NAMES[id] ?? id. office-hours-auto-mode's Bun test is named
'AskUserQuestion surfaces when --disallowedTools AskUserQuestion is set',
so the filter matched 0 tests and the shard failed (#3077 eval-slices (6),

Garry Tanandcapy-ai[bot] 52eec18c63 v1.91.42.0 fix(cso): lift the snapshot file-count ceiling, bound report lists, actionable capacity errors (#3078)
* fix(cso): give snapshot.json its own 16 MiB cap and actionable capacity errors

/cso refused any repository with more than about 3,100 tracked files before
an audit started: the snapshot manifest was pretty-printed and both its
writer and every reader capped it at the shared 1 MiB private-state limit
(#3068, manifest half of #2993).

- snapshot.json is written compactly and capped by SNAPSHOT_MANIFEST_LIMIT
(16 MiB), a constant local to the snapshot module.
- readJson takes a per-artifact cap; readSnapshotManifest is the only
reader of snapshot.json and passes the snapshot cap through recovery and
the private-file size check. MAX_STATE_FILE and MAX_OUTPUT stay 1 MiB.
- The manifest-cap and 64 MiB source-cap errors name the bound cap, the
measured value, what counts toward it, the next step and #2993. A single
file that would cross 64 MiB now hits the same message before it is read.
- Older pretty-printed manifests still load (recheck, inspect).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(cso): bound per-entry report and evidence lists with omitted counts

Lifting the manifest cap alone moved the failure into the audit: report.json
listed one transformation per transformed entry plus one coverage gap or
exclusion per omitted entry (1.97 MB for 5,000 nested paths), and
sensitive-evidence.json grew per scanner hit (1.68 MB), both under the
1 MiB private-state cap that saveReport and the start stdout share.

- boundedList (state.ts) keeps the leading entries that fit a byte budget,
measured as the pretty-printed JSON they add at their nesting depth, and
ends the list with one summary carrying the omitted count.
- report.json snapshot lists share one 256 KiB budget (gaps 128 KiB,
transformations 96 KiB, exclusions 32 KiB); coverage counts and status are
computed before trimming, so they stay exact.
- sensitive-evidence.json keeps 512 KiB of per-file evidence plus an
  omitted-count record.
- Snapshot coverage and publicSnapshotPath no longer scan the whole manifest
  per entry.

Test: 5,000 long nested paths, mostly excluded or withheld, 999 files with
scanner hits, driven through start, inspect, submit, finish and recheck.

* fix(cso): admit Git path listings above 1 MiB during snapshot capture

With the manifest cap lifted, the next file-count wall was the shared 1 MiB
process-output cap on Git's path listings: ls-files/ls-tree --name-only
failed at roughly 15-20k files (23,749-file grafana/grafana fails there) and
the comparison-base ls-tree near 8-10k.

runProcess lets raw callers, whose output is never printed or persisted,
opt in to up to 16 MiB; git() forwards an optional cap, and the four
snapshot listings pass SNAPSHOT_MANIFEST_LIMIT. Every other caller keeps
the MAX_OUTPUT clamp.

* docs(cso): explain the capacity refusals and file the follow-ups

docs/troubleshooting.md#cso-capacity covers the snapshot-list cap, the 64 MiB
source cap and the tracked-symlink refusal. TODOS.md records the symlink
design call, the #2993 aggregate cap, the full-manifest inspect output and
repair-bundle transformations.

c7554824cb v1.91.38.0 fix(ios-qa): iPads work, a dropped route no longer relaunches the app, loud boot-token write (#3076)
* fix(ios-qa): allow physical iPad devices in tunnel bootstrap (#2743)

(cherry picked from commit d6d43934ef)

* fix(ios-qa): iPhone + iPad on USB names each UDID instead of picking one

With GSTACK_IOS_TARGET_UDID unset, tied default ranks used to pick the first
device devicectl listed, so a QA session could drive the wrong device. Tied
candidates now fail with multiple_devices, list each device and UDID, and
print a ready export line. Device selection moves into selectDevice() so the
route-drop recovery can re-resolve with the same rules.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(ios-qa): a dropped CoreDevice route no longer relaunches the app (#1975)

Rotation deletes the app's one-shot boot-token file, so the daemon's old
503/504 path (invalidate + full bootstrap) could only get a new token by
running devicectl launch --terminate-existing, wiping the in-app QA state at
every route blip.

A route failure now goes through recoverTunnel(): re-select the device with
the bootstrap rules, re-resolve the tunnel address for the pinned UDID, check
the unauthenticated /healthz owner, then probe /state/snapshot with the cached
bearer. Outcomes:
- bearer accepted: keep the session (new address if it moved)
- 401: full bootstrap from the fresh boot token (app was relaunched)
- app confirmed absent: bootstrap, which launches it normally once
- different device or an address devicectl cannot tie to the UDID:
bootstrap; the old bearer is never sent there
- route still down with the app running: surface the error, keep the session

Concurrent failures on one tunnel share a single refresh. Mutations whose
response was lost are still never replayed. The CLI wiring moves into
deviceTunnelSource() so tests drive it against a simulated device.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(ios-qa): a failed boot-token write is loud instead of silent (#1837)

StateServer.start() wrote the boot-token file with try?, so a failed write
left nothing on disk and no trace anywhere, and the daemon then relaunched the
app trying to mint a token that could never be written. The write is now a
do/catch that logs the path and error (never the token), logs a NOT READY
bootstrap line, and reports boot_token_error on /healthz. bootstrapTunnel
reads that field and returns boot_token_unavailable naming the device-side
cause instead of running --terminate-existing.

Also drops the stale comments claiming the token was written to os_log.
Template and fixture stay byte-identical.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(ios-qa): type the iPad acceptance test's mDNS stub

ResolveImpl returns Promise<string[]>; resolving null tripped the test-code
typecheck ratchet. A rejected lookup exercises the same resolve_failed path.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* docs(ios-qa): iPad support, GSTACK_IOS_TARGET_UDID, route-drop recovery, known limits

- iPhone-only description, triggers, and prerequisites now cover iPads, and
say how to pick one device with GSTACK_IOS_TARGET_UDID.
- Phase 2 no longer tells you to capture a boot token from os_log (it has
not been logged for many releases); it describes the 0600 tmp/ file and
the NOT READY signal (#1837, #1735).
- Source-control guidance for the generated DebugBridge/ package (#1735).
- Failure table: route drops keep the session, multiple_devices, unwritable
token file, and the one relaunch a daemon restart still costs.
- Known limits: SwiftUI DragGesture and /swipe on iOS 26 (#1975), the
iOS 26.3.1 /elements data from PR #1755, unverified iPad Stage Manager.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* docs: iOS QA how-to and skill index cover iPads and route-drop recovery

The how-to still said the boot token was readable from os_log and that a
route drop re-bootstraps; both are now accurate, with iPad selection,
GSTACK_IOS_TARGET_UDID, and three new failure rows.

* v1.91.37.0 fix(ios-qa): iPads work, a route drop no longer relaunches the app, loud boot-token write

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* docs(ios-qa): keep issue numbers out of model-read skill text

The archaeology lint rejects issue references in generated SKILL.md text;
the Known limits entry keeps the iOS version and drops the number.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* chore: re-slot to v1.91.38.0 (v1.91.37.0 claimed by #3075)

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(ios-qa): device-change recovery test without an IPv4-mapped address

The Windows lane could not reach a fake StateServer through
::ffff:127.0.0.1, so the iPad half of the device-change test timed out. The
fake device now swaps identity on its one ::1 listener, which is also closer
to production: every StateServer answers on the same port.

* fix(context-restore): offer newer checkpoints from nested repos and task worktrees

/context-restore could present an older checkpoint as the latest with no
error in two layouts:

1. A session started in a non-git parent saved from a nested repo, so the
checkpoint went to the nested repo's bucket. Restore in the parent now
lists checkpoints of other buckets whose project_root lies below the cwd
(BELOW_CWD; the 200 newest are read, and from $HOME only each project's
newest). /context-save also leaves a JSON pointer (checkpoint path, bucket,
project_root) in the starting directory's bucket when $CLAUDE_PROJECT_DIR
is set and files elsewhere; restore lists those as POINTER, in or out of git.
2. A plan checkpoint on the main tree's branch shadowed a newer checkpoint
saved in a ticket worktree. When another branch holds a newer checkpoint
for the same task (a worktree below the main tree, read from the new
worktree: stamp; equal normalized titles; or a shared #123 / ABC-123
token in titles or branch names), restore prints NEWER_TASK and the skill
shows both and proposes the newer one. Generic titles (wip, checkpoint,
save, notes, todo) never match on title alone, and the worktree rule
applies only when restoring from the main tree, so sibling worktrees
   still never shadow each other.

The branch-first ordering itself is unchanged.

* fix(redact-prepush): list each MEDIUM finding with rule, file line and fix

The pre-push hook printed only a MEDIUM count, which nobody could review
without re-running the engine by hand. It now prints one line per finding:
rule id, file and file line, and a fix, never the matched value.

Line numbers come from the hunk headers (combined --cc headers included), so
a finding in a later hunk or past a scan-chunk boundary points at the real
file line. Findings from the per-commit catch-up path name the commit whose
file line it is, so content removed before the pushed tip is still locatable.

Plan items: CEO-7 (summary half), DX-9 (fix lines), ENG-8 (line mapping).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(redact-prepush): stop flagging your own email and already-public authors (#3060)

The hook never passed the engine's selfEmail / repoPublicEmails, so every
push that added the pusher's address, or a commit author's, raised a MEDIUM
pii.email warning for an address the destination already shows.

The hook now reads `git config user.email` and one bounded `git log` of
author and committer addresses, raw and mailmapped (%ae %aE %ce %cE): the
history of every pushed tip (those commits become public with the push),
each remote tip being replaced, and, for a new branch or an unknown remote
tip pushed to a configured remote, that remote's own tracking refs. A URL or
unknown destination never borrows tracking refs, and another remote's
history never counts.

The read stops at 50,000 commits (the newest authors are used) and 5 seconds.
An error or the timeout passes no history and, when an email finding is
listed, prints one line saying suppression was limited for this push. The two
inputs fail independently. HIGH findings and every other rule are untouched.

Controls: a stranger's address, an address only another remote knows, a URL
push next to a matching remote, and a first push of a repo with no history
all still report.

Plan items: A3, CEO-7, CEO-23 (A3 half), DX-9 (partial history), ENG-8
(history scope, mailmap).

* feat(redact-prepush): per-address allowlist for pii.email (gstack.redact.allowEmail)

`git config --add gstack.redact.allowEmail <address>` allows that address for
MEDIUM pii.email findings only, so a team alias or support address that has
to be in the repo stops warning without reaching for --no-verify. It never
affects HIGH findings or any other rule, and the pii.email fix line names it.

Controls: a different address still reports, and a HIGH secret next to an
allowed address still blocks.

Plan item: DX-17.

* docs(troubleshooting): pre-push pii.email findings, exemptions and allowlist

Plan item: DX-12 (pre-push touchpoint).

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(redact-prepush): assemble fixture addresses at runtime

The email fixtures were literals, so this branch's own push listed 47
pii.email findings in its test file. Build them from a helper instead, the
way the neighbouring tests build credential and IP fixtures.

* fix(browse): offer the user's signed-in tab, name silent aside repl failures

Rules (rendered into every browsing skill, kept short):
- Rule 1: before the first openTab, offer the user's tabs on the target
origin (title and origin only; the rest of listBrowserTabs() stays
private) and attach only after the user confirms one.
- Rule 4: a second sign-in wall after a confirmed sign-in means the session
is bound to the tab or URL; offer that tab instead of another sign-in.
- Rule 6 no longer suggests an early return; rule 7 says a fast [ok without
  GSTACK_STEP_OK is an abort.

Cookbook (browse, devex-review): a "Silent failures" list (evaluate returns
only serializable values, no top-level return, locator(sel).press(key), an
empty DOM read is about the selector until the screenshot and the request's
response say otherwise, read toggle state before clicking) and a signed-in
tab recipe that prints only target-origin tabs and never closes the user's tab.

Parity caps for design-consultation, land-and-deploy, qa and the browse
skeleton are raised to the measured sizes, with notes.

* feat(codex): run gstack-codex-probe as a command, one subcommand per probe

Generated skills now execute bin/gstack-codex-probe (select-model, check-auth,
show-sandbox, check-sandbox, probe-model, check-version, show-first-use-notice,
run-with-timeout, log-event, log-hang, help) and capture its KEY: value lines,
so no helper state crosses into the calling shell and the preflight works from
bash, zsh or sh. select-model keeps --model > GSTACK_CODEX_MODEL > config.toml
precedence; run-with-timeout supervises with the existing gtimeout/timeout/
watchdog chain and returns 124 on a deadline; an unknown subcommand exits 64.

Sourcing keeps working for skills rendered before the upgrade and prints one
deprecation line naming /gstack-upgrade; it and the zsh self-locate block are
removed in a release on or after 2026-10-21 (DX-18). Parity caps raised with
measured notes for the longer capture lines.

* feat(gstack-post): one argv-only, redaction-scanned write path for PR and issue text

bin/gstack-post posts a PR/MR or issue comment, a review-comment reply, a
PR title or body, a new PR/MR or a new issue from agent-written files. Every
value reaches gh/glab as its own argv element in --flag=<value> form or on
stdin, never through a shell string, and a target is a number or a URL of
this repository on the same host.

The helper owns the redaction scan: it reads each file once, scans those
exact bytes (visibility from config, then gh/glab; the pusher's own email
suppressed) and sends the same bytes. HIGH is refused with exit 1 and no
confirmation can post it. MEDIUM exits 2 with RULE:/LINE:/PART: lines and a
TOKEN: digest of the bytes, destination, operation and findings; only
--confirm <token> for those exact bytes posts, so any edit needs a new
confirmation. gh/glab failures exit 3, usage errors 64. It also carries the
projectCards REST fallback and the title read-back that /ship did by hand.

The logic lives in lib/gstack-post.ts behind an injectable runner, so its
tests run in the Windows free lane; the stub-gh executable test is POSIX.

* fix(autoplan): guard verifies dispatches on Claude Code 2.1.29x (schema strip, unflushed call, named causes)

Cause A: Claude Code journals the model's raw Agent input but hands
PreToolUse the schema-parsed input; with the fork-subagent gate on (the
interactive default in 2.1.292) run_in_background is dropped, so the journal
record never deep-equaled the payload. nativeToolInput now drops only that
documented strip for Agent. Agent input is bound on both paths: keys limited
to prompt, description, subagent_type (general-purpose) and
run_in_background; a model override or any other key is denied.

Cause B: when a ready journal lacks the current call, the guard evaluates
the payload placed after the last journaled record instead of polling for a
record that may arrive only with the tool result. A phase transition counts
a report only when a later journaled record follows it (and, on the journal
path, it is not in the current call's message). One batch rule covers both
paths; a background reviewer keeps the invocation armed until its
completion notice.

Every outcome now has a stable reason code, disposition and anchor
(autoplan/bin/guard-reasons.ts). UC1: Claude-Code-caused states
(unrecognized shape, journal lag, rewritten journal, oversized record,
unchecked-version schema strip) allow with a visible warning; integrity
failures stay denials. No denial advises a bare retry. Every guarded
decision appends one content-free line to analytics/autoplan-guard.jsonl,
trimmed to 1,000 lines past 256 KiB.

* fix(autoplan): publish each phase report with a no-op call; foreground requests are conditional

Phase-close step 6 sends the report in a message whose only tool call is
the Bash no-op ': autoplan-published <phase>', and step 7 makes the next
guarded call in a later message, so the guard sees the report flushed
before the next phase entry regardless of Claude Code's journal timing.

Every site that asked for run_in_background: false now asks for it only
when the Agent schema offers it and says how to recover when a dispatch
runs in the background anyway (wait for its completion notice). Parity
caps rise by the measured bytes.

* feat(ship, spec, review): PR and issue text goes out only through gstack-post

/ship: Step 18 writes the title into an agent-written file. An existing PR's
title is piped from gh/glab through gstack-pr-title-rewrite.sh --stdin (new),
so neither the agent nor a shell string carries it; a new title is written
with the file tool and its version prefix checked in the file. Step 19
composes the body as before and publishes with gstack-post pr-create, or
pr-body then pr-title, which scan the exact bytes they send. The separate
title and body scans, the hand-rolled REST fallback and the title self-check
are gone (the helper does all three). Title preparation moved into the
pr-body section, so the skeleton shrinks.

/spec files its issue with gstack-post issue-create and drops the pre-issue
re-scan; Greptile replies use gstack-post reply and pr-comment.
scripts/resolvers/redact-doc.ts loses its PR and issue sinks, and a test
keeps any template from rendering one again.

Tests that pinned the old scan-then-gh wiring now pin the gstack-post wiring;
the publication tests run the real compose and publish blocks against a stub
gh with the real helper (HIGH in title or body refused, MEDIUM token flow).

* ci: pin Claude Code 2.1.292 in the CI image

2.1.292 is the npm latest users run. Its interactive default turns on the
fork-subagent gate, which strips run_in_background from the Agent schema
and backgrounds reviewers; CI on 2.1.284 never saw the /autoplan guard
break. The image tag is a content hash, so every eval workflow picks up
the new image without a workflow edit.

* fix(brain-sync): one drain lock for every writer to the artifacts repo

gstack-artifacts-init, gstack-brain-restore, gstack-gbrain-source-wireup
and the skill-start merge ran git in $GSTACK_HOME without the drain's lock,
so the drain could never tell an abandoned .git/index.lock from a live one.
They now take the same mkdir lock (bin/gstack-brain-lock.sh): the drain and
the skill-start merge skip when it is held, user-run writers wait for it
(GSTACK_BRAIN_LOCK_WAIT, default 120 s), and a child of the holder (the
drain's daily worktree advance, restore's wireup) inherits it by token.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(brain-sync): hold back only the flagged file, keep coupled artifacts whole

One file with secret-shaped text used to block every push, silently. The
drain now attributes scanner hits to files in one pass over the staged diff
(split on its diff --git headers), holds back each flagged file with the
rest of its coupled group, unstages it, and requires the remaining staged
diff to re-scan clean; anything short of that falls back to a full unstage
and status blocked. Held files stay queued and are re-scanned every drain.

- Status file gains held_count, drainable, last_drain_at, last_push_at and
held[] (path, scanner rule, dependents, both fixes); new status held.
- Coupled groups come from an explicit map (decisions.jsonl and
decisions.active.json): a queued member stages the whole group, a group
stages only when its snapshot matches its log in the index, a skipped
  member stops the whole group.
- --unskip-file undoes --skip-file and queues the path again.
- An index lock older than 10 minutes is removed under the drain lock; a
younger one, a failed git add, a failed commit (hooks included) keep the
queue and report status error. A verified empty staged diff is the only
"nothing to commit". git add runs once, batched.
- The scanner reads bytes, so undecodable content no longer crashes it into
a false clean; matched text never reaches the status file.
- An empty-queue run with unpushed sync commits stays push_failed.
- Privacy-held duplicate records collapse to one per path; --status counts
a spool too deep for a shell glob.
- --attention prints skill start's fixed attention lines from the sync

* feat(skill-start): ARTIFACTS_SYNC attention lines for a stuck sync

Skill start now prints one fixed attention line per sync-status-table row
that applies (held, blocked, push_failed, error, unknown, stale-push,
stale-drain), each naming gstack-brain-sync by absolute path. Values from
the status file are clamped (status code to the table, counts to digits,
timestamps to their shape), so the status message, file paths and scanner
output never reach the line. The preamble tells the model to surface the
line in one sentence and continue. Skill-start tests are generated from the
table in bin/gstack-brain-sync, and docs/gbrain-sync-errors.md carries the
same table.

* fix(outside-review): severity words in label position count as findings

The Oct 7 /autoplan run's Codex CEO and DX reviews labeled seven and eight
findings High/Medium without [Pn] tags, and the classifier printed
VERDICT: clean / FINDINGS: none for both. lib/outside-review-result.ts now
reads severity words in label position (Severity: High, a leading High: /
High — / [High], **High**, a table cell): critical/high block like P0/P1,
medium/low are advisory. Words in prose (high-level, low-risk, no critical
findings) never count. A review or structured response with neither tags,
labels nor an explicit no-findings statement is unverified, never clean
(DX-11, ENG-10). Design-direction proposals move to a new proposal gate so
they keep needing only their Recommendation line, and review prompts ask
for a severity per finding. Fixtures reproduce the CEO/DX label shapes, the
eng [P1] output, a table and prose negative controls; the stored Codex panel
keeps its verdicts. docs/troubleshooting.md explains how the verdict is read.

* feat: gstack-doctor readiness report and setup --status Codex row (B5)

bin/gstack-doctor prints one row per check without starting a skill:
install root (the registry rows behind ./setup --status), state root, Bun
against the floor, hook parse check, Codex mode and self-locate via
gstack-codex-probe subcommands, the cached model probe with its age
(--live runs the paid probe-model), artifacts-sync health from the status
file with the status code clamped, the browse bundle, Claude Code version,
the largest session journal and the last five schema-1 guard reason codes.
Rows are ok, warn, not configured or fail; only fail exits non-zero; fix
lines name helpers by absolute install-root path.

./setup --status gains a read-only Codex row (bin/gstack-codex-status.sh,
shared with the doctor) and ends with the doctor's absolute path.

* docs: bug-report template asks for doctor output; README troubleshooting entry; doctor test in the Windows lane

The template gives the Claude Code doctor path and a lookup for other
hosts and names no bare gstack- command (asserted in
test/gstack-doctor.test.ts). The doctor test runs every script through
explicit bash argv, so it joins the curated Windows free subset.

* fix(question-tuning,plan-tune,gbrain): free text reaches helpers only through agent-written files

Question tuning no longer pipes the question summary through printf: a
registered id needs no summary, and for an unregistered id the agent writes
the summary to .gstack/tmp/qt.txt and passes --summary-file (new). The
helper resolves a relative path against the git top level, removes scratch
files under .gstack/tmp, and asks normally when the file is missing.

/plan-tune tune events carry the user's own words again: they go into a
file the agent writes and reach gstack-question-preference --write through
--free-text-file (new), still behind the injection filter and the
user-origin gate.

docs/gbrain-write-surfaces.md drops its heredoc templates for the
agent-written page file on stdin, matching the per-skill save block, and the
generic save block stops showing a quoted --content argument.

The tuning prose was tightened so every preamble stays within its size cap.

* test(evals): daily autoplan-schema-canary on the latest Claude Code

A periodic rule case installs the latest published Claude Code with
install scripts off, verifies registry signatures, and runs one headless
session with the fork-subagent gate on whose PreToolUse hook records an
Agent and a Read payload. Each payload must match its journal record under
the guard's own comparison, keep the journal's key set apart from the
documented run_in_background strip, and stay inside the Agent allowlist;
a background Agent's completion notice must keep the shape the guard reads.
evals-periodic.yml runs it daily on its own schedule and concurrency group
(read-only token); the weekly census also includes it. A free test proves
the comparison fails on an injected strip.

* fix(doctor): read the artifacts sync held count from held_count, the field the drain writes

* fix(artifacts sync): the init and restore pre-commit hooks catch Bearer/Basic/Token-prefixed JSON auth headers like the drain does

* feat(greptile-early): detect Greptile, ask once on public repos, open the early draft, wait for the review

bin/gstack-greptile-early backs /ship's early Greptile PR:
- detect: a root .greptile/ folder (which takes precedence), greptile.json,
or a Greptile comment on the repo in the last 90 days; triggerOnDrafts from
the effective root config; repo visibility; public-repo consent. It prints
the signal, that /ship will push and open a draft now, the 10-minute wait
cap and the absolute gstack-config command that turns it off.
- consent yes|no: the public-repo answer, remembered per repo in the
  project's state.
- open: opens the draft through gstack-post with a body saying /ship opened
it early, and posts one @greptileai comment through gstack-post when the
config does not set triggerOnDrafts: true. An open PR is left alone.
- wait: polls every 30 s for a completed Greptile check run, a Greptile
review of the head commit or a summary naming it; each call returns within
about 90 s and reports timeout 10 minutes after the PR opened.

gstack-config gains ship_greptile_early (default true, invalid values
rejected): on automatically whenever the repo uses Greptile, off per user.
The logic is in lib/greptile-early.ts behind an injectable runner, so its
fixture-repo tests run in the Windows lane; an end-to-end test drives the
real helper and gstack-post against a stub gh.

* feat(ship): early Greptile PR at Step 6.5, bounded wait at Step 10 (#3020)

When the repo uses Greptile, /ship pushes and opens a draft PR right after
its free tests pass (new lazy section greptile-early, Step 6.5), so Greptile
reviews while Steps 7-9 run. A public repo asks once, remembered per repo;
declining keeps today's order. Step 10 waits for that review with
gstack-greptile-early wait and takes the existing unavailable route on a
timeout, never a claim of zero comments. Step 18 replaces the provisional
title, Step 19 marks the draft ready unless a draft was requested, and a
/ship that stops after Step 6.5 leaves the draft open with one comment
saying why.

ship's union cap rises to 1.5275 with the measured note (B4 +836 bytes,
B2 +4,732 bytes); the step-numbering guard allows 6.5; ship goldens are
refreshed for B4 and B2.

* fix(autoplan): the publication no-op is 'true autoplan-published <phase>'

Measured on Claude Code 2.1.292 in a default-settings interactive session:
a bare ':' Bash command asks for approval ('This command requires
approval'), and after a background reviewer's completion notice starts a
new turn the skill's allowed tools no longer cover it, so the phase-close
no-op prompted the user. 'true' with arguments runs without approval and
prints nothing. The guard only needs a later journaled record, so the
command changes everywhere it is named.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(evals): live autoplan-guard-pty case on the pinned Claude Code

A gate-tier rule case (one trial, haiku) drives an interactive PTY
session with default settings through a project skill that carries
/autoplan's own hook block: init, Phase 1 entry, a CEO reviewer dispatch
that sends run_in_background, a prepared close packet, the report
published with the no-op, and the Phase 2 entry in a later message. It
reads outcomes from the guard decision log (three verified allows) and
fails if the no-op raises a permission card; other cards are approved and
printed.

* fix(autoplan): bounded, append-tolerant owned-journal read (A2)

The publication guard no longer reads the whole parent journal or refuses
it at 32 MiB. lib/claude-owned-journal.ts streams the bytes present at open
to the last complete line (pass one), keeps a content-free index per record
and decides ownership, ancestry and causal order on it with the same checks
and reason codes as before. Pass two reloads in full only what the
evaluation reads (autoplan/bin/guard-journal.ts): every literal init and its
result, the current call, and inside the invocation window (from the latest
human turn at or before the bound chain's first init) guarded and
close-packet Read results, structured Bash results, error results, assistant
text after the first close Read and active-plan mutations, plus the earlier
Read a dedup reply cites even before the window (ENG-4).

- lib/claude-journal-records.ts: per-record event extraction, record
metadata and the causal-order checks, shared by readPlanCountTranscript
and the bounded reader. The batch reader keeps its whole-file read and cap.
- Appends after open never fail a read; same device and inode and no
shrink are required (identity). A second read in one hook invocation must
  hash bytes [0, first size) the same, or it is `rewritten` (UC1 unverified);
the unrecognized-shape double read uses that prefix hash. `changing` is
  removed (REMOVED_CODES; its anchor stays).
- OWNED_RECORD_MAX_BYTES (32 MiB): only one record over it is too_large.
OWNED_RETAINED_MAX_BYTES (32 MiB): over it is the new oversized_invocation
  fallback denial. GSTACK_TRANSCRIPT_TEST_MAX_BYTES and
  GSTACK_TRANSCRIPT_TEST_RETAINED_BYTES can only lower them.
- ENG-5: evaluation groups uses and results by tool id once (no per-tool
  rescans).

CEO-25 / ENG-5 spike (120 MiB synthetic, 31,242 records, 4 vCPU Linux,
Bun 1.4.2): a full streaming rescan (parse every line, small index, sha256)
took 254-326 ms at 147-161 MiB peak RSS; a pass-one index cache under the
state root (load cached index, verify a 64 KiB tail hash, parse the tail)
took 26-29 ms at 47 MiB. The rescan meets the 1 s budget, so the cache is
not adopted: it would add trusted persistent state and invalidation for no
budget need. A backward tail read cannot prove competing_root or cycle, so
it was not a candidate.

* test(autoplan): journal-read benchmark and its dispatch-only workflow (CEO-2, CEO-6)

scripts/measure-journal-read.ts builds two synthetic 120 MiB parent journals
around a real /autoplan invocation (mixed: ~28k records, most bytes inside
the invocation; large-invocation: ~89k records with a current invocation of
many small records), keeps appending while the guard reads, and measures one
hook call in a fresh process (wall ms, peak RSS). The workflow runs it on
ubuntu-latest and macos-latest on workflow_dispatch only.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* test(evals): autoplan-long-session, a resumed >100 MiB session enters Phase 1

CEO-15 with ENG-6's observer: a real session is compacted, its journal
padded past 100 MiB with owned records before the compact boundary, and
the resumed interactive session runs the scripted phase boundary; the
guard decision log must show a verified Phase 1 allow. A free test proves
that observer on a padded journal through the lower-only bound seams
(the PTY counting reader refuses the same journal), with a sidechain
control that must deny.

Garry Tanandcapy-ai[bot] db745675bd v1.91.33.0 feat(ship): measurement bar with explained-red and pooled decisions, weekly flaky-test sweep on main (#3059)
* refactor(eval-history): share pass-rate history loading as loadPassRateHistory()

eval:pass-rates' main block loaded local dirs or the weekly trial-outcomes
artifacts inline. Move that into an exported function returning records,
scope and fetch errors, so the off-ship sweep reads the same history the
same way. Behavior unchanged.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(ship-measure): the measurement bar and the weekly off-ship sweep CLI

Decide each diagnostic measurement with one bar (scripts/lib/measure-bar.ts):
MEETS at 9/10; MEETS-qualified at 8/10 when every red is a mechanical
provider error, judge noise or a cited model miss; EXTEND at 7/10 (or an
unqualified 8/10) for one more batch on identical inputs, decided once on
the pooled 20; BELOW otherwise and for any timeout, hang, regression or
fixable cause. Behavior uses 12 trials (11/10/9; 22/20 of 24); judge counts
median-panel outputs. A batch with more than 30% provider-evidence failures
is void and redispatched once. Below-strict reds nobody classified return
needs-classify.

ship-measure gains classify (append-only records; provider refused without
evidence), decide and extend (refuses a changed identity, names the field,
never a third batch), exit codes 5 and 6, and a pooling identity (frozen
tree, runner, CLI, model, CI image, Bun, policy and harness versions).

ship-measure sweep ranks main's gate cases by their cost to the all-green
probability from eval:pass-rates history, measures the top K with the same
runner and bar under a weekly cap shared with the last 7 days of sweep
reports (ship_measure_sweep_cases, ship_measure_sweep_budget_usd), and
writes a report with the predicted all-green probability before and after.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* feat(ship): the measure loop decides by the bar's exit codes

Step 4 branches on MEETS/MEETS-qualified, needs-classify (with the
qualifying and fix-round classes), EXTEND once and BELOW; the rest of the
loop prose is tightened to offset most of the growth. ship's union size
cap moves from 1.495 to 1.4975 (measured 281,058 / 187,706 = 1.4973).

* ci: weekly eval-sweep workflow on main

Mondays 12:00 UTC and on dispatch (k, cap_usd, dry_run): runs
ship-measure sweep in the published CI image by digest with the census's
paid-eval setup, downloads the last 7 days of sweep reports first so the
weekly cap holds across runs, uploads the report and captures, and writes
the job summary. Read-only grants; it never pushes or opens a pull request.
One sweep at a time repository-wide, never cancelled mid-spend.

* ci: classify eval-sweep.yml, the sweep CLI and actionlint labels as free-only PR inputs

None is read by a PR-gate case, so a change to them selects no paid case

inventory).

* fix(ship-measure): the selection preflight accepts name-mode cases

A case registered beside others in its file lists its trial shard as
<file>#<id>~t1, not <file> (trial 1), so the preflight refused every
name-mode case (for example ship-exploratory-late-input) before any paid
trial. Accept both forms.

* test(sweep): the fake single-case command is a Bun script, so it runs on Windows

The Windows free lane runs test/ship-measure-sweep.test.ts; CreateProcess
cannot exec a shebang shell stub, so every fake trial errored and the cap
stopped the sweep after one trial.

* test(plan-mode-no-op): the CEO sub-test reviews a fixed pasted plan

Without a plan, plan-ceo-review reviewed the checkout's branch diff, so its
pre-question work (audit and landscape searches) grew with whatever PR the
gate ran on. On this PR it timed out at 300s in 3 of 10 trials while still
researching the diff (ship-measure baseline 7/10). With a fixed small plan,
the same way the devex sub-test already runs, it measured 10/10 (CEO
sub-test 83-255s). No budget or threshold changed.

Co-authored-by: capy-ai[bot] <230910855+capy-ai[bot]@users.noreply.github.com>

* fix(ship-measure): each diagnostic trial gets its own EVALS_RUN_ID

Evidence-retaining cases (functional QA, docs faults) refuse to start
without EVALS_RUN_ID, which only CI sets, so measuring them locally ran
0/10 sessions. qa-functional-cli-report, red once in this PR's gate,
measured 10/10 once the trials started ($11.13).

+8 28f1385eac v1.91.32.0 fix: Codex works from zsh, free text never enters shell commands, honest failures (#3057)
* fix: sourced helpers locate themselves under zsh (#3024)

Skill blocks source bin/gstack-codex-probe and bin/gstack-egress-lib.sh into
the host's shell. Under zsh (macOS default) BASH_SOURCE is empty, so Codex
model selection resolved /../scripts/... and every outside pass reported
CODEX_MODEL: invalid; the egress lib resolved to the caller's cwd. Resolve
the file once at source time (BASH_SOURCE under bash, %x under zsh), and
replace the bash-only ${!name} indirection in the auth probe. A free test
derives every sourced helper from the generated skills and sources each
under bash and zsh.

* fix: reply and issue templates never pass untrusted text in double quotes (#3046)

greptile-triage's reply APIs posted -f body="<reply text>" while the
templates quote SHAs and diff code in backticks; in double quotes bash ran
each span (replies posted as 'Fixed in .', and diff or reviewer text ran as
a command). The pre-existing-failure issue template and /spec's issue title
had the same shape. All three now go through quoted heredocs. The
generated-bash lint gains untrusted-in-quotes and now also covers runtime
reference docs in skill directories (review/greptile-triage.md).

* fix: a Codex usage limit is its own state, not 'inconclusive; proceeding' (#3051)

The model probe matched only model and broken-install errors, so a usage or
rate limit fell through to MODEL_PROBE_INCONCLUSIVE: Codex's own message
(with the reset time) was dropped, the hint pointed at model config, and
every later skill re-probed for 30 s. The probe now returns
MODEL_QUOTA_EXHAUSTED (exit 4) on a failed, non-timeout call that names a
usage limit, rate limit, insufficient_quota or 429, relays Codex's line
verbatim and caches it for 15 minutes. Preflights map it to
CODEX_MODE: quota_exhausted (missing coverage, no further Codex calls), and
classifyOutsideReview reports a mid-run limit as unavailable (quota) with
its own troubleshooting anchor.

Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com>

* test: record ship/review union sizes after the quota branch and issue template

* fix: Codex watchdog freezes the command before reaping its children

The bash-native watchdog (stock macOS, no timeout binary) ran pkill -P on
the command's children and then killed the command. Between the two, the
command continued past its killed child and printed output after its
deadline. Stop it first. The test's minimal PATH also lacked pkill, which
the watchdog uses to reap children (stock macOS ships it), so the orphaned
child held stdout open and the capture waited out its sleep.

* fix(autoplan): an oversize session journal is a hard too_large denial, not a retryable identity failure (#3050)

The guard capped journal reads at 32 MiB and reported a longer journal as
'identity', which the hook treats as deny-and-retry. A journal only grows,
so /autoplan could never enter Phase 1 in a long session, and the
troubleshooting row sent people chasing symlinks. Oversize journals now
report too_large as a hard cause that names the size and the supported
fallback (run the reviews by hand or start a new session).

Co-authored-by: venkatareddyg <venkatareddyg@users.noreply.github.com>

* fix(deps): clear the weekly OSV findings (#3040)

Raise overrides to fixed releases: ip-address 10.7.3 (our own pin was the
vulnerable 10.3.1; reachable through socks in browse's proxy path),
fast-uri 3.1.8, hono 4.13.13, proxy-addr 2.0.8, @modelcontextprotocol/sdk
1.31.0, sharp 0.35.5, and in lib/diagram-render dompurify 3.4.16 (bundled
by mermaid) and source-map-js 1.2.2; the render page is rebuilt. Three
advisories without a usable fix get justified, expiring ignores: braces
(build-time only, excalidraw -> sass -> chokidar; a test checks the page
carries no chokidar code), sprintf-js (roarr's constant-format logger) and
katex (needs pre-existing prototype pollution; fix outside mermaid's range).
osv-scanner with the repo config: no issues found.

* fix(autoplan): name a shell-built snapshot init as the cause of the missing-evidence denial (#3045)

The guard binds a phase entry only to a literal snapshot-init Bash call.
When the model resolved the tool path into a variable, chained or piped the
call, init ran and succeeded but the denial said 'Complete the existing
snapshot init step', which reads as already done, so the run gave up. When
no init binds but a Bash call looks like one, the denial now says the shape
cannot be bound and prints the literal re-run (answers reused:true). The
binding rule is unchanged, and the skill says to run init on its own.

Co-authored-by: CharlesBerg85 <CharlesBerg85@users.noreply.github.com>

* fix(browse): chain reads piped stdin synchronously, so Windows runs the flow (#3039)

On Windows, an awaited Bun.stdin.text() inside the CLI's un-awaited main()
let the process exit 0 before reading stdin: a replayed flow reported
success with nothing sent to the daemon. Read fd 0 synchronously (skipped
for a terminal); empty input now gets the server's usage error and a
non-zero exit.

* fix(browse): clearer config-mismatch refusal and a WebGL note for the macOS GPU flags (#3030)

A plain call refused because another session started the daemon with
--headed/--proxy was told to 'apply --proxy/--headed', flags it never
passed. It now says who started the daemon and offers both remedies. The
BROWSER.md env table notes that the default macOS GPU flags disable WebGL
and that GSTACK_DISABLE_GPU=off restores it for WebGL QA.

* fix(ship): skip Greptile triage when no PR exists yet (#3020)

/ship's first run on a branch dispatched a triage subagent only to learn
there was no PR. Step 10 now checks for a PR inline (gh's 'no pull requests
found') and records that Greptile runs on the PR once it exists; other gh
errors still dispatch so the child can report unavailable.

* fix(router): route only to loaded skills, and say when not to invoke (#3018)

The root router listed every skill and ended 'When in doubt, invoke the
skill'. Current Claude models follow that literally: they tried to invoke
skills a user had turned off in Claude Code's skillOverrides (which gstack's
own disabled_skills render cannot see), and borderline requests tipped into
full workflows. The router now routes only to skills in the session's
available-skills list and names when to answer directly; the CLAUDE.md
routing block skill-start offers says the same.

* fix: spawned sessions never auto-grant consent or publish off the machine (#3016)

Spawned sessions auto-choose the recommended option except for destructive
or irreversible choices. Main already withholds the artifacts-sync consent
prompt from spawned sessions; the remaining gap was any other prompt whose
recommended option grants a consent or syncs data off the machine. Every
spawned directive (skill-start's block, the escape sentence, the Conductor
deny reason and the AUQ-error fallback) now names that as irreversible. A
consent the user already gave interactively stays in force.

* test: fix the spawned-block slice and type the #3045 re-init events

* fix(redact): TypeScript types and JSX expressions are code, not env.kv secrets (#3048)

A credential-named parameter or prop with an unquoted code value
(`session: SessionState,`, `key={turn.requestId + turn.role}`,
`apiKey={settings.apiKey}`) cleared the entropy gate and blocked pushes as
a MEDIUM env.kv finding. One rule replaces the PR's two keyword heuristics:
a value that reads as a code name (no digits, word-like camelCase or dotted
parts) with a code terminator, or a brace expression of such names with no
quoted literal, is source syntax. Paired controls keep firing on quoted,
brace-wrapped, concatenated, YAML-bare and random-letter literals.

* fix(setup): refuse to register a hook that does not parse (#3032)

Claude Code runs gstack's hook shims through /bin/sh; a shim that does not
parse exits 2, which for a PreToolUse hook blocks the tool call in every
session until someone finds the file. Before any hook registration is
healed or written, setup now runs bash -n on each registered shim and a Bun
parse of its TypeScript entry, and stops with the file, line and fix. A
lean rewrite of the PR's idea (one gate block plus a test that runs it).

* fix(browse): Windows runtime roots start the server from the source checkout's bundle (#3026)

On Windows a host runtime root (~/.codex/skills/gstack and the other
env-var hosts) holds file copies of browse/dist and no node_modules, so
its server-node.mjs could not import playwright and every browser skill
died (the module-scope server.ts half was fixed in v1.91.19.0). setup now
records the source checkout in every runtime root's .source-path, and the
CLI runs that checkout's bundle when the adjacent one cannot reach
playwright. Verified by unit layout tests and the Windows CI lane, not
live on Windows.

* fix(copilot): on Windows, run bash blocks in Git for Windows Bash (#3047)

Copilot CLI on Windows ran gstack's extensionless helpers straight from
PowerShell, which opened an app picker, and bare bash can resolve to WSL.
The Copilot tool glossary (rendered into every Copilot skill) now says to
run each bash block in Git for Windows Bash by its full path and never to
translate a block into PowerShell. The PR's PowerShell repair skill and
README section are not taken.

* test: record ship's union size after the Greptile no-PR check

* fix: keep issue numbers out of model-read Copilot text; seed the new routing sentence in the autoplan fixture

* docs: Oct 6 fix-wave plan, reviewed through /autoplan

* fix(autoplan): every oversize-journal path reports too_large with a recovery that keeps the work

Review follow-up to the #3050 fix. The summed batch read and the narrowed
parent loop now report too_large with the real size instead of a plain
error naming a hard-coded 32 MiB. The denial names the size and the cap,
and gives the recovery that keeps work (/context-save, new session,
/context-restore, /autoplan) with a docs anchor. A test seam that can only
lower the cap replaces the 33 MiB fixture.

* test(autoplan): negative controls and more shapes for the shell-built init denial

* fix(redact): decide TS/JSX source syntax by file context, not by value shape

Review follow-up to the #3048 fix. The word-shape heuristic also exempted
real secrets made of words (API_KEY=VelvetRiverOrbitSunset; in a .env).
The scanner now takes the file path (ScanOptions.sourcePath): the pre-push
hook scans the pushed diff per file with its +++ b/ path, and gstack-redact
--from-file passes its path. Only in a TypeScript/JSX file is an unquoted
value after ':' or a JSX brace of names treated as source; the same line in
.env/YAML, or with no path, is scanned as a literal, and the jwt pattern
still fires in .ts files.

* docs: gstack's own CLAUDE.md routing block matches the new router wording

Live check (Claude Code 2.1.284): a skill set to "off" in skillOverrides
is absent from the session's skill list while its sibling is listed, and
both are listed without the override, so 'route only to skills in your
available-skills list' holds.

* fix(sync-gbrain): don't offer a call-graph build for a source with no code pages (#3029)

Step 3.5 offered --dream for any source with pages, including one that
holds only markdown (synced with gbrain's default strategy), where a build
cannot help, and the injected CLAUDE.md guidance told future sessions to run
--dream for any count: 0. On gbrain >= 0.60, code-def for an impossible
symbol answers .status (probed live against gbrain 0.60.96: ready for a
code source, out_of_scope for a markdown-only one). Step 3.5 now probes the
pinned source first, reports the cause instead of asking on out_of_scope,
and keeps today's offer when the probe is unknown (older gbrain, an error).
gstack's own sync uses --strategy code, so the PR's soft-delete claim does
not apply to gstack's path.

* fix(setup): register the hooks that parse, refuse the rest with file:line (#3032)

The landed gate stopped setup before any registration, so one unparseable
hook left every other hook unregistered. On the Claude install the canonical
root is the checkout Claude Code runs hooks from, so there is no staged swap
to abort: setup now registers the hooks that parse, refuses the ones that do
not with file and line, finishes the install and exits non-zero, saying it
is a gstack bug and that a skipped hook keeps running from the checkout.

bin/gstack-hook-check derives its list from setup's _hook_command_path calls
plus the autoplan phase-publication hook (a test keeps them equal), parses
each shim with its shebang's interpreter, and bundles each TypeScript entry
with its local imports (packages external) into a temp dir, so a broken or
missing import, or a missing entry, fails.

Co-authored-by: BenjaminDSmithy <BenjaminDSmithy@users.noreply.github.com>

* fix(auto-update): hold an update whose hooks do not parse, before the checkout moves

bin/gstack-session-update fast-forwards the live checkout before setup runs,
and Claude Code runs hooks straight from it, so setup's gate cannot protect
an auto-update. After the Bun floor check the updater extracts the incoming
revision with git archive and runs bin/gstack-hook-check on it; a failure
holds the update like bun-too-old, records hook-does-not-parse with file:line
in the update status, and the next session start says the current hooks keep
running. Fixture-remote tests cover a broken shim, a broken imported module
and a parseable control.

* fix(browse): chain with no flow is a usage error before any daemon boots (#3039)

Empty piped input, a terminal with no arguments, and a readFileSync(0)
error (EAGAIN, EOF) now print the chain usage with the cause and its
troubleshooting link and exit 1 before ensureServer(), instead of sending
an empty argument to a daemon the CLI booted for it. Tests run the real CLI
with no daemon and no browser, plus the stub-daemon flow.

Co-authored-by: VaibDeveloper <VaibDeveloper@users.noreply.github.com>

* fix(browse): refuse a runtime-root CLI and checkout server from different builds (#3026)

A Windows runtime root runs the source checkout's Node server bundle through
.source-path. When the root's CLI and the checkout's bundle carry different
browse/dist/.version builds (the checkout was rebuilt without refreshing the
root), the CLI now refuses to start the server with both builds, the
./setup fix and its troubleshooting anchor. Unversioned dev builds are not
called skew.

Co-authored-by: ubay887 <ubay887@users.noreply.github.com>

* ci(windows): real isolated ./setup install, then a piped chain flow through the installed browse

windows-setup-e2e built the checkout but never ran ./setup. It now installs
the codex host into an isolated HOME (a runtime root copy with no
node_modules), then from an unrelated directory pipes a two-command chain
flow at a loopback page into the installed browse and asserts the page text,
and that empty piped input exits 1 with the usage error. Covers the
checkout-bundle path and the piped-stdin path end to end.

* test(lint): free text never enters a generated command (structural rule)

The generated-bash lint's untrusted-in-quotes rule (gh/glab/git/curl text
flags in double quotes only) becomes free-text-placeholder: any <placeholder>
anywhere in any command, in double, single or no quotes, and in heredoc
bodies (quoted or not), unless it is on the reviewed identifier allowlist
(test/helpers/placeholder-allowlist.ts) with the grammar a value must match.
Quoted-only identifiers (URLs, selectors, paths printed by gstack) must sit
inside quotes. Backslash-newline continuations now read as one command, so
a flag and its value on separate lines are still seen together.

* fix: reply, issue and spec text travel as agent-written files, never through a heredoc

Replaces the quoted-heredoc bodies with CEO-12's file mechanism: a bash
block creates a private mktemp file in the project's .gstack/tmp/ (kept out
of git through .git/info/exclude), the agent writes the text with its
file-write tool, and the next block posts with -F body=@file, --body-file
or "$(cat file)". A heredoc ends early at any body line equal to its
delimiter; a file has no such line. When the file was never written the
block refuses with the path and the command to send it by hand, and it
deletes the file after a successful post.

- review/greptile-triage.md: replies; the fetched JSON moves from fixed
/tmp/greptile_*.json paths to a mktemp directory; REPO and PR are
re-derived per block instead of read from a previous shell.
- Test-failure issues: title and body files, platform case on
github/gitlab, assignee only when it is a valid login.
- /spec: one draft file feeds the audit hash, every redaction scan, the
issue and the archive; the archive no longer expands <title>/<body>
inside an unquoted heredoc; the decision-log JSON is built by jq from
  files.

Location verified against Claude Code 2.1.284 (Dockerfile.ci pin):
project-local .gstack/tmp is writable under acceptEdits with no prompt and
keeps mode 0600; $TMPDIR and the gstack state root prompt even under
acceptEdits.

* fix(ship): Greptile triage runs only when a PR exists; every other case says why

PR: unknown (gh missing, not logged in, a non-GitHub remote, or a failed
lookup) used to dispatch the triage child anyway. The Step 10 check now
tests gh, its login, the remote and the PR in that order, and every case
but 'PR: exists' skips the dispatch and records
'Greptile: not run (<reason>); runs on the PR once it exists'.

* fix: question-tuning commands carry no free text

The question log took the summary in single quotes and the preference write
took the user's own words; an apostrophe ended the quote and the rest ran as
shell. The summary is now a kebab-case slug (an identifier with a grammar),
and the optional free_text field is no longer passed from skills (the
binary still accepts it).

* fix(cso): Windows finds docker.exe only under known-folder roots and says transport is unsupported (#3028)

On Windows /cso looked for docker only in Git's directory and System32, so
Docker Desktop was never found and the gap read as 'not installed'. It now
resolves Program Files, Program Files (x86) and the Windows directory through
PowerShell's [Environment]::GetFolderPath (not the process environment),
checks each candidate's real path against those roots and rejects reparse
points. lib/cso/docker.ts admits only unix:/// endpoints, so a trusted
docker.exe reports 'native Windows Docker transport is not supported yet;
static assessment only'; one outside the roots is refused as outside the
trusted install locations, not overridable. Both carry an anchor. Native
named-pipe transport is not implemented.

* fix: sourced helpers share one self-locate block and fail loudly when they cannot locate themselves (#3024 rework)

The codex probe and the egress lib now carry one byte-identical block:
BASH_SOURCE under bash or %x under zsh, made absolute with $PWD, else a set
GSTACK_ROOT that holds the helper. Anything else (dash, sh, a shell that
cannot name the file) prints "gstack: cannot locate <helper> (shell: ...)"
with the docs/troubleshooting.md#sourced-helper-location URL and returns
before any bash- or zsh-only syntax. The Claude-only default path and the
$(pwd) fallback are gone.

The generated Codex preflight dot-sources the probe and, when it cannot load,
reports CODEX_MODE: helper_unavailable with the helper's own line instead of
falling through to not_authed. The class guard enumerates nested sources,
makes real resolver, auth, state-root and egress-home calls under bash and
zsh (absolute, relative then cd, bare name via PATH), covers dash and an
unknown shell, runs the full preflight, and lints BASH_SOURCE to the shared
block plus other bash-only constructs.

* fix(cso): a Maven or Gradle repository is Java, not Python (#3028)

With no supported stack metadata, preparation defaulted to Python and asked
a Java repository for requirements.txt. A root pom.xml, build.gradle or
build.gradle.kts now names the stack as Java (unsupported for runtime
preparation, so the prerequisite says so), and Python helper scripts or a
root requirements.txt beside it no longer make it Python.

* fix(browse): BROWSER.md says plainly how to enable WebGL; the mismatch refusal links Daemon discipline (#3030)

The GSTACK_DISABLE_GPU row now states the effect and the switch: set
GSTACK_DISABLE_GPU=off to enable WebGL (off is the only recognized value).
The reworded config-mismatch refusal links BROWSER.md's Daemon discipline
section. Free tests pin both.

* test(windows): name the B2, B4, B5 and B12 tests in the curated Windows lane

The runtime-root bundle tests lived in browse/test/config.test.ts and the
Copilot glossary test in test/host-config.test.ts, both excluded from the
Windows lane, so the two Windows-only fixes were never checked on Windows.
Move each into its own Windows-safe file (unchanged assertions) and list it,
the piped-stdin chain test and the /cso docker.exe discovery test as named
Windows coverage in scripts/lib/windows-curation.ts.

* fix: the free-text block exits loudly when mktemp fails; spec and redact tests follow the file mechanism

The redact resolver test, the #2679 mktemp guard test, the /spec invariant
and the spec secret-sink harness pinned the heredoc transport. They now pin
the agent-written file: the harness runs the draft-file block, writes the
draft the way the file-write tool would, then runs the real scan fence, with
every HIGH/MEDIUM/scanner-failure assertion unchanged.

* test: hostile payloads through the generated posting blocks, identifier grammars, every quoting shape

- The lint reads a whole command, so an unquoted multi-word placeholder is
caught; negative controls cover double, single and no quotes, quoted and
unquoted heredocs, continuation lines, and quoted-only identifiers used
  unquoted.
- Every allowlisted grammar rejects quote-breaking characters (and shell
operators and whitespace when used unquoted); git accepts 'fix;id' and
'fix`id`' as branch names, the ref grammar does not.
- Greptile replies, test-failure issues (GitHub and GitLab) and /spec filing
and archive run from the real generated text with stub gh/glab: a body
with backticks, $(...), an apostrophe and lines equal to EOF and the old
heredoc delimiters arrives byte for byte, nothing in it runs, and the temp
file is gone. An unwritten file is refused with its path and the manual
  command, and nothing is sent.

* test(ship): the PR check precedes the Greptile dispatch, and each skip names its reason

Runs Step 10's generated check against stub gh for gh missing, not logged
in, a non-GitHub remote, no PR, a failed lookup and an existing PR. Fails on
the previous text (which printed PR: unknown and dispatched).

* feat(ship): ship-measure runner for the measure-then-fix loop (A6)

scripts/ship-measure.ts owns /ship's diagnostic measurement of a red eval
case: per-kind aggregation (rule trials; behavior panels of 3, each at its
own 2 of 3 plus 90% of trials; judge outputs scored by 3-sample panels,
standalone judge ids accepted), one artifact directory per baseline,
repair round and trial, a diagnostic label with verdict null that refuses
the eval history dir, an estimated admission budget that reserves every
concurrent trial and reconciles actual cost, the ask-once rule, the
repair-round limit, the unmeasured skip and the per-case PR table.

Free-suite flakes (CEO-1): 'free' reruns one shard's exact file list N
times, rerun 1 alone as the baseline and the rest in parallel at
max(1, floor(cores / shard concurrency)), each with its own HOME, state
root and flake ledger, GSTACK_FREE_RETRY_FLAKY=0, under a wall cap that
reports a partial count. test:ubicloud --diagnostic runs the same on one
VM with the retry off.

bin/gstack-config gains the DX-7 keys with their CEO-9 defaults and
rejecting validators. Free tests drive every kind, the budget stop, the
unmeasured skip and the free reruns with fake runners.

* fix: a Codex usage limit is quota_exhausted, a plain 429 is rate_limited, and only Codex's own error lines count (#3051 rework)

The probe's quota signature used to treat any rate limit, "too many
requests" or 429 as quota, and read the whole output, which echoes the
prompt. Now only Codex's trailing ERROR: / stream error lines are read.
A usage-limit or insufficient_quota line is MODEL_QUOTA_EXHAUSTED (exit 4,
cached 15 minutes; quota wins over a 429 that names insufficient_quota); a
plain 429 is MODEL_PROBE_RATE_LIMITED (exit 0, state rate_limited, never
cached), reported as CODEX_MODE: unverified (rate_limited) so the review
still runs and decides.

The quota HINT says how many minutes gstack skips Codex and how to retry
now (GSTACK_CODEX_PROBE_RETRY=1 skips the negative cache for that probe, or
delete <state root>/.codex-model-probe), with the full troubleshooting URL.
classifyOutsideReview reads the same trailing lines: a mid-review quota is
unavailable (quota_exhausted), a mid-review 429 unavailable (rate_limited).
New gate row rate_limited and docs anchor codex-rate-limited; the quota row
documents the retry. Tests cover each line shape, the echoed prompt, exit
0, timeout, retry recovery, probe/classifier parity, and two fresh-shell
preflight blocks where the second makes no paid Codex call.

* ci: the eval image installs zsh, and the macOS lane sources the Codex probe from zsh

Dockerfile.ci adds zsh so the sourced-helper guard runs (and fails loudly
when zsh is missing) inside the eval container too. The macOS named
regressions job sources bin/gstack-codex-probe from zsh in an unrelated
directory and runs _gstack_codex_select_model exec (no paid call), then runs
the sourced-helper guard on the macOS runner. A free test pins both lanes and
runs the macOS step's script locally.

* test: deterministic regression for the Codex watchdog freezing the command before reaping its children

The existing stubborn-child test caught the reap-then-kill race only some of
the time (2 of 3 runs on the pre-fix watchdog). A pkill that sleeps after
reaping widens the window between reaping the children and killing the
command, so a watchdog that does not SIGSTOP the command first always lets
it print "late" after the deadline. Fails 3 of 3 on the pre-fix watchdog,
passes 3 of 3 on the current one.

* test: live codex smokes run codex in a private HOME and CODEX_HOME

codex-resume-flag-semantics and codex-e2e-sol-scope probe the codex CLI (the
latter at module load, in every free run), and every codex run writes
$CODEX_HOME/tmp/arg0, so with a real codex on PATH they wrote into the
developer's ~/.codex. Both now give codex a private HOME and CODEX_HOME and
remove it afterwards. A free test runs each file against a stub codex that
records HOME and CODEX_HOME and asserts neither is the caller's.

* feat(ship): a red eval runs classify, measure, fix, re-measure, gate once (A6)

Step 6 no longer stops at the first red paid eval: it reads the new lazily
loaded measure section and runs the loop for each red case, and Step 5
measures a failed free-suite shard (diagnostic reruns, retry off) before
the triage can call it pre-existing. The section states the diagnostic-
not-verdict rule, the per-kind table, the ask-once and named-red stops,
the unmeasured label and the honest-fix wording (observed k/n after fix
at <cause>). A free test pins the steps and their order in the generated
skill and checks every command and flag it names exists in the runner.

Ship's parity cap rises to 1.455 (measured 272,944 / 187,706 = 1.4541).

* docs(testing): diagnostic measurement is not a verdict; per-kind measure table (A6)

* test(ship): seeded-flake paid case for the measure loop (A6, periodic, behavior)

A fixture repo with no remote documents a single-case eval command and a
gate command, both free local stubs. queue-priorities claims trial slots
with atomic mkdir (reset per ship-measure round) and fails slots 2, 5 and
9 while src/priorities.js leaves its seed-shuffled list unsorted, printing
failure_cause and failure_detail that name the line. The paid case gives
/ship's generated Step 6 and measure section to a live session and asserts
it measured the case alone, fixed the named line without touching the
eval, re-measured at least 10 passing trials on the sorted file, and only
then ran the gate once; any question other than the spend question is a
contract violation.

A free test measures the fixture through the real ship-measure CLI: 7/10
unsorted, 7/10 after an unrelated edit with the gate still red (ENG-13's
negative control), 10/10 after the sort with the gate green.

* test(cso): the missing-scanner case points Docker at a dead socket

'an optional scanner prerequisite does not turn completed manual coverage
partial' assumed no usable Docker. On a machine with Docker and a pulled
gitleaks image the scan completed and the test failed (also on main). The
scan step now gets a DOCKER_HOST that cannot connect, which is the
prerequisite-missing condition the test describes.

* fix: Aside prompts travel as agent-written files, never inside the command

The open-ended reading recipe (browse, devex-review), scrape's fuzzy-intent
read and the shared web-research block put the model's question, query and
reply format inside a double-quoted _aside_exec argument, where backticks and
$(...) from page or repo text would run. The agent now writes the prompt into
a private .gstack/tmp file; the send block rebuilds the path from the printed
basename, refuses when the file is empty, and passes "$(cat "$PROMPT_FILE")".
The fixed read-only sentence stays in the shell source.

aside-driver pins move from the inline prompt to the file-based send block.

* fix: design briefs and approval feedback travel as agent-written files

Design briefs went into $D variants/generate/check as a double-quoted
argument, and approval feedback was spliced into a single-quoted echo of JSON,
so a quote, backtick or $(...) in either broke the command or ran. The agent
now writes each brief into a private .gstack/tmp file and the block passes
--brief "$(cat "$BRIEF_FILE")" (the CLI's --brief-file expects a JSON
brief, so plain text uses --brief). variants and the later check reuse the
same brief file. approved.json is built with jq -n from --arg values and
--rawfile feedback, so the feedback is data. The office-hours mockup flow
creates its brief and feedback files in one block.

design-approval-flow now writes the feedback file (with quotes, backticks and
$(...) in it) instead of substituting <FEEDBACK>, and checks the file is
removed after the save.

* fix: carve office-hours on every host to stay under the 160,000-byte ceiling

With its brief, feedback and research prompts moved into files, the inlined
office-hours render is 161,329 bytes on Codex and 160,302 on Factory, over
SKILL_BYTE_CEILING, so gen-skill-docs --host all fails. Per the C4 policy
(carve every skill whose external render exceeds the ceiling), office-hours
joins ship and plan-ceo-review in CARVED_ON_EVERY_HOST. The Claude render is
unchanged; external hosts read its sections relative to the installed skill.
external-host-carve and qa-lazy-sections now include office-hours in their
carved-skill lists.

* chore: regenerate after the free-text mktemp guard

* fix(codex): prompts and review focus travel as agent-written files

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

Challenge and consult prompts, and the /codex review focus text, are written
by the agent into private .gstack/tmp files and fed to Codex on stdin or read
back with cat; no heredoc carries them. Consult's new and resumed runs share
one block (and one parser) so no placeholder stands in for the parser; a
resumed run reads its session id from .context/codex-session-id and checks it.
Adds the A3 group 2 placeholder allowlist block.

* fix(benchmark-models): an inline prompt goes through a prompt file

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix(land-and-deploy): PR head repository and branch travel as data

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

The branch check reads headRepository/headRefName from gh's JSON into quoted
shell variables; ls-remote, the confirm-first delete and the local delete use
those variables instead of model-substituted names. The postfail test now pins

* fix(office-hours): the reviewer receipt must match the receipt grammar before the check runs

* fix(setup-gbrain): cleanup-orphans collects the PAT with read_secret_to_env

* fix(ship): PR body drafts and rejected test paths are agent-written files

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

The PR body is written into two private drafts (through ## Documentation, and
from ## Test plan on); the scan block splices Step 14.5's section between them
into the scanned temp file and deletes the drafts. Rejected test paths are read
from a written list, and paths outside the repository are skipped. Tests that
ran the old heredoc blocks now write the drafts and list first.

* fix(sync-gbrain): orchestrator flags must match an allowlisted grammar

Co-authored-by: dgrant <dgrant@users.noreply.github.com>

* fix: the gbrain page save and external-host placeholders pass the free-text rule

The gbrain save block passed the whole page through a quoted heredoc inside
--content; the agent now writes it to a mktemp file and gbrain put reads it
on stdin. The Claude Code outside-voice runtime root and session mode, and
the feature slug, join the allowlist with grammars; unused entries are
dropped, and a test keeps every allowlisted placeholder tied to a real use.

* fix: skill-specific Aside research queries travel as prompt files

investigate, office-hours, plan-ceo-review, review, plan-eng-review,
design-consultation Phase 2, the shared test bootstrap (B2) and QA's test
bootstrap each carried their own _aside_exec call with the model-filled query
inside a double-quoted argument. Skills that already carry the Web research
block now give only the prompt-file text and use that block to create, write
and send it; design-consultation, the shared bootstrap and QA create the file
and send it with the new {{ASIDE_RESEARCH_SEND}} resolver, which is the same
send block the research section uses (read-only sentence stays in the shell).

aside-driver's B2 pin and qa-browser-preservation's bootstrap contract move
from the inline call to the prompt file and the shared send resolver.

* fix: prose design commands pass briefs and feedback as data

The DESIGN_SETUP command list taught `iterate --feedback "..."`, and
design-consultation's late-change and plan-design-review's slop-regenerate
lines showed `--feedback "<feedback>"` / `"..."`; design-shotgun's retry
showed `generate --brief "<brief>"` and `evolve --brief "<brief>"`. Feedback
now reads "$(cat "$FEEDBACK_FILE")" from an agent-written mktemp file
(the setup note says how), and the shotgun retry reads the variant's own brief
out of briefs.json with jq instead of retyping it.

* test: record parity sizes after the free-text file mechanism; refresh ship goldens

Measured union and skeleton sizes for ship, plan-eng-review,
plan-design-review, office-hours, design-consultation, review, qa, browse
and design-shotgun after every posting, prompt and brief site moved to
agent-written mktemp files; the ship goldens pick up the question-tuning
slug and the dropped free_text field.

* test: parity caps for the combined wave head (rounded up past the measured ratio)

* fix(plan-ceo-review): the mode handoff helper reminds the model to lead its chat with the line (#3038)

Periodic run 37272185151's auto-decide-preserved red replays as the Step 0E
handoff-line class: the model ran gstack-ceo-mode-handoff --auto, got the
exact line, then opened its chat with a heading and a paraphrase, so the
user never saw the auto-decide line (the terminal collapses tool output)
and the detector saw no outcome. The helper now prints a one-line reminder
on stderr beside the line; the detector accepts the line followed by that
reminder and nothing else. The ship Greptile check test reads its template
(test-of-test ratchet).

* test: every troubleshooting link in shipped code resolves, relative or full URL

* docs(todos): Oct 6 fix-wave follow-ups

* v1.91.32.0 release notes: Codex works from zsh, free text never enters shell commands, honest failures

* fix(plan-ceo-review): show the mode handoff line through a PostToolUse hook, not model compliance (#3038)

The stderr reminder beside the handoff line measured 1/10 on
auto-decide-preserved: the model still opened its chat with a paraphrase
('I've set review mode to HOLD SCOPE...'), so the user never saw the line and
the detector saw no outcome. /plan-ceo-review now declares a Claude-only
PostToolUse hook that shows the helper's exact line as a system message.
Diagnostic measurement on this head: 10/10, the line visible in every trial
while no trial's chat led with it. The reminder is reverted.

* fix: type the handoff hook's line filter

* fix: repairs from the integrated free suite

- The self-locate markers looked like heredocs to the pipe-deadlock guard;
they are plain comment fences now.
- The spawned block keeps the canonical 'conservative non-destructive'
phrase the cross-surface drift guard pins.
- The free-text block's mktemp refusal says 'mktemp failed', as every
  /codex temp file must.
- The batch transcript read reports sizes in MiB.
- docs/TESTING_INTERNALS.md gets a real 'Ship measure' heading for its
  anchor.
- The early-question harness admits /plan-ceo-review's handoff hook by its
exact command (it only adds a system message); any other PostToolUse Bash
  hook is still refused.
- Paid file count and the unconsumed-file example follow the new seeded
  case; ship goldens refreshed.
- The CSO scanner test points Docker at a dead socket, so a machine with the
  pulled image does not scan.

* test(cso): a host with no Docker binary is also a no-scan host

The missing-image scan test failed on a Docker-less VM (docker is not
installed in a trusted directory). Every accepted message is a reason the
scan cannot run here; the not_assessed, helper-owned artifact assertions are
unchanged.

* ci: split the USERPROFILE assignment from export (SC2155)

* fix(browse): Windows runtime roots read .source-path as a native path (#3026)

The new windows-setup-e2e journey showed the installed browse could not
start its server. setup runs under Git Bash and wrote .source-path as an
MSYS path (/d/a/gstack); Node read it as \d\a\gstack on the current drive,
found nothing, and fell back to the root's own bundle, which cannot import
playwright. setup now writes cygpath -m, and the CLI maps an MSYS drive path
for installs written by an older setup. The journey prints the daemon and
startup logs when the chain fails.

* fix(ship-measure): defects from the parallel #3034 plan review

- A trial passes only with a JUnit testcase that executed and passed; a
--case that selected nothing also exits 0.
- measure runs a no-cost --list preflight that must plan the case's own
  file before any paid trial.
- Diagnostic trials drop EVALS_CACHE_DIR/EVALS_CACHE_RUNTIME_ID, so they
never reuse a stored judge pass or publish one the gate could reuse.
- Gate once: when CI runs the gate on push, the push is the gate run; the
skill no longer also starts eval:bg:pr (a second paid gate).
- ubi-runner: down fails unless the VM is confirmed gone, and nothing
sweeps other runners' VMs by default (UBI_GC_HOURS opt-in).

* test(windows): the seeded-flake fixture drives a POSIX shell stub, so it stays off the Windows lane

CreateProcess cannot exec the fixture's ./evals.sh shebang script (status
null on windows-free-shard 6). The measure runner is covered on Windows by
ship-measure.test.ts, which uses in-process fake runners.

* fix: the free-text block refuses an unset temp dir instead of writing at /

The rendered block ran mktemp "$_GT/..." with _GT unset in the portability
test; on Linux that fails at /, but Git Bash on Windows can write to its
install root, so the block went on (windows-free-shard 3). It now uses
${_GT:?}, so an unset directory always fails closed with the mktemp
message.

* test: pin the guarded ${_GT:?} temp-file form

* test: the Greptile check's gh stub uses a portable shebang

On Windows the absolute bash path has a space, so the stub never ran and
every case read 'gh not logged in' (windows-free-shard 1).

* test: the Greptile check runs with the native PATH and the gh stub first

The PATH-only stub bin could not run its stub on Windows (every case read
'gh not logged in'). Stub cases now keep the real PATH with the stub first;
only the no-gh case drops directories holding a real gh.
